The management of safety has undergone a number of radical changes in the past 100 years. The most important paradigm shifts occurred through the work of Heinrich, followed by the advent of the safety systems era and, more recently, the behavioural safety approaches that have become increasingly popular around the world. Each of these æapproachesÆ to safety has left an indelible mark on the way we target safety improvements, helping decide which interventions are deployed and how they will be measured. They have also resulted in a number of æmythsÆ about safety management, each of which will be discussed, analysed and challenged in this paper. This paper highlights the complex problem of æcomplacencyÆ, that has arisen out of the increased levels of regulation and consequent perceived protection that workers enjoy in the work place. It also looks at a phenomenon called æaccident migrationÆ which results from a superficial treatment of accidents with changes to rules and procedures. The most recent approach to safety is generally known as behaviour-based safety, which flowed from the human sciences and quality management era. This approach has had a dramatic impact on the way safety is managed around the world, but still falls short in a number of key aspects, most notably the area of risk awareness and risk understanding. Many workplace accidents occur simply because the risk is unidentified, underestimated, not understood or ignored. This paper proposes a new direction for safety, called competency-based safety. It is based on cognitive psychology aimed at combating the consequence of modern protection. A case study is presented detailing the deployment of this approach throughout Placer Dome.

The Wild Dog gold-silver-copper deposit represents a new ore-deposit type in Papua New Guinea. The telluride vein mineralisation is associated with a Tertiary caldera and has similarities with that from the Baguio district, Philippines, Vatakoula, Fiji, and the San Juan Mountains, southwestern Colorado. The deposit, discovered in July 1983, has negligible stream silt and concentrate anomaly, and a float-train limited to within 300m of the prospect. The comparatively recent exposure of the vein system may explain the limited stream expression of the deposit. At least three major veins occur in the deposit; the vein system strikes north- northeasterly, dips 78¦W, and has a maximum strike length of 1350m. Veins are enveloped by a wallrock dominated by an assemblage of quartz-illite-pyrite. Four types of silicification related to multiple vein filling events have been defined on the basis of field relationships, drill-core intersections, petrology and fluid inclusion study. To December 1986 a total of 22 diamond drill holes (2304m) and 40 reverse circulation drill holes (1833m) have been completed on the deposit, defining a 750m zone of interest. The upper 30m of the deposit is oxidised. Much of the high-grade mineralisation in both the sulphide and oxide portions of the deposit is restricted to ore- shoots which strike parallel to the overall vein structure and are vertical to steeply westward dipping. They are defined geochemically by the lg/t Au cutoff and geologically by the relative increase in the vein density of auriferous grey-black sulphidic silica, the final vein filling event. Metallurgical studies and the results of close-spaced drilling have demonstrated the very good repeatibility on duplicate assays, and the extremes of grade variability of mineralisation over very short distances. The good assay agreement on duplicate samples .s indicative of fine, dispersed gold.

The Renison Bell Tin Mine on the West Coast of Tasmania has recently installed a new underground ore handling system incorporating underground crushing, conveying and hoisting facilities. Contracts for the sinking of the shaft and the development of the mine were let and managed by the operator, Renison Limited. Hardcastle & Richards was responsible for the design, procurement and construction management of the fixed plant and equipment including civil, structural, mechanical, electrical and instrumentation services. The system demonstrates worldÆs best practice in design and incorporates a state-of-the-art integrated control system allowing the entire system to be controlled by a single operator located in the underground crusher chamber. A rapid construction period was achieved, despite the difficult and constrained underground environment, without affecting the ongoing operations of the mine. The mine, which was established in the mid-1960s, had previously operated by trucking ore to the surface via a decline. Truck haulage distances and costs had increased as the mine became deeper over the years. By the 1990s it had become apparent that, in order to remain economically viable, it was necessary to reduce truck haulage distances and allow the mine to go still deeper to access orebodies such as Rendeep beneath the current workings. The Internal Shaft System achieved this by eliminating approximately 5 km of truck haulage. It allows the production rate to be increased to up to 1 Mtpa compared with a limit of 628 kt per annum by direct truck haulage to surface. The access it provides to the Rendeep ore and other deep orebodies greatly increases the amount of ore which can be economically mined.

