This paper presents an evaluation of tailings dam incidents from the 1994 USCOLD Tailings Dam Committee report. Comparisons are also made with water storage dam incidents from USCOLD reports and recent tailings dam incidents. The evaluation results show that tailings dam incidents are due to a number of factors including overtopping, static and seismic instability, seepage and internal erosion, and foundation conditions, and that no particular type of tailings dam or operational condition is more susceptible to incidents. In addition, the results confirm that tailings dams should be located, designed, operated, and reclaimed with a thorough knowledge of site conditions, mill operation, site monitoring, and tailings characteristics.

THIS paper offers examples of some of the typical calculations which occur in the ventilation of metal mines. The fi.rst section of the paper discusses the problems associated with natural-draft pressure; the second section presents the more common of the calculations concerned with the operation of mine fans; the third section illustrates the integration of the individual problems covered by the preceding sections into the larger problem of determining the operating characteristics of a fan for a particular mine duty. There is a very definite limit to the extent to which pure arithmetic can be successfully applied to the solution of min<; ventilation problems. As mines advance from the exploration, development, and initial production stage to the rime when underground workings become exceedingly complex, there is a considerable increase in the relative importance o theory and judgment in the solution of ventilation problems. These calculations illustrate the application of mathematics to ventilation, but they also in-elude examples of instances where arithmetic cannot be conveniently applied, and important estimates, based on judgment compounded of experience and a knowledge of the theoretical consideration involved, must be made. The inclusion of such estimates is intended as a guide and a warning to the reader; a guide in assisting him to recognize and estimate his own incalculables; and a warning that the solutions must not be blindly applied to his apparently similar problems. These sample calculations are intended to assist the reader in applying the theory contained in the standard texts on metal-mine ventilation. They have been selected from the ventilation experience of a British Columbia mine.

Every paste backfill system is typically unique, this being due to the particular tailings characteristics, the backfill recipe and requirements of mine planning. Key to successful transport to the underground workings through the boreholes and the distribution pipelines is the quality of cemented paste, being free of lumps and recipe components uniformly mixed. Certain types of mixer require that the mixing environment is required to be intensive as possible. In this sense, the present paper point out some of the general aspects of paste plant design, a short discussion on paste recipe and process equipment, followed by a technical comparison between a batch mixing and a continuous mixing process, currently applied to the production of cemented paste tailings for mine backfill.

Foreword Following the explosion in the Hillcrest mine, which occurred in the evening of Sunday, September 19th, 1926, and in which two men lost their lives, the writer was invited by the Hon. J.E. Brownlee, Premier of Alberta, to conduct an investigation into the causes of the disaster, and to make such recommendations as seemed advisable with a view to minimizing as far as possible the chance of future explosions in this and similar Alberta coal mines. Accordingly, the mine was visited, and extensive investigations were made underground on October 21st, 2.2.nd, and 25th, in company with Mr. John T. Stirling, Chief Inspector of Mines for Alberta, Mr. Moses Johnson, District Inspector for the Crowsnest Pass District, Mr. J as. A. Richards, Inspector of Mines, Edmonton, and also Mr. Thomas Davies, manager, and other officials of the Hillcrest mine. Hearings of the coroner&apos;s inquest were also attended on October 20th and 23rd, when testimony regarding the explosion was given by various witnesses. The following paper summarizes my general observations in tracing the place of origin of the explosion, and my efforts to determine the cause of ignition. Certain recommendations arc also made, and the writer invites discussion on these and the other matters dealt with in the paper.

Despite increasing competition from mechanical methods of tunnelling, the drill and blast method is often still the most viable method of excavating tunnels in strong and abrasive rock. To advance a tunnel using drill and blast, explosives are loaded into boreholes in the rock and detonated according to a prearranged sequence. On detonation of the explosive, the rock is fractured and displaced from its original position, leaving behind the desired void. Among the secondary effects of a tunnel blast are ground vibrations caused by elastic disturbances that propagate away from the blasted tunnel face. The possibility that such ground vibrations may cause permanent damage to property and substantial nuisance has generated significant opposition to the use of drill and blast, particularly in urban areas. It is therefore essential that the blasting engineer is able to predict Peak Particle Velocity (PPV) vibration levels at locations in the vicinity of the blast site. Such predictive capability enables the engineer to design blasts so as to ensure that ground vibrations can be kept within acceptable levels. The objective of this project was to analyse data obtained from the ground vibration monitoring program undertaken during the drill and blast excavation of a hard rock tunnel in Central England. Seismograph recordings of tunnel blasts contain a vast amount of information that cannot possibly be expressed using a single PPV value. The main focus of the study was to investigate whether predictive capability could be improved by using scaled distance models derived for selected PPV components relating to specific detonator timings in a given tunnel round. This study has established a method of analysis that can unlock this information for predictive purposes. By permitting the extraction of sub-event Peak Particle Velocity (PPV) values and allowing Scaled Distance (SD) models to be derived for individual period numbers or groups of period numbers, the method has the potential to vastly improve PPV prediction from tunnel blasting. The method established could also help diagnose blasting problems and help target blast design modifications, resulting in improved efficiency and excavation rates.

