"Safety performance in many companies and even industries has stalled in the last decade. Accidents rates are at a “plateau” and yet, serious accidents and fatality rates are not. In more dramatic cases, such as in the BP Texas Refinery disaster, organizations that have “exemplary” safety statistics, suddenly have a catastrophic or multi-fatality event occurring. Classic examples are the Piper Alpha disaster and NASA’s Challenger and Columbia disasters, the BP’s Deepwater Horizon Rig disaster in the Gulf of Mexico. Looking into the causes of these accidents provides an insight into the events and deficiencies that led up to the accident, but what are the common features in the organization’s mindset, or its culture? What characterizes these organizations’ decision-making, their approach to safety and to risk and are there features that can be delineated? The research and review presented in this paper covers a period since1995, which started in the Australian resource industry and since then covered international events. The features of these organizations are summarized as the seven deadly delusions of near zero organizations (NZO) that suffered unexpected disasters, based on extensive research by the author. More recently, the massive West Fertilizer plant explosion in Texas, and the Train Disaster in Lac-Mégantic, Quebec, Canada underlined the same trends. Organizations may experience calamities not because they are bad, or unsafe, but because they have become “cult-like”. INTRODUCTION The train disaster of 6 July 2013, in the small town of Lac-Mégantic in Quebec, Canada, in which 47 people were killed, had many strange pre-cursorsso-called “black swan” events that have occurred in many different industries. Black swan events are so rare, that there is almost zero chance that they will ever occur again, but they are so calamitous that the responses to these are emotional and irrational, (but necessary) and in those responses, the seeds for other, different black swan events are put into place. It is not what we don’t do, or do wrong, that causes modern disasters, it is what we do. The same things in the culture that make us successful, may also cause the disasters. We slowly, incrementally and insidiously ‘float’ into disaster. One such Black Swan event: The Piper Alpha Oil Rig disaster, killing 167 men in the North Sea on the 6 September 1988, was one of the most pivotal events in safety around the world. It changed the thinking and focus of governments, whole industries and led to numerous books and papers. It resulted in new legislation, textbooks and a critical self-examination by the oil and gas industry."

In response to numerous requests for information as to the status of internal-combustion mine locomotives of the Diesel type that use heavy or non- volatile oil for fuel, with special reference to their safe use in coal mines, the United States Bureau of Mines is issuing this report. Data mere obtained, on application of such locomotives to cod mining only in Belgium, France, Germany, and Great Britain. The locomotives are used in coal mining in other countries, as well as in potash, iron, and other kinds of mining, but their chief development has been in coal mining in Germany, France, and Belgium. Recently popular interest in the Diesel type of engine for motive purposes was aroused in this country by the successful application of Diesel engines to railroad locomotives hauling fast transcontinental trains, and, according to published statements, at a low cost of fuel per mile of run.

The East Bay Municipal Utility District is undertaking seismic upgrades of the Claremont Tunnel in the San Francisco Bay area. The tunnel carries treated water across the active Hayward Fault to supply a major metropolitan area. Studies indicate the tunnel could be severed by fault offset during a major seismic event. A design approach to achieve limited operation of the tunnel following fault displacement has been established and design details are currently being evaluated. This paper describes the project, the design approach, and summarizes significant construction constraints.

