There has been an unprecedented rise in the fatality rate in the Australian mining industry over the last few years. The factors behind this rise are not clear; however, it is noted that the increase in fatalities has occurred during a fall in commodity prices.This paper examines prevention methods and systems to deal with hazards that have the potential to result in either single or multiple fatalities. Managing the risks associated with mine ventilation is used to illustrate the principals behind the approach.The paper also touches on the broad science of human error and proposes a multifaceted approach to controlling and preventing fatal risks.A summary of the steps involved in developing an effective principal hazard management plan is provided.CITATION:Clough, A J, 2015. Current attitudes by exploration companies towards working on another party's exploration permit or equivalent vary widely. That this is so is due to the virtual absence, on the one hand, of anything in the mining legislation in any jurisdiction that establishes what are the exclusive rights to explore, as opposed to apply for a mining tenement, that the grant of such an authority conveys on the holder, and on the other hand, of any code of ethics or set of guidelines to proper practise. By contrast, in most states and territories, if not all, it is an offence to enter onto a mining tenement, other than to cross it, without the consent of the holder, although until recently most field personnel would have been unaware of this and most explorers would have transgressed at one time or another. A similar vacuum exists where aerial surveys are concerned. There are in fact no guidelines. It has always been assumed that a party wishing to carry out such a survey was free to do so, without even an obligation to notify the holders of any tenements or permits in the area to be surveyed, let alone make available to them any data from the survey. Concepts of 'geophysical trespass' now being tested in the US also raise the possibility that data won from adjoining ground using geophysical techniques may be privileged. Attitudes now appear to be changing. In the absence of legislation or any code of practise, what does the industry regard as ethical behaviour? Do we need to define the rights and obligations of various parties more clearly, and if so, what should they be and to what extent will it be necessary to confirm them by legislation as opposed to adopting a code of practise? Will the existence of such a code in fact improve corporate practise?

It is always difficult to sum up an open forum adequately and instantaneously, so, with the privilege of hindsight, and also with some time to reflect, I am exercising the Chairman's prerogative to put together my view of the overall conclusions. Hopefully, these may provide a focus for each Institute to formulate its own programme with respect to mineral industry education. First, in answer to the question, "Should there be Schools of Mines?", there was a very clear 'yes'. However, I reiterate what I said in the introduction to this forum, specific professional education for the mineral industry exists only because the mineral industry sees a need and wants it. If the mineral industry should decide that it does not need professionals with specific mineral industry training, then such training will cease to exist. As alluded by Dr Hackett, if such training ceases to exist, one must then question the need for Institutes of Mining and Metallurgy. Now, there's some food for thought! Specialist schools of mines (metallurgy and geology) can only exist in an environment of genuine interaction with, and support by, industry. For their part, schools of mines, used here in a generic sense, require leadership that is strong but responsive to industry needs. It is the leader that puts his stamp on a school, and, to a large extent, determines whether that school is 'good' or only 'mediocre'.

