The traditional measure of Lost Time Injury Frequency Rate (LTIFR) as a measure of improvement in safety performance is recognised as inadequate and misleading, lulling managers into believing that safety is improving. It is a reactive measure, and steadily declining LTIFRs around the world have failed to be accompanied by a similar reduction in the fatality rate. This paper confronts the industry with the attitudes of some mining managers, such as one that allowed fatalities to occur in the case of a gold mining company at a rate of one death per 150 000 ounces of gold produced while claiming their safety record has improved, as shown by a falling LTIFR, and viewing fatality reductions as a long-term goal instead of an immediate priority. A quantum shift in management attitudes is required to focus on proactive measurements to detect the indicators of an impending disaster, and to predict conditions conducive to fatalities. Corporate CEOs are challenged to attend the funerals of all workers killed in the workplace. CEOs who fail to attend such funerals send a message to employees that the CEOÆs priorities lie elsewhere, and that profits take priority over workersÆ lives. The dichotomy of whether mining companies are run for the benefit of shareholders or whether companies owe social responsibility to the community is examined. The role of individual responsibility for personal safety is examined, together with the psychology of employees in the mining industry. A number of proactive measurable indicators are suggested to stimulate discussion on developing robust measuring tools to predict and therefore prevent fatalities.

"INTRODUCTION This report is one of a series. 4. 5. 6/ concerned with presenting historical development, and operating data on secondary-recovery projects in various States to stimulate interest in sound secondary-recovery programs. Preceding reports in this series have dealt exclusively with descriptions of and results obtained by water flooding in three states, but no recent publication of comparable purpose describes gas-injection 7/ practices. The authors intend to present material relevant to the development and operation of gas-injection projects in Illinois, with particular emphasis on those producing from oil-bearing horizons that had been depleted to a point near the economic limit of operation by past oil-recovery methods.Most operators realize that the energy expended on the movement of fluids through gas-expansion- or gas-cap-type oil reservoirs to producing wells must be supplemented to attain maximum recovery of the oil present. Under favorable conditions, many operators choose to maintain oil-propulsive forces at or near their original magnitude by injecting extraneous fluids into a reservoir or recycling produced fluids early in the life of a field. More common, and less desirable, is the practice of replenishing these forces after primary producing methods become unprofitable. The lattes practice has been adopted in many stripper fields in Illinois and most of the oil-producing States during the last 40 years, but other old oil fields, and oil properties within those fields which were partly exploited by secondary--recovery methods, also may offer favorable opportunity for further development. To encourage and, aid operators in the development of gas-injection projects in the old oil fields of the United States, four projects have been described in detail in this report."

"Since 2005, longwall coal mine operators in Pennsylvania (US) have been required to collect extensive hydrologic and biological data to document pre- and post-mining conditions of aquatic resources (streams and wetlands) overlying the subsidence control plan areas. Continued operation of the longwall mine is dependent upon empirical data demonstrating that the undermined aquatic resources have either been maintained or restored to an accepted range of pre-mining conditions. This study used quantitative hydrologic methods and comparative biological metrics to examine response of aquatic resources to longwall subsidence at the Bailey Mine located in Greene County, Pennsylvania. This study analyzed data from over 25 miles of streams and over 8 acres of wetlands that traverse the 4,779-acre study area. Hydrologic analysis of flow intermittency suggested that there are variable degrees of change to headwater streams immediately following mining; however, the majority of the effects are reversible through streamflow mitigation measures. Furthermore, the post-mining discharge from streams can be restored to within the range of pre-mining conditions the majority of the time. Biological assessments showed that greater than 95% of assessed stream length has maintained its ability to support benthic macroinvertebrate communities following mining and intervention (i.e., mitigation). Comparative metrics of the benthic macroinvertebrate communities showed that the biological community in 48 of the 50 sample sites is being maintained or restored across both high- and low-gradient streams. The effectiveness of intervention was further substantiated by the similarity in recovery times between streams having no intervention (median = 2.1 years, 95th percentile = 4.9 years) and streams with intervention, when recovery times were measured from the date of the intervention (median = 1.7 years, 95th percentile = 4.7 years). Changes to individual wetlands were observed between the pre- and post-mining assessments, but cumulatively a net gain of 7% in wetland acreage was realized. Hydrologic assessments were used to infer whether there was a potential that streams had been affected by mining; however, the ultimate test of stream recovery is based on biological metrics that establish the relative quality of the biological communities following mining. Overall, the hydrologic balance is being protected and the aquatic life use of streams is being maintained within the Bailey Mine’s subsidence control plan areas."