In 1972, with the West Lyell Open Pit winding down, mining operations transferred underground. All development tunnelling until the end of the decade was carried out by a fleet of ten pneumatic Jumbos.A decision was made in 1980 to purchase an electric-hydraulic Jumbo and a second was purchased in 1982. Their introduction changed the face of development at Mount Lyell. Significant new demands were placed on main-tenance sections, particularly on electrical reticulation.Subsequently, two further electric-hydraulic Jumbos were purchased from Agnew Mining in 1984, and successfully overhauled and commissioned. As well, two electric-nydraulic production drilling rigs have been purchased. Only one of the original pneumatic Jumbos remains in service.

In order to achieve optimum results for money spent in construction of new projects there is a need for mining developers to pay greater attention to cost control. Control is achieved by establishing a frame of reference, a method of checking and a means of remedial action. The frame of reference is a well coded set of budget estimates which graduate into definitive estimates as design is completed, backed up by drawings, specifications and bills of quantities. The method of checking is provided by cost engineering and quantity surveying input, at both feasibility study and design and documentation stages. Remedial action follows if the checking process reveals a need. In large complex projects where construction completion time is often of prime importance, control of costs can sometimes be relegated to second place. To avoid this it may be necessary to include specialists in cost control in project management teams. Quantity surveyors are trained specifically in construction economics and can provide develop- ers and project engineers with valuable support at all stages of construction.

The discovery of minerals in any community, especially precious minerals, gives a sense of hope and optimism about development prospects within the community. However, the euphoria surrounding such a find could be short lived if the wrong perception is created at the inception of the project, that the exploitation of the mineral resource will pose a serious problem to the community. The socio-economic impact can be negative or positive but if not properly handled, the negatives will outweigh the positives. Illegal mining activity, popularly known as Galamsey in Ghana, is a typical example of the negative impact of the mineral industry. Galamsey activities have resulted in an increase in school drop-out rate, pollution of water bodies affecting entire communities bringing a lot of health problems, destruction of farm lands, destruction of rail lines resulting in transportation problems, armed robbery, prostitution, just to mention a few. The improvement of the water supply system in the surrounding communities, implementation of sustainable development programs, improvement in the infrastructure, are just a few examples of positive socio-economic impact.Factors responsible for the socio-economic and environmental impact include: the unwillingness of most companies to invest substantial resources to deal with environmental issues, lack of adequate professionals with the requisite appreciation for environmental issues related to their activities, realisation that support of surrounding communities is key to successful operations, the now globally accepted view that multinational companies should invest in the socio-economic lives of the surrounding communities, etc. In order to ensure the prevention of the negative impact of mining activities on local communities, there should be a paradigm shift in the way the training for the new crop of mining and metallurgical engineers have been structured in the past, social contract with the local communities must be drawn and implemented; it must not be an imposition on the communities but rather a contract arising out of extensive interactions between the industries and the local communities. Environmental awareness has reached a level where communities affected by the minerals industry, to a very large extent, know what they should tolerate and what they should not. There is therefore the need to ensure that effective systems are in place to exploit mineral resources without damaging the environment.

The following is a discussion of a new approach to safety when developing or applying a new mining technology. In the form of an engineering process, the following approach had its inception in the military and aviation industries and is also very strongly evident in the chemical industry.The process has been developed and applied in the mining industry by the author. It is called 'Incremental Development and Management of New Technology'.The process is a composite of carefully selected System Safety Management, System Engineering and Financial techniques and methods. It consists of modules which can be modified to suit the requirements of a specific project. Modules provide an organised framework of activities to facilitate a gradual acquisition of pertinent knowledge and information and to enable progressive evaluation or verification of produced outcomes against specified requirements.