Traditional mechanised resin rock bolting in hard rock underground mines requires the installation of resin capsules into a pre-drilled hole prior to the rock bolt being spun into a hole to mix the resin and set the bolt in place. In poor rockmass conditions, especially at depth or in highly altered faults and shears, the condition of the holes often deteriorates as the poor ground around it unravels and falls into the hole. This results in partially or fully blocked holes that either damages the resin capsule when they are being inserted into the holes or prevent it entirely until the hole is continuously cleaned of debris. This deterioration of the hole condition, and continuous need to clean and re-drill holes, also increases the diameter of the hole which in turn affects the continuously mechanically coupled anchorage mechanism of the rock bolt to the rockmass as there is simply not enough resin in the hole to fill the annulus between the bolt and the rock. This results in a large increase to critical heading cycle time and, more importantly, results in the rock reinforcement not performing as per design specification and increases the risk of instability. To overcome the quality control issues with resin bolt installation in poor ground, a single pass drill, install, inject and tension self-drilling resin bolt application was designed and implemented for use in such conditions. This system allows the operator to inject a two part polyester resin through a self-drilled hollow bolt to mix and fully encapsulate the rock reinforcement. The self-drilling bolt allows for post injection of the hole after the bolt has been installed into the rock and gives the operator manual or automated control on the amount of resin used in the hole to ensure complete encapsulation and continuously mechanically coupled anchorage to the rock. This allows the reinforcement to work as designed and provide the Geotechnical Engineer extra quality control confidence in these conditions. The implementation of this single pass resin bolting system resulted in significant decreases to the resin bolting cycle time and rock bolt quality control failures. In this paper the design, implementation and embedment into the development cycle will be discussed and the in situ quality control checks used to validate the increase in rock bolt installation quality will be shared. Tumbling mills are grossly inefficient, using only 5 per cent of the total energy supplied for actual breakage. A significant proportion of this inefficiency is unavoidable owing to geometric limitations; rotating drum systems are prone to exhibiting certain regions (energy barriers) that resist particle entry thereby restricting the transfer of energy. Though such systems are inherently inefficient, improved modelling schemes may lead to slightly enhanced efficiency through better microscale predictions of key breakage drivers (stress, force). Central to achieving such incremental advances is the development of a universally accepted rheology for dense granular flows. Armed with a better understanding of these macro-scale ingredients, classical process modelling offers an efficient, spreadsheet friendly framework for recovering the spatial distributions of the comminution drivers. At this real-time level of process control, it should be possible to tune comminution operations towards more energy efficient configurations – the ultimate goal of this research. The present work employs validated DEM data to recover the full continuum field which will ultimately serve as a basis for calibrating process model outputs based on different rheological hypotheses. Such a continuumbased approach captures tensor properties like the shear stress and strain rate – both of which are necessary to developing a dense granular rheology. Moreover, DEM captures this data at the momentum transfer timescale of 10-6 s – no other measurement technique is capable of this level of spatial and temporal resolution. Importantly, the confidence in this DEM data is strengthened because it has been validated at the continuum scale using Positron Emission Particle Tracking (PEPT) kinematic measurements. Validated DEM data is derived from batch experiments conducted within a 300 mm ID laboratory drum using three and five millimetre glass beads at 60 and 75 percent critical speed; such mill speeds are typically encountered in the tumbling mill industry. Dimensional analysis is conducted using critical rheological parameters (shear rate, shear stress, granular temperature etc), which are key to not only developing a granular rheology but ultimately will be used to tune our mechanistic models.

Techniques for the optimisation of open pit design and development are now accepted as standard practice in the mining industry. These techniques are used to analyse the impacts on project profitability of factors such as product price, slope design criteria and equipment selection. A fundamental assumption used in this type of analysis is that dump and stockpile designs and sequencing are predetermined. The cost of haulage for a typical truck shovel mining operation is in the range of 25 - 50 per cent of mine operating costs. Substantial cost savings can be achieved by optimisation of dump design and location of associated road and ramp systems.Factors which affect both the operation and total cost of dumping include; Ramp gradients and location, road design, dump placement and shape, dumping height, slope design (interim and final), slope stability, environmental requirements and equipment selection. This paper discusses these factors and their effects on the total mining operation and project value. Reference is made to case studies in which some of these factors have had a significant impact.

The mmmg of alluvial gold involves many deposits of theMesozoic, Tertiary, and Quaternary Ages in processing. Majorfeatures of the composition of such alluvial deposits are the lowcontent of gold and the small size of the gold particles. Atpresent, the gold-miners treat alluvial deposits in which a portionof fme gold (0.25 mm) exceeds 45 per cent. It is expected that by2000 these resources will be the main deposits mined (Zamyatinet aI, 1991).Under real industrial conditions the losses of minus 0.25 mmmetal are 50 per cent and more. The low level of recovery of finegold is observed not only at concentrators equipped with sluices,but also at concentrators equipped with modern diaphragm andpistonless jigs (Table 1).

