In the 10 years from 1998 to 2008 the annual number of fatalities in the Australian mining industry has plateaued (MCA, 2007-08 and abc.net.au website). Despite significant efforts to eliminate fatalities, this is not a comfortable statistic in this very public performance indicator.GHD has been working with the mining and minerals processing industry to identify the key risks that lead to fatalities utilising a semi quantitative risk assessment technique (SQRA®), initially developed to assist Major Hazards Facilities prepare Safety Cases (OH&S, 2007-08) in response to the Longford Incident in 1998 (Dawson and Brooks, 1999).Workshop cross functional teams are assembled from the sites and led by the GHD facilitators to identify the incident types that can have fatal consequences, the causes of those incidents and the controls that are utilised (or could be utilised) to reduce the risk. The risks for each incident type are quantified to produce a risk profile for the facility and the critical controls – the controls that have the greatest impact at reducing the risk of fatality – are selected. These controls are assessed for their adequacy and improvement actions are identified. Prioritisation of the actions is achieved by re-estimating the incident risk profiles.A review has been undertaken using a small sample from the more than 100 individual studies that GHD has completed at mining and minerals processing sites. The five most significant risks reported by sites are:1. Fall from Heights,2. Vehicle/ Vehicle and Vehicle/Pedestrian interactions,3. Entanglement,4. Molten Metals and Materials, and5. Electricity.The most significant reductions in risk identified were found to be by implementing engineering controls. Examples include:1. Chutes to contain splashes and spills of molten materials,2. Redesigning instrument and equipment layouts to take personnel away form high risk areas,3. Designing remotely operated equipment to remove personnel away from high risk areas,4. Proper fire fighting equipment provisions such as deluges around flammable materials, and5. Adequate risk assessment to ensure equipment is designed to handle the real situations and not a design case to meet an arbitrary specification.All of these cases and many of the actions identified in the studies could have been handled through better design and construction early in the life of a project rather than retrospectively fitted after operations have begun.

A new procedure has been developed for the direct determination of metals loaded on activated carbon. It involves preparation of a suspension of the powdered carbon in water, followed by direct sampling of the suspension with a micropipette,and analyses by atomic absorption spectroscopy using carbon rod atomization. Advantages of the method include the simplicity of sample preparation and the ability to provide a complete metal analysis in 1 - 2 hours. This enables carbon-in-pulp operators to respond quickly to changes in plant efficiency arising from carbon fouling. The successful application of the method has been demonstrated for gold, copper, nickel and iron. Other elements to be surveyed include calcium, magnesium and silicon.

A significant part of the reserves of the Southern Coalfield lie beneath major water storages and their impounding dam structures. One of the functions of the NSW Government's Dams Safety Committee is to specify the particular mine layouts which can be used beneath the stored waters according to certain guidelines. A study was made of examples of the two different methods of extraction used under the stored waters as proposed by the mining company according to the guidelines and approved by the Dams Safety Committee. Using both pillar monitoring and mathematical modelling approaches it was shown that the recovery of coal from these layouts could be improved while still ensuring the long term stability of both the underground support pillars and the body of the strata. The integrity of the water storages and their impounding structures would therefore not be Affected.

The Waterview Connection will complete a strategic Auckland motorway route linking the south, west and north. The project will include 2.1 km of ‘undrained’ driven tunnels, 500 m of drained cut-and-cover tunnel and tunnel portals.The project is aligned beneath a stream valley of Oakley Creek, which is incised through alluvium and in places overlain by landfill, between Mt Albert volcano and a ridge formed in alternating sandstones and siltstones. The resulting topography, geology and hydrogeology of the tunnel alignment are complex.The Assessment of Effects required estimation of the likely settlements resulting from the tunnel construction and assessment of their effects on the buildings, services and other features. A conservative approach was taken when estimating settlements in order to provide an upper bound estimate of the consequent effects. There are three sources of these settlements: mechanical settlement from the tunnel excavation, mechanical settlement from the displacement of retaining walls and consolidation settlement from the groundwater drawdowns during construction and, in some cases, in the long-term.The settlements from these sources were calculated independently and then combined to identify the most critical effects. These combined settlements were used to assess the potential damage to the buildings, services and other features above and around the tunnels. The combined settlement results generally followed the anticipated trough shape centred on the underlying tunnel but with some noticeable effects relating to the underlying geology and hydrogeology.The effects on buildings were typically assessed as negligible with some areas of very slight to moderate damage predicted. Effects on services are anticipated to be minor but some large and older pipes will require specific monitoring during construction. A monitoring programme is proposed to allow the actual settlement and effects to be compared to those estimated with appropriate responses should those effects significantly differ.This paper describes the methods used to calculate and combine the settlements and assess their effects.

