Arc flash generally refers to the dangerous exposure to thermal energy released by an arcing fault on an electrical power system, and in recent years, arc flash hazards have become a prominent safety issue in many industries. This problem however, has not been effectively addressed in the mining industry. MSHA data for the period 1990 through 2001 attributes 836 injuries to “non-contact electric arc burns”, making it the most common cause of electrical injury in mining. This paper presents results from several elements of a recent NIOSH study of arc flash hazards in mining, and provides information and recommendations that can help reduce these injuries. Characteristics of past arc flash injuries in mining are first outlined, such as the electrical components and work activities involved (based on MSHA data). This is followed by a review of important concepts and terminology needed to understand this hazard. Next, methods for identifying, measuring, and managing arc flash hazards on a power system are covered, with emphasis on recommendations found in NFPA 70E, Standard for Electrical Safety in the Workplace. Finally, results are presented from a detailed arc flash hazard analysis performed on a sample mine electrical power system using IEEE 1584-2004a, focusing on components and locations presenting severe hazards as well as engineering solutions for reducing the risk to personnel. As a part of a wartime program to aid in the development of known ore bodies and in the discovery of hitherto unknown deposits of strategic minerals, the Bureau of Mines investigated the feasibility of diamond-drilling the Christmas copper deposit, in Gila County, Ariz., to increase the reserves of milling ore. Upon recommendation of the examining engineer, drilling was begun in September 1942, from the bottom (770) level of the mine. One month later, in cooperation with the Bureau, the Federal Geological Survey started geologic mapping of the mine workings. Since extensive and costly development would be required, the War Production Board concluded that ore below the bottom level could not be mined at that time because of the wartime shortages of equipment and man-power. In January 1943, drilling was moved to the 300 and 400 levels to locate reserves readily accessible for continued production of fluxing ore. This drilling, which was under the supervision of L. G. Marshall, was completed in January 1944.

The European Union (EU) completed a new directive 2002/44/EC1 called the Physical Agents Directive (PAD) that establishes action and limit values for hand-arm and whole body vibrations. The directive specifies that: “…workers shall not be exposed above the exposure ‘limit value’.” 1 and “…once the exposure action values … are exceeded, the employer shall establish and implement a programme of technical and/or organisational measures intended to reduce to a minimum exposure to mechanical vibration and the attendant risks…” 1 The PAD limit value is effective for new machines starting July 6, 2007 and for used machines by at least July 6, 2010. These requirements apply to the users of machines, but machine manufacturers will be challenged to provide machines and information to help the users comply with the directive. Caterpillar manufactures machines with the goal of enabling our customers to comply with all regulations dealing with health and safety. Caterpillar designs all of our machines to provide a safe, comfortable and productive work environment. This study was to determine if seat suspensions could provide a reduction in the vibration environment experienced by operators of Caterpillar mid sized (<50,000KG) Track Type Tractors Methods Seat manufacturers were asked to provide seat suspensions that provide improved isolation over and above current seat suspension. Each supplier was provided with ride profiles and was asked to demonstrate the vibration reduction on a shaker table. Two suppliers provided suspensions that were compared with the current seat suspension in a field study. Three full factorial experiments were conducted. The first experiment was to evaluate overall suspension performance for four operations. The second experiment was to determine the benefit of the adjustable vertical damper at three different levels and the third experiment was to determine the effect of the fore/aft and side/side isolators. Six operators were used for the study. Acceleration was measured at the seat base and at the operator seat pad. Both the transfer function and the ISO 2631 RMS ride values were used to determine the seat suspension’s effectiveness of isolating the operator from vibration. A structured questionnaire was used to determine the operators’ subjective assessment of the seat suspensions. Results There was significant operator-to-operator variability in the vertical direction (>35%), however there was little variability in the fore/aft and side/side direction based on seat base

"INTRODUCTION In cooperation with the Boston Edison Co. and the Boston Consolidated Gas Co. of Boston, Mass., a survey has been conducted continuously since 1929 by the Bureau of Mines to determine the general hazards of combustible gases and vapors in manholes of the Boston Edison Co. The New England Telephone & Telegraph Co. also has cooperated in the investigation, and tests have been made in its manholes from May 1, 1933, to October 15, 1937. Yearly reports. 6/ on the results obtained since the beginning of the survey have been published.The methods and procedure of conducting the survey in 1937 were the same as heretofore."

