This report summarizes the results of a research project entitled "Fatal Accident Data Base Development." This project is the last project under Contract No. S0110601. Volumes I and 1I, "Industrial Engineering Study of Hazards Associated with Underground Coat Mine Production," summarize the original industrial engineering project and the Fatal Accident Report Analysis project. These projects were completed and accepted by the U.S. Bureau of Mines on December 10, 1971. Both volumes of the report have been made available by the Bureau to the public through the U. S. Department of Commerce technical information service. Another project under the same con tract, entitled "Equipment Operations Safety Study, ' was completed in January 1972 and the report was accepted by the U. S. B. M. at that time. The analysis in this Natal Accident. Data Lose Development report is closely related to analysis in both of the earlier reports. The original project involved an industrial engineering and safety of production operations for a representative sample of underground bituminous coal mines. The contactor was required to make recommendations 'in work practices, mining methods, equipment, and safety practices which can effectively reduce the fatality rate from rib and roof falls.' (Longwall mining methods were excluded from the study.) Time study and work element analyses were performed in over 50 mines as part of the field data gathering effort. Another requirement of the study was to develop sets of multiflow. Process charts, which are diagrammatic illustrations of the time-related interaction of mining, cycle job elements.

The U.S. Bureau of Mines Information and Analysis Directorate collects worldwide data on virtually every commercially important nonfuel mineral commodity. These data form the base for tracking and assessing the health of the minerals sector of the U.S. economy. The Bureau's data collection activity was instituted by the 47th Congress in an appropriations act of August 7, 1882 (22 Stat. 329), which placed the collection of mineral statistics on an annual basis. The most recent authority for the U.S. Bureau of Mines survey activity is the National Materials and Minerals Policy, Research and Development Act of 1980 (Public Law 96-479, 96th Congress). This act strengthens protection for proprietary data provided to the U.S. Department of the Interior by persons or firms engaged in any phase of mineral or mineral-material production or consumption.

These volumes report on the study initiated by the U. S. Bureau of Mines to develop and apply a methodology to quantitatively assess the probability and potential benefits of technological innovation over the next twenty-five years. The potential was recognized for new technology to solve copper industry problems in the United States. Objectives of the study were to: (1) Develop a methodology to obtain the opinions of mineral experts about future events -their likelihood of occurrence, and, conditional on occurrence, when and to what extent they are likely to occur; (2) Identify, select, and describe potential technological innovations which would have significant impact on copper production; (3) Implement the methodology and obtain the opinions of the mineral experts regarding selected copper technologies; and (4) Analyze the expected future benefits of successful technological innovations :n the copper industry. Although individual technologies are continuously examined throughout the copper 'community', this study is the first known comprehensive forecast of future copper technology. This study is also believed to be unique 1n that it represents the consensus opinion of over sixty expert copper technologists on the expected use of innovative technology in the industry over the next 25 years. Three electrostatic field sensing proximity warning devices were tested using full-scale cranes and power line systems, a scaled model facility, environmental testing facilities, and theoretical/numerical analysis. The design objective of each device is to assist in safe crane operation near overhead power lines. Several design deficiencies (varying from device to device) were observed, including inadequate overlap in sensitivity ranges, utilization of vacuum tubes and electro-mechanical parts, temperature sensitivity and inaudible alarms. In addition, the testing showed major device limitations including sensitivity to boom orientations and length, polarization dependent responses, and unreliable operation in the presence of multiple power line circuits. The multiple power line circuit problem is intrinsic to electrostatic sensing devices. The remaining deficiencies and limitations are correctable within a state-of-the-art design. This investigation is not intended to evaluate the safety aspects or policy of utilizing proximity warning devices. Rather, the purpose is to determine the performance characteristics of such devices in relation to the criteria specified in proposed MSHA regulation 77.805-4 (i. e., the devices must alarm at a distance of 10 feet from the power line).

This volume was prepared in support of an evaluation of the Advancing Oil Shale Mining Technology Program performed by The Aerospace Corporation under a Bureau of Mines contract. The evaluation is described in part within a companion volume entitled Benefit-Cost Analysis. The economic mine models outlined herein provide a quantitative basis for the assessment of program benefits. Reference is made to estimated oil shale mine costs to delineate the beneficial impacts that can be expected to accrue for the future implementation of program developed technology. Estimated mine cost data are provided for surface, underground, and modified in situ oil shale mining operations.