Supergene lateritic iron ores are uncommon when compared with the vast tonnages of BIF-hosted microplaty haematite ores of the world or with the dominant supergene mimetic martite-goethite ores of the Hamersleys. In Australia and to a lesser extent in India and South America, mimetic replacement of gangue minerals by iron hydroxyoxides is a major factor in forming supergene iron ore from banded iron-formation (BIF) and results in the superb preservation of all of the original BIF features. This includes primary banding down to the finest inclusions, as a result of pseudomorphing of the BIF gangue at depth by goethite. The enrichment as a whole is an absolute accumulation of Fe at depth, dissolved from the surface BIF, and a removal of the gangue elements without loss of the original BIF mineral textures. Thus the ores grow upward to meet the downward destruction of the surface BIF by erosion. By contrast the supergene lateritic ores including the 'blue dust ores' are residual ores resulting from the relative accumulation of Fe. In the supergene lateritic ores derived from BIF, removal of the gangue is accompanied by repetitive solution and reprecipitation by secondary haematite and goethite. The final product of lateritisation of BIF and BIF-derived ores is typically complex, due to successive overprinting of earlier stages of alteration during the process, and because of the different precursors. This progressive lateritic modification tends to produce a varied surface complex including massive, vermiform, and botryoidal, or nodular aluminous/siliceous goethite and haematite, variously known as ferricrete, duricrust, canga, or 'hydrated zone hardcap'. The significant difference in these ores to the supergene mimetic martite-goethite ores is that they grow downward, with the total destruction of any of the parent BIF or BIF-ore textures.

The Mount Whaleback iron ore deposit at Newman, Western Australia comprises the largest known continuous iron ore deposit in Australia, originally with a Measured Resource in excess of 1700 Mt of hematite ore. The deposit occurs within the Hamersley Province which forms part of the Pilbara Block in the NW of Western Australia (Figure 1). The Mount Whaleback iron ore deposit, together with a number of discrete iron ore bearing satellite orebodies in the Newman district, contributes to the Newman Lump and Fines products. Extensive quality control systems are utilised from mining to shipping to ensure that each product meets a narrow geochemical target grade range.

The ore mined from the Lead Lode of The Zinc Corporation leases consists of a fairly granular mixture of galena, marmatite, chalcopyrite, tetrahedrite, pyrrhotite, pyrite, and lollingite with a gangue association of calcite, quartz, bustamite, rhodonite, mangan-hedenbergite, garnet, fluorite, mangano-calcite and apatite. The silver content of the ore occurs in three minerals tetrahedrite, dyscrasite, and pyrargyrite, the last two mentioned occurring in microscopic association only. Other metallic and gangue minerals are also present in this complex mineralogical association, but do not influence the milling operations.Fortunately, the silver minerals favour the galena and chalcopyrite as their host minerals, thus facilitating the recovery of both copper and silver in the lead concentrate. The ore from the Lead Lode assays from 14 - 18 per cent lead; 3.0 - 4.5 oz silver per tonne, and 9.5 - 12.0 per cent zinc.A parallel lode, known as the Zinc Lode, is well developed in The Zinc Corporation leases. This lode assays from six to eight per cent lead, 1.5 oz silver per ton, 18 - 25 per cent zinc with a siliceous gangue. The common Lead Lode gangue minerals, calcite, rhodonite, bustamite - are very rare in present underground exposures. No metallurgical work was done on this ore during the pilot plant operations.The aim in the milling process is to produce:1. A lead concentrate with the maximum economic recovery of lead, silver, gold, copper and antimony, and minimum zinc content.2. A zinc concentrate with the maximum economic recovery of zinc, cadmium, and cobalt, and minimum iron, arsenic and fluorine content.3. A waste residue containing sufftcient granular material to be utilised as underground fill.In considering the economics of the milling practice the above three requirements plus consideration of the tariffs submitted by the purchasers of concentrates were the deciding factors in the evolution of the present milling practice.Towards the end of 1924 The Zinc Corporation Limited added to and adapted the old zinc tailings plant for the treatment of 5000 tons of crude ore per week. This plant, which operated until January 1936, was in general principles similar to the standard Broken Hill methods of ore concentration, whereby the galena was recovered by the gravity concentration methods of jigging and tabling with selective flotation concentration of the residual galena and marmatite. One important difference from other Broken Hill plants was that the primary grinding of the jig tailing was performed with flint-loaded mills operating in closed circuit with revolving screens. Two primary grinding sections were installed, each consisting of two 5 ft diameter by 10ft long tube mills loaded with flint pebbles working in closed circuit with four 3 ft diameter by 3 ft wide revolving screens covered with 22mesh Toncap woven wire screen with 0.57 mm opening. Each section carried a circulating load of 28.3 tons per hour for a jig.