The authors have presented their ideas for developing a powder sampling device in the I-Corps™ program sponsored by the National Science Foundation (NSF). This is an intensive customer discovery program developed to increase the economic impact of the research sponsored by NSF.The I-Corps™ program requires performing at least 100 interviews with potential customers in various industries. These interviews are essential to establish the need for a particular product or solution, and identify potential commercialisation partners and competition. The interviews have revealed the absence of Theory of Sampling (TOS) principles in many companies. The interviews are designed to identify the difficulties associated with current sampling practices that do not meet TOS principles.The I-Corps™ program is a significant challenge for scientists who often avoid discussing a concept in fear of losing their research idea. The program teaches how to obtain information without releasing the important intellectual property related to a product development idea. The I-Corps™ methods have been used to discover the prevailing ideas on sampling in various industries.I-Corps™ also teaches how to present ideas to investors; another significant challenge for a scientist. "In today’s challenging climate of reduced staffing, limited funding and uncertain markets, we are called on to operate more productively and ‘be more innovative’. As an industry, we are quick to identify our practice areas as a mark of our role in the machinery of the mine value chain. We proudly note our profession – ‘geologist’, ‘mining engineer’ or ‘accountant’ – and demarcate our responsibilities and even our physical space. We even subdivide our disciplines into sacred entities such as ‘logging geologist’, ‘resource geologist’ or ‘structural geologist’ to prevent any blurring as though somehow the confusion will cost valuable disciplinary ground. How can we function professionally to heed the call to be more productive and more innovative?The purpose of this paper is to underscore what is expected from project evaluation, examine the ways in which we work as an industry and explore alternatives used in other industries and environments.The paper begins with a summary of project evaluation expectations and the ways in which disciplines typically interconnect when conducting project evaluations. Next, the paper examines alternative work frameworks and making a distinction between multidisciplinary, interdisciplinary and transdisciplinary frameworks. In-practice examples of these frameworks in alternative industries and contexts provide evidence for application as well as an opportunity to examine their pros and cons.The paper closes with a call to re-examine working practices in the mining industry, with a view to enhancing productivity and heeding the call for greater innovation.CITATION:Coombes, J, 2016. "Although it is well known and easily experienced that the feed size distribution influences the throughput and stability of operation of a semi-autogenous grinding (SAG) mill, the details are poorly understood since it is often only the input and output measurements that are used to identify unfavourable in-mill conditions. Load cells and power monitoring are sometimes used but are generally poor indicators of the mechanisms that lead to such unwanted conditions.At the MMG Century operation a 12-microphone array around the SAG mill using non-contact acoustic measurement methodology has been used to diagnose the in-mill conditions to facilitate improved throughput and stability of operation. Use of acoustic measurement and control has provided a demonstrated increased throughput of at least 20 per cent as well as improved stabilisation of the mill. Since the information derived from the acoustic noise from a mill allows discrimination of the behaviour of the steel balls and the rock, a number of in-mill conditions can be identified with signals that include toe and shoulder angles, water holdup and the ball to rock ratio.Feed size variations, measured online with an imaging system, have been used to determine their effect on the internal charge behaviour. It has been found that circumferential segregation of steel balls and rock occurs as well as poor mixing of large rocks. Both of these effects are not conducive to optimal breakage rates of different size fractions. Longitudinal segregation effects also occur. This paper presents and discusses these results and illustrates how the new acoustic knowledge can be used to manage and improve mill operation and stability.Citation:Pax, R and Cornish, B, 2016. The transition from low stress to high stress conditions is a challenge eventually faced by most mines in Australia that extend to depths greater than (generally) about 1000 m. In areas with strongly adverse 'stress versus depth' gradients, such as some parts of Western Australia, significant stress-related problems have occurred at depths as shallow as 300 m below surface. The actual depth at which the 'transition' occurs is a function of the local pre-mining stress field, the strengths of the various rock types hosting the deposit and the layout and sequencing adopted in the mine. Stress-related damage can result in increased support costs, and delays associated with rehabilitation and disruption to production. In the worst case, where strong seismicity and associated damage occurs in active mining areas, 'exclusion' zones and 're-entry' protocols may be required. These can severely disrupt production. A key aspect of managing the transition is the recognition of the onset of stress-related ground behaviour. Damage mapping can be used to develop understanding of the relationships between mining activities and the local and regional response to them. Carefully documented case histories provide a basis for calibrating numerical models. This paper discusses some of the typical styles of ground behaviour observed in higher stress conditions in underground mines. A simple damage classification scheme is described to assist in 'damage mapping' of mine tunnels. An example of damage mapping is presented to illustrate an approach commonly used for recording stress-related damage.