The U.S. economy made significant progress in 1984. The current-dollar gross national product (GNP), the market value of the Nation's output of goods and services, increased almost 11 percent over that of 1983 to slightly more than $3.66 trillion (table 1), and the real gross national product increased almost 7 percent.2/ Such rapid growth in real GNP has not occurred since the early 1950's. Real final sales increased only 5 percent during 1984, and real business inventories increased $24.8 billion ($57.9 billion in current dollars). Total employment of 106.7 million was an all-time record, and the unemployment rate declined from 9.5 percent in 1983 to 7.4 percent in 1984. During 1984 the U.S. dollar showed great strength on international markets, contributing to large current-dollar negative net exports of goods and services (-$62.5 billion) and even larger negative net exports of merchandise (-$107.6 billion). The strong dollar and the substantial increase in net imports of goods and services helped reduce U.S. inflationary pressures and provided a major source of loanable funds for financing domestic private investment.

The complex organic chemicals in coal differ from those in petroleum mainly by the ratio of carbon atoms to hydrogen atoms; this ratio is about 2 for coal and 1 for petroleum. By changing the ratio, coal can be converted into compounds similar to those found in petroleum. In. very simplified form, omitting here the special conditions required, the paths by which this can be done are: [ ] Part or all of the coal is first gasified by reaction with steam and oxygen, either in a gasifier or while the coal is still in place underground. The resulting gas is mainly hydrogen and carbon monoxide, with impurities that .must be removed before further processing by one of the following routes.

In connection with a Bureau of Mines research program on utilization of Cuban lateritic ores, a comprehensive review was made of the chemical literature on nickel and cobalt. Initially, the review had as its objective the selection, from the voluminous literature dealing with the chemistry of these two metals, of only those references to reactions that were especially pertinent, or at least closely related, to the field of extractive metallurgy. Later, the coverage was extended to include references to the techniques and plant applications of extractive metallurgical procedures for the recovery of nickel and cobalt. In this report, the period from January 1929 to July 1955 has been covered, as it seemed logical that recent literature should be of greatest value. This cutoff date was established in order that the bibiliography could be issued at an early date, inasmuch as currently much work is in progress on the lateritic ores. Later, a supplement will be prepared extending the coverage to about 1900.

This report presents research results obtained in the investigation of deviation control of boreholes in the rotary drilling process. The approach taken in this research is the passive method of placing stabilizers in the bottom-hole assembly to control deviation. Extensive use was made of the drill string mechanics theories of Lubinski and Woods. Analog, digital and hybrid solutions to variety of boundary value problems were investigated in order to determine the best models for bottom-hole assemblies. Field drilling data was analyzed and the results obtained from various models were compared to the actual drilling results. Various accomplishments of this work have been published in open energy industry literature, with more publications to follow.

The value of U.S. mineral raw material output reached a record $63.1 billion in 1975. This high exceeded the 1974 value of $55.2 billion by 14 percent. Among the three major mineral categories -fuels, metals, and nonmetals -only fuels showed an increase in value. Both the metal and nonmetal categories showed declines in value. The significance of this nearly $63 billion mineral output can be noted in the fact that it is the basis for the subsequent production of over $270 billion a year in energy and processed materials. Of the 78 individual mineral commodities included in these 1975 totals, only 17 registered gains in output, while 45 registered value increases, reflecting higher prices.

We describe inconsistencies in disease and illness reporting in U.S. mining, identify under-reporting of disease and illness in U.S. mining, and summarize selected disease and illness in U.S. mining from 1983 through 2001. Methods: We summarized information on mining-related disease and illness data for the years 1983-2001 from the Mining Safety and Health Administration database (MSHA). Results: Discrepancies exist in types of information collected by the Centers for Disease and Control, the National Institute for Occupational Safety and Health, and the Mining Safety and Health Admin¬istration database. Several factors, including a worker's fear of losing his or her job, health insurance, or other job-related benefits contribute to under-reporting of disease and illness information in the US mining industry. Conclusions: Since 1997, both number of workers employed in mining and disease and illness rates have decreased; however, the highest disease and illness rates in mining continue to be coal worker's pneumoconiosis and hearing loss.