The Kisladag Mine, Turkey, is a large porphyry gold deposit which is mined by open pit methods. In operation for over ten years, Kisladag provides an excellent example of how attention to a geologic feature, namely hydrothermal alteration, became a very effective predictive geological tool for planning and operation. The alteration domains were initially used as a geological tool to assist with establishment of resource model domains and to guide delineation drilling, but they were soon recognised to be correlative to geotechnical characteristics (affecting pit slope angles) and metallurgical performance (heap leach recoveries). Recent developments in spectral processing allow the systematic quantification of complex overlapping alteration phases and have greatly improved the ability to rapidly define internally consistent alteration domains for geotechnical and geometallurgical purposes.Alteration at Kisladag comprises a central core potassic (K-feldspar-biotite) zone, a tourmaline-white mica enveloping zone, an advanced argillic lithocap on the deposit’s eastern side and an overprinting pervasive argillic zone. These alteration zones and their distribution were based initially on strictly qualitative, visual mineralogical logging of diamond drill core. With the need for more accurate and consistent identification of the alteration mineral suites, particularly the phyllosilicate phases, logging was augmented by systematic measurement of spectral data using a Terraspec instrument.Recently the Kisladag spectral database (over 85 000 readings) was reprocessed by the newly developed methodology (aiSIRIS™) that allows relative quantification of the spectrally-responsive minerals. Visualising this transformed spectral mineralogy at various mineral concentration thresholds using implicit modelling software (Leapfrog®) led to the creation of consistent mineralogical and alteration domains that are used for the definition of geotechnical and geometallurgical parameters. An implicit modelling workflow enables these domains to be easily updated as new data is collected while maintaining geological integrity of the model.CITATION:McKinley, S, Baker, T and Öztas, Y, 2017. En el proceso de clausura de labores explotadas en a mina subterránea se emplean estructuras de cerramiento, con la finalidad de que estos sectores alcancen la seguridad física, química y protección ambiental en el interior de la unidad de producción y sus alrededores. Sin embargo, uno de los problemas que se presenta durante la explotación de la mina, responde a el abatimiento de la napa freática, donde los sistemas de drenaje captan y conducen el agua por las galerías ubicadas en los niveles inferiores de las minas, generando el Drenaje Ácido de Roca. Por ello, el presente texto señala la construcción de las estructuras de cerramiento proyectadas y construidas en un nivel freático para así reducir el oxígeno requerido para la formación del DAR. Asimismo, se muestra la estructura proyectada y construida en el Nivel 490 de la unidad de producción Julcani, perteneciente a la Compañía de Minas Buenaventura S.A. A y la aplicación práctica de los métodos usados para su diseño.

BARCUS AND SONS, INC. PROJECT PROFILE: 108-10 in. diameter AugerPiles installed to an approximate depth of 32 ft. Piles were installed through Stiff Clayey Fill and Medium-Dense Sands to Decomposed Shale. Piles were drilled to a refusal criteria of 1 ft. / 3 min. Piles were installed using a Barcus Mantis AugerPile rig which is large enough to install piles 25 to 50 ft. long outdoors. The Mantis is also ideal for indoor and low overhead installations. On top of being extremely versatile, the Mantis is also very economical to mobilize. The existing structure is supported on H-Piles. The AugerPiles were furnished and installed for less than the material price of the H-Piles. Design loads were under 40 tons which eliminated the need for a load test. AugerPiles were selected for minimal vibration, limited noise, and economy.

Editor’s note: This is the second part of a two-part series on longwall mining in the United States. Part one was published in the December 2005 issue of ME (pp. 21-26). It provided a history of longwall coal mining in the United States. It also discussed the technology involved in the productivity gains and operational improvements. Boyd anticipates that U.S. coal production will increase by 61 Mt (67 million st) (6 percent) from 2004 to 2010. A combination of higher surface and underground mine production will be required to meet the projected market demand. Boyd’s projections of coal production by major mining methods are shown in Table 1.

INTRODUCTION The Macassa Division of Lac Minerals Ltd. is a high grade gold mine that has been operating in Kirkland Lake, Ontario for the past 51 years. It produces approximately 120,000 tonnes of ore per year, averaging approximately 18.7 grams per tonne (0.45 ounzes per ton) of gold. Over the years, the ore body has been developed to a point where mining takes place below the 1,220 m (4,000 feet) elevation and 1,830 m (6,000 feet) west of the original shaft. The mine is currently serviced by two surface and two winze shafts. (Figure 1). The ore body extends below the 1,930 m (6,400 feet) Level which is the bottom of the existing mining operation. An ambitious underground exploration program has substantially increased these reserves. Concurrent with the discovery of greater reserves, has been rapidly escalating mining costs, particularly labour which constituted 66% of the total cost in 1981. These costs continued to escalate as the major proportion of mining was carried out to the west, far away from the existing hoisting systems. This required three lift hoisting for men, materials and ore. I n addition to the increased travel time and ore handling expense, increased humidity and temperature and ventilation and compressed air losses have added to the inefficiency of the operations. As a result, studies were initiated in 1980 to assess the future of the Macassa operation.

