Incident investigation is of utmost importance in most high-risk industries and is often used to improve organisational safety. This study was undertaken to examine the content of past incident investigation reports to determine the effectiveness of the incident investigations. The study uses a semi-quantitative method to assess the effectiveness of incident investigations in the Ghanaian mining industry by evaluating the quality of past investigation reports. The assessment tool consists of five elements with several indicators and rating scales for assessing the quality of an investigation report as a measure of the effectiveness of the investigation. The method was applied to 304 investigation reports of three Ghanaian large-scale gold mines, and the results correlated with incidence rates of the mines to determine if any relationship existed. The results showed that the mines differ significantly in the quality of their investigation reports, suggesting differences in the effectiveness of their investigations. In addition, the incidence rates of the mines negatively correlated with some elements of the assessment tool. In general, the method was found useful and revealed areas where improvement is needed.

Once completed, the Montreal Réseau Express Métropolitain (REM) will be the fourth largest automated transportation system in the world. The REM represents construction costs of approximately 7.0 billion Canadian dollars. The project consists of 67 km of twin tracks over four branches connected to downtown Montreal. The project includes 26 stations with 3 underground stations in downtown Montreal. One of the underground stations was built using the NATM method with thin permanent shotcrete initial and final liners separated by a sprayed on waterproofing membrane. The 2 other underground stations were built with the cut and cover approach including one using permanent secant pile wall with permanent walers and struts as the support of excavation and the final perimeter wall of the station. The project also includes the rehabilitation and enlargement of the Mont Royal Tunnel. This 100-year-old double track tunnel is about 5 km long. The REM also consists of 3.6 km new TBM tunnel connecting downtown to the Montreal International Airport through saturated soft ground and karstic rock. By the time of RETC, the construction of REM underground structures will be completed. This paper presents the design and construction aspects of the underground structures of this mega project.

Slope monitoring has been an important issue in static terrestrial lidar for some time. Several solutions for monitoring the progress of a slide have been devised over the years. All of these solutions have required users to be in the field operating the lidar equipment. This paper presents a new system developed using the Optech ILRIS-HD or ILRIS-LR lidar and the scripting language of PolyWorks to produce an autonomous slope monitoring system. This system has proven invaluable in detecting and alerting mine workers to potential danger within their mine sites. It has already been deployed in two mine sites and is scheduled to be installed at a third.

INTRODUCTION The wet shotcreting system utilizing steelfiber microsilica shotcrete with remote controlled equipment (The Robocon System) was developed in Norway 7-8 years ago. Norway was the natural origin for this system due to several factors. Almost 30% of all underground hydro-power stations in the world are located here. These projects created a need for high productivity shotcreting rigs for the rock support work in the caverns and in the connecting tunnels. During the period of 1976-1980, more than 1000 miles of tunnels were completed in Norway. Secondly, the utilization of microsilica in concrete was also developed in Norway due to a concentration of the world's ferroalloy p ants there. Robocon has presently placed approximately 110,000 m (150,000 cu. yds.) of this superior shotcrete, and has consequently an extensive experience in the shotcreting method. During a relatively short period of time (1975-1980), Robocon had a change from hand-operated, dry-process shotcrete to remote-controlled, wet-process shotcreting, because of improved productivity, reduced rebound, safer and cleaner working conditions. The introduction of microsilica and water-reducing admixture made it possible to produce wet-process shotcrete of high strength.

Doyon mine, Quebec, Canada evaluates the possibility of desulphurizing their tailings to produce material to be used in as cover over the acid generating tailings disposal site. Laboratory tests were conducted and showed that a single-layer cover made of desulphurized tailings placed over reactive tailings can prevent the generation of acid mine drainage. Modelling with the software Vadose/W was undertaken to optimize the design parameters, namely cover thickness, water table position, and residual sulphide content of desulphurized tailings following laboratory work. It was found that the optimal configuration includes a 1 m desulphurized tailings cover containing 0.8%S, with the water table kept at the interface between the reactive tailings and the cover. To maintain the cover performance to reduce oxygen flux towards acid-generating tailings even in the event of a long dry period, a sand layer is suggested to protect the cover from excessive evaporation. It is possible to reach the environmental objective chosen for Doyon mine (10 mol/m2/year) with the protective sand layer, even in extremely dry conditions.