"A large-scale panel destress blast was fired at Brunswick mine in the fall of 1999 in order to reduce the ground stresses in a critically important neighbouring mining region. This work, sponsored by the Mining Division of CAMIRO and Brunswick mine, consisted in heavily choke-blasting — and subsequently leaving in place — a massive waste sulphide pillar in the southwest end of the 1000 m level. This paper describes the approach and design methodology used to try to maximize breakage in the rock mass in the area to destress, while minimizing damage to critical infrastructures in the vicinity. The loading operations, as well as the results of the blast — in terms of its field performances, as recorded by near-field blast vibration monitoring equipment, visual postblast assessments and measured impact on the regional stress field — are discussed as well. A brief comparison between the measured stress changes and inelastic numerical modelling of the test site is also presented.Un tir à grande échelle de relâchement des contraintes dans un pilier a été effectué à la mine Brunswick à la fin de l’automne 1999 dans le but de diminuer les contraintes de terrain dans une très importante zone de production adjascente. "The IREDES1 Special Interest Group (SIG) for drill rigs has been active since its launch during MineExpo 2000 in Las Vegas, Nevada, United States. SIG for drill rigs is responsible for the development of an IREDES drill rig profile, including the exchange of production planning, performance, and quality reporting data between various kinds of drill rigs and the databases used by the operating mine. During 2001, a first draft of the drill rigs profile was jointly prepared by drill rig suppliers, mining companies, and third-party suppliers and reviewed during several SIG meetings. The profile for prototyping projects was released in mid-2002; the first official IREDES release was based on feedback received from these projects.In 2002, LKAB and Atlas Copco decided to carry out an IREDES prototyping project using the Simba L6C production drill rig. The goal was to verify the drill rig profile and identify potential problems prior to the official release. This was done by implementing the IREDES prototype version into LKAB’s production planning and Simba rig control systems. The Simba rig control system was designed to handle drill plan and logging information coming from the mine planning system. In parallel to this, the mine production system was adapted for drill plan export and logging data import in IREDES format.This paper presents the most recent results of the IREDES prototyping project. Results from late 2002 show that the basic architecture of the drill rig profile works and that a common language for data exchange, IREDES, will be a central requirement for future mine automation.As a result of the IREDES prototyping project, the IREDES standard was approved at the IREDES board meeting held at the 2003 CIM Annual Conference and Exhibition. Soon thereafter, IREDES compliant products, including drill rigs and mine production planning systems, were put on the market."

The No. 10 metro line of Shanghai city is 36 kilometers in length and will pass through the operating airport runway by using tunnel boring machine with earth pres-sure balance mode. Although there are examples through world that tunnels pass through airport runway and tunnelling underground is not a difficult undertaking technically, there is rare experience in China for the similar projects especially in Shanghai saturated soft soil region. Shanghai soft earth is famous for its high compressibility, high rate of water content, weakness in strength, large deformation and different characteristics in earth strata. Besides, the whole construction process will be under ultra-large airplane dynamic load on the surface ground over the tunnel, which increases the risks of construction process. This paper evaluates the feasibility of building the metro tunnel under such an extremely serious condition, especially the subsidence of ground surface caused by advancing of tunnel boring machine, which also is the most important control target for the safe operating of the runway. Furthermore, this paper discusses the effective measures to keep the settlements in the permissible level during the whole working period for the tunnel from aspects of plan, design and construction.