The method for smelting zinc in horizontal retorts developed several centuries ago is still in use. This process consists essentially of the reduction of zinc oxide by carbon at 1,200° to 1,400° C. in a fire-clay retort. The reaction is highly endothermic, and a high radiation loss occurs in transferring the heat of reaction through the fire-clay walls. In recent years, silicon carbide retorts with much better heat-transfer characteristics than fire clay have been supplanting the old retorts. The practical impossibility of recovering the carbon monoxide formed in the reduction causes a large loss of fuel. Intermittent smelting is done in small retorts that are manually charged and discharged, and the furnace crews must be specially trained if efficient results are to be obtained. During the past 20 years several processes have been introduced that have substantially improved the horizontal-retort process. One of these, developed by the New Jersey Zinc Co., consists of a continuous process in which a briquetted mixture of zinc sinter, reducing agent, and binder is coked at 750° to 900° C. and charged into a vertical silicon carbide retort. A supply of displacement air or exhaust combustion gas is introduced at the base of the retort at an appropriate pressure in order to keep the zinc vapor from condensing on the cooled residue in the lower part of the retort. Any .oxidizing constituents in the displacement gas are converted to carbon monoxide by reaction with the carbon in the charge. The spent residue is continuously removed at the bottom of tl1e retort. The chief advantages of this process are said to be increased output per man-day, improved working conditions, high recovery of zinc, increased thermal efficiency, and long retort life.

The Bureau of Mines investigated the availability of flake and high-crystalline natural graphite from 4 domestic and 25 foreign mines and deposits in 11 market economy countries (MEC&apos;s). Demonstrated resources were estimated to be 95 million metric tons (Mmt) containing 5.61 Mmt of recoverable flake and high-crystalline graphite products. Fifty-five percent (3.17 Mmt) of the total recovered product is estimated to comprise plus 100-mesh (150-µm) flake pro-ducts and "lump" and "chip" high-crystalline products, designated as "graphite A" in this study; 45 pct (2.44 Mmt) of the total is minus 100-mesh (150-µm) flake products and "dust" high-crystalline products, designated as "graphite B." Total production costs, including all costs needed over the life of each operation, were determined at a 0-and 15-pct discounted-cash-flow rate of return (DCFROR) for both graphite A and graphite B products. For a 15-pct DCFROR, 1.74 Mmt (54.9 pct) of the graphite A products is available at $600/mt or less and 2.13 Mmt (67.2 pct) is available for $800/mt or less. For the same DCFROR, 1.08 Mmt (44.3 pct) of the graphite B is available at $200/mt or less, and 1.71 Mmt (70 pct) is available at $325/mt or less. No domestic resources were determined to be economic at present prices. A sensitivity analysis of the relative economics of the Alabama graphite deposits under various operational parameters is presented in an appendix. All evaluations are in January 1984 dollars.

This is the final report on a research program to develop a pilot plant gasifier for slagging gasification of lignite at high pressures and to obtain process data under various operating conditions. An operable pilot plant was developed, and the test results show that lignite is well suited for the process. Tests were run at pressures from 80 to 400 psig, and the results indicated that pressure had little effect on gas composition or the material requirements per unit gas produced. Gas production rates as high as 26,000 scfh/sq ft of reactor area were obtained at 300 psig, but limitations on auxiliary equipment prevented full-capacity operation at 400 prig. In several tests lignite having a 37-percent moisture content and 2,500° F ash-fusion temperature was successfully gasified. Refractory materials were found to have a short life in contact with high-temperature slag, and a water-cooled taphole was developed. Exploratory tests with coking coal were unsuccessful.

