The most frequent causes of accidental injury and fatality are carelessness and ignorance; and the most common accidents are haulage accidents, fall, drowning, and fire. However, in this modern day and age, severe injury has become the dire consequence of an ever-increasing number of industrial and mining accidents as well as sport-, military- , etc. Mine accidents have declined dramatically in number and severity through decades of research, technology, and preventive programs. However, preventing recurrence of disasters like those of the past remains a top priority requiring constant vigilance by management, labor, and government. The goal of the present research is to investigate and identify root causes associated with types of mining accidents (victim being caught by an equipment, bodily motion, over exertion, etc) and nature of injuries such as (sprain and strains, fracture, dislocation, amputation, etc). Further, we hope to discover the impetus of the root causes occurring more and more often in this modern day of technology and quality awareness. With the different dimensions of accidents and injuries brought into the fold, there is the need for researchers and mining operators to comprehend and better still visualize significant patterns of associations among these variables of interest. We assume that, there is a high correlation between many dimensions of nature of injuries and types of accidents. This suggested a principal component analysis to reduce the dimensions for easy and amenable interpretation.

Composite piles with precast concrete tapered pile tips (TPT Composite Piles - U.S. and foreign patents) are a new and important addition to the piling types available to the construction industry. TPT Piles were invented by Stanley Merjan, Vice President of Underpinning & Foundation Constructors, Inc.. U. F. C. installs these piles in the Northeast United States and licenses piling contractors in other parts of the country. TPT Piles have been developed for use in sandy subsoils at depths to 70 feet or greater. The TPT system (see Figure 1) is simple. A pile stem of conventional material (shell, pipe, steel, wood, precast concrete) is connected to a socket formed in a precast concrete tapered pile tip (TPT). This composite assembly is then driven as a unit by ordinary pile driving methods until adequate driving resistance (established by formula or load tests) is met in a suitable soil strata. This driving resistance develops from the displacement and densification produced by the TPT. Many combinations of tip shapes and stems are available to suit particular job requirements with respect to subsoil conditions and pile capacities. TPT Piles should be considered for use on a wide variety of jobs. The advantages offered by a TPT installation include: ? High capacity piles in marginal soils ? Minimal lengths required to produce these capacities ? The use of conventional pile driving equipment and procedures ? Pile inspection techniques that are the same as for ordinary pile driving Furthermore, TPT piles are economical due to a combination of a low cost per ton of pile capacity and high productivity, both of which lead to significant savings in foundation costs and time required to complete foundation work.

"A new process is disclosed for the recovery of gold and/or silver values from activated carbon wherein they are adsorbed as cyanide complexes. A unique chemistry allows the new process to use mill process water containing an inexpensive, non-hazardous compound as the eluant. The new process has an elution period of 5 hours at 98°C. The gold in the eluant can be recovered by electrowinning or zinc precipitation. The new elution process is simple, reliable, and provides an eluted carbon that may not require thermal regeneration before reuse. Retrofitting the new process to existing processes such as Zadra, Anglo, and others will not require significant capital expenditures.IntroductionThe efficient and reliable elution process for gold/silver from activated carbon is a very important process in the gold mining industry. Researchers at the U.S. Bureau of Mimes monitored rock mass response to mining of an experimental underhand longwall stope in Hecla Mining Co.'s Lucky Friday Mine, Mullan, ID. This stope design, the Lucky Friday underhand longwall (LFUL), was proposed as a means of controlling rock bursting while also allowing increased mechanization of mining operations. Instruments were specifically manufactured to monitor three geomechanical factors (backfill performance, ramp stability, and rock mass response) that directly affect the success of stope design. Despite considerable difficulties with instrument installation and survival in the harsh mine environment, especially within the stope fill, sufficient information was collected to suggest that the LFUL method was successful in achieving project goals. This conclusion, in addition to favorable economic and operational evidence, led the mine to adopt the method throughout the mine. Indeed, this study and mining experience suggest that underhand longwall mining could be considered a feasible mining method for rock-burst-prone ground throughout the Coeur d'Alene District. Further Reports of Investigations (RI) in this series present in-depth analyses of the effects of backfill on rock bursting and calibration of a stope numerical model.

