"The U.S. National Institute for Occupational Safety and Health (NIOSH) has been researching refuge alternatives (RAs) since 2007. RAs typically have built-in pressure relief valves (PRVs) to prevent the unit from reaching unsafe pressures. The U.S. Mine Safety and Health Administration requires that these valves vent the chamber at a maximum pressure of 1.25 kPa (0.18 psi, 5.0 in. H2O), or as specified by the manufacturer, above mine atmospheric pressure in the RA. To facilitate PRV testing, an instrumented benchtop test fixture was developed using an off-the-shelf centrifugal blower and ductwork. Relief pressures and flow characteristics were measured for three units: (1) a modified polyvinyl chloride check valve, (2) an off-the-shelf brass/cast-iron butterfly check valve and (3) a commercially available valve that was designed specifically for one manufacturer’s steel prefabricated RAs and had been adapted for use in one mine operator’s built-in-place RA. PRVs used in tent-style RAs were not investigated. The units were tested with different modifications and configurations in order to check compliance with Title 30 Code of Federal Regulations, or 30 CFR, regulations. The commercially available relief valve did not meet the 30 CFR relief pressure specification but may meet the manufacturer’s specification. Alternative valve designs were modified to meet the 30 CFR relief pressure specification, but all valve designs will need further design research to examine survivability in the event of a 103 kPa (15.0 psi) impulse overpressure during a disaster. BackgroundThe Mine Improvement and New Emergency Response Act of 2006, known as the Miner Act (U.S. Department of Labor, 2006), was enacted in the wake of three mine accidents involving explosions or fire that claimed 19 lives that year. Intended to help improve underground coal mine accident preparedness, the Miner Act includes provisions that target mine safety issues in areas such as emergency response planning; adoption of new technology; training and education; and mine safety standards enforcement. Section 13 of the Miner Act specifically directed the U.S. National Institute for Occupational Safety and Health (NIOSH) to provide for research into the effectiveness and viability of refuge alternatives (RAs) for underground coal mines, and the U.S. Department of Labor to act on the results of such research, as appropriate. These mandates culminated in the 2009 adoption of changes to the Title 30 Code of Federal Regulations, or 30 CFR, mining health and safety regulations (U.S. Department of Labor, 2008), requiring underground coal mines to supply mine emergency RAs and associated components so as to provide a life-sustaining environment for persons trapped underground. Such RAs can be either selfcontained mobile units or built-in-place facilities. The regulatory changes also include provisions establishing requirements for the approval of RAs and their components by the U.S. Mine Safety and Health Administration (MSHA), among which are numerous criteria for providing a safe breathable atmosphere under positive pressure within the RAs. One criterion for maintaining a safe RA atmosphere is the inclusion of an air pressure relief valve that will activate at a maximum of 1.25 kPa (specified as 0.18 psi, or approximately 5.0 in. H2O), or as specified by the RA manufacturer, above mine atmospheric pressure in the RA (U.S. Department of Labor, 2008)."