In the past several decades, many major equipment manufacturers have ventured into developing new technology for underground hard rock excavation. This includes variety of configurations for full and partial face mobile excavation units. Mining industry prefers machines that can excavate noncircular, flat invert profiles and be able to make tight turns in hard rock. The need to use disc cutters for cutting hard rock has impeded the development of such machines due to the need for gripper system or large mass of a machine to support the force requirements of the disc cutters. Most of the prior attempts have been partially successful and the operations have faced some practical limits and challenges. Among the units that have been successfully used, one can name mobile miner, which was used for rock excavation in some projects. A redesigned mobile miner, called the “Remote Vein Miner,” or RVM, has been developed by Epiroc for underground hard rock mining in Hecla’s Lucky Friday Mine. This equipment potentially opens numerous opportunities for innovation in civil tunneling projects, where the operational flexibility and accessibility of a TBM’s limited tunnel path, design of the profile, and cost are limiting factors. This paper explores the application of the RVM in tunneling projects, its advantages and possible areas of innovation in tunnel planning and design.

Libby, Montana is the site of a large vermiculite deposit that was mined between 1920 and 1990 to extract vermiculite for commercial applications such as insulation, gardening products, and construction materials. The Libby vermiculite deposit also contains amphibole minerals including tremolite, actinolite, richterite, and winchite. Historically, Libby mine workers experienced high exposures to amphibole structures, and, as a group, have experienced the health consequences of those occupational exposures. It has been suggested that Libby residents also have been and continue to be exposed to amphibole structures released during the vermiculite mining operations and therefore are at increased risk for disease. The Agency for Toxic Substance and Disease Registry (ATSDR) conducted two epidemiological-type studies of residents living in Libby and the surrounding areas to assess these risks. The Environmental Protection Agency (EPA) collected and analyzed exposure data in Libby and used those data to project risks of asbestos-associated disease for Libby residents. The EPA has placed the Libby Asbestos Site, which includes the mine and the town of Libby, on its National Priority List of hazardous waste sites in need of clean up. This article presents a review of the exposure studies conducted in Libby and an analysis of health risks based on the data collected in those studies. Libby mine workers have experienced elevated levels of asbestos-associated disease as a consequence of their occupational exposures to amphibole structures. Libby residents? exposures typically are substantially lower than mine workers? historical exposures, and the health risk projections for residents are, accordingly, substantially lower. © 2007 Published by Elsevier Inc. "The paper describes in general the objectives, goals as well as theoretical background of "METSI"-project proposal submitted to German BMBF (Federal Ministry of Education and Research). The METSI project is a starting point for technology and know-how transfer of German water management and water treatment technologies to South African Mining industry. "METSI" is Setswana, one of the eleven official languages in South Africa, and means "water". It stands also for "Management System for the Implementation of Environmentally Sound Water Supply Technologies in South African Mining Areas with Involvement of all Relevant Stakeholders". The project focuses on development, design and implementation of mathematical models that consolidate and evaluate existing data and generate substantiated projections of "water" amount and quality. METSI covers a wide range of scientific, technical, social as well as economic questions related to "water in mining"."

Kinetic models are the most important tool for predicting and evaluating the performance of flotation circuits. Gilsonite is a natural fossil resource similar to an oil asphalt, high in asphaltenes. Here, in order to determine the kinetic order and flotation rate of a gilsonite sample, flotation experiments were carried out in both rougher and cleaner stages. Experiments were conducted using the combinations of oil–MIBC and gas oil–pine oil, with one test without collector and frother. Five kinetic models were applied to the data obtained from the flotation tests using MATLAB software. Statistical analysis showed that the results of the experiment with oil–MIBC were highly in compliance with all models. Kinetic constants (k) were calculated as 0.1548 (s−1 ) and 0.0450 (s−1 ) for rougher and cleaner stages, respectively. Rougher and cleaner tests without collector and frother also matched all models well (R2 > 0.98), with k values of 0.2163 (s−1 ) and 0.284 (s−1 ), respectively. The relationship between flotation rate constant, maximum combustible recovery, and particle size showed that the maximum flotation combustible recovery and flotation rate were obtained in the size range of −250 + 106 μm in the rougher and cleaner stages. The combustible recovery and flotation rate were higher in the rougher flotation process than in the cleaner stage.