The Bureau of Mines has developed a prototype nonincendive explosive rock-breaker charge that can be fired unconfined in underground bituminous coal mines without the danger of igniting a flammable atmosphere that might be present. At present, unconfined shooting in underground coal mines is prohibited, but there are situations where the use of such shots would yield an overall improvement in safety. The charge consists of l-1/2 lb of permissible water gel explosive in the form of a short cylinder 7 inches in diameter and 7/8 inches high, surrounded by a l/2-inch-thick layer of damp salt, and encased in latex rubber reinforced with cheese cloth. The latex rubber housing provides a charge package that is strong enough to resist rough handling yet is pliable enough to conform to an irregular stone surface. A charge of this shape was found to be more effective at breaking rock than charges with lined or unlined cavities. Incendivity tests in a gallery have shown that the prototype charge will not ignite a flammable methane-air atmosphere when fired. Rock- breaking tests made with limestone boulders indicate that the charge will satisfactorily break stone slabs weighing 1 to 2 tons, and that two or more charges might be effective for slabs weighing up to 10 tons. In cooperation with the Mine Safety and Health Administration (MSHA), a test protocol for the certification of these charges as "permissible" for use in flammable atmospheres is being developed.

"This study reports a new property of the important NAD(P)H-dependent flavoenzymes, glutathione reductase, lipoyl dehydrogenase and ferredoxin- NADP oxidoreductase, that can catalyze a one electron reduction of metal ions such as chromium(VI) and vanadium(V). During the enzymatic reduction process, molecular oxygen is reduced to H2O 2 , which reacts with the reduced metal complexes to generate hydroxyl radicals. Since the hydroxyl radicals have been suggested to play an important role in Cr(VI) toxicity, this study provides a basis for a recent observation that Cr(VI) mutagenesis is strongly oxygen dependent. These results also point to an enzymatic pathway for the metabolism of some metal ions and concomitant generation of hydroxyl radicals. l. INTRODUCTIONThe purpose of this communication is to report that some important NAD(P)H-dependent flavoenzymes (glutathione reductase, lipoyl dehydrogenase and ferredoxin-NADP+ oxidoreductase) can catalyze one electron reduction of metal ions such as chromium(VI) and vanadium(V) and simultaneously reduce molecular oxygen to generate Hi02 and, eventually, the ""OH radical. This finding is significant because (i) the known substrates for the flavoenzymes are organic species and not metal ions, and (ii) the report on the enzymatic formation of the ""OH radical provides a new pathway for understanding the mechanism of genotoxicity of some non-ferrous metal ions. For example, it has been suggested that OH radical is likely to be the major species responsible for Cr(VI) genotoxicity [1-5). The 'OH radical is generated through the reaction of Cr(V) complexes with cellular Hi02 [4,5]. Cr(V) itself is known to be generated from Cr(VI) in various biological systems, in particular, microsomes [ 6), mitochondria [7] and mitochondrial electron transfer chain complexes [8]. However, no specific enzyme responsible for the Cr(V) generation has been identified except possibly glutathione reductase [4]. In this study we have used ESR and spin trapping techniques to show that the flavoenzymes can generate Cr(V) and provide a catalytic redox cycle for 'OH formation in such reactions. The U.S. Bureau of Mines investigated a method of using inert gas to prevent the formation of explosive gas mixtures in auger highwall mining of coal. A combination of gasoline and diesel engine exhaust gases was introduced into the auger drill hole using a short section of pipe located at the collar. Gas samples were taken and analyzed on-site with infrared detectors for oxygen, carbon dioxide, methane, and carbon monoxide. Evacuated bottle samples were also taken and analyzed by gas chromatography in the laboratory. These gas results were analyzed for explosibility. Coal samples from various depths were used to obtain the gas content of the coal using the modified direct method. Personal expo- sure to carbon monoxide was also monitored. The highest methane level observed was 9.55%. The levels of inert gas (carbon dioxide and nitrogen) were sufficiently high to prevent any ignition of the methane. Results showed that for all conditions during mining, gas concentrations were nonexplosive. The gas content of the coal was about 0.30 cm3/g. The maximum personal time-weighted average Sample for carbon monoxide was 20 ppm. This system provides a safe, inexpensive, simple method for preventing explosions during auger mining.