Paper written on project work carried out in partial fulfilment of B Tech (Engineering Metallurgy) degree This investigation relates to a multi-purpose electrolytic system. Its primary purpose is to electrowin gold from high or low concentration gold cyanide solutions and/or from gold sludge, and to convert the recovered gold into a gold (alloy) electroform or electrode at the gold mine itself. These operations are carried out sequentially and in situ in the same electrolytic cell. Electrorefining of the gold recovered may also be done in the cell. The system applies to silver as well, and may be adaptable to base and platinum group metals. It could also be used in environmental applications, to remove undesirable metals present in aqueous solution at very low concentration. The work to be discussed forms part of recently filed patent applications. Subsequent and future developments of the system are briefly outlined. Observations of dislocation structure in niobium are presented using two techniques. Triangular pits developed by electroetching are confined to orientations in the stereographic triangle between the (111) pole and planes of the (1ll) zone and require the presence of carbon. Some unusual electroetching phenomena at other orientations are noted. A chemical etch of dislocations decorated by the addition of carbon to 50 ppm reveals the complete dislocation line structure in a surface layer up to 30 p deep, with a resolution of less than 1 p. This effect is independent of surface orientation. The structure of subboundary nets may be resolved for misorientations up to about 1'. Dislocation structures found in strain anneal grown crystals, such as helices, prismatic loops, and high-density areas, are observed. The measurement of dislocation density is discussed and values obtained in different ways are compared. IN the course of some work on the response of refractory metals to thermal-mechanical treatments, it was desired to examine the defect structure of relatively low dislocation density crystals of niobium in bulk form. Surface and bulk methods for the observation of dislocations have been reviewed by ~melinckx,' and include etch pitting, decoration, and various X-ray techniques. The dislocation line structure has been developed by decoration in a few metals, although observations were limited to certain orientations, or by the coarseness and nonselectivity of the precipitate. The most commonly used technique is etch pitting. In niobium, etch pits have been developed chemically'0," in material of high impurity content and electrolyti-cally'' in relatively pure material. In this paper, etch pits developed electrolytically, and the dislocation line structure of a surface layer developed by decoration and chemical etching, are examined, and observations are presented from both techniques. EXPERIMENTAL METHODS Our starting material was electron-beam-melted niobium, swaged to 4 or -in.-diam rod. Chemical analysis is given in Table I. Single crystals were grown by a strain anneal technique using 1 to 2.5 pct of strain and growth temperatures of 1700" to 2300"~.'~ Some crystals grown by zone melting were also examined. The desired surface orientation was exposed either by spark machining or by sectioning with a cut-off wheel and prepared for etching by electro-polishing.

The mechanized mining cycle using loaders and shuttle cars had its peak in terms of usage in the mining industry during 1950 through 1960. In Brazil, this system is still used at underground coal mines in southern Santa Catarina coal basin. With the aim of investigating the benefits of a new mining scheme using the room and pillar method for these local mines, a computational simulator model was developed in order to investigate the mining cycle. This paper presents the methodology used to develop the simulator and the results that the simulation shows regarding impact of the proposed new scheme on mining productivity.

"Underground mine fires can significantly influence mine ventilation airstreams, in some cases throttling or even reversing airflows. As a result, the performance of a metal and nonmetal mine stench fire warning system, which depends on the ventilation to carry the vital warning signal, under fire conditions is different from performance under nonfire conditions. The safety of underground miners can be jeopardized if the warning signal is delayed. This Bureau of Mines report describes research to investigate fire and stench warning system interactions. A computer model is presented that permits quantitative analysis of stench warning signal delays as a function of fire location and intensity. The results of a case study involving computer simulations of stench distribution in a hypothetical mine network subject to various fire exposures are also discussed. This case study illustrates a technique for identifying the areas within a mine that are subject to unacceptable warning signal delays, thereby enabling preemptive action by mine personnel, such as redeployment of stench injectors. INTRODUCTIONFires are an ever-present threat to the safety of underground miners. Since the smoke and toxic gas produced by a mine fire can be spread rapidly by the mine's ventilation system, mine evacuation must be accomplished as quickly as possible in the event of fire. In metal and nonmetal mines, the most common means of passing the fire warning signal to each miner is the stench system. The typical stench system utilizes ethyl mercaptan, a highly odoriferous organic compound, injected on the surface into the compressed and/or ventilation airstreams. Upon smelling the stench, workers evacuate the mine according to an emergency preplan.Although the stench system has been used successfully for over 60 yr, it has several serious shortcomings, owing to certain chemical properties of ethyl mercaptan and to certain performance characteristics and limitations of present injection systems.Recent Bureau research succeeded in upgrading the overall safety and effectiveness of the stench system through the use of a superior stench odorant and the development of improved stench injection equipment (1).4 This research has also investigated a specialized computer simulation model capable of calculating the precise concentration of stench in any mine ventilation network branch at any time after stench release (2). An adaptation of the computer model now enables the analysis of stench system and ventilation system interactions under the influence of a mine fire."