"The solubilities of rhodium (Rh) in a molten silver alloy in doré-smelting conditions for treating copper refinery slime at temperatures of 1,000 to 1,300 °C were measured using a high-temperature equilibration-quenching technique and direct phase analysis by electron probe X-ray microanalysis. The results obtained indicate that the solubility of rhodium in molten silver is very limited: only 0.04 to 0.26 weight percent at 1,000 and 1,300 °C, respectively. The thermodynamic properties of rhodium in molten silver were estimated from the solubility data. The compositions of the solid rhodium alloys in equilibrium with molten silver-rich doré metal were also measured in this study. The excess rhodium in the doré alloy will precipitate during the refining as solid, greater than 99 weight percent Rh.IntroductionSilver refining is a core step of the processes of anode slime treatment and precious metals recovery in copper refineries (Hait, Jana and Sanyal, 2009; Ludvigsson and Larsson, 2003). It separates the oxidic materials, such as nickel oxide (NiO) and barium sulfate (BaSO4), from metallics after the de-coppering and de-selenation steps of the anode slime, and refines the silver bullion from the undesired elements present (Cooper, 1990; Hyvärinen, Lindroos and Yllö, 1989). Doré metal is a silver-rich alloy which after the smelting step will be refined electrolytically, and the remaining constituents of gold as well as the platinum group metals (PGM) will be subsequently recovered in their own processing stages.Because of low concentrations of PGMs in copper, they are typically soluble in the doré metal in anode slime treatment. This can be concluded from their binary phase diagram data with silver (Massalski et al., 2007). One exception is sometimes encountered in the anode slime smelting operations if the smelter feedstock is rich in rhodium, as may be the case if the raw materials contain secondary raw materials such as e-scrap. The solubility of rhodium in solid silver is less than 0.04 weight percent (Królas and Sternik, 1995), and its solubility in molten silver is highly uncertain. The only experimental information in the literature (Królas et al., 1993; Elliott, 1985) suggests that the silver-rhodium alloy system at 960 °C forms a eutectic, which locates less than 2 °C below the melting point of pure silver (Arblaster, 2015). Early observations (Rössler, 1900) propose that rhodium is insoluble in molten silver even if solid silver-rhodium solutions have been recently produced on a nanoscale (Kusada et al., 2010).The aim of this study is to measure experimentally the solubility of rhodium in molten silver at doré metal smelting and refining temperatures. The silver alloy used to mimic the doré alloy contained silver and about 1 weight percent of the alloying elements copper, gold, palladium, rhodium and tellurium. The experiments were carried out in metal-slag equilibria with nitrogen-oxygen mixtures as the atmosphere, employing an equilibration-quenching technique and using direct phase composition analysis by electron probe X-ray microanalysis (EPMA) (Jak, Hayes and Lee, 1995)."

Over-exposure to noise remains a widespread, serious health hazard in the U.S. mining industries. Most other categories of illnesses and injuries associated with mining have improved, with the exception of hearing loss. Mine Safety and Health Administration (MSHA) coal noise data from 2000-2004 show that the continuous mining machine is first among all equipment with 35% of the noise overexposures. Joy Mining Machinery, in collaboration with the National Institute for Occupational Safety and Health (NIOSH), is conducting research to reduce noise generated by continuous mining machines. This paper describes a ?JOY Designed for Noise Reduction? (JOY-DNR; Patent Pending) tail section as a noise control for reducing the noise overexposures of continuous mining machine operators. Underground results show a 45% and 65% noise exposure reduction for the operator when compared to a standard machine. Utilizing this newly developed noise control, along with previously proven controls, will provide continuous mining machine operators an opportunity to be within the MSHA-Permissible Exposure Limit (MSHA-PEL).