This Bureau of Mines paper describes the methods and practices used at the Peck quarry of the Crab Orchard Stone Co., Inc., one of the leading producers of dimension stone in the Crossville, Tenn., area. The product is marketed under the registered trademark "Crab Orchard Stone". Another large producer of stone from the Crossville district sells its product under the registered trademark of "Tenn-Quartz". Both companies, and a number of other companies and individuals who quarry and process dimension stone from the same general geological formation, are members of the Quartzite Producers Association of Tennessee. The salable product is shipped throughout the country for use as architectural building stone. In 1959, 653,008 cubic feet of stone was produced in the Crossville area and was sold for $1,414,405. Terms and descriptions used in this report are those of the quarry and building trades. The offices, pitching sheds, wire saws, railroad siding, and storage areas of the Crab Orchard Stone Co., Inc., are on a side road 0.3 mile north from a point 2.2 miles east of Crossville, Tenn., on U.S. Highway 70. The quarry is on this same side road 2.0 miles south of Highway 70. Crossville is 62 miles west of Knoxville and 65 miles northeast of Chattanooga, Tenn.

Musculoskeletal injury is a term used to describe a wide range of soft tissue disorders which affect the nerves, tendons, and muscles. Common examples include lower back pain, tendonitis, and carpal tunnel syndrome. The majority of these injuries are not the result of sudden mishaps, but usually develop gradually from repeated wear and tear. Symptoms may first appear after weeks, months, or even years. Symptoms may result from many types of activities, performed at work or at home, and it is often difficult to attribute a single event. In fact, it is more common to identify the factors which may have contributed to the development of the condition. The terms repetitive strain injuries or cumulative trauma disorders (CTDs) have been commonly used to refer to disorders that have occurred due to work related activities (Putz-Anderson, 1988; Fraser, 1989). Three main risk factors contribute to CTDs: force, repetition, and awkward postures. Any one or combination of these may contribute to the development of CTDs. Therefore, the design of equipment in conjunction with the required tasks should attempt to reduce these risk factors. Examining the layout of the work area to help identify tasks which may contribute to cumulative trauma is necessary. In the fall of 1946 and spring of 1947 the Bureau of Mines put down five inclined diamond-drill holes totaling about 3,100 feet of bore to test for the possible eastward extension of the Royal Princess ore deposit in Jo Daviess County, Ill. Attention of the .Bureau was first directed to this property by the Illinois State Geological Survey. The history of production of the Royal. Princess mine and reports as to the size and nature of the deposit were considered justification for investigating the trend of the mineralization. This report describes the work done by the Bureau of Mines and presents the data obtained.

Probabilistic design is gaining wider acceptance in the rock engineering community since it allows more rigorous determination of risk relating to ground fall or excavation instability. Risk analysis can be conducted by various means, but the basis is formed by either objectively or subjectively determining probability of occurrence of an event. In the case of rock engineering, this event is either instability or excavation failure. Rock mass classification systems provide objective analysis of data collected on a typically subjective basis that also relate closely to excavation stability. A probabilistic analysis technique is presented that uses statistical distributions of rock mass and material properties, ground support fixture specifications, stress conditions, opening geometry, and ground support installation quality to more rigorously determine probability of failure for an underground opening and the subsequent risk to personnel.

The U.S. Bureau of Mines has designed and tested a system called the Bag and Belt Cleaner Device (B&BCD) to reduce dust levels in and around the bag conveying and stacking process. The device physically cleans either 22.7 kg (50 lb) or 45.4 kg (100 lb) paper bags by using a combination of brushes and air jets. It is completely self-contained and is kept under negative pressure by a baghouse to ensure that dust and product removed from the bags during cleaning does not flow into the work environment and contaminate workers. The bags travel through the device on a chain conveyor, which permits any product or dust cleaned from the bags to fall into a hopper at the bottom of the device and be recycled back into the process via a screw conveyor. Once exiting the B&BCD, the outside of the bags and the conveyor are essentially product and dust free. The B&BCD was evaluated at two mineral processing plants to determine reductions with the device in use. The results of both field evaluations showed that the amount of product removed from the outside of the bags varied from 77 to 93 pet.