To help reduce the Nation's dependence on imported chromite, the Bureau of Mines is conducting research on the use of dolomites as an alternate material. Dolomite is a plentiful domestic resource and offers certain advantages as a refractory raw material. A review of the literature has indicated that there are many sources of high-purity dolomite in this country and that European nations use a greater proportion of dolomite refractories, primarily in steelmaking, than the United States, The Bureau of Mines characterized 14 domestic dolomites as to chemistry, density, mineralogy, microstructure, and thermal behavior, to develop baseline data on their suitability as refractory raw materials.

Barite, the natural sulfate of barium (BaSO4), is the most important barium mineral. Normally it is white (opaque to translucent), but impurities sometimes render it yellow, gray, blue, brown, red, or black. Brown or red stains are commonly caused by iron oxide. Barite is unusually heavy (specific gravity, about 4.5); in fact, it is one of the heaviest of the nonmetallic minerals. It is brittle and fractures unevenly. The hardness of barite ranges from 2.5 to 3 but is usually about 3. It can be scratched by a copper coin. Pure barite contains 65.7 percent BaO and A.3 percent S03. Much of the crude barite ore mined, however, contains impurities; thus it has a lower content of barite and consequently less barium oxide. Commercial analyses show its total BaSO4 content. Barium also occurs in nature as the mineral witherite (BaCO3), which, however, is not common in the United States, and commercial deposits have been found only in California. Witherite is a yellowish-white, light-gray, or Kite mineral with a specific gravity of about 3, a hardness of 3 to 4, and an uneven to subconchoidal fracture.

The monetary metals , gold and silver , have recently been the object of such general interest that the United States Bureau of Mines has received many inquiries regarding them . Though requests for technologic information have been numerous , they have been exceeded in number by those concerned with economic aspects of these metals . The nature of the latter communications indicates that the public is not generally aware that various Bureau publications are available which deal with the economic situation of gold and silver . For this reason , it is deemed advisable to present a brief résumé of the papers which are obtainable from the Bureau or the Superintendent of Documents , together with the text of three articles recently prepared by the Director , reprints of which are not otherwise available . The Bureau of Mines compiles figures showing mine production of gold and silver , by districts and States , and cooperates with the Bureau of the Mint in determining the total annual output in the United States as a whole . 3 It recently established an annual canvass to determine the consumption of silver in the arts and industries of the United States , and published an economic paper covering such use in 1928 and 1929 , followed by an information circular on silver consumption by industry during the 2 years subsequent . A 4-year record is thus provided for those interested in utilization of silver . The Bureau has also published summarizations of all available data upon world production since 1493 of gold and of silver . It has , furthermore , rendered available a compilation of production- data of silver mines of the world , and an analysis of it , showing the proportions of silver and other metals contained in all the ores from which silver is derived . " All of these papers are concerned solely with putting into readily usable form statistical data and other factual material ; they do not offer theories or express opinions upon controversial questions related to these metals . Three other recent papers concerned with gold and silver are presented below in order to put them in shape for distribution by the United States Bureau of Mines . This document presents a description of the results of a project aimed at producing a broad description of current mining training programs and an evaluation of their effectiveness with respect to reducing mine injuries. Aggregate training and injury data were used to evaluate the overall training effort at 300 mines as well as specific efforts in twelve categories of training course objectives. From such evaluations, recommendations were derived with particular emphasis being placed on identifying program characteristics that tend to be associated with effective training. Recommendations were also formulated with reference to role changes in industry and government interaction with respect to training programs, new or additional training programs, changes in the types of course materials and training methodologies, and recommendations concerning new areas for investigation.

A 9-month study of the uses of wire rope associated with U. S. underground coal mining is reported. Rope applications are broken down into hoisting and nonhoisting applications where "hoisting" refers to transport in vertical and inclined shafts. The former receives the greatest emphasis since it represents the most critical rope application from the stand-point of production and safety. The discussion of hoisting practice includes aspects related to the mine operators as well as those related to the equipment builders and wire rope manufacturers. U. S. state and Federal laws regulating hoisting and wire rope use are also presented and discussed. Canadian and other foreign hoisting practice and laws are included. The state of the art in electromagnetic nondestructive inspection of hoist ropes is presented. Capabilities and use of one Canadian electromagnetic device are discussed in great detail. Conclusions are drawn regarding the present state of wire rope and hoisting technology in U. S. coal mines. Recommendations are made for improving rope practice, in general, and hoisting, in particular, consistent with greater safety.