What made this success possible was safety training done with programmed instruction materials-which, incidentally, US Borax also uses for vocational training.

Two mine-fire experiments were conducted in the National Institute for Occupational Safety and Health's (NIOSH) Safety Research Coal Mine (SRCM) at the Pittsburgh Research Laboratory (PRL) which demonstrated that smoke from diesel-fuel fires of 500 kW and 660 kW heat-release rates in a return airway can develop, without causing a complete air flow reversal, into a roof layer that can migrate upwind forming a counter flow to the primary airflow in a crosscut. Subsequently, smoke can penetrate into an intake airway and create a hazardous atmosphere in the intake airway upwind from the fire. Visibility conditions less than 13 m were created by the smoke in the intake airway downwind from the crosscut. Modeling of the event with a three-dimensional, time-dependent, computational fluid dynamics (CFD) program correctly represented the smoke movement.

Results of laboratory studies on the flotation of marmatite (zinc sulphide) with new reagents, which were developed to avoid the use of copper sulphate, are presented.Episulphides were chosen as the basis for the reagents because of their reported special chemical affinity for zinc and cadmium. Attempts were made to solubilize and introduce functional groups to episulphides with organic and inorganic acids, ammonia, phenol, aniline, diethylamine, and pyridine. As a class of reagents, the episulphide-amine combination was generally found effective in zinc sulphide flotation to a greater or lesser degree. Best results were obtained with ethylene sulphide-aniline combination. Although anilino-ethyl mercaptan is the reported reaction product of aniline and ethylene sulphide, it was concluded that the effective zinc sulphide flotation with ethylene sulphide-aniline combination is not entirely due to the formation of the mercaptan.The economics of the possible usage of ethylene sulphide-aniline reagent are discussed.

An implicit numerical method is presented for simulating the differential and algebraic relations governing one-dimensional three-phase flow in porous media. The method is based upon compositional representation of the hydrocarbon system. Variable physical properties and water-oil capillary forces are included in the formulation; however, the effect of gravity is ignored. The effects of changing composition and mass transfer are considered through the use of known phase behavior concepts and correlations. The differential and algebraic equations and the numeric approximations to these equations are presented, and the computing algorithm is discussed in detail. This method is used to simulate a single-phase, two hydrocarbon-component gas displacement laboratory experiment. The method is also applied in simulating a solution gas drive and a gas injection problem using a reservoir oil represented as a nine-component mixture. For comparative purposes, the latter two problems also are examined with a one-dimensional volumetric model. The results of the simulation of the laboratory experiment demonstrate the relevance of this method to problems wherein the effects of mass transfer significantly enter the displacement process. The comparison of the results of the volumetric and compositional methods applied to a reservoir system illustrates not only the advantages and benefits derived from examining the hydrocarbon system composition, but also the necessity of this approach in application to a general reservoir problem. INTRODUCTION The advent of large memory, high-speed digital computers enabled reservoir engineers and mathematicians to pool their knowledge in the development of sophisticated techniques for the prediction of oil and gas reservoir performance. These techniques have progressed from volumetric and trend analysis calculations to the development of "mathematical models", or reservoir simulators. These latter models, in general, utilize finite difference approximations to the rather complex partial differential equations that mathematically describe the physics and thermodynamics of fluid flow in porous media. Many forms of this "model" type are available today, and much of the recent literature has been devoted to either improving known methods or developing new techniques for use in these prediction methods. The more advanced techniques in use at this time consist of the solution of differential and algebraic relations that describe a reservoir geometry in one, two or three dimensions and that incorporate reservoir hydrocarbon and water fluid systems that obey specified physical laws, having properties that might easily be measured in the laboratory. This type of model utilizes formation volume factors and other volumetric data, expressed directly as functions of pressure, to represent the reservoir fluid system. Experience gained with this type of simulator, commonly known as a "beta-factor model", has indicated that the approach has many outstanding defects, most of them due to the assumption that those properties measured in the laboratory completely describe the behavior of the fluid system throughout the life of the project under study. The effects of varying hydrocarbon composition and mass transfer between the phases of this hydrocarbon system must, by definition, be ignored. These defects, which may often be ignored in