Silver and cupric ions have been proven to play the roles of catalysts to promote the oxidation kinetics of chalcopyrite in acid solution. In this study, the interaction between silver ions and cupric ions during ferric leaching of chalcopyrite in sulfuric acid solution has been investigated. The electrochemical study by Tafel analysis showed a significant boost of dissolution rate for chalcopyrite dissolution in Fe3+- Cu2+ containing sulfuric acid solution after Ag+ was added in the solution. For 10 ppm Ag addition, the dissolution rate of chalcopyrite increased to 7.94 ×10-6 A/cm2 from 3.2 ×10-7 A/cm2, when Ag+ was absent. On the other hand, the effect of Cu2+ on the dissolution of chalcopyrite in Fe3+-Ag+ containing sulfuric acid solution was not obvious from electrochemical measurements. The chalcopyrite leaching tests in different acid solutions showed that silver addition had an obvious catalytic effect on promoting chalcopyrite dissolution. Comparing the leaching rates of chalcopyrite leached in Fe3+ containing, Fe3+-Ag+ containing, Fe3+-Cu2+ containing and Fe3+-Cu2+-Ag+ containing sulfuric acid solutions, it was found in Ag+-Fe3+-Cu2+ containing sulfuric acid solution, the leaching rate was highest among all the aqueous acid systems. Further leaching tests suggested that with increase of the Ag+ concentration from 0 to 20 ppm, the leaching rate of chalcopyrite in Ag+-Fe3+-Cu2+ containing sulfuric acid solution also increased and reached 57.62% with 20 ppm Ag+ addition after 24 h. INTRODUCTION Chalcopyrite is the most important copper mineral in copper hydrometallurgy and it accounts for nearly 80% of the universal copper resource (Olvera, Rebolledo, Mora, Dixon, & Asselin, 2018). The hydrometallurgy of chalcopyrite has been intensively studied with emphasis on solving the slow dissolution kinetics of chalcopyrite in acid environment. A variety of aqueous systems, such as sulfate (Hiroyoshi, Miki, Hirajima, & Tsunekawa, 2001), chlorides (Liu, Chen, & Yang, 2017), and ammonia (Hua et al., 2018) have been studied to improve copper leaching. From another perspective, chalcopyrite leaching studies have indicated that the mechanism of chalcopyrite dissolution in acid solution is essentially electrochemical (Li, Kawashima, Li, Chandra, & Gerson, 2013). Therefore, extensive studies have recently been carried out to explain the dissolution kinetics of chalcopyrite in different media by various electrochemical techniques.

An experimental blast furnace in normal operation was quenched by substituting cold nitrogen for the hot blast air. More than 2,200 samples from the stack, bosh, and hearth were taken; 500 of these were selected for detailed analysis. Deductions are made as to the patterns of burden movement and gas flow; the reduction of iron oxides, including a comparison of ore and sinter; and the behavior of sulfur above the smelting zone. Metal and slag beads from the bosh were analyzed, as were samples from the hearth metal and slag. A brief description of a petrographic examination of the quenched burden is included.

Zirconium, formerly distinctly a rare element, is rapidly becom- ing of rather extensive commercial importance. Economic features of the zirconium industry, together with general information regarding the metal and its compounds, are discussed in a separate paper, to which this paper is a supplement. In addition to giving a brief discussion of the four or five de- posits that so far have proved to be commercial sources of the element, this circular summarizes available data (including some hitherto unpub lished material from the files of the Bureau of Mines) upon other occur- rences, including many of scientific interest only, of the two commercial. minerals, zircon and baddeleyite, and also data upon some of the minor minerals, such as eudyalite and cyrtolite, which have attracted addition- al interest in recent years because they are frequently richer in hafnium than the more common zirconium minerals.