"INTRODUCTION Sporadic beneficiation tests have been made on the Birmingham (Ala.) red iron ores for the last 55 years. Now, gravity-concentration plants are in operation, of tables are in use. A major problem, as seen by the present writers, is to obtain a coarser concentrate and add to the recovery without increasing the quantity of slime. The oily practicable solution found so far is to recover, by means of concentration (hydraulic) classifiers, the coarse concentrate normally lost in the table middling and, by a similar method, to recover middling particles from the table tailings The size and shape of the coarse concentrate Particles combine to cause them to report in the table middling rather than in the table concentrate. Similarly, mineral is lost in the tailing. Because of this, the nature of the ore, the deportment jerking table, plant tests on classifiers and tables, and the suitability of small concentration classifiers to treat table zonal middlings are discussed herein.The work was done by the Southern Experiment Station, Bureau of Mines, in cooperation with the University of Alabama, Tuscaloosa, Ala."

Keywords: Pyrometallurgy, Wits University, research Wits Metallurgy celebrated its centenary in 2004. The department has been at the forefront of education and research and has contributed greatly to the metals and minerals industry, both in South Africa and abroad, in terms of human resources and knowledge generation. A significant number of industry and academic leaders worldwide are Wits Metallurgy graduates. Since its early beginnings, pyrometallurgy has always been part of the curricula and research offered by the department. Pyrometallurgical activities started to expand tremendously from the middle of the 1960s, especially by the formation - together with Mintek - of the Pyrometallurgy Research Group, which continues its internationally recognised research activities today. The research efforts have been concentrated on problems associated particularly with ferroalloy, steel, stainless steel production, and PGM/Cu-Ni sulphide smelting. In recent years, research has been further expanded to physical modelling of pyrometallurgical reactors. Almost all of the specific research projects were handled from a fundamental point of view, and hence a large number of MSc(Eng) and PhD degrees were achieved by candidates under the supervision of the Pyrometallurgy Research Leaders. Moreover, a significant portion of the research results were published in internationally recognized journals and conference proceedings. Among these, Infacon and the Molten Slags, Fluxes and Salts Conference series are worth mentioning. The studies from a fundamental point of view include thermodynamics, phase equilibria, transport phenomena, kinetics, and process dynamic aspects of high temperature processes and systems, both in the solid and liquid states.

Anisotropy is a characteristic of intact foliated metamorphic rocks (slates, gneisses, phyllites, schists). ?esting was used to establish the anisotropic behaviour of selected gneissic rocks. The effect of anisotropy on various mechanical properties (strength, deformation) and dynamic properties (wave velocity) was examined. The rock samples were also tested at successively higher confining pressures in order to evaluate the effect of anisotropic behaviour at high overburden pressures. L’ anisotropie characterise les roches foliées (schistes, gneiss, phyllades). Dans ce travail des essais en laboratoire ont été exécutés afin de déterminer le comportement anisotropique des roches gneissiques. L’ effet de l’anisotropie a été examiné par rapport aux propriétés méchaniques (résistance, deformation) et propriétés dynamiques (vélocité ultrasonique). En outre des essais des roches ont été exécutés a des compressions latérales successivement plus grandes afin de déterminer le comportement des roches sous grands charges. Anisotropie ist eine Eigenschaft der geschieferten Gesteine (TonSchiefer, Gneis, Phyllit, Marmor). Versuche wurden durchgeführt, um anisotrope Verhalten von gneis Gesteinen zu ermitteln. Der Einfluss von Anisotropie auf mechanische (Festigkeit, Dehnung) und dynamische Eigenschaften (Wellegeschwindigkeit) wurde bestimmt. Die Dreiaxialversuche wurden unter grossem allseitigen Drücken entnommen, um das anisotrope Verhalten auf grösse herrschende Spannung zu bestimmen.

In most cases, slope movement monitoring data are collected on a temporal basis and displacement or velocity is plotted against time in order to assist the engineer to make judgements about slope behaviour. Once an unusual behaviour pattern has been established, the data are then analysed to estimate when the key inflection points occur in the data, when the slope movements start to accelerate and, ultimately, when the slope is expected to collapse. The identification of these inflection points is sometimes difficult to achieve with the naked eye. This paper presents the application of a trend analysis technique, known as cusums, to assist in this process. The authors have presented a range of data from various real life examples which highlight the effectiveness of the cusums technique. A key aspect of the cusums technique is the use of a constant (K) against which data trends are assessed. This constant is best derived from slope performance data at each site, as each rock mass will behave in its own unique way. The authors have experienced slopes that have had threshold values ranging from 0.1mm/day through to 10mm/day, and therefore a conservative approach should be adopted when assessing the performance of slopes.