Weathered mine waste consists of oxidized primary minerals and chemically unstable secondary phases that can be sources of readily soluble metals and acid rock drainage. Elevated concentrations of metals such as Cd, Cu, Fe, Mn, Ni, Pb, and Zn are observed in deionized water-based leachate solutions derived from complex sedex and Cu-Pb-Zn mine wastes. Leachate (USGS FLT) from the Elizabeth mine, a massive sulfide deposit, has a pH of 3.4 and high concentrations of Al (16700 ug/L), Cu (440 ug/L), and Zn (8620 ug/L). Leachate from the sedex Faro mine has a pH of 3.5 and high concentrations of Al (2040 ug/L), Cu (1930 ug/L), Pb (2080 ug/L), and Zn (52900 ug/L). In contrast, higher-pH leachates produced from tailings of polymetallic vein deposits have order of magnitude lower metal concentrations. These data indicate that highly soluble secondary mineral phases exist at the surface of waste material where the samples were collected. Sulfide minerals from all sites exhibit differential degrees of weathering, from dissolution etched grain rims, to rinds of secondary minerals, to skeletal remnants. These microscale mineral-dissolution textures enhance weathering and metal leachability of waste material. Besides the formation of secondary minerals, sulfide grains from dried tailings samples may be coated by amorphous Fe-Al-Si minerals that also adsorb metals such as Cu, Ni, and Zn.

Until a few years ago tunnel construction was based exclusively on experience. In recent years, however, numerical models have increasingly become an integrated part of tunnel design [2]. These models permit good evaluation of the tunnel's support behavior even before it is designed. In particular, numerical methods are highly suited for the development of new construction methods. The prerequisite for this is the application of high-performance pre- and postprocessors, permitting the design engineer to come to a quick interpretation of the analysis results. In future, 3D analysis will take on increasing significance, for which reason this paper also deals with this type of models.

Economically favored Cr and Cu leaching out process from spent power plant fly ash (PPFA) had been investigated. PPFA, bearing of C, S, V, Ni, Cu and Cr, has been widely used as an assistant fuel in cement industry. Recent environmental regulation in S. Korea prohibits using PPFA as a fuel. Many researchers have also studied on extraction V and Ni from PPFA. Due to low concentration of V and Ni, 2% and 1%, respectively, economical and environmental friendly process could not be found. In this study, Cu and Cr have been removed in an economical way and which satisfied with KS Extraction Test. The rest unburned carbon, V2O5 and NiO2 in residue were found to be applicable for indoor and outdoor colored bricks or deodorant panel. Various parameters such as pH, lixiviates and time on leaching behavior were investigated in order to leach Cr and Cu remaining C, V and Ni. In the extractive industry the term ‘continuous improvement’ (CI) usually means procurement of the latest state of the art equipment. This is generally perceived as the only way to add further value to the supply chain, with the driving philosophy of increased tonnage to dilute operational costs. However, not all market places can facilitate this approach. RJB lives in a fiercely competitive market and has targeted the standard efficiency drivers of rationalisation and stock management with a vengeance; and also operate CMMS (Computer Maintenance Management System), RCM (Routine Condition Monitoring)and CBM (Condition Based Maintenance) methodologies. Ultimately though, these systems are only as good as the people who operate and manage them and to add further value without procurement, people must be re-introduced back into the operation. Through TPM (Total Productive Mining), that is what RJB have set out to do.