Introduction In view of the continuing depletion of natural resources, requirements for energy conservation, plant optimization and hazardous waste management, and environmental consciousness, recycling and secondary recovery of materials are being developed with increasing interest Many secondary recovery processes are being developed that apply mineral processing technology. Gravity separation, magnetic separation and flotation have all emerged as explicit methods for recovering residual values from various process streams and hazardous constituents from waste streams. Recently, a technique for concentrating metallics has been successfully reintroduced. The concept of the alternating magnetic field eddy-current separator was developed over a century ago using rudimentary electromagnets (Patent 400317 issued to Thomas Edison on March 26, 1889). Widespread acceptance was, however, hindered by high costs, marginal results and poor economic incentives for secondary recovery. With the recent increased interest in secondary recovery and the development of high-energy rare earth permanent magnets, the eddy-current separator has now gained prominent acceptance. The separation forces imparted by an eddy current separator are the function of the magnetic field intensity and the alternating frequency of the magnetic field. The evolution of permanent magnets has provided a cost effective alternative for the generation of high-intensity magnetic fields. High-energy rare earth permanent magnets have substantially reduced the capital and operating costs over the electromagnetic circuits employed in antecedent eddy-current separators. Specifically, in recent years, the strength of permanent magnets has increased several fold with neodymium-boron-iron rare earth magnets now providing an energy product of 35 million gauss-oersted. Figure 1 shows the evolution in the strength of permanent magnets. The development of these rare earth magnets has led to the design of circuits possessing a magnetic force an order of magnitude greater than that of conventional permanent magnetic circuits. The eddy-current separator has been successfully applied in several metal sorting and recovery operations. Most common is the sorting of metal from shredded automobile scrap and municipal waste. With the advent of high-frequency, high-intensity magnetic fields, the separator has advanced to the point where it has direct application in the beneficiation of fine sized metals. Relatively fine sized (-1/4 in.) metal bearing slags and spent foundry casting sands as well as precious metal hearing electronic scrap demonstrate excellent metallurgical response to the eddy-current separator, and applications have progressed to finer materials and more selective separations. Theory Electrical currents are induced in all conductors when exposed to an alternating magnetic field. The induced current generates a magnetic field in the conductor that opposes that of the alternating magnetic field. Figure 2, is a schematic illustration of generated eddy currents. In this particular example, the alternating magnetic field is produced by a series of permanent magnets mounted on the circumference of a rotor. The permanent magnets have alternating polarity. As the rotor revolves, an alternating magnetic field is produced, and the rate of revolution determines the frequency of the alternating magnetic field. When a conductor, such as a metallic disc in the illustration, is placed in the alternating magnetic field, a closed loop current flow occurs. The current loop in the conductor produces a magnetic field that is a mirror image of the alternating magnetic field. At any point in time, the induced magnetic field in the conductor directly opposes the alternating magnetic field. This opposition of magnetic fields produces an instantaneous repulsion in the conductor. The conductor is consequently repelled from the rotor. The alternating magnetic field has no effect on nonmetallic materials as they pass through the magnetic field and discharge the rotor in a natural trajectory. [ ]

Much of what is reported was published elsewhere over a year ago (i.e., in their reference: Chi, G., Fuerstenau, M.C., Brandt, R.C. and Ghosh, A., 1996, "Improved comminution efficiency through controlled blasting during mining," International Journal of Min¬eral Processing, Vol. 47, p. 93). There is nothing unusual or significant about a 10% or more difference in grindability between two materials of apparently similar origin and other attributes (such as chemical assays, hardness and specific gravity). Consider also that: • the materials came from different locations in the pit; • only two sets of four blastholes were considered per blast: • spatial orientation with respect to the face and other blastholes was also altered; • only two pairs of data were collected; • the impact of sampling error and bias, • both of which could be significant in this case; and • normal experimental error in parameter measurement and calculation. It is clearly inappropriate to draw any conclusions with regard to cause-and-effect relationships without first presenting a thorough error analysis. A substantial number of randomly selected test and control areas might provide meaningful statistical evidence of the claimed effect of the variables being studied. Even that appears doubtful, however, because Figures 3 and 4 show about as much variation in crushing energy for the two "regular" conditions vs. regular and altered conditions. On a separate, but also important issue, broad experience with autogenous/semiautogenous milling has shown that smaller or weaker feed material results in reduced tonnage. ? Reply Thank you for the opportunity to respond to the comments of Messrs. Mclvor ant Fink. The points raised are addressed in the order of presentation. As the commenters noted, there are parallels between this article and a previous manuscript. The reason for these complementary articles is that the audiences for the two publications are different, although there is obviously some overlap. We believe that the different audiences each profit from the technical aspects of the two manuscripts. We are not in agreement that a grindability difference of 10% is insignificant. The laboratory work is corroborated by data obtained at plant scale, which confirms that an increase in throughput of the same magnitude (between 5% and 10%) is achieved when the blasting pattern is modified, i.e., reduced from 9 to 8 m (29.5 to 26.2 ft). This increase in throughput translates into substantial power savings for ore grinding. No differences in appearance in the rock were noted between the two adjacent areas. Rock in both areas belonged to the same rock mass/block, which was quite homogeneous and without geological features in between that could create drastic differences in comminution characteristics. Given the resources, we would have sampled from all of the holes. Additional data would certainly have been desirable, as would have been additional funding to expand the study. Their comment, "spatial orientation with respect to the face and other blastholes was also altered," is not clear. Figure 1 does not show the initiation sequence. Without the initiation sequence, we do not understand how the commenters concluded that spatial orientation with respect to the face and other blastholes was altered. Deep open-pit mines are becoming increasingly common in the highly mineralized arctic and sub-arctic regions. Air inversion is a frequent occurrence in these regions, and is exacerbated by complex geometry of the open pit and the surrounding natural topography of an open-pit mine. The resulting inversion cap is known to contribute to the degradation of air quality in the open pit, resulting in loss of production. This paper discusses the development and validation of a three-dimensional model that simulate the flow of air and the transport of gaseous contaminants in an arctic open-pit mine and the analyzes effect of the geometry of the mine and the slope angle of the pit on the contaminants profiles in the mine.

