Risk Management has been applied relatively slowly in the mining industry. Except for the risk concept application in financial considerations, the application of Risk Management in safety management has been far from satisfactory. When applied it was treated in a global occupational safety and health sense. Regardless of how much the mining industry has progressed over the years it is still, in many respects, `state-of-the-art' business. The safety aspects of mining are well known and yet little understood by the public. Regardless of what has been achieved so far and how much effort has been put into it, mining is regarded as an unsafe and somehow rude business. Safety records of the mining industry vary from State-to-State. Latest mining related disasters in PNG and Queensland and some other fatal accidents support this public perception. They purport a need for change in managing safety in the mining industry. The latest mining fatalities have raised the issue: What must be done to improve safety standards? Duty of care, risk management and system engineering concepts can provide some answers. Risk Management principles which have been advocated for some time provide many answers and explanations and can make safety management more constructive and workable. What has been missing so far is that this concept has neither been applied nor has been modified for its practical application and purpose on a larger scale. A risk management approach to the management of safety and health in the mining industry is addressed, herein. This approach does not attempt to provide the solution. It does, however, provide a basic understanding of what Risk and Risk Management are all about. Rational management of risk is addressed in the life cycle and system contexts. A system of continuous improvement is applied. There is always a chain of events, outcome, exposure, consequence(s), and consequence(s) value(s). It means that development of an event takes place in time and space. Dynamic circumstances have been created in or within the system. It is a decision-making process designed to provide a methodology which is directed not toward a mathematical model but toward easy use and application in dynamic working environment, by decision-makers who can be anybody from either a risk taker or a risk creator. The emphasis is placed not only on arriving at the best practical, possible and fair decision but also on structured and systematic acquisition of knowledge about the system, its hazards and associated risks, hence increasing the decision maker's ability to manage the decision-making process related to the management of risk.

The Wongawilli system of mining has been in use for over 30 years and has enabled high production rates and acceptable extraction ratios to be achieved in seams less than 4 m in thickness. A modification of the Wongawilli system has been developed to permit the use of a continuous miner and mobile equipment to extract a coal seam 6 m thick. Following physical and mathematical model studies the new mining method is now undergoing field trials at Collinsville, Queensland. Progress of the field trials to date has achieved 99% extraction of the upper slice by remote control operation of the continuous miner and established the feasibility of installing resin grouted wooden dowels in the coal floor to reinforce the coal septum. The method is explained in the text.

In-pit tailings storage can provide many advantages when compared to typical above-ground tailings storage facilities (TSFs). As regulations become more restrictive and existing mines expand into new pits, the motivation and opportunities for in-pit tailings disposal is increasing.The Langer Heinrich uranium orebody follows a palaeo-river channel and is mined as an open pit operation. The mine pits intersect two main aquifer systems, one at the surface and one at the base of the orebody/pits. The tailings storage methodology is full in-pit disposal in multiple TSFs for the life-of-mine. Continuity of the initial hydrogeological conditions is incorporated in the design by the use of the pervious surround concept and the reinstatement of the lower aquifer (palaeochannel aquifer) at the base of the pit. Containment embankments are required to facilitate the staging of the TSFs development and are constructed using waste rock liberated during the mining of the pits. All of the TSFs are equipped with decant structures and an underdrainage system is included to improve water recovery, enhance consolidation and increase tailings density.This paper presents the design of a unique in-pit tailings storage solution for the Langer Heinrich mine in Namibia owned by Paladin Energy Ltd.CITATION:Hore, C and Luppnow, D, 2015. Risk exposure in tunnel projects is extremely high due to various factors such as variability of subsurface conditions, selection of inappropriate design and tunnelling methods, improper risk assessment on adjacent surface and subsurface structures and utilities, and inadequate contractual practices. The risks in tunnelling must be borne by the contractor, designer and the owner of a tunnelling project. There have been a number of improvements in the last ten to 15 years in the manner in which tunnel projects are developed and implemented in terms of their design, construction, financing and operations. One significant improvement has been the manner in which underground conditions and their risks are being managed. Owners are less willing to transfer the full responsibility to the contractor for managing the underground conditions, although this is a familiar manner in which underground major urban projects have been previously contracted and managed. The owners of tunnel projects in Singapore have advanced the risk management process for their projects through geotechnical interpretative baseline reports (GIBRs), geotechnical data reports (GDRs) and project specifications (PSs). This paper discusses industry practice and its development as well as the application during the preparation and use in Singapore on the Deep Tunnel Sewerage System (DTSS) and the Marina Coastal Expressway (MCE) project in Singapore. The DTSS trunk sewer is 48 km and ranges from 3.3 m to 6 m finished diameter up to 50 m below the ground. The underground conditions ranged from Old Alluvium, strengths of about 1 MPa to competent Bukit Tima granite, with strengths of over 230 MPa. The total route length of the MCE is about 5 km. About 3.6 km will be constructed underground, consisting of dual five-lane carriageways. The underground construction will be cut-and-cover, and 2.6 km will be constructed about 13 m underground, and 1 km of tunnel will range from 13 m to 24 m as the tunnel passes under a 420 m wide reach of Marina Bay, whose crossing will be constructed as a two-phase cofferdam.