Segnit's article puts forward the views that the rocks associated with the Broken Hill lode are simply highly metamorphosed sediments, tbat metasomatism on a large scale was not an important factor at Broken Hill and that the rocks and lode were contemporaneously metamorphosed.In particular, the garnet quartzite, the most abundant rock in the limited section of the mine examined by Segnit, is described by him as a simple, metamorphosed manganiferous sediment.These views, while consonant with those of H. F. King (1958), are drastically opposed to those of myself and others e.g. Gustafson et al. (1950, p. 1383) who state that the most extensive hydrothermal alteration in the district is in and close to the zinc lodes in the Zinc Corporation and New Broken Hill Consolidated Ltd. mines. Segnit's conclusions conflict with 1. the absence of any manganiferous sediment in the Broken Hill district except in close proximity to the lode,2. the clear cut metasomatism of the dolerite dykes which indicates that the ore is younger than the dykes and therefore post-metamorphism, and 3. the petrofabric study of den Tex (1958) which provided independent evidence that the ore in its present form post-dates all events of a diastrophic nature in the area.The original sediments of the district were mudstones and calcareous sandstones with 2 to 5 per cent CaO. Deposits of manganese oxides and silicates in the Tamworth area were re-examined. The major deposit at Danglemah consists chiefly of rhodonite and the rare silicate tephroite. It is considered that the manganese was deposited with the original sediments, the silicates being formed later during the intense folding and low-grade metamorphism of the enclosing rocks.INTRODUCTIONMany small deposits of manganese oxides and silicates occur along a belt of eastern New South Wales and Queensland.Some fifty occurrences, most of which have been subject to mining activity, were described by Lloyd (1943) and some of this information was summarized by Knight (1947). The ores have been mined on a small scale since 1884. Small supplies of battery grade ore were won from a number of deposits, while larger quantities of metallurgical grade were sold to the steel industry. Maximum production was in 1918, when 6,500 tons were mined.The siliceous nature of many of the ores and their occurrence in the neighbourhood of granite batholiths led earlier investigators to consider the deposits to be of hydrothermal origin, associated with the granite intrusions. The present work questions this viewpoint.In this investigation, three of the chief deposits in the Tamworth area were seen. These were at Danglemah, which is being mined at present, and Mount Jasper and Manganese Mountain, which were mined in the past. In 1993 after over 110 years of mining operations, production ceased at the Broken Hill North Mine. Following the cessation of operations a rehabilitation program was developed to satisfy the requirements of the various government departments. This program has been implemented progressively since mine closure. The paper will discuss: primary aims and objectives of the rehabilitation program, implementation of the program, rationale behind rehabilitation objectives, and problems encountered in implementation of the program. To-date the primary rehabilitation of the mine has involved the capping of redundant tailing dams with inert waste rock to prevent the contamination of the receiving environment. A Water Management Plan has been developed to contain all run-off from the mine area in an extreme storm event. A cultural heritage assessment of the leases has been undertaken to ensure the rehabilitation is consistent with the historical significance of the site and community expectations. All redundant material from the mine has been sold in a æno reserveÆ auction and the workshop area has been relocated to the Southern operations. At present the only active areas on the mine are a maintenance area for equipment for the Potosi open pit, and a primary crusher for Potosi ore which is railed to the Southern operations for further processing. Upon completion of the rehabilitation program, Pasminco Broken Hill Mine intends to relinquish the mining leases to the Department of Mineral Resources.