"Lithium is a soft, silver-white alkali metal with atomic number 3. As of 2015, the end use of lithium was estimated to be as follows: lithium-ion batteries (35 percent), ceramics and glass (32 percent), lubricating greases (9 percent), air treatment (5 percent), continuous casting mould flux powders (5 percent), polymer production (4 percent), primary aluminum production (1 percent) and other uses (9 percent) (Jaskula, 2014). Lithium is found in brine lake deposits containing lithium chloride and in hard rock pegmatite deposits. It can be sold in a range of product types; as mineral concentrate, brine, compound (lithium carbonate or lithium hydroxide) and metal depending on its intended end use (Goonan, 2012; Jaskula 2014).In the 1990s, the United States and Chile were the largest producers of lithium globally, primarily from brine operations (Ober, 1994). Most of the lithium compounds produced during this period were consumed in the glass, ceramics and aluminum industries. In the 1980s and 1990s, there were two major lithium operations in the United States: brine operations at Silver Peak, NV and a spodumene mine and lithium carbonate/lithium hydroxide plant in Kings Mountain, NC. The Kings Mountain operations ceased production in 1991 and were eventually dismantled in 1994. Brine operations were also active in Argentina and Chile, while the largest lithium ore deposit in the world was operated at Greenbushes, Western Australia. During this period, it was predicted that major shifts in demand for lithium would come from the success and widespread use of electric vehicles (Ober, 1994).Lithium is still produced from the brine operations at Silver Peak. But in more recent years, Chile has become the world’s leading producer of lithium carbonate from brines. Australia has maintained its position as the world’s largest producer of lithium mineral concentrate, the majority of which is exported to China to produce lithium carbonate and lithium hydroxide (Jaskula, 2014).A spodumene concentrate was also produced at the Tanco Mine in Manitoba, Canada beginning in the 1980s through to 2010 when lithium operations were suspended due to fluctuations in the lithium market price (Martins et al., 2013)."

A master composite (Master Comp) sample was prepared from three pegmatite zones from the Zoro Dyke D1 for mineralogical analysis and heavy liquid separation (HLS). The mineralogical examination was conducted on a subsample that was crushed to approximately P80 = 600 μm. The sample consists of spodumene (10.5 percent), quartz (29.3 percent), plagioclase (29.0 percent), K-feldspars (21.3 percent), micas (5.1 percent), tourmaline (2.9 percent) and Fe-Mn-phosphates (0.1 percent). Liberation of spodumene (88 percent) and gangue minerals, including quartz (89 percent), feldspars (94 percent) and micas (83 percent), is very good at this P80 . The average lithium (Li) concentration is 3.7 percent in spodumene, 2,196 ppm in micas, 1,001 ppm in tourmaline and 115 ppm in K-feldspars. Spodumene accounts for 96 percent of the total Li in the sample. The mineralogical data indicate the potential to recover spodumene by flotation with minimal Li losses due to other Li carriers. The HLS was conducted on a subsample that was crushed to smaller than 6.4 mm, with the smaller than 600 µm fraction removed from this test. The HLS results indicate it is possible to produce a high-grade lithium concentrate, with close to 6 percent Li2O, after the rejection of iron silicates with magnetic separation.