EnergyAustralia is a state-owned corporation in the state of New South Wales, whose responsibility includes ownership and management of an electricity distribution network. Expansion of the high voltage distribution network in the Sydney central business district (CBD) includes the requirement to run an additional five 132 kV feeders over 1.7 km from south of the CBD to a new 132 kV substation in the northern section of the CBD. A driven tunnel is currently being excavated in Sydney sandstone beneath Darling Harbour to contain the new 132 kV feeders. Tunnel construction methods involve both roadheader and tunnel boring machine (TBM) excavation. This paper describes the design development of the cable tunnel, discusses the use of a geotechnical baseline report as an integral part of the contract documentation and provides an update of the current construction works.

In many places young marine sediments are to be found at high altitudes, as evidenced by the existence in the North Island of Pliocene shell limestones which form prominent escarpments at altitudes of about 3000 ft. inthe central part of the Island, and remnants of the same formation found on the summits of the northern Ruahin.e Range at altitudes greater than 4000 ft.

The commonly used SAB, SABC and run-of-mine (ROM) ball mill circuits can achieve high throughputs with good operating stability. However, these circuits consume a substantial amount of grinding media, which can have a substantial impact on the OPEX and the æcarbon footprintÆ, specifically when viewed as an æembodiedÆ energy input. On the other hand, fully autogenous (AG) circuits can operate at much lower operating costs, but with limited throughput. The control of AG mills, fed by ores with hard and soft components, can also be challenging, due to the sensitivity of the mill to the ratio of hard to soft components.In this paper, a multi-component modelling approach was used to demonstrate that the harder component should be used as grinding media, and its use should be kept to a minimum to avoid reduction in the mill throughput. There is an optimal blend of hard to soft, and any optimal blending condition is both ore-dependent and mill-specific. Establishing this blend is challenging, and consequently discourages the use of autogenous mills. The new modelling approach addresses this challenge by providing a quantitative prediction of the influence of blending soft and hard components in a mill. This prediction can then be used to establish the required blend for maintaining a stable and efficient operation in autogenous grinding mode.CITATION:Bueno, M P and Powell, M S, 2012. The Cawse nickel project is a new generation pressure acid leach (PAL), solvent extraction and electrowinning process used to produce LME grade nickel metal and cobalt sulphide concentrate containing 40 per cent cobalt by treating nickel and cobalt oxide ores from a lateritic ultramafic profile. The project is located 50 kilometres NW of Kalgoorlie in Western Australia. The commercial success of the Cawse nickel operation is dependent in part upon the ability of the principal ore type to be beneficiated. A simple process of rejecting lower grade material on the basis of particle size achieves this. The process is called upgrading and allows subgrade material to become commercial ore grade and also allows higher ore grades to be processed in the early parts of the operation. The success of the upgrade process is dependent upon the physical and chemical properties of the ore being processed and the nickel grade of the ore. The upgrade ore type represents >80 per cent of the Cawse nickel resource and is hosted mostly within a limonite unit, represented by goethitic clays in the upper saprolite zone. In the upgrade processing circuit the ore is crushed and slurried. It is then passed over a series of screens that remove all material over 0.5 mm. This ensures that coarse material is removed from the main ore stream. Upgrade is reliant on selectively rejecting lower grade or barren material, which can include silica, magnesite and relict saprock. To predict leach feedgrade, the grade control estimates the head grade and upgrade percentage for all ore delivered to the run of mine ore pad. To do this reverse circulation (RC) drill holes are logged for ore type, assayed for a range of elements and undergo simulated upgrade testwork. The percentage and grade of material rejected through the upgrade circuit has the most influence on upgrade per cent. This reject is determined by the physical properties of the rock mass and drum scrubber parameters. The correlation between nickel upgrade and reject can be established to predict upgrade from the known mass reject. Parcels of upgrade ore milled to date closely follow projected correlation trends. Upgrade circuit results have been excellent, with average nickel grades upgrading by 34 per cent, cobalt by 23 per cent, with 55 per cent mass passing.