The Ambuklao and Binga Rehabilitation Project is a 175 MW hydro power project currently under construction across dual sites in the north region of Luzon Island, Philippines. When it was built in 1956, the Ambuklao Dam harnessed the second largest hydro-electric reservoir in Asia. However, due to landslide erosion and prolonged silt infiltration caused by an earthquake in 1990, the power plant has been rendered inoperable since 1999.To bring the power generating facility back to life, the project scope entails the rehabilitation and construction of civil structures, shafts and tunnels. A major component of these works is the decommissioning of a section of the seven metre diameter headrace tunnel by installing a concrete plug. The headrace tunnel is part of a closed waterway system that connects the dam via a shaft, exhibiting a hydrostatic pressure of 13 bar (1300 kPa) at the location of the new tunnel plug. In addition, the power station ball valves have seized ajar and the damaged intake riser is unsafe toaccess thereby requiring the plugging operation to be carried out under live flowing conditions.Working under hydrostatic pressure and live conditions, the headrace plug will be an underwater tremie pour operation that includes drilling and concrete injection through blow out preventer caissons installed in the floor of an underground chamber above the tunnel. The purpose of this paper is to describe the key engineering and planning components involved in this operation including pre- and post-grouting, chamber crane, sonar, local water compensation, jet flushing, drilling, caisson installation, and concrete design, production, delivery and placement. The success of this task rests on the strategic preparation and operational management of risk and safety in a challenging rural environment.

Assessing the uncertainty and risk in mineral resource estimation is required for best practice and is implied by the public reporting codes in the categorisation of mineral resources and reserves. One industry best practice is quantifying the risk on future production for a given period, which can be done using two geostatistical approaches: conditional simulation and drill hole spacing studies based on estimation variances. Conditional simulation can provide a more complete and flexible answer to the problem, but it takes a fair amount of time to complete. Drill hole spacing studies are simpler, faster and can provide reasonable assessments of mineral resource uncertainty.This paper is a presentation of the principles, theory and practice of drill hole spacing studies. Various types of risk assessment are considered. Difficulties encountered in practice are discussed, and solutions are proposed. The important parameters impacting the results are reviewed. The key issue which determines the use of imaging for explosives and mining research is the question - `What information will I gain by using imaging technology?' This paper will address that question from the point of view of a researcher and cameraman working in industrial imaging and photography. The paper will establish a common set of reference standards for high speed imaging and photography which researchers can apply when deciding which type of imaging technology is appropriate. It will present an overview of current image analysis technologies and practices in use across a range of appropriate scientific fields and draw lines of commonality of approach between these fields and the field of explosives and mining analysis and research. Further to this, the paper will examine the different technologies from an image quality reference point, ease of operation, cost, and reliability parameters. Finally this paper will present an argument for the hybridisation of established and new technologies which could provide a more beneficial approach to the use of imaging as a tool of research and analysis in the explosives and mining industries. This will include a novel approach to high speed filming and analysis in the field which should deliver high quality images in a time frame which is more appropriate to urgent action and solutions.