The nickel exploration boom of the late-1960s and early-1970s led to the discovery of numerous komatiite-hosted nickel sulphide deposits, associated with variably serpentinised bodies of olivine-rich cumulates. Subsequent research has shown the importance of volcanological processes to understanding the genesis of these deposits. Crucial developments have come through consideration of the fluid dynamics of magmas and magma-crystal mixtures, through observation of the dynamics and geometry of modern day basaltic lava flows, through application of theory and experimentation on the kinetics of crystal growth from magmas, and most importantly from extensive detailed field mapping of komatiitic sequences in the Yilgarn Block and elsewhere. From these studies has emerged a new integrated view of the volcanological setting of komatiite-hosted deposits, and a recognition that a complete continuum of deposit types exists. Nevertheless, komatiite associated magmatic nickel deposits generally fall into two overall types based on the nature of the sulphide ore. In the first type the bulk of the ore is either massive Fe-Ni-Cu sulphide or a variable mixture of massive sulphide and an ore characterised by olivine crystals in a continuous matrix of sulphide occupying between 30 and 75 per cent by volume of the rock (called Matrix Ore). Nickel grades of massive sulphide ore range from 2 - 20 wt% and those of the matrix ore generally fall in the range 1 - 5 wt% (average 2.5 wt%) Ni/Cu ratios of the sulphide fraction are generally >10.

THE absence of any locality maps of the district of Morobe has been seriously felt by mining engineers engaged on the goldfields, and as it was evident at the beginning of operations that the administration would be unable to make good this deficiency, ascheme for making rapid traverses was worked out, and in all subsequent travelling the necessary records were kept for the construction of a reliable reconnaissance map of the Bulolo goldfield and its approaches (see Plate 1.). Since all the field work had tobe carried out without encroaching on primary duties, the compilation of sufficient data was a somewhat lengthy business. It was found that in this r;ough country a pedometer was useless, as it was impossible to calibrate it for the variation in length ofpace between climbing semi-precipices and walking over level country. Further, the native tracks "meander" so excessively that the only method of calculating air-line distance is by timing, and a knowledge of what the timing represents, and until a 10kg experience had been gained in the time taken to cover known distances in various types of-country, one could not feel competent to estimate sufficiently accurately. It was found that reliability could be attained only when walking at nearly top pace. Inregard to aeroplane traverses, results were reliable only when the air was fairly still.BEARINGSIn arriving at the bearing between main traverse points, the method used was to keep a "boy" about two chains- behind when marching and sight back on him, standing, at about two-chain intervals. The magmatic arcs of SE Asia and SW Pacific contain some of the worldÆs major magmatic-hydrothermal gold deposits. Most gold deposits in SE Asian and SW Pacific arcs formed during periods of plate reorganisation rather than during periods of normal or steady state subduction. These plate reorganisations were caused initially by the collisions of the Australian Craton with the Philippines-Halmahera Arc and the Ontong Java Plateau with the Melanesian Arc at ~25 Ma. A second Mid-Miocene period of mineralisation accompanied plate reorganisation following maximum rotation or extrusion of Indochina and cessation of spreading in the South China Sea at ~17 Ma. However, the vast majority of deposits from New Zealand to Taiwan formed since 7 Ma during an important period of tectonic reorganisation that accompanied and followed changes in the relative motion between the Indian-Australian and Pacific plates between ~8 and 3.5 Ma. Magmatism in unusual tectonic settings produced the most abundant and largest deposits with many deposits associated with high-K calc-alkaline, shoshonite, adakite and alkaline magmatism. In particular peak mineralisation appears related to melting of sub-arc lithosphere that has been previously modified by subduction. During large-scale plate reorganisation this can occur towards or at the end of a period of normal subduction, following arc collision or accompanying subduction reversal at approximately the same time in different parts of a complex system of arcs such as in SE Asia and the SW Pacific.