The commonly used exact methods (1-2) to determine the WL measure the total a activity or the total number of a counts during three preselected time periods. Due to the half-lives involved (TRaB = 26.8 min, TRaC = 19.7) I these methods take 3S minutes to complete the a activity Ra measurements. Methods using the total a counts, obtained at times less than about one RaB half-life after the end of sampling run into statistical 214 problems. These methods are almost blind to RaB (Pb214) as a consequence of its relatively long half period (see above) and the fact that RaB is a ß emitter. The way out of this dilema is to measure RaB directly through its ß activity. Since RaB's daughter Rae (Bi214) is also a ß emitter, the two f3 spectra have to be separated to obtain RaB alone. How this is accomplished 1s described in subsequent sections describing the theory of the IWLM and its calibration. 2.0 THEORY The IWLM samples air for two minutes and counts this airs ample for two minutes starting 3 sec after the end of the sampling period. The instrument has three channels. The lower a channel, the upper a channel, and one ß channel. The total RaA counts observed are accumulated in the lower a channel, the total RaC? counts are stored in the upper a channel. The total ß counts from RaB and RaC are recorded in the ß channel. These three quantities are functions of NA, NB, and NC, the unknown concentrations

Of the 345 km long Taiwan High Speed Rail Project, connecting the cities of Taipeiand Kaohsiung, the 23 km alignment on Contract C230 required the construction of5755 m of shallow mined tunnels ranging from 137 m to 1285 m in length. All nine of these tunnels were mined using NATM methodology. On September 6, 2002, a collapse occurred in Tunnel T08 that affected approximately 50 m of tunnel. There was little warning. However, the crew was evacuated approximately 45 minutes prior to failure and thus, no injuries or loss of equipment. This paper will discuss the mining methods, jet grouting ground support, and the instrumentation selected for the rehabilitation and monitoring of the tunnel.

A series of analyses of possible mining excavations associated with the development of copper nickel deposits of the MINNAMAX site in Northeastern Minnesota is presented. Specific areas such as roof and pillar stability and excavution orientations are addressed. The majority of these analyses were conducted using a boundary element method but some cases were also analyzed using the finite element method. Comparable results were obtained using the two methods but the boundary element method required less data preparation time and computer run time. Results of the analyses, as well as indicating the capabilities of geotechnical numerical models, provide some insights into behavior around multiple excavations in jointed rock.

Results are presented of a census of the machines used in surface coal mines in the United States. The numbers of each machine type and model reported to be in use are tabulated for large and small mines in the Eastern, Central, and Western regions. Also tabulated are the average daily usage times of the major machine categories and the numbers of machines that have acoustical and other cabs. Projections of the total numbers of primary production machines are given. It is found that heavy tracked dozers and wheeled front end loaders constitute the most prevalent machine types, and that generally a very few models of machines made by one or two manufacturers constitute a major fraction of the population in each machine category. About 70 percent of all machines are reported to have some kind of cab, and about half of these are reported to be acoustical gabs; it is noted that acoustical cabs generally came into significant use since about the early 1970's. It is observed that the percentage-wise mixes of equipment types used in the various categories of mines differ significantly for only a few types of machines, notably for heavy dozers and loaders, and for haulers, highway trucks, and internal-combustion engine-powered shovels and draglines.