A new comprehensive insitu instrumentation technique named this RCC Method has been developed for rapid assessment and improvement of the structural stability of underground openings. When applied to tunnel construction, the degree of present and prospective safety of underground openings can be determined in quantitative terms within 10 to 20 minutes of microcreep observation. When applied to mining, roofs and floors can be stabilized by controlling the stress conditions. This stress control method has eliminated the need for intensive roof bolting and extensive floor renewal in all cases where it was applied. Use of this technique is illustrated by field examples from the Seikan Undersea Tunnel of Japan and deep potash mining in Canada. NEW INSTRUMENTATION TECHNIQUE: THE RCC METHOD Failure problems in underground excavation usually arise in weak non-elastic grounds where conventional rock mechanics theories of elasticity are not only useless but also misleading. Presently, discontinuity planes in the ground are being considered a factor in assessing these failure problems, and while this development is interesting academically, it is not of much use in actual excavation work, where it is nearly impossible to identify and quantize all existing planes of discontinuity. Even if such an extraordinary task could be accomplished at an underground excavation site, effective use of the data would be difficult and impractical for rapid excavation work, with the possible exception of rare cases of ideally elastic ground.

The Hemlo Gold District, located on the north shore of Lake Superior, was discovered in 1981 and will become one of the largest gold mining camps in Canada. Three major Canadian mining companies are presently developing large orebodies and are scheduled to commence production at various times through 1985 and 1986. This paper describes the types of shaft sinking equipment and technology being used by contractors in constructing the required shafts to depths below 1000 metres. The principal shaft design criteria, types of shaft sinking machinery and relevant manpower allocations are discussed.

Mining is big business in Ontario. The province is Canada?s leading mining region. It consistently ranks among the top 10 jurisdictions worldwide for exploration spending, reaching about C$1 billion in 2011. More than 26 percent of all exploration and appraisal dollars spent in Canada are invested in Ontario. Minerals produced include nickel, copper, platinum group elements (PGE), cobalt, zinc, diamonds, gold and silver. It was reported that there are more than 600 mining exploration projects in the province. And, recent discoveries of chromite in northern Ontario?s Ring of Fire will add to the province?s mineral production in the coming years.