The North Fiji Basin (NFB) is delineated by the Vitiaz trench to the North (paleo sub duction zone, Brocher, 1985), the Vanuatu is- land arc (called before New Hebrides arc) to the West and South, the Hunter Fiacture Zone to the South-East and the Fiji islands to the East (Brocher, 1985 ; Larue et al., 1982 ; Carney and MacFarlane, 1982 ; Falvey, 1978). The NFB is characterized by shallow water depths (about 3 km) (Chase et al., 1982 ; Scla- ter et al., 1972 ; Karig, 1971), high heat flow (average of 2.29 HFC) (MacDonald et al., 1973 ; Halunen, 1979), strong positive free air anoma- lies (Worzel, 1965 ; Kogan, 1976 ; Luyendick et al., 1974 ; Solomon and Biehler, 1969) and a geoid anomaly (Malahoff and Larue, 1979) inter- preted as a geoid swelling. Few refraction data reveal a relatively thin crust and sediment co- ver, and the existence of low speeds in the up- per mantle beneath the NFB (between 7,1 and 7,7 km/s) (Larue et al., 1982 ; Raitt, 1956), which could indicate the existence of active spreading zone(s) in this area (Barazangi et al., 1974). Global seismicity of the SW Pacific clearly de- fines the.limits of the NFB, as well as the Lau basin (Sykes et al., 1969). But one can note the low level of shallow seismicity of the NFB (Ham- burger and Isacks, in press) which concentrates in three distinct trends :

For the past 150 years, drilling and blasting has been an integral part of the underground mining industry in Australia. Ever since Busby's Bore was developed using black powder to supply Sydney with water, miners have been drilling holes, filling them with explosives and blasting the rock into a manageable size. The drilling was fairly straight forward but the explosives were always a mysterious commodity. It worked, but how and why? The explosives industry is now working much closer to the mining industry to dispel these mysteries and also to aid the advancement of explosive technology. This paper looks at the results of these advances in the form of explosives, accessories and blasting systems.

Operational grinding mills emit significant acoustic emissions which are a consequence of the dynamic environment prevailing inside them. From a technical perspective, the noise presents an opportunity to determine some details of the processes occurring inside the mills. The noise generated is not random and can be attributed to a range of events that occur inside these mills, consequently non-contact acoustic determinations of in-mill behaviour provide powerful insights into the operation of primary comminution tumbling mills. To enable a logically coherent picture to be formed of the processes inside a mill, arrays of microphones have been used. This approach allows a clearer picture of the in-mill behaviour of the charge to be accessed. In contrast to single microphone approaches, usually located near the expected charge toe position, an array of microphones allows much more useful information to be obtained from grinding mill noise.This paper will discuss and present data obtained from microphone arrays positioned at the toe and shoulder locations of a mill, and how this information can then be used for the optimisation of grinding mills in real-time.CITATION:Pax, R A, 2012. "Pierre Gy showed in his early works that the results in estimating the variance of the mean value of continuous lots depends on sampling mode (random, stratified or systematic), except in the theoretical case that the measurand is randomly distributed in the lot. The experimental variance estimate calculated from samples taken from the lot is independent of sampling mode but depends on sample size and the type of the lot heterogeneity. Variography can be used to estimate the variance of the lot mean for different sampling modes.Gy also defined weighting error, which is made if a simple average is calculated from the assays of samples taken from a continuous object (eg from a process stream) where the flow rate varies with time and is correlated with the concentration of the analyte in the stream.The effect on sampling uncertainty of the three sampling modes and their interaction with the following factors in estimating the uncertainty of the mean concentration is studied using simulated lots, and lots having heterogeneity patterns similar to those of real data sets, including the effect of: the type of heterogeneity (random, grading and clustered heterogeneity patterns) increment size and number employing composite sampling weighting error.