The major slime forming minerals in coal seams are generally montmorillonite, illite and kaolinite, and due to the consistant trend to full seam mechanisation in New South Wales and Queensland these minerals remain in the feed to a coal preparation plant. After coming into contact with water their size degradation is very rapid, and therefore they mostly report to the fine coal cleaning section of the plant. Froth flotation, being the most effective method of beneficiating fine coal, is widely practised in these two major coal producing states of Australia. Plant experience and laboratory tests suggest that the presence of these slimes in the flotation circuit depresses the flotation of fine coal. Although the deleterious effects of slimes on the flotation behaviour of minerals is well known, the mode of their adsorption at the mineral surface has been a subject of controversy for some time. In this paper, on the basis of the surface properties of slime forming minerals and coal, a model has been put forward to explain the adsorption of slimes at the coal surface. The degree of slime coating has been examined by scanning electron microscopy. Laboratory tests have revealed that with the appropriate use of polyvalent phosphate electrolytes these slimes can be liberated from the coal surface. Just attrition and desliming are not enough to ensure complete liberation of slimes. Coal, after treatment with these electrolytes and then deslimed, floats readily with a common frother, such as MIBC. Modifications in the conventional flotation circuits have been suggested to ensure an efficient flotation of fine coal.

The Hunter Fracture Zone can be traced into the Fiji group, and onshore SE Viti Levu in the form of NE-trending morphostructures. NW- trending morphostructures offset the NE elements. Structural mapping of Suva Harbour indicates that the NE elements are grabens bordered by faulted monoclines. Faulting is characterised by block-rotations and dip-slip displacements. The NW morphostructures are faults displaying elements of both dip-slip and strike-slip movement. Structural overprinting suggests that the NW faults represent the youngest phase of deformation, and current seismic activity occurs on this trend. A conceptual, geomechanical model of the deformation invokes toppling and wedge failure at the edge of the Fiji platform, translating material downslope into the Hunter Fracture Zone. The interference pattern produced by the superposition of the NW on the NE structures consists of a series of circular features bearing superficial resemblance to volcanic centres. Known mineral deposits in SE Viti Levu correspond closely to these features. The formation of the circular features postdates mineral genesis, suggesting that the features provide a focus for the enhanced exposure of mineral deposits rather than a source of mineralisation. There is a close association between mineralised exposures and interference patterns in extensional structures in SE Viti Levu, and a correlation between these structures onshore, and the Hunter Fracture Zone cum South New Hebrides Trench offshore. Future mineral exploration in the SW Pacific will benefit by closer consideration of the geologic structure of Pacific islands bordering the extensional regimes of deep oceanic trenches.

The objectives of grade control remain unchanged but new methods are being developed to attain them. Since the early-1990s, an improvement in the power of mine site computers has made the geostatistical tools of conditional simulation more accessible. Optimisation of the grade control process on mine sites now involves two stages: optimisation of the sampling regime and optimisation of the interpretation of ore and waste boundaries. Optimisation of the sampling regime first requires an understanding of the nature of the mineralisation and in particular the impact of particle size on the sampling precision. This relationship, termed the Fundamental Error of sampling, enables an optimum subsampling protocol to be defined for the sample preparation and assaying. However, the actual sample collection method, whether drilling or trenching, must be correctly selected and the sampling grid should be optimised. The relative costs and benefits of various alternative sampling methods and grids can be determined using the method discussed. The use of the grade control data can also be optimised. In the past ore and waste boundaries were often defined using a linear interpolation algorithm that smoothed the data (such as polygonal, sectional, inverse distance squared or even kriging methods). Frequently the relative impact of ore loss and dilution could not be estimated. Conditional simulation to generate equiprobable alternative maps (realisations) can now be adopted to address these issues. A transfer function is then used to evaluate the similarities and differences between the maps. The Ore Block Optimisation (OBO¬) process is a transfer function that produces a three-dimensional model of the optimal ore block outlines. This provides a clear guide for the mining geologist to refine and mine. The nature of the transfer function and the inputs and outputs are discussed. In the past, reconciliation has been the `acid test' for any grade control system. Any ore blocking method based on conditional simulation can make use of the exhaustive data set to ensure that reconciliation between prediction and production will improve significantly. The OBO¬ method provides a guide to the mining geologist to ensure that the best ore blocks are mined. Misclassification between ore and waste is reduced, ore loss and dilution are minimised (and are measurable) and ore is optimally assigned to stockpiles of various cut-off grades. These advantages significantly enhance mine profitability and control of the mined grade.