Newmont Mining Corporation owns and operates the Phoenix Mine near Battle Mountain, Nevada, USA. The existing milling facility has operated since 2006. Following the startup of the milling operation, and coupled with the upswing in the copper market, Newmont realized that Phoenix needed a process option to treat lowgrade transition and oxide copper ores. This meant that potentially revenue generating ore was being mined as waste. With no system in place to treat the transition and oxide copper, work began to determine the most feasible approach. After evaluating multiple options, the flow sheet was set for a run-of-mine (ROM) heap leach pad and solvent extraction/electrowinning plant (L-SX/EW). This paper will discuss the synergies and challenges of developing a copper L-SX/EW plant at the Phoenix copper-gold mill.

This paper describes the application of a cable disc elevator to continuous lift ore vertically from underground mines. Application of this system requires the tensions developed within the cable must remain within the carrying capacity of the cable including applicable safety margins. A critical element occurs at starting the system when it is fully loaded which requires the force developed by the system to exceed the static friction forces. This paper describes the laboratory rig developed to investigate the static friction forces. Details of the results of tests on three different ores for both dry and wet conditions are given. These results are discussed.

The North Singhbhum Mobile Belt (NSMB) lies between two crustal-scale shear zones, the South Purulia Shear Zone (SPSZ) at the north, and the Singhbhum Shear Zone (SSZ) at the south. The Babaikundi-Birgaon Axis (BBA) is an east-west trending secondary shear zone in the NSMB lying south of SPSZ, which is known for the occurrence of gold along with other sulfide minerals within the quartz reefs. The present study has been carried out at north of Ichadih village, which lies on the BBA. The present study attempts combined electrical resistivity tomography (ERT) and self-potential methods along a profile of 470-m length in the Ichadih area to map the potential mineralized zones. Initially, ERT sections generated by Wenner, Wenner- Schlumberger, dipole-dipole, and joint inversion of the combined array have been analyzed, and results are validated through the analysis of model resolution. A prominent low resistivity (10–100 Ωm) associated with quartz reef/limonitic quartz reef at a depth of approximately 30–85 m, encircled by a high resistive (more than 10,000 Ωm) formation associated with metasedimentary rock, has been delineated using joint inversion of the combined array. Further, analysis of self-potential (SP) anomaly using particle swarm optimization inversion also confirms the prominent low SP anomaly at a depth of 77.3 m, which is analogous to the low resistivity anomaly identified in the joint inversion of the combined geo-electric array. Subsequent analysis of a complex synthetic geo-electrical model analogous to the present field situation proves the efficacy of the joint inversion of combined arrays. As a whole, joint inversion of multi-array data is established as an efficient technique for better subsurface imaging in case of the complex subsurface structure. In-depth analyses confirm the possibility of sulfide mineral-rich deposit (quartz reef/limonitic quartz reef) which requires further geological drilling and lithogeochemical sample analysis, for possible future exploration.

Researchers from the National Institute for Occupational Safety and Health (NIOSH) developed a light-emitting diode (LED) area luminaire called the Saturn and conducted a laboratory study using a Fletcher High Dual-boom Mast Feed (HDDR) roof bolting machine. The Saturn luminaire was designed to (1) enhance floor illumination to enable better detection of trip hazards in the interior spaces of a roof bolter and (2) reduce glare that has typically been an issue of concern on roof bolters. This paper reports on the results of achieving the second objective. The existing roof bolter lighting was the baseline and was compared with three versions of the Saturn luminaire relative to light intensity (100%, 75%, and 50%). Discomfort and disability glare data were obtained from 30 participants that comprised three age groups. Discomfort glare perceptions were obtained using the De Boer rating scale, and disability glare was quantified by using Mars Letter Contrast Sensitivity tests. Discomfort glare was reduced at least 3 levels with all Saturn versions. Also, a predictive model was used to estimate discomfort glare, and the results were similar. Disability glare was the least for the Saturn’s 50% intensity, and all Saturn versions had significantly less disability glare than with the baseline lighting. Veiling luminance was calculated and used as another indicator of disability glare. Veiling luminance was 28 to 42 times greater with the baseline lighting as compared with that of the Saturn lighting. Lastly, visibility levels were calculated. The Saturn versions were 4 to 6.5 times better in terms of visibility level.