Every day, around the world, thousands of tons of ore are loaded onto ships completing the miningproduction process and starting a multimillion-dollar economic transactions process. These transactions are based on the supply of tons of a commodity with certain specifications of assays of the main element (or physical properties) and concentration of contaminants, which are finally verified and evaluated by buyers, and also are considered ‘as true’ during Reconciliation process. On Sample Stations, this ‘true’ is based on a few kilograms of sample, often representing thousands of tons, that are sent to laboratories for further sample preparation and chemical analysis, where finally assay information is obtained from a few grams of pulverised sample, and are used for further production, planning, operational and financial-marketing decisions. Different International Standards (depending on the commodity) are used to establish the methodology to be followed, to set-up and obtain samples from the Sample Stations, and the specifications are visually inspected to give an estimation on the performance of the sampling point. This paper seeks to highlight with examples, the importance of the implementation of a continuously monitored Quality Program (QAQC and QM) on Sample Stations, focused on the parameters that directly affect the quality and representativeness of the samples. By this implementation, a proactive approach and better-informed decisions can be taken for mining companies to: a) quantify/improve production and reconciliation results, and b) to supply the right specifications of shipped ore, increasing the value of the Resources. Finally, laboratory results are normally used as an indication of the quality of the information applied for the economic transaction, but it should be considered, clarified and highlighted that the laboratory will only process and analyse a sample obtained from a Sample Station, and potential problems on bias or precision may occur in stages prior to the laboratory reception. Hence the importance of the implementation of a quality assurance program that protects multi-million dollar decisions.

At Newmont’s Jundee Operation, exploration and resource geologists collaborated to produce an open pit resource model. Using a variety of modelling and estimation techniques but no new data, an uneconomic underground target was converted into a potentially economic open pit evaluation. During an industry downturn, it is more important than ever to efficiently share expertise, ideas and resources to problem solve and produce meaningful project outcomes. The two departments brought together different methods and ideas, and the process showed that certain methods worked better than others in different situations, thus the appropriate method was applied to each part of the evaluation. The results were used to create new procedures and improved lithology modelling and resource model estimations. Adaptability of the geology departments and collaboration such as this are key to ongoing improvements and future mine life at Jundee.CITATION:Dudgeon, K, Paradis, J and Topping, M, 2014. The iron and steel industry is experiencing an increase in the use of iron and steel commodities within societies, decarbonisation pressures, depletion of high iron ore grades and high energy consumption. This necessitates scrap metal recycling, low-grade ore utilisation using the induction or electric arc furnace to achieve the global target of net zero CO2 emission. The available highquality scrap metal cannot sustain the steel industry but the world has unused hematite-goethite ores. It is the aim of this research that this resource be utilised using the induction furnace to sustain the steel industry in an environmentally friendly manner. Indurated pellets prepared from the hematite-goethite ore, 5 wt per cent limestone and 5 wt per cent coal addition were smelted in the induction furnace at 1350 ± 50°C with 25 mins holding time. The effects of basicity and carbon content were also investigated to optimise the smelting parameters. The results showed that the careful control of the basicity, and iron-to-carbon ratio maximised total metal recovery. The optimal basicity ratio was determined to be 1.3 and a carbon to iron ratio of 1.4 achieved a total metal recovery of 73 per cent. The metal recovery was closely related to the estimated slag melting temperature. The basicity ratio of 1.3 achieved the lowest slag melting temperature estimated to be around 1380°C based on the CaO-SiO2-Al2O3 phase diagram. The recovered metal had a comparable chemical composition and microstructure to that products from an ITmk3 furnace.