Numerous recent analyses have shown that future demand for a broad range of critical commodities is unlikely to be met by new discoveries and recycling alone. The only practical alternative source to address this shortfall comes from orebodies which are currently either uneconomic or inaccessible. A common assumption is that a simple rise in commodity price will render these deposits economic. The mining industry has a long history of innovation allowing economic production from increasingly low grade and technically challenging orebodies. Industry “roadmaps” have been produced on a regular basis, with the aim of identifying future innovations which will allow this progression to continue. At the same time, there have been relatively few systematic analyses of the actual range of challenges facing undeveloped orebodies. The aim of this study is to analyse the mix of challenges faced by a group of undeveloped orebodies in order to better understand the relative importance and necessity of innovations applicable to the deposits, with the ultimate aim of determining the relative value and impact of possible innovations to the economic viability of those deposits. Mechanical damage leads to the increase of effective stress of sediments, and pore closure hinders the discharge of methane gas. Ignoring the influence of mechanical damage leads to an excessive prediction of hydrate gas production. Therefore, a fully coupled model considering mechanical damage-chemical decomposition-gas-water two-phase seepage-temperature was developed in this study, and the model was verified by Masuda experiment. Based on the fully coupled model, the temporal and spatial evolution of the whole process of hydrate decomposition under the influence of three initial absolute permeabilities, three pressure drops and three water saturations were considered. The results showed that: • The gas production rate reached the peak value in a short time after depressurisation, and the peak value increased with the increase of the absolute initial permeability. The larger the initial absolute permeability was, the faster the damage variable reached the peak value. The porosity increased gradually before the damage variable reached its peak, and the gas flowed more easily. • When the pressure drop was low, the hydrate remained stable in solid form. As the pressure drop increased, the hydrate gradually began to decompose. The greater the pressure drop was, the faster the initial decomposition rate of the hydrate was. So the damage variable increased rapidly and the porosity increased continuously, resulting in a large gas production rate at the initial stage of decomposition. • The larger the initial water saturation was, the smaller the effective stress wss, resulting in the slow growth of the damage variable and the small change of porosity.While the gas relative permeability was small, it resulted in a smaller gas production rate, and hydrate decomposition was slower. • The damage variable showed a process of rapid growth first, then slowly increased to the peak, and then gradually decreased to a stable state. When the damage was considered, the hydrate decomposition time to the steady state was longer, and the gas production was about 80.68 per cent of that without considering the damage.