Investigations over several years have disclosed that the principal manners of occurrence of shuttle-car injuries are in order: 1. Victims in contact with roof or crossbars. 2. Victims in contact with ribs or timber legs. 3. Victims falling from, or jerked from, shuttle cars. 4. Victims in contact with overhanging brows or protruding pine for pin timbers, or water lines. 5. Contact with persons or collisions with parked equipment. 6. Victims injured by roof falls or falling timbers caused by knocking out roof supports with shuttle cars. 7. Victims injured by runaway shuttle cars. 8. Victims injured while loading or unloading timbers or other materials from shuttle cars. These recurring types of shuttle-car accidents reflect the physical conditions of the working sections, certain safety deficiencies in shuttle-car maintenance and design, and unsafe practices by operators.

The number of completed petroleum refineries in the United States continued to decline in 1938, but the total capacity to process crude oil increased materially. This information was obtained in connection with the Bureau's annual canvass of refinery capacity as of January 1, 1939. In spite of the addition of a half dozen plants in Illinois and the inclusion in this year's list of several more "pot-stills" in Montana, the total number of plants, and inactive, declined from 551 on January 1, 1938, to 538 on January 1, 1939. was entirely in shut-down plants, as the number of operating plants rose from both active The decline 431 to 435. Yost of the plants dismantled were situated in Wyoming and Texas, parts or several from the latter being transported to Illinois. About 10 more East Texas refineries were dismantled in 1938, leaving about 15 plants in the field, only 8 of which were operating on January 1, 1939. Although the total completed capacity increased from 4,351,151 barrels on January l, 1938, to 4,508,555 barrels on January l, 1939, the operating capacity declined, and the inactive capacity increased about 50 percent, or from 380,955 to 574,770 barrels. These shifts in capacities reflect the comparative slump in the refining industry in 1938 after the prosperous year 1937. Daily average crude runs in 1938 were 3,192,000 barrels, from which it may be figured that the operating ratio for the active units was about 81 percent of crude and for all completed equipment 74 percent. The highest daily throughput in 1938 was about 3,400,000 barrels, at which time the reserve capacity represented in the active equipment was at least 500,000 barrels, in addition to about that much capacity shutdown. In an emergency, much more reliance could be placed on the reserve capacity of the active equipment, as much of the shut-down capacity is obsolete at present specifications and prices. Regarding obsolescence, the refiners reported 418,165 barrels of their idle capacity in good condition on January l, 1939 and 156,605 barrels in poor condition. However, it is probable that the former reflects an optimistic viewpoint. Refinery activity, as represented by new construction and dismantling, declined materially in 1938. The new capacity added in 1938 was 378,350 barrels compared with 394,680 barrels in 1937. Probably 1939 will see a much larger decline, as the new capacity under construction on January 1, 1939, totaled only 142,250 barrels compared with 283,020 barrels the preceding year. The quality of hafnium metal produced by the Kroll process depends upon the purity of the hafnium tetrachloride. This investigation of methods for purification of hafnium tetrachloride was undertaken to aid in producing a higher purity hafnium metal than is currently produced by the Kroll process, The quantities of metallic impurities in hafnium metal were decreased by contact with molten alkali-metal chloride salt. Contact was achieved by a semicontinuous process of bubbling gaseous hafnium tetrachloride through a molten salt bath or by a batch process, first fusing the tetrachloride with a combination of alkali chlorides and then heating the mixture to sublime the hafnium tetrachloride. Low-melting mixtures of sodium, potassium, and lithium chlorides were used to purify hafnium tetrachloride. About 30 percent of the initial hafnium tetrachloride was trapped during purification with alkali- metal chloride salts. The 95-percent confidence limits (6)4 for the amount of oxygen decrease 60 and 34.3 parts per million when the batch purification is combined with a reduction so that both take place in the same equipment,

Results of initial research by the Bureau of Mines are given on the dust explosion hazard of float coal and on the related factors of occurrence, rate of production, sampling techniques, transport, and deposition of float coal. Tentatively, it is suggested that where float coal deposits prevail, a mini-mum of 80 percent incombustible be maintained in the top 1/8-inch layer to neutralize the explosion hazard of the surficial coal deposit.