Introduction Guar gum is a natural nonionic polysaccharide. Guaran, the functional polysaccharide in guar gum is a chain of (1?4)-linked ß-D-mannopyranose units with a-D-galactopyranose units connected to the mannose backbone through (1?6) glycosidic linkages. The poly-mannose chain is randomly substituted with galactose units at a mannose-to-galactose ratio of 1.8-1.0 [1]. Cheng et al. reported that the molecular weight of natural guar gum is 1.935 • 106 [2]. Robinson et al. found that the molecular weights of five grades of commercial guar gums ranged from 4.4 • 105 to 16.5 • 105 [3]. Guar gum is frequently used as a depressant of naturally hydrophobic gangue minerals (e.g. talc) in the flotation of PGM-bearing ores, and according to Harris et al. a wide variety of modified guar gums are available for this application [4]. The Canadian potash industry utilizes natural guar gum as a blinder of water-insoluble slimes in the flotation of potash (sylvinite) ores, a process that is carried out in a saturated KCl/NaCl brine. The role of the polysaccharide is to prevent a primary amine collector from adsorbing onto these unwanted fine minerals. Hydrophobic interactions between basal cleavage planes and the polymer appear to be involved in guar gum adsorption on talc [5,6]. Liu and Laskowski stressed the importance of metal impurities on mineral surfaces that can serve as the active sites for polysaccharide adsorption [7]. This postulate was consistent with the later observation that, in addition to hydrogen bonding, the adsorption of guar gum onto talc was strongly dependent on the surface concentration of magnesium sites [8,9]. In this case the adsorption process was visualized as metal-polysaccharide complexation facilitated by the cis-configuration of the hydroxyl groups of mannose monomers. Hydrogen bonding and chemical interactions with metal sites were also suggested by Rath and Subramanian in their studies on guar gum adsorption on biotite mica [10]. In a recent study, Wang et al. demonstrated that hydrogen bonding is the main driving force behind guar gum adsorption onto talc [11]. Their data also show that the adsorption density of guar gum on alumina is about five times lower than the adsorption density on talc. Pawlik and Laskowski reported that the amount of guar gum adsorbed on illite was independent of ionic strength over a wide range of salt concentrations [12]. The adsorption density of the polysaccharide on illite did not measurably change with ionic strength from that of distilled water to about 50%-saturated KCl/NaCl potash brine (~3N KCl/NaCl). However, guar gum adsorption on dolomite dramatically decreased over the same range of ionic strengths, and no reliable explanation of that phenomenon was proposed [12]. The water structure-breaking or structure-making capabilities of background ions are usually ignored when discussing adsorption mechanisms of polymers. The concept of “structure breakers” and “structure makers” was first introduced by Gurney, and Frank and Wen who postulated that each ion is surrounded by three distinct regions of water structure [13,14]. In the first layer, water molecules are tightly bound to the ion. The second region extends farther away from the ion and is referred to as the region of structure breaking. Only at larger distances, where the ionic field is weak, water molecules form the “normal” ice-like structure. It is also now recognized that the first layer can be further divided into the primary and secondary hydration shells. Small, strongly hydrated ions reinforce the “normal” structure of water and the region of structure breaking disappears. In contrast, large less-hydrated ions disturb the ice-like structure and generate an extensive region of structure breaking. Experimentally, these two effects can readily be distinguished through viscosity measurements. Increasing concentrations of structure-breaking ions result in a decrease of the viscosity of aqueous solutions. On the other hand, increasing amounts of structure-makers significantly reduce the viscosity of water. Alkali metal cations form a sequence known as the Hofmeister series. In many developing countries, asbestos is still used for friction products, insulation, and asbestoscement products. However, there is very limited modern data evaluating the airborne concentrations of asbestos fibers in operating asbestos plants. We have conducted an Industrial Hygiene survey, using state of the art PCM and TEM analysis, of a Bolivian asbestos cement plant that uses crocidolite during the production of roofing shingles and water tanks. The study is the first to use modern analytical measurements in an uncontrolled asbestos environment. Thus, this study offers insight into the historical airborne exposures at asbestos plants operating before modern dust control measures were introduced or airborne fiber levels regulated. The results suggest that fiber concentrations are far in excess of those for which current models would predict the onset of asbestos disease and mesothelioma. Significantly, the plant and community do not have an elevated incidence of asbestos disease or mesothelioma. Airborne fiber levels during comminution and fiber sizing ranged from 100 to as much as 1,000 f/cc. Fiber levels in the mixing, forming and finishing areas ranged from 0.5 to 9 f/cc. Based on the number of overloaded samples and estimates of their concentration, historical estimates of exposure may be underestimated by an order of magnitude. Also, an excellent correlation (1:1) between the PCM and TEM fiber concentrations (for fibers _0.25 um diameter) were obtained, raising questions about the conventional wisdom that the PCM systematically undercounts fibers. The results of this study further indicate that current models may overestimate the risk of asbestos disease and mesothelioma.

Over the last 25 years, through adversity and a growing level of risk mitigation, Australian Longwall mines have been using higher and higher capacity cable bolts for secondary support. A recent survey revealed 85% of Australian longwall cable bolts are a 70 tonne capacity pre-tensioned hollow post groutable system. Mines avoiding pumping cementitious grouts are sometimes using pre-tensioned resin encapsulated solid cable bolts of over 90 tonne capacity. Results from various industry and University research has led to manufacturer's fine tuning the construction of their cable bolts. A review of this research explains how smooth wire construction is now dominant. Aggressive shear has also led to some mines using 8 tonnes pre-tension instead of the previous common 20 tonnes pre-tension. Lower pre-tension acceptance is now leading to a greater use of threaded cable bolt end fittings, these are tensioned to 8 tonne using the drill motor torque. A recent development is the TEX cable bolt, a slim line threaded end fitting that fits with the drill hole like a standard rock bolt. This cable is being installed at the coal face as primary support off bolter-miners. The laboratory and in-situ testing of the TEX cable is presented. In a bid to improve gateroad development rates and ground support in the last several years, a 100 tonne tensile capacity Goliath cable system was developed. The latest development in resin anchored solid cables is a 110 tonne capacity cable bolt, with laboratory and in-situ testing results presented.