Mining is experiencing a difficult period of low profitability and declining productivity. To achieve sustainability, mining companies are challenged with improving output, quality and safety while decreasing costs. At the same time, organisations are having to manage a workforce in which individuals no longer accept a traditional style of management, employees prefer more flexible employment, and diversity within teams is becoming the expected norm.Increasing organisational diversity has been shown to have a positive impact on performance and therefore sustainability. Gender diversity and equality between working men and women is the area most studied in this regard. Executives are becoming more aware of the correlation between gender diversity and performance with some larger organisations having dedicated resources to increase the number and status of women in their organisations. Despite this, limited improvement in gender equality is apparent globally over the last decade with some industries, including mining, going backwards. So why is this the case and what other strategies are available?In 2015, the United Nations Women National Committee Australia (UN Women NCA) in conjunction with Autopia released a white paper discussing the lack of visible change in gender equality despite the widespread implementation of merit-based personnel management strategies. Using research from around the world and thoughts from influential government, education and business leaders, the paper ‘Rethinking Merit’ debunks the myth of the meritocracy. It provides a number of recommendations for organisations committed to improving their performance and sustainability through diversity, including a variety of real-life company examples, instead of relying solely on merit-based decision-making.This paper takes the recommendations of ‘Rethinking Merit’ and applies them to the mining industry. It shows the relevance that rethinking merit has for the industry and why applying the recommendations can result in an increase in diversity, therefore enabling greater performance and sustainability of mining companies. Hydraulic transport of coal has been used for many years on the West Coast of New Zealand to extract small blocks of steeply dipping coal. Recent development at Strongman 2 Mine has seen the use of high pressure monitors to cut coal and a corresponding increase in the scale of production.This paper provides a brief introduction to hydraulic mining, describes the present system used at Strongman 2 and discusses future potential development.

In April 2000 Mount Isa Mines installed hoist ropes of a new design on the double-drum winder at their P49 shaft. The ropes, CASAR Duroplast, were about three times as expensive as the previously used triangular strand ropes. The P49 shaft is situated at Mount Isa MinesÆ George Fisher Mine, located some 30 km north of Mt Isa. This shaft used the tried and proven 6 x 33 triangular strand style ropes. The rope life attained by these ropes at the time was acceptable due to many years of satisfactory performance. The following study indicates the improvement of service life using CASAR Duroplast rope in P 49 shaft, which depends on shaft and winder parameters of that particular shaft and cannot necessarily be generalised to other shafts.