TRANSVERSE rupture tests have been commonly used in mechanical investigations of brittle materials. The specimens are simple, easy to prepare, and loading presents no difficulty. Owing to the complexity of stress distribution in bending1 one can determine stresses rigorously only for the elastic range. The bend-test results, however, are at times converted into tensile-strength data, determined as maximum fiber stress. The assumption is made that for small plastic deformations the linear stress-strain relation applies. This problem was experimentally investigated by Bach3 who tested cast iron in bending and in tension and found that the maximum fiber strength obtained in bending exceeded the experimental tensile strength by a factor depending on the geometry of the tested piece. The factor varied from 1.57 for a rectangular cross section to 2.12 for a circular cross section of the tested beam. Since this work was carried out 60 years ago, the cast iron used could be expected to have been highly brittle, thus the extent of plastic deformation prior to fracture very small. We have therefore carried out a number of tests on magnesium-modified cast iron with an average elongation of -5 pct. It was hoped that the data obtained from the tests would serve as a guideline for comparing data obtained in tension and bending for brittle materials. Mechanical properties were measured at room temperature on an Instron tensile machine at a crosshead speed of 0.05 in. per min. The three-point bend test employed cylindrical bars 0.20 in. in diameter with a 1.0-in. span. For the tensile test, cylindrical tensile specimens with a gage length of 0.85 in. and a diameter of 0.160 in. were used. Results are summarized in Table I. Membrane technology has been available for over 36 years, but has been used sparingly in the general mining industry. However, recent developments in polymer chemistry, spiral wound element construction, pretreatment equipment and techniques and an expanded understanding of membrane fouling and cleaning techniques have dramatically improved the reliability and robustness of membrane based systems in the mining industry. Harrison Western has expanded the use of this “new” technology to enhance the use of membrane technologies to separate metals from large volume heap leach mining solutions containing copper, zinc, and iron, gold or silver. Moreover, membrane technologies are ideally suited to fractionations that add value to processing fluids from refineries like separating metals from acids or concentrating acids and producing high quality process fluids. Finally, wastewater from Acid Mine Drainage (AMD) or, as in the case of the Newmont Yanacocha Mine in Peru, the treatment of excess barren leach solution created during the rainy season, can be cost effectively processed to meet World Bank Standards for surface discharge. Polymeric membranes, specifically thin-film composites, have been developed that will purify, fractionate or concentrate gold, silver, copper, zinc or nickel. Recent applications have been developed for spiral wound modules that operate across the pH spectrum from 0 to 14. Systems using membranes have been constructed that operate on feed streams from 140° C to 300 centipoise. Opportunities exist to apply membrane technology to concentrate gold, silver, copper, zinc or nickel using thin film composite membranes. Engineered membrane types can operate across the spectrum from low pH (<2) acid applications to high pH > 10 cyanide applications. Incorporating HW EMS™ membrane technologies into current mining operations could allow one to increase production because the membrane technology would be used to pre-concentrate heap leach solutions, and therefore, increase utilization of existing extraction and refinery capabilities.

ABSTRACT It has been suggested that effective disaster prevention and response in mining requires that elements of emergency management be incorporated into everyday operations, planning, and decision-making, much in the same way that the industry approaches routine risk management. Since Congress passed the MINER Act in 2006, the newly mandated structural and technological advancements in mine emergency response preparation have been made and are apparent, but measureable improvement in mineworker emergency preparedness is difficult to ascertain. To begin exploring whether measurable indicators of mine workforce preparedness can or should be addressed within a risk management framework, NIOSH researchers examined the datasets from two separate research efforts—one which measured the organizational safety climate and self-reported safety performance, and the other which measured individuals’ perceptions of their own self-escape competency. INTRODUCTION In response to the mine disasters in 2006, the United States Congress passed the Mine Improvement and New Emergency Response Act (MINER Act), which strengthened existing regulations [30 CFR, Part 48] and introduced new provisions to improve mine emergency response. Since that time, new technological and structural features, such as mandated redundant communication systems, personnel tracking systems, and lifelines in primary and secondary escapeways, have been installed in underground coal mines across the country. Additionally, mine operators are required to develop emergency response plans, provide adequate supplies of breathable air throughout the mine, provide more frequent and realistic expectations training, and are required to immediately notify the Mine Safety and Health Administration (MSHA) in the event of an accident. While the structural and technological advancements in emergency preparedness are readily observable and easily measured, levels of individual mineworker preparedness remain difficult to assess. There is a general consensus that deficiencies in competence in the non-routine tasks required for individual miners to effectively respond to a mine emergency likely remain (e.g., National Research Council [NRC], 2013), and NIOSH research continues address this concern (Haas, et al., 2015; Hoebbel et al., 2018). To maximize an operation’s capacity to effectively prevent disasters and/or respond to emergency events, it has been suggested that individual and emergency management efforts be incorporated into everyday operations, planning, and decision-making in much the same way organizations manage routine risks to health and safety (Haddow et al., 2017). To the authors’ knowledge, the mindful intersection of routine risk management policies and practices and emergency prevention and response readiness have yet to be empirically studied within the mining industry.

En ese sentido, el presente trabajo describe el avance del proceso de extracción en el proyecto minero Ivan, perteneciente al yacimiento de la minera Rayrock a través de las técnicas de lixiviación de minerales oxidados y sulfurados de cobre y del manejo innovador en las operaciones de extracción por solventes y electro-obtención.