The revival of the mining industry at Captain's Flat after a long period of years of inactivity has attracted attention to the importance of the ore-deposits of that field as potential sources of supply of lead and zinc. Mindful of the necessity for taking stock of the State's mineral resources, the Department of Mines of New South Wales as part of its programme of geological investigation, carried out a survey of the oredeposits and associated country rocks during 1938 and 1940.Surface geology was mapped in detail and all available exposures were examined and logged. Unfortunately, and this condition militates against the formulation of conclusions because of insufficiem.t data, some of the older workings, notably in Keating's Section, were inaccessible and the logging of levels and stopes is by no means as complete as could be desired. However, with the progress of development and exploration, further information of geological value will come to hand for incorporation in supplementary reports. It may be expected that, under such circumstances, modification of certain statements herein expressed may be necessary but it is not anticipated that any vital alteration will be occasioned.Within the State of, New South Wales, Captain's Flat ranks second to Broken Hill as a pqtem.tial producer of lead and zinc. In addition, large quantities of iron pyrites are available for the manufacture of sulphuric acid which finds application in the superphosphate industry.The mines are now controlled by an operating company known as Lake George Mines, Ltd., incorporated in New South Wales, the parent organisation being the Lake George Mining Gorporation Ltd., registered in London.Over £600,000 has been spent by the Company in exploration, mine development, surface plant, housing and water 'conservation, while the State' Government constructed a branch railway from Bungendore at a cost of £200,000. The various activities consequent upon these development and construction programmes and the prospect of employment in the industry has led to a rapid increase in the population of the township.The State, generally, has benefited to the extent of a substantial contribution to the relief of the unemployment problem, and it is hoped that reasonable permanency will be assured to some hundreds of workers for many years.

Converter slag assaying 3.7 per cent Cu is produced by Mount Isa copper smelter at a rate of up to 180 000 tonnes per annum.Priority is given to the processing of ore, however concentrator capacity periodically exceeds that of the mine and the opportunity is taken to treat converter slag. It is processed in batch campaigns, typically through the Copper Concentrator, but the Hilton Concentrator has also been used. Slag and ore are kept as separate as possible, since each has an adverse effect on the flotation performance of the other.

Improving liquid blending operations in production plants is of paramount importance in many industries like mining, oils, petrochemicals and food. For accurate blending of different types of process streams, like homogeneous, heterogeneous, cold or hot streams, it is essential that the composition can be accurately monitored in real time. Advances in energy dispersive X-ray fluorescence (EDXRF) technology offers the possibility to apply XRF for online elemental analysis. In this paper we discuss an online EDXRF analyser for accurate blending control in many industries.EDXRF has the advantage that multiple elements can be measured simultaneously over a wide concentration range of wt per cent down to a few mg/kg, for elements between sodium (Na) and americium (Am) in the periodic table. To reach the desired accuracy and precision in an online XRF analyser, it is important to bring a representative and homogeneous sample of the process flow into the optical path of the analyser. The combination of a laminar flow cell design and advanced EDXRF technology leads to low detection limits and high performance. The obtained precision and accuracy allows for much tighter process control. Cave mining is of interest to the mining community as it offers a chance to extract large, deep orebodies with low operating costs and high production rates. It allows for extracting large low-grade orebodies, which may not have previously been considered for mining. Cave mining is done either top down (ie sublevel caving) or bottom up (ie block/panel caving). Both approaches rely on gravity, stresses and weakness of the rock to fracture and achieve a controlled collapse.Block caving starts at the base of the orebody. It progresses upward and across through continuous collapse of the roof. To achieve this, a critical hydraulic radius must be exceeded. This is accomplished by blasting drawbells, through which the caved ore passes to the production level. An entire level above the drawbells (undercut level) is excavated to help initiate the cave. The undercut level ensures the connection between drawbells and achievement of the required hydraulic radius.Conventionally, drawbells are generated by drilling and blasting several times from the production level as well as the undercut level. Mucking between each shot is required to generate enough void space for the rock to swell. The need for several blasts necessitates some downtime between each blast. Also, the cost of developing and maintaining the undercut level can constitute up to 35 per cent of the cost of mining the panel.A new blasting technique was developed to create the drawbells in a single blast from the production level. The technique ensures the connection of the drawbell to the neighbouring ones, thus, the critical hydraulic radius is reached and propagation of the cave achieved without the need for the undercut level. The technique has the potential to reduce the capital cost of a block caving operation, increase its feasibility and reduce time to reach the ore and consequently the payback period. As the mining sector increasingly embraces the sustainability agenda, and implements thorough environmental reporting in support of its case for a continued æsocial licence to operateÆ, there is a need for ever-clearer metrics for key areas of impact. It is clear that natural waters are both the predominant pathway and main recipient of the environmental impacts of mining. A new approach to summarising the impacts of mining operations on water quantity and quality is proposed, adapted from an existing generic framework, which is increasingly used to quantify the æwater footprintsÆ of cities and nations. This includes both direct water use and ævirtual waterÆ use, implicit in goods and services used by the mine. In contrast to the existing formulation of æwater footprintÆ, it is here argued that it makes hydrological sense to express the water footprints of mines in areal terms, as equivalent catchment areas. It is also important in the mining sector to capture systematic changes in the scale of impact, which can be anticipated and predictively quantified over the full life cycle. This includes the essentially infinite post-closure phase, during which æremanent water consumptionÆ occurs, either through physical water loss (eg by evaporation from pit lakes) or by ongoing contamination of sizeable flows of natural waters. Methods for assessing the water footprints of mines are now available, and the time is increasingly ripe for them to be applied in earnest in environmental reporting. An example application to a mine in a humid subtropical area of South America is presented. In that case, the mine site itself extended over 106 ha, whereas the maximum water footprint exceeded this by a factor of 27. Although the water footprint can be expected to decline by a factor of ten immediately after closure, sustained evaporation from pit lakes and tailings dams with water covers will gradually increase the remanent water consumption rate, such that the final post-closure water footprint remains about 25 per cent of that encountered during the peak of mining.