All industrial water users are required to hold Water Rights, which are in effect licences to take, use, dam, divert or discharge water. Water Rights are obtained through a legally defined process and any potentially affected party has the per to object to their granting. A special tribunal hears the evidence for and against an application, and issues its decision, which is reviewable in a higher court on Appeal. Rights, if granted, are issued subject to conditions and, in the case of mining discharges, limits would be set on flaw rate, suspended solids concentrations, turbidity, pH and the concentrations of any potential toxicants. This paper presents a case study of an integrated, catchment wide water management plan for Lake Waahi, that was prepared by the author, and others while the author was in the employ of CoalCorp, and its predecessor, State Coal Mines. At Cliffs Natural Resources Ltd (CNR) crushing and shipping sample stations, precisions are estimated for the sampling, preparation and measurement stages of iron ore sampling in accordance with ISO 3085 (2002). In this Standard, gross duplicate samples are prepared at each of the three stages, yielding eight measurement assays. Appropriate calculations of the differences between assay pairs are then made to estimate precision of each stage of sampling. The International Standard for checking precision in iron ore sampling, ISO 3085 (2002) prescribes a method for identifying outliers that is ambiguous and possibly inappropriate. The procedure also makes an unnecessary implicit assumption, that the differences between assay pairs are normally distributed. The Anderson-Darling statistic is used to demonstrate that in some cases the assay pair differences have distributions significantly different from normal. When applied at CNR as specified, the Standard leads to precision estimates well below the expected values, thus overestimating the sampling method's capability and limiting the opportunity for real process improvement. This paper suggests an improved method of estimating sampling, preparation and measurement precision. A bootstrap procedure is used to estimate confidence limits for the calculated precision estimates. The proposed method would be suitable for testing blasthole precision. The discussion is supported by extensive simulation modelling of realistic data.

Gold and copper mineralization in the Parkes area is hosted by a meridional sequence of Late Ordovician to Early Silurian igneous and sedimentary rocks on the central northern edge of the exposed Lachlan Fold Belt. The mineralized area straddles two zones separated in part by the Parkes Thrust and exhibiting different styles of deformation and mineralization (Figure 1). The western zone is characterized by gently folded rocks, with shallow dips and weak to non-existent axial plane cleavage. Faulting is restricted to directions oblique to bedding. Numerous occurrences of minor copper mineralization are known, but of recent national importance is the discovery of the Coradgery- Goonumbla porphyry copper-gold deposits by Geopeko Ltd. A resource in excess of 250 million tonnes has been delineated (Jones, 1985), and this represents the first porphyry deposit of sufficient grade and tonnage to be potentially mineable in New South Wales. The eastern zone is much more strongly deformed. Folding is tight with strongly developed axial plane cleavage. Faulting is abundant both parallel and oblique to bedding. Quartz veining is also abundant, and a narrow belt of gold mineralization adjacent to the Parkes Thrust stretches from Forbes in the south, through Parkes and Peak Hill, to Tomingley in the north. Over 31 tonnes of gold have been produced from this gold belt, mostly before 1914, and now three deposits show good potential for redevelopment - Peak Hill, and the London-Victoria and Lachlan lines of lode.