The Velvet gold orebody (153 Koz Au) is located near the Kanowna Belle gold deposit in the Boorara Domain of the Kalgoorlie Terrane, a subdivision of the Archaean Eastern Goldfields Superterrane (Swager et al, 1990). Gold mineralisation at Velvet is hosted within an intrusion of intermediate composition that broadly follows a sub vertical hanging wall splay of the Fitzroy Shear Zone.Velvet was discovered in 2003 during an underground exploration drilling program targeting potential strike extensions to the Kanowna Belle deposit. Diamond drill hole KDU1595 returned a downhole intercept of 36 m grading 4.6 ppm Au approximately 500 m west of the deposit and at a vertical depth of 700 m below surface in the immediate hanging wall of the primary Kanowna Belle host structure, the Fitzroy Shear Zone. Subsequent drilling completed following the initial discovery, from both surface and underground collar positions, failed to replicate this result.In 2014, KDU1595 was resurveyed gyroscopically and the mineralised intercept correctly located. Additional drilling was undertaken to follow-up the repositioned discovery hole. Challenges included poor drilling angles through the Fitzroy Shear Zone, excessive deviation during drilling and the unknown geometry of the target. Navigational drilling helped to successfully penetrate the Fitzroy Shear Zone and in April 2015, drill hole KDU3234 returned a significant downhole intercept of 45 m grading 5.6 ppm Au.Additional drilling success resulted in a decision to develop a 600 m drive to serve as a dual-purpose access for further exploration and mining. This was a potential risk considering that the orebody geometry was unknown and all significant drilling results were clustered within a 50 m × 50 m panel. Development commenced in June 2015 and the first ore cut taken in April 2016. Geological mapping and structural interpretation, together with multi-element and spectral analysis of drill samples has enabled several other target areas to be identified for exploration. The Velvet mining area is critical to continued production at Kanowna Belle. Geological understanding must advance as mining progresses to ensure continued resource growth and optimal extraction of reserves.CITATION:Lorusso, G, Rogers, J and Tripp, G, 2017. Rehabilitation, reclamation and restoration are all terms used to describe the repair of land after mining. Here, restoration is used to mean restoring the land's original use, capability or productivity. Effective restoration is dependent on good planning before mining commences and must include a clear definition of post-mining objectives. The physical condition of the land within the mining licence was that of an imperfectly drained soil in permanent pasture, supporting a seasonal supply dairying unit. During the mining process, the soil layers were separated, with the topsoil stockpiled separately. As part of the restoration process, the land was recontoured to encourage surface drainage, with topsoil spread evenly over the recontoured tailings. Low grond pressure machinery was used to minimise soil compaction, with light discing and harrowing used to increase water infiltration. A permanent pasture was sown, with weed species controlled by spraying, both before and after pasture establishment.  Soil testing has been used extensively to ensure there were no nutrient deficiencies that good compromise pasture establishment and performance. Controlled grazing practices were used to ensure that the sown pastures became well established.

London Underground is the oldest and one of the deepest Metros in the world. The temperature of the Underground has been increasing over the last 100 years, mainly as a consequence of increases in train service. Further increases in train service are planned in the coming years to meet the predicted growth in passenger demand. Unabated, the additional service will increase temperatures on the Underground. London Underground Limited (LUL) has therefore set up the Cooling the Tube Programme (CTP) to tackle the issues of thermal conditions on the Underground. Some of LUL?s studies and cooling methods have borrowed experience and techniques from the mining industry; some of methods used or proposed may in turn be of interest to the mining community. This paper presents a background into the sources of heat on the Underground network, the manner in which the heat is presently dissipated, the analysis of future temperatures and criterion and a number of means and methods being used or proposed to cool the network in an environmentally sensitive manner.

Our subject, "Safety, Legal, and Contracting," is a range of topics which covers a broad spectrum, but is obviously Intended to focus our attention upon certain particular aspects of tunnel problems. These are all fascinating and complex subjects, and others are much more qualified to discuss to discuss the fine points involved. Put after some 45 years' involvement as an inspector, engineer, contracting officer, and especially for the last 22 years as a contractor's estimator and manager, I have developed some strong and definite convictions to share with you. I hope it will serve to get you started thinking. My first tunnel experience was in a soft ground tunnel which was dug by hand, literally. The crown or arch back was taken out, a pebble at a time, by bare hand. The face was constantly breast-boarded, and the mucking was all done by hand shovel. My first rock tunnel was drilled by hand held drifters from the muck pile. Since then, most tunneling techniques have improved greatly. The jumbos, booms, muckers, and moles have taken over many of the back-breaking chores, and reduced the manpower requirements.

The Lake Hodges to Olivenhain Pipeline project is part of a 40-megawatt pumped storage project linking two reservoirs as part of the San Diego County Water Authority’s $939 million Emergency Storage Project. Seeking to expedite project completion, the project was advertised as Design-Build, a first for the owner. Awarded to Kiewit Pacific with Parsons as designer, the 1,750m (5,700ft) long, 4.25-m (14 ft) horseshoe tunnel was excavated with conventional drill-and-shoot technique. The mining method was well suited to deal with the shear zones, the very abrasive rock, and 13.3% and19.6% grades. At the apex of the alignment, a 60m (200ft) by 3.6m (12ft) diameter raisebore shaft with surge chamber to protect against pump malfunction was constructed. Upon completion of mining, the tunnel was lined with a 3m (10 ft) diameter high strength steel pipe. Scheduled to come on-line by January 1, 2008, the Lake Hodges to Olivenhain pumped storage project is valued at $108 million, delivering a reliable source of water in emergency situations and electricity-generating capability to meet the growing demands of the San Diego region.