Social licence to operate is developed and kept through managing both operational impacts and the relationship between local community and company. However, CSIRO research has demonstrated that in determining how accepting community members are of a mining operation, relational factors, such as procedural fairness and trust, are much more important than operational impacts, such as impacts on employment and the environment (Moffat and Zhang, 2015). Companies seek to develop relational factors through their engagement with local community; such engagement has many aspects, including, initially identifying stakeholders and positioning the company as part of the local community (Lacey et al, in prep; Thomson and Boutlier, 2011). These relational factors involve the social identities of people living in the community. Social identities are a part of people that shape their beliefs, based on their connections with various groups in their lives (Hogg et al, 2004).An engagement process, intentionally or not, inevitably interacts with the social identities of stakeholders (Crane and Ruebottom, 2011). For example, traditionally, at the very beginning of engagement processes, community members are assigned to categories using conventional stakeholder mapping and categorisation methods, often based on assumptions about stakeholders’ interests, which equates to assumptions about stakeholders’ social identities (Colvin, Witt and Lacey, 2016; International Council on Mining and Metals, 2012). Yet, there has been limited empirical inquiry into whether such categories reflect the social identities of stakeholders and if there is a bearing on relational factors, including, the perceived fairness of consultations and trust in mining companies (Crane and Ruebottom, 2011). Also at this time, companies commonly seek to create a presence in communities, for example, joining Chambers of Commerce and participating alongside community members. Again, there has been limited empirical inquiry into whether community members and companies identifying with the same overarching group and thus sharing aspects of social identity, influences relational factors (Crane and Ruebottom, 2011). In this study, we examined the effects of changing aspects of social identity by changing how stakeholders were identified and companies were positioned, on how stakeholders’ perceived a new mining operation, specifically how they perceived the procedural fairness of the proposed consultation process and trusted the company, two factors that underlie a company’s social licence to operate.CITATION:Carr-Cornish, S and Moffat, K, 2016. This study introduces a new probabilistic approach to seismic stability analysis of embankments and slopes. The approach is based on Sarma's solution, which is modified through presentation of the seismic force by its horizontal and vertical components; i.e. the force may have an arbitrary inclination. The probabilistic modelling includes Monte Carlo simulation of the shear strength parameters of the soil mass, which are assumed to have a normal distribution. The probability of failure is determined based on the seismic co-efficient, which is treated as a random quantity. The probabilistic approach is illustrated through a solution of a numerical example from an opencast coal Mine.

The Northern Sewerage Project (NSP) is an upgrade to one of MelbourneÆs ageing, under-capacity trunk sewers. It will reduce the volume and frequency of wet-weather overflows to the Merri and Moonee Ponds Creeks and facilitate new housing developments in the northern suburbs of the city. A total of 12.4 km of tunnel and eight vertical access shafts will be excavated as part of the two project stages: Stage 1 (NSP1) will be owned and operated by Melbourne Water (MW) and Stage 2 (NSP2) will be owned and operated by Yarra Valley Water (YVW). The owners require the project to have a service life of 100 years, a requirement significantly beyond many of the current Australian Standards, which aim to achieve service lives greater than 60 years but are typically assumed to achieve 40 to 60 years. Because of the potentially corrosive natures of the external environment along the tunnels and the internal sewer environment, durability was a key concern for NSP1 and NSP2. Stringent specifications were applicable to the tunnel lining systems. This paper describes the corrosive environments impacting NSP1 and NSP2, explains the key design differences between the two stages and details the lining systems considered and selected to achieve the clientsÆ desired 100-year service life. The project design is complete and construction began in 2007.

Longwall faces producing in the order of 20,000 tons per day of raw coal are presently running in the Houilleres du Bassin de Lorraine of Charbonnages de France. Production levels of twice that level pose some technical and economic problems of coal cutting, ventilation, coal and equipment transportation in the.face and from the face to the sur- face. The peak productions of the equipment in use today, the results of trials with new equipment, the utili- sation of forecasting models (some in the form of software), allow us to fo- resee feasable technical solutions to these problems. They will first be considered in the best possible context (thick- ness, dip, non gassy, soft coal, good strata, no faults). This will give. the necessary conditions to reach daily output'of that level. A sensitivity analysis of these- figures to the effective natural con- ditions will then be undertaken and the implication of such a project on health and safety (dust, climate ...) will be carefully considered. A concise technical and econo- mical approach will permit us to fore- cast the economical interferences of such production units and specially the consequences of sudden variations of the output (downtimes, failures, etc). From this investigation, goals for technological development and re- search areas will be outlined to be able to reach this production target in the XXIst century.