Bulk explosives provide the energy to move a rock mass and reduce it to an optimal size (fragmentation). The intensity of energy needed depends on several factors including a customer’s geology and end objectives. An energy mapping tool has been developed to help customers to determine their bulk explosive needs based on these objectives.At one end of the energy map dealing with high-energy products, a new range of bulk explosives products for surface mining have been developed. With these products, it is possible to provide blast designs with up to nearly triple the relative bulk strength (RBS) of ANFO, and address primary focus areas for mining operations, including the reduction of drill and blast costs, improvements in production rates, better face advance through improved broken stock management, better drill productivity, improved milling efficiency and lower energy costs in surface mines.In this paper, field trial results from the first level of new high-energy products are presented. These products have an RBS 2.25 times that of ANFO. Industry variations of a smaller diameter (89 mm) pumped product for civil quarrying applications, and for large diameter (311 mm) hard-rock metalliferous mines are presented. For both cases, the run-of-mine fragmentation levels obtained with the new bulk explosives are contrasted with the fragmentation profile generated with the currently accepted product range applied in similar geological conditions. The new products show increased fragmentation across the measured size distribution, with reduction in P50 values of 20 and 37 per cent over the currently used product for the 311 mm and 89 mm diameter blastholes respectively.CITATION:Wilkinson, D, Rigby, G, Norgard, J and Thomson, S, 2015. ‘Every mine is a pilot’ is an attitude which encourages the mining paradox of optimistic mine plans and equipment under performance; propped up by an environment lacking accountability. The premise that ‘every mine is a pilot’, appears to underlie the development and operating approach to most mines. It is the hypothesis of this paper that many mining companies, development teams and their managers largely ignore what is done elsewhere. Anecdotally, most planners don’t know whether the selected rates are high or low (rates based on theoretical models tend to be high – certainly higher than what most companies can achieve). Mines then consistently operate the equipment at rates well below targets; often more than 20 per cent below. Further to this paradox, mine performance data demonstrates the average mine is currently operating their equipment at levels of output not much more than half the output of those in the top ten per cent. On the whole mines don’t know the potential of their equipment and therefore don’t put appropriate plans in place to do better. After all, when ‘every mine is a pilot’ there is little reason to look to one’s peers for an indication of potential. This paper will outline why every mine is not a pilot and how mines do value some knowledge, but not as much as they should. This paper proposes a two-pronged approach to address this issue in the area of equipment performance. This approach is to:1. engage in better forecasting of equipment rates and better subsequent performance through benchmarking against industry standards2. provide conditions for more accountability.These parameters are linked and proper implementation of them will address the losses which many shareholders and financiers have incurred when investing in companies with unrealistic mining plans.CITATION:Lumley, G I, 2016. Computer simulation is a well accepted tool for modelling the behaviour of large and complex operating systems. It is a particularly useful technique in complex operations where there are multiple interactions between the inputs and outputs of various unit operations. It can be utilised during the early stages of a project as part of the business case definition phase or the early engineering phases, or alternatively to help understand the various options available in an upgrade or expansion. With the design of a new processing plant to handle a different ore type at Lihir Gold Limited (Lihir), there was concern that the existing conveying system could separately handle the increased tonnages. A simulation model was constructed which allowed Lihir to examine the impact on the conveying system of the new process plant, as well as investigate the effect of different trucking options. As is often the case, once the simulation model was built, further understanding of the downstream bottlenecks was able to be investigated, including the sizing of critical equipment.

Standards in the public reporting of resources and reserves in Australia have dramatically improved since the implementation of the Australasian Code for Reporting of Mineral Resources and Ore Reserves (The JORC Code) in 1989. Whilst the JORC Code states clearly that Mineral Resources and Ore Reserves are imprecise calculations and should be treated as estimates, there is strong evidence that many people regard them as fact, including some accountants, directors, auditors and investors. Mineral Resources and Ore Reserves are key inputs in the valuation of a mining company and the determination of financial performance. Geological knowledge is the foundation upon which Mineral Resources, Ore Reserves and investment decisions are made, however this information carries with it substantial uncertainty. Case studies and company reports have established that errors derived from inadequate geological inputs have led to reduced profitability in operating mines and in extreme cases the premature closure of mine operations. Whilst there are many causes of mine closure and reduced profitability, including commodity prices and exchange rates, the cumulative effect of all geological uncertainty contributes significantly to the erosion of the credibility of mining within the investment community. There are a number of methods used to characterise and model uncertainty in Mineral Resource and Ore Reserve estimation. Empirical mining indexes, sensitivity analyses, Monte Carlo simulations and Conditional simulations are used to identify and estimate geological uncertainty, notably the quantification of uncertainty in grade and tonnage estimates. These techniques have not generally been applied to estimating geological uncertainty associated with key Ore Reserve inputs such as appropriate mining technique, equipment selection, production rate, mining dilution, stability requirements, materials handling, metallurgical processing, product quality and waste storage. New software tools are needed to model the entire mining process with all its geological and other interdependencies, enabling all uncertainties to be separately identified and estimated. By its very nature, mining involves the assumption of significant technical, financial, political, environmental and social risk. Success comes by understanding and managing these risks and consequently the mining industry will never choose to reduce the level of geological uncertainty to nil. The ultimate aim of improved identification and estimation of geological uncertainty is to quantify inputs into the Ore Reserve process, allowing a company to assess the risks and manage these accordingly. This approach is relevant to feasibility studies for new projects as well as life-of-mine planning through to the weekly schedule at existing mines.