Disa Technologies Inc. provides innovative, sustainable solutions for mineral processing and remediation through its patented High-Pressure Slurry Ablation (HPSA) technology. HPSA processes solid feed material in slurry form, propelled by high pressure pumps through opposing nozzles that generate impinging jets within a collision housing. This process induces particle-to-particle collisions in a high energy zone, creating intergranular fractures along grain boundaries. In our batch scale and continuous operating testing, we have found that controlling key processing variables such as nozzle velocity, slurry solids percentage, and the number of passes can allow for controlled size reduction and maximum mineral liberation. Since 2018, Disa Technologies has demonstrated the versatility of HPSA by testing it on diverse materials, including phosphate, copper, uranium, vanadium, zinc, rare earth elements, lithium, nickel, mineral sands, gold tailings, and more. The intent of this paper is to present an overview of our test results describing in detail impacts on size reduction, recovery, and grade with a focus on HPSA's adaptability across ore types. Looking forward, it explores the potential to process materials such as DR-grade iron, clay, PGMs, bauxite, and various tailings. Additionally, it examines planned advancements in HPSA technology, including optimized nozzle spacing, higher capacity models, improved tonnage-to-footprint ratios, enhanced maintenance features, and expanded nozzle configurations.

The Mineral Resources Institute of The University of Alabama conducted research on the removal of sulfur from coal using a combination of gravity concentration and flotation methods. The research utilized the gravity and centrifugal force capabilities of the Humphreys spiral to remove part of the pyritic sulfur and ash. Flotation of the Humphreys spiral coal concentrate removed additional sulfur and ash to yield products that conform to Federal air quality standards for sulfur oxide emissions. The clean coal products recovered by the combined process contained 7.7 to 8.0 percent ash, 0.12 to 0.16 percent pyritic sulfur, and 0.78 to 0.96 percent total sulfur. Pyritic sulfur rejection by the process was about 95 percent. In addition to obtaining good recoveries of high quality coal the tests demonstrated that the pyrite in the wastes could be isolated as enriched flotation products by the combined gravity - flotation apporach. Pyrite concentrates analyzing 25 to 33 percent sulfur were recovered. Potential benefits of the pyrite flotation step are recovery of a product of possible market value and the elimination of the pyrite from the waste ash which could be a potential acid water problem when impounded.

Roof bolting typically follows the extraction of a commodity to help keep the roof from collapsing. During 2004 to 2013, roof bolter operators had the highest number of machinery-related injuries, accounting for 64.7 percent, at underground coal mines. This paper analyzes U.S. roof bolter fatal and nonfatal lost-time injury data at underground work locations for all commodities from 2004 through 2013 and determines risk indices for six roof bolting tasks. For fatal and nonfatal incidences combined, the roof bolting tasks in order of the highest to lowest risk index were bolting, handling of materials, setting the temporary roof support (TRS), drilling, tramming, and traversing. For fatalities, the roof bolting tasks in order of the highest to lowest risk index were handling of materials, setting the TRS, bolting, drilling, traversing, and tramming. Age was found to be a significant factor. Severity of injury, indicated by days lost, was found to increase with increasing age as well as with increasing experience, largely due to the confounding of age and experience. The operation of the roof bolting machine used in underground mining should be a research priority given the high frequency and severity of incidents. The results also suggest that temporal factors may exist, so additional research is warranted to better understand these factors and potentially develop interventions. This research provides a datadriven foundation from which future research can be conducted for safety interventions to reduce the frequency and severity of incidences involving the roof bolter activities of bolting, handling of materials, and setting the TRS.