This paper discusses the geotechnical condition of bedding shears and small- and large-scale faults and shears in Archean banded iron formation (BIF) bedrock, based on observations of core and televiewer images. Information that can be collected includes the orientation of defects, infill thickness, strength and material type, defect shape and surface roughness.The spacing and continuity of bedding shears may be critical to slope design where performance is structurally controlled with bedding dipping out of a wall. The intensity of development of the bedding shear is a useful indicator of the amount of shearing a bedding surface has been subjected to. This can allow for engineering judgement to be used in adopting peak or residual shear strengths along defect surfaces when developing the design.The spacing and continuity of bedding shears may also contribute to the blockiness and degradation of a rock mass where the structural orientation is otherwise favourable. Additionally the condition, thickness and strength of infill of major fault structures can affect slope design and performance, depending on orientation.Comparison of the condition of structures observed in core with the corresponding in situ televiewer image has been made to assess what characteristics are most accurately captured by each technique and whether the additional information gathered from using the televiewer technique is justified by the cost. Electron probe microanalysis (EPMA) and quantitative X-ray diffraction (QXRD) techniques are capable of providing high-quality quantitative information regarding the phase mineralogy and distribution of alumina within iron ore samples. The techniques, however, use different methodologies in determining the mineralogical information (ie structural versus chemical), which introduces some limitations. In this study, EPMA was used to identify and quantify the deportment of alumina within mineral phases in iron ore samples (-1 mm crushed) sourced from different suppliers. Results were directly compared with quantitative mineralogical data derived from QXRD to provide an evaluation of the relative merits of both techniques. The four ores studied contained alumina either in the form of gibbsite, aluminosilicates (eg kaolinite), or associated with goethite.For minor phases, gibbsite and kaolinite, the abundance trends measured via QXRD were generally reflected in the EPMA data. Agreement was less satisfactory for the more abundant goethite phase (both low- and high-Al forms). The discrepancy may be the result of sampling issues arising from textural features such as grain size, and also a systematic overestimate in the amount of Al-rich goethite when hematite and Al-bearing phases are finely intermixed and the EPMA step size is coarse. A second difference in the EPMA results compared to the QXRD data was in regard to quantification of low-Al and high-Al goethite phases. The sensitivity of the chemical-based EPMA was better for distinguishing between high- and low-Al goethite although the results remain overprinted by potential errors introduced via sampling.Care should be taken when directly comparing results from each method, although in general the data should be considered complementary, as each approach provides slightly different results and interpretations regarding the overall mineralogy.CITATION:Pownceby, M I, MacRae, C M and Torpy, A, 2017. National Grid needs to maintain a connection between two existing electricity sub-stations (Beddington & Rowdown) in the Croydon (south London) area, and allow the replacement of two 275 kV cable circuits that are buried under roads. This is to be achieved using a single 400 kV cable circuit within a 10 km (6.25 mile) long, 3 m (9.8 ft) diameter high voltage cable tunnel which is being driven through water bearing Chalk, at depths ranging from 5 m (15 ft) to 80 m (262 ft), with four access shafts(12.5 m (41 ft) in diameter) ranging in depth from 10 m (31 ft) to 40 m (131 ft). The Chalk in this area acts as a source of public drinking water supply (PWS) and is protected by areas designated Groundwater Source Protection Zones. To minimise the groundwater risk the design and construction of the cable tunnel and shafts has paid particular attention to the construction materials used, including testing of these construction materials prior to use, developing construction methods (including the design of the Earth Pressure Balance TBM) and maintaining a groundwater monitoring & testing programme prior to and during construction to ensure the integrity of the Chalk aquifer is maintained as a drinking water source. This paper describes the tunnel &shaft design and construction in relation to these particular groundwater environmental issues which set new precedents for tunnel assessment, design and construction requirements in the United Kingdom.