The Kibali Gold Mine is currently comprised of several open pits, a 3.5 Mtpa underground mine and a 7.5 Mtpa processing plant. Development of the underground operation commenced in December 2012, with initial access established through twin surface declines, followed by the commencement of a blind sink 8 m diameter rock hoist shaft in March 2013. Stope production started in December 2014 with ore and waste trucked to surface. The 755 m deep blind shaft sink was completed in August 2015 together with the cutting and equipping of three main station levels. Two of these levels provide access for the establishment of the underground materials handling system as well as the main dewatering system for the mine. This paper covers the process of determining the optimal location of the Kibali hoisting shaft relative to project-specific critical factors, and then focuses on the design and implementation journey associated with the off-shaft development that essentially encompasses the Material Handling System (MHS) for the mine. The journey starts from the findings of the previous owners Feasibility Study (FS), a subsequent Optimised FS, followed by detailed engineering through to implementation. It was identified during implementation that a significant change in the designed off-shaft layout was required following detailed underground geotechnical and hydrogeological investigations. In addition, the paper provides details on the MHS performance since commissioning to steady state, and on key learnings relating to the overall project process.

"An EXTENDED ABSTRACT is available for download. A full-length paper was not prepared for this presentation. The Misima gold mine, located on Misima Island within the Milne Bay Province of Papua New Guinea, was operated by Misima Mines Pty Ltd, a subsidiary of Placer Dome Asia Pacific Limited (Placer), between 1988 and 2004. During this period, the mine produced 3.7 Moz of gold and 22.2 Moz of silver. In June 2012, the Special Mining Lease for the property was surrendered and incorporated into an Exploration Licence (EL). WCB Resources Ltd (WCB) entered into an exploration farm-in agreement with Pan Pacific Copper covering this EL.In June 2013, WCB published a new mineral resource of the Umuna deposit at Misima. All the drill hole data used for estimation was historical data collected by the previous owners of the Misima mine between 1977 and 2001. No new drilling was carried out by WCB. Confidence that the data could be relied on was provided by detailed interrogation of the existing data, reconciliation of the new resource model against production data and significant input from individuals who had previously been involved with the data collection, resource estimation and production activities of the Misima mine during its operation.This extended abstract will consider the challenges associated with locating, retrieving and validating historical data to use as inputs to produce an updated Mineral Resource estimate and comment on the material value of appropriate data storage, documentation and record keeping through the life of a project. It will also comment on the value of WCB obtaining access to the intellectual knowledge of many professionals that were former employees of Placer. "Phytostabilisation with tolerant plant species has been advocated as a cost-effective solution for sustainable cover of tailings from metal mines and bauxite refinery. The development of sustainable plant ecosystems on such wastes requires not only tolerant plant species compatible with local climatic conditions, but also the formation of stable and functional root zones through systematic processes to develop functional soil matrix and horizons (eg A-B-C). However, the process requirements for reconstructing functional root zones in tailings are poorly understood. Based on a review of current literature and industry experience in tailings revegetation, the present paper conceptualises three key phases in the process of root zone development in amended tailings: hydro-geochemical stabilisation resulting from the interactions among tailings characteristics, amendment measures and local climatic conditions the rehabilitation of soil biological capacity the establishment of ecological (carbon and nutrient) linkages between the rehabilitated root zones and the above ground plant communities.Key research is required on: the relationship between hydro-geochemical stability and mineralogy and geochemistry of tailings, in response to rehabilitation treatments under local climatic conditions the relationship between soil biological capacity and hydro-geochemical stability in the root zone the transition of soil microbial communities specific to exogenous organic matters into those required for mineralising litters of in situ plant communities.CITATION:Huang, L, Baumgartl, T, Edraki, M and Mulligan, D R, 2012.