The mining of magnetite iron ore in Western Australia will generate large volumes of waste rock and tailings over many decades. The need to dispose of these materials will result in massive waste landforms that will require significant planning and management. The formation of comparable man-made landforms is relatively common, as exemplified through past practice in gold and iron ore (haematite) mining. Unfortunately, the performance of these landforms, with respect to long-term stability (erosion resistance), the release of low pH seepage water and aesthetic impact, has commonly been disappointing and environmental legacies have been created as a result. Lessons that have been learnt from past, large mining operations indicate that there is a need to rethink the methods of waste disposal and landform development applied to magnetite mining. The environmental compliance criteria applied to magnetite mines for the storage of tailings and waste typically include minimising dust generation (particularly as the tailings and mine waste may contain asbestiform minerals), control of silt, minimising visual intrusion and management of potentially acid forming materials. In addition, the climate of Western Australia and the paucity of both surface and groundwater in the vicinity of the magnetite mines frequently necessitate water consumption to be minimised. This paper discusses the potential to co-mingle magnetite mining waste streams in a variety of ways to concomitantly satisfy the environmental compliance criteria and the operational requirements of the mine. These methods include co-mingling of coarse tailings and waste through dry-stacking, deposition of tailings as a paste, co-disposing of all wastes to form an integrated landform and the co-mingling of all waste streams to produce PasteRockTM (a blend of tailings paste and crushed waste rock). The paper demonstrates how these techniques can be applied to the magnetite mining industry to provide better outcomes than might be achieved using more conventional techniques, currently employed in large-scale mining operations.

The Project Specification and Technical Requirements documents associated with several recent large road tunnel projects within Australia have specified the application of AS/IEC 61508 – FunctionalSafety of Electrical/Electronic/Programmable Electronic Safety Related Systems at very high performance levels for the various control and fire life safety systems associated with such facilities.This paper seeks to explore the implications of referencing this standard within the road tunnel services context in terms of practicality, complexity and cost, whilst providing some examples of how a high safety integrity level (SIL) level can theoretically be achieved given current hardware availability.

In the present work, the microstructure and mechanical properties of structural mortar from glass insulators were studied by X-ray fluorescence, X-ray diffraction, infrared spectroscopy, microhardness and compression testings. The investigated glass insulators were manufactured between the 1960 and 1990 years and had been used in 230 kV transmission lines. The results evidenced that the structural mortar of glass insulator was replaced from Portland to aluminous cement along the years. The crystalline phases of the mortar were identified and mechanical properties were correlated.

Consent to develop and operate a new coal mine in New South Wales (NSW) will be conditional upon a number of environmental controls. In NSW, applications for any new major development associated with coal mining are determined by the Minister for Planning and Environment. The environ- mental conditions placed on the development consent, the coal mining lease and the approvals, and licences under the pollution control legislation amount to a substantial component of any new project's commissioning considerations. The nature, scale and location of projects and the principles of pollution control on which conditions of State Pollution Control Commission (SPCC) approvals are based have a considerable influence on project planning. The design, construction and operation of coal mines in NSW has been progressively modified over the past 15 years, to incorporate increasingly stringent requirements for environmental management. These requirements are in turn, a reflection of a greater community demand for environmental protection.