"The San Juan Basin, New Mexico contains many heavy mineral beach-placer sandstone deposits of Cretaceous age. Beach-placer sandstone deposits are concentrations of heavy minerals that formed by mechanical concentrations (i.e. settling) of heavy minerals on beaches or in longshore bars in a marginal-marine environment. They have high concentrations of several elements, such as Ti, Zr, rare earth elements (REE), Sc, Y, U, Th, Nb, Ta and Fe. These elements are becoming increasingly more important to industry, because they are used in the manufacture of devices our society uses every day, i.e. cell phones, computers, and alternative energy devices, like solar panels. It is unlikely that any of the beach-placer sandstone deposits in the San Juan Basin will be mined in the near future because of small tonnage, low grades, high iron content, and distance to processing plants and markets. However, as the demand for some of these elements increases because of increased demand and short supplies, the dollar value per ton of ore rises, enhancing deposit economics. Current studies at the New Mexico Bureau of Geology and Mineral Resources are focused on the Apache Mesa (formerly Stinking Lake) deposit near Dulce, New Mexico. INTRODUCTION Beach-placer sandstone deposits are accumulations of heavy, resistant minerals (i.e. high specific gravity) that form on upper regions of beaches or in long-shore bars in a marginal-marine environment. They form by mechanical concentration (i.e. settling) of heavy minerals by the action of waves, currents, and winds (Fig. 1; Bryan et al., 2007). Modern examples are found along the Atlantic Coast, USA (Koch, 1986; Carpenter and Carpenter, 1991), southeastern Australia (Roy, 1999), and Andhra Pradesh, India (Rao et al., 2008), where they are mined for titanium, zircon, and monazite (a Ce-bearing rare earth, REE, mineral). Detrital heavy minerals comprise approximately 50-60% of these sandstones and typically consist of titanite, zircon, magnetite, ilmenite, monazite, apatite, rutile, xenotime, garnet, and allanite, among other minerals. Most of these minerals have a high specific gravity exceeding 4 and are dark colored, giving the sandstones a dark color, resulting in them being called black sandstones. Although beach-placer sandstone deposits are found in strata of all ages; the deposits in the San Juan Basin in New Mexico are restricted to Late Cretaceous rocks belonging to the Gallup, Dalton, Point Lookout, and Pictured Cliffs Sandstones (Fig. 2; Table 1; Chenoweth, 1957; Houston and Murphy, 1970, 1977). The beach-placer sandstones are black, dark gray, to olive-brown, resistant to erosion, and radioactive due to radioactive zircon, monazite, apatite, and thorium minerals. Anomalously high concentrations of Ti, Fe, Nb, Th, U, Zr, Sc, Y, and REE are characteristic of these deposits. Similar Upper Cretaceous heavy mineral, beach-placer sandstone deposits are found throughout Montana, Wyoming, Utah, Arizona, and Colorado (Dow and Batty, 1961; Houston and Murphy, 1970, 1977; Zech et al., 1994)."

Coal froth flotation cells typically receive little attention in coal preparation plants. They process only a small portion of the total coal feed (perhaps 5-1 0% of the total plant feed in the United States), they are viewed as a way to take the load off the refuse thickener, and their operation is not well-understood by cleaning plant operators. Operating parameters are set based on the equipment supplier information, and reagent dosages are set at the recommendation of the latest chemical vendor. The cells then operate day in and day out, recovering coal and producing a product that may not meet ash or sulfur specifications or that may be the wrong percent solids for downstream dewatering operations. If improvements can be made to this circuit, then the whole plant will benefit. Paying attention to rate and residence time and the wealth of data from the literature or from a series of targeted laboratory tests can give operators the key to improving the performance of froth flotation cells AND overall plant performance. This paper provides an overview of froth flotation rate and residence time data and gives practical suggestions for specifying and using laboratory tests to improve froth flotation performance. Suggestions for improving froth cell performance are also given based on available literature data.