Dilution reduction is one of the areas that offer metal mines an opportunity to increase revenue and reduce costs, resulting in enhanced profits. Effective dilution control programmes require interaction on a multi-disciplinary level. This paper outlines recent initiatives undertaken at Konkola Mine to reduce dilution and increase revenue. The increase in revenue arising from reduced dilution and high factors between in situ and delivered to mill grades were quantified and amount to close to U$ 1.0 million per one per cent improvement in grade factor at a 75c/lb copper price. Sources of dilution from open and in?stope mining methods practised at the mine were identified and a multi-disciplinary dilution control forum set up to control these. Among the major achievements were the introduction of low dilution in-stope mining methods; the introduction of Gemcom geological and mining planning software to replace traditional pencil and paper methods, leading to better in situ estimation and production planning; enhanced control in open stopes through dilution awareness among mining production personnel; location of critical secondary development ends in correct positions; enhanced blast hole control; tracking of development waste by metal tracers and monthly tally; implementation of a cavity monitoring system to check over-breaks in open stopes; and setting up of a stope history database. The results from the cavity monitoring system showed that stopes with over-breaks of 5 metres or less could be controlled to break along the hangingwall contact.

"INTRODUCTION For the past six years the United States Bureau of Mines in cooperation with the University of Alabama, Tuscaloosa, Ala., the Missouri School of Mines and Metallurgy, Rolla, Mo., and certain operating companies of the Florida land-pebble phosphate district have conducted an investigation to determine the amenability of phosphatic sands to concentration. 4 This paper discusses briefly an investigation of the concentration of fine phosphatic sanads of the land-pebble district of Florida. The process used - namely, the tabling of material previously given a selective oiling treatment - is new in its application to the phosphate industry, although old in principle.The feed to the standard phospidate washery is a hydraulically wined mixture of phosphate pebbles, quartz sand, and clay in which the quartz sand grains are segregated in the fine sizes. Me standard method of concentrating (washing) consists of wet-screening. at about 20 mesh to recover the plus 20-mesh material. 5 Lately some of the washeries have introduced hydraulic classifiers for sizing of the screen undersize to recover the coarser spigot products, but even this additional treatment allows much of the fine phosphate to be wasted."

This paper introduces the technical characteristics, transformation scheme and production practice of the project in which Copper P-S Converter was replaced by Bottom Blowing Continuous converting for the first time, and introduces the technical achievements obtained by the project. The application practice proves that bottom blowing copper continuous converting technology can completely solve the problems of fugitive off-gas pollution in the ladle transportation, large amount of flue gas and fluctuation of flue gas caused by P-S converting. Comprehensive energy consumption and operating costs of producing blister copper, safety and environmental risks are greatly reduced, and economic, environmental and social benefits are significant. It is one of the preferred technologies for upgrade and to replace P-S converting technology. INTRODUCTION At present, more than 80% of copper production capacity is still using the P-S converting technology with a history of more than 100 years. During P-S converting process, melted copper matte needs to be transported by ladle, and it needs to rotate the furnace body several times, causing a lot of serious environmental and safety risks such as SO2 flue gas emission and low-altitude SO2 pollution. In addition, the high operating cost caused by low rate of air blowing, the short life of the refractory materials and high energy consumption during P-S converting become big problems. Other problems such as large amount of flue gas, low SO2 concentration, large fluctuation of flue gas volume and SO2 concentration, and discontinuous flue gas cause high cost of subsequent flue gas treatment are also needing to be solved. In order to solve these problems during the P-S converting, metallurgical engineer in various countries has spared a lot of efforts to develop continuous blowing process for decades.