Paper presented at the IAGOD international symposium on mineralization related to mafic and ultramafic rocks held in Orleans, France, 1-3 September 1993. Phases of the Ir sub-group are present either as inclusions in or interstitially to chromite and are unaffected by (or only altered in situ during) hydrothermal processes. By contrast, almost all phases of the Pd sub-group are located in a secondary textural setting - in fractures in chromite or between fragments of brecciated chromite. The different settings indicate that the mineralisation episodes were associated with magmatic and hydrothermal stages respectively. The presence of an included Pd-group mineral in a hydrothermally Pd-group mineralised chromitite sample suggests that the hydrothermal Pd-group minerals were at least partly derived by remobilisation from magmatic deposits in the chromitite pod. These observations indicate that Pd-group elements are more mobile than Ir-group elements under hydrothermal conditions and point to the potential for hydrothermal platinum and palladium mineralisation in favourable environments

"Copper Mountain’s concentrator was commissioned in May 2011. The circuit consists of a 34’ x 20’ EGL SAG mill, a Raptor 900 pebble crusher and two 24’ x 39’6” ball mills. Each mill is equipped with a 2 x 8500 HP dual pinion drive system. The plant was designed to process 1585 mtph of SAG feed. From startup, the mill struggled to meet the required production rates. Over time, a major initiative that involved blasting and fragmentation, optimization of the SAG mill in terms of discharge grates, ball size and slurry density, and operation of up to three mobile pre-crushing plants resulted in significant increases in throughput. In parallel, a number of modeling tools, including JK SimMet, were run based on plant benchmarking exercises. Samples collected at the mine site during ramp up confirmed the very competent nature of the ore with drop weight - A*b values of less than 24 being recorded on a variety of plant feeds and lithologies. After due consideration including demonstration of proof of concept, a new pre-crushing circuit incorporating an XL2000 cone crusher and associated screening equipment was designed, constructed and commissioned at the site in August 2014. This paper reviews the history of the original circuit design, the various optimization steps taken and impacts, together with the final pre-crushing circuit and net results.IntroductionThe Copper Mountain Mine is located approximately 20 km south of Princeton, British Columbia, Canada (refer to Figure 1). The region has a long history of copper mining dating back to 1923. The Copper Mountain camp occurs in the 'eastern volcanic belt' of the Nicola Group (Monger, 1989). It is an alkali copper-gold deposit in which the volcanic strata are intruded by early Jurassic dykes, sills, irregular plugs and zoned plutons (Woodsworth et al., 1992). The stratified rocks are relatively fresh with slight metamorphism in the local contact regions."

"IntroductionAt TransAlta Utilities Corporation's Highvale Mine, located west of Edmonton, Alberta (Fig. I and 2), dragline stripping has proved to be a practical and economical method of overburden removal and future plans have been based upon the continued use of draglines (Fig. 3) as the major stripping equipment (Williams, 1979, Nikols et al., 1986). However, long range planning has shown that the capacity of draglines alone will be exceeded in the early 1990s as mining progresses into deeper overburden cover (Tapics, 1983). Strip ratios will increase from I.3 bank m!/ t of coal in 1985to 4.5 bank m3 / t in 1995. During this period the depth of overburden will increase substantially and the areas to be mined will be characterized by weak soils including glacially displaced overburden. This increased overburden depth in relatively weak soils will make it impractical to _continue side casting by draglines without prestripping in advance. In 1983, a major investigation was initiated to determine what additional overburden removal equipment was appropriate to the Highvale operations, and recognizing the long delivery times of this major equipment. when it would be required. This paper describes studies and tests which were undertaken in 1983 and 1984 to select prestripping methods and equipment for the future operation of the mine.Previous planning studies have shown that with the current burn schedule and with the economies of progressing from low cover to high cover, as has been practiced in the past, overburden volumes will exceed the capacity of the existing draglines in the early 1990s (Fig. 4). These previous studies had placed reliance on (I tandem dragline operation for future overburden removal requirements up to a depth of 45 m throughout the Highvale Mine. New drilling together with the associated geotechnical studies and some detailed planning on the practicality of using tandem draglines, created concerns about their universal application. Even without considering geotechnical conditions there was a depth limitation to tandem dragJines and substantial prestripping would be required to achieve the ultimate depths."

Numerous studies conducted in the 1960's and 1970's concluded or implied (1) that a high accident rate in terms of severity and frequency exists among new (first year) operators of mine machinery equipment, and (2) that lack of experience and training is a major cause of this high accident rate. Further studies refined this premise to show that the major causes of accidents among first year operators are: (1) lack of manipulative skills; (2) failure to follow/adhere to routine procedures; and (3) failure to recognize and respond properly to hazards. This report describes the design, development, and manufacture of a shuttle car operator training system, including a program of instruction which: ? Is economically feasible. ? Addresses accident data and specific causes of accidents. ?