Incident investigation is of utmost importance in most high-risk industries. This study was undertaken to study the content of past investigation reports to determine the effectiveness of the incident investigations. The study uses a semi-quantitative method to assess the effectiveness of incident investigations in the Ghanaian mining industry by evaluating the quality of past investigation reports. The assessment tool consists of 5 elements with several indicators and rating scales for assessing the quality of an investigation report as a measure of the effectiveness of the investigation. The method was applied to 304 investigation reports of 3 Ghanaian large-scale gold mines, and the results correlated with incidence rates of the mines to determine if any relationship existed. The results showed that the mines differ significantly in the quality of their investigation reports, suggesting differences in the effectiveness of their investigations. In addition, the incidence rates of the mines negatively correlated with some elements of the assessment tool. In general, the method was found useful and revealed areas where improvement is needed. INTRODUCTION The importance of learning from incidents is well documented in the literature. For instance, some scholars regard learning from incidents as important for improving safety [1,2]. Thus, many organisations spend considerable resources in identifying lessons from incidents and subsequently on the process of improvement that follows the identification of lessons. It is therefore important for such organisations to determine the effectiveness of their learning from incidents processes, in order to identify specific areas where improvements are needed, so that interventions can be as specific as possible. The learning from incidents process models the plan-act-check-do approach of management systems, often consisting of four major stages, namely; data collection and incident analysis, planning interventions, implementing interventions and follow-ups and evaluation [3,2]. The investigating and analysis of incidents have been regarded as one of the vital stages of the learning process since it is the stage where lessons are identified [3,2,4]. It has also been noted elsewhere in the literature that results of the previous stage of the learning process are important for the next stage [3]. For example, ineffective incident analyses could result in the planning of ineffective remedial measures. One of the primary aims of incident investigations is to identify lessons from previous incidents which when implemented can prevent future recurrences or minimise the effects of future incidents. There is thus the need for organisations to assess the quality of their investigations periodically since it is one of the means they adopt to improve safety and the quality of remedial measures to some extent depends on the effectiveness of the investigations. Although previous studies [5,2] have assessed the effectiveness of the learning process, and some aspects of the learning process, such as incidents reporting, there is less focus on the effectiveness of incident investigations. The research described in this paper was therefore conducted to contribute to this gap, focusing more closely on incident investigation reports.

The Tek Process can produce masonry products such as bricks, roof or wall tiles, and block from virtually any inorganic waste. In the process the aggregate raw materials, portland cement, water and common chemicals are first mixed together; then mechanically subjected to several thousand pound pressure per square inch. This causes a novel chemical reaction between the cement, chemicals and water; producing the "patented" Tek adhesive binder which holds the aggregate together in a stone-like mix. Simultaneously, the desired product shape is formed. The building modules can have strengths of 8000 p.s.i. (pounds per square inch) with completely sound aggregates utilizing air curing for a period of 28 days. With most aggregates, 3000 p.s.i. is achieved within 24 hours. Eventual strengths usually exceed 5000 p.s.i. with almost any raw material. Water absorption ranges from 5-7.5 percent. Both absorption and strength figures are superior to the most rigid of any and all A.S.T.M. specifications for the highest quality bricks. In most locations, the product (when brick shaped is called Tekbrik) can be produced significantly more economically than can comparable grades of clay or other bricks, be- cause almost any inorganic waste materials or pollutant can be used as a raw material. The process results in a lower manufacturing cost as compared to clay bricks; fuel cost to operate a clay kiln represents a substantial portion of the total cost. Capital expenditure for a Tek plant also is lower due to the large investment in clay kilns. Transportation savings also are important, clay brick factories must be very large because the kilns are uneconomical unless they are large. Most bricks are manufactured at the sites of clay deposits in relatively uninhabited areas. The cost of transportation to the job site represents a considerable percentage of the selling price.