Helium has probably aroused more varied scientific curiosity than any other single element. Discovered by spectroscopic examination of the sun during an eclipse in 1868, helium is now known to exist not only in the sun and other stars but on earth occluded in minerals, accumulated with natural gases in pockets in the earth's crust, and in the atmosphere surrounding the earth. Astronomers study its presence in the various stars of the universe for clues to their origin and history. Geochemists and geologists study its occurrences in rocks, minerals, and meteorites to determine the age of the earth and the manner of its formation. Pathologists study its effect on living tissues in search for the ideal combination with oxygen for artificial breathing mixtures in medicine, diving operations, and space vehicles. Chemists study its physical properties and use it to trace the sequence of events in reactions in which it does not participate. Geneticists and bacteriologists use it for special studies. Physicists study its behavior when liquefied and the behavior of materials cooled to nearly absolute zero by its use. Nuclear physicists learn more about the universe through study of the nuclear reactions that produce it. Through study of helium and greater knowledge of its origin and properties, science continues to unlock some of nature's more closely guarded secrets. Unknown on earth before 1895, and not discovered as a minor constituent of natural gas until 1905, helium with its simple atomic structure--a nucleus with two electrons orbiting in a closed shell--is a unique element. It is next to the lightest gas, only hydrogen is lighter. It occupies position number 2 in the Periodic Table of the Elements and heads the list of so-called "rare" or ?noble? gases. Most of the naturally occurring helium has an atomic mass of 4.003874 (oxygen = 16). A trace (1.7 x 10-5 mol percent) is the light isotope helium-3 whose atomic mass is 3.0169800 Unstable isotopes of mass numbers 5 and 6 have been authenticated. Isotopes of mass numbers 7 and 8 have been reported, but their existence is less well established. All the heavier isotopes are radioactive and are the product of nuclear reactions.

Studies were made on the projectile impact sensitivity of explosives by subjecting explosive samples to the impact of metal projectiles fired from a 0.50- caliber gun . The projectiles , in the form of right cylinders , made plane surface to plane surface contact with the explosives . Pressures in the explosive were investigated with an expendable pressure transducer in some instances . The physical model of the projectile - explosive interaction describes , in principle , the duration of the peak shock pressure in terms of the projectile and explosive geometry and the steady- state penetration of the explosive . Of the explosives studied , liquid hydrogen- solid oxygen and liquid oxygen - solid or liquid hydrocarbon fuel were the most shock sensitive ( requiring an initiating shock of about 1.0 kb or 987 atmospheres pressure ) ; cast TNT was the least sensitive ( requiring an initiating shock of more than 110 kb pressure ) . Measurements of pressure in the explosive showed some moderate space resolution by the transducer and gave pressure distributions in agreement with expectations from the model of the shock processes . The appearance of the recovered projectiles and the calculated initiation shock pressures suggest that the solid explosives were initiated directly to high- order detonation , whereas the liquid explosives were probably initiated to low- order detonation-- with a subsequent , later transition to high- order detonation .

This is the fifth of a series of annual reports4/ of technologic investigations conducted by the Bureau of Mines on the occurrence, properties, mining, preparation, and uses of coal. The research of the Bureau is carefully coordinated with the requirements of the producing and consuming industries, with a view to conservation of resources of coal in mining and use, and conservation of the health and lives of workmen employed in these industries. Such coordination is facilitated by the wide range of specialization of the several groups on engaged in studies concerning coal. Here are reported correlated studies ranging from paleobotanic analysis of the formation of coal as it occurred some millions of years ago to the effect of mechanical methods of mining on present-day employment in mines. As this is a summary of the current work of the Bureau of Mines, so the Bureau likewise has provided a condensed review of the outstanding current work of others on research and technology of coal 5/.

Chemical alteration of the compressive strength of sandstone has been investigated by the U.S. Bureau of Mines (USBM). Successful development of this technology would offer an attractive alternative to the methods now used for stress control in mines. Sandstone cores were stressed to failure under uniaxial compression at two different strain rates. Specimens saturated with either distilled or tap water showed an average 14% reduction in stress at failure compared with those dried in vacuum. Samples saturated with dilute solutions of aluminum chloride, hydrochloric acid, and polyethylene oxide showed no statistically significant difference in failure stress compared with those saturated with water. By contrast, compressive strength of the cores was increased some 7% by saturation with the nonpolar solvent carbon tetrachloride. No correlation was found between zeta potential and compressive strength.