Previously published stream quality data and AMD (acid mine drainage) analyses have been collated into a database (DAME û Database for Assessment of Mine Environments) and additional samples including NMD (neutral mine drainage) have been collected from coal mine drainages on the West Coast. Variations in the chemistry of mine drainage samples can be related to four factors: 1.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; regional geology (Brunner Coal Measures or Paparoa Coal Measures) 2.&nbsp; &nbsp;&nbsp;&nbsp;&nbsp; mine type (open cast or underground) 3.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; hydrogeology 4.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; local geology &nbsp; In general mines hosted in Paparoa Coal Measures produce NMD whereas mines hosted in Brunner Coal Measures produce AMD with highly variable chemistry. Differences in coal mine drainage chemistry between mines hosted in Brunner and Paparoa Coal Measure sequences can be measured with acid-base accounting geochemistry. Correlation of ANC (acid neutralising capacity) and MPA (maximum potential acidity) analysis of rocks with mine drainage samples is substantially better than correlation of NAG (net acid generation) analysis with mine drainage chemistry. &nbsp; It is likely that NAG analysis over estimates acid generation potential of some samples due to release of organic acids that are not released under environmental conditions. In general, differences in mine drainage chemistry between Brunner and Paparoa hosted mines are attributed to the effect depositional environment and diagenesis on coal measure composition. Exceptions to generalisations relating mine drainage to Paparoa or Brunner host rocks are likely where local geological conditions cause increased deposition of pyrite in Paparoa rocks and additional analyses are required to confirm interpretations. &nbsp; Based on the interpretation of these results a mine drainage chemistry hazard model has been constructed. The hazard model predicts likely variations in mine drainage chemistry based on the four factors listed above. The hazard model has implications for mine related water quality risks. For example, ecotoxicity, human health affects and selection of remediation techniques are all sensitive to mine drainage chemistry.

1. The major sources of heavy metal ions in wastewaters of the Coeur d&apos;Alene Mining District are effluents from metallurgical processing plants. Effluents from mine operations provide a secondary source for heavy metal ions. Based on data analyzed by the Federal Water Quality Administration, mercury and arsenic are escaping from some mill waste ponds, but they are in the suspended phase. 2. Metallurgical process effluents are combined with mine waste effluents in tailings ponds, consequently, the quality of the tailings pond decant water is highly degraded. The major effect of combining metallurgical process effluents with mill wastes is a drastic change of pH in the tailings pond. This chance may be an increase or a decrease depending on the metallurgical process. The effluent from an electrolytic antimony plant drastically increases the pH in the tailings pond permitting the removal in solution of antimony via the pond decant system. The addition of low pH effluents to the tailings ponds results in a net decrease in pH which makes lead, zinc, cadmium, and copper more soluble, thereby permitting their removal from the pond via the decant system.

Recognizing this fact, the Bureau has a major on-going research effort to address this problem area. This report summarizes the results of a research project entitled "Fatal Accident Data Base Development." This project is the last project under Contract No. S0110601. Volumes I and 1I, "Industrial Engineering Study of Hazards Associated with Underground Coat Mine Production," summarize the original industrial engineering project and the Fatal Accident Report Analysis project. These projects were completed and accepted by the U.S. Bureau of Mines on December 10, 1971. Both volumes of the report have been made available by the Bureau to the public through the U. S. Department of Commerce technical information service. Another project under the same con tract, entitled "Equipment Operations Safety Study, &apos; was completed in January 1972 and the report was accepted by the U. S. B. M. at that time. The analysis in this Natal Accident. Data Lose Development report is closely related to analysis in both of the earlier reports. The original project involved an industrial engineering and safety of production operations for a representative sample of underground bituminous coal mines. The contactor was required to make recommendations &apos;in work practices, mining methods, equipment, and safety practices which can effectively reduce the fatality rate from rib and roof falls.&apos; (Longwall mining methods were excluded from the study.) Time study and work element analyses were performed in over 50 mines as part of the field data gathering effort. Another requirement of the study was to develop sets of multiflow. Process charts, which are diagrammatic illustrations of the time-related interaction of mining, cycle job elements.

The results of a pile load test on a 1200 mm dia and 25 m long bored Cast-in-situ pile in soft rock is reported. The subsoil has a high N value of above 60 from 7 m to 21 m. The soft rock was observed below 21 m and N value exceeded 100. The theoretical calculations showed that the safe carrying capacity will be about 410 t. But the actual load test on the pile indicated that the allowable load is only about 140 t. This is attributed to the fact that the soil is cemented and loses strength due to remoulding caused by pile installation. It is concluded that great care should be exercised in determining the carrying capacity of piles in cemented soils and soft rocks and that &apos;N&apos; value alone should not be used.

The use of augercast piles has been limited by the engineer&apos;s concern for the structural quality of the installed pile. The nature of the pile is such that its final structural continuity is very dependent on the skill of the personnel that do the installation. Thus, NDT methods have been used to verify integrity and when problems are indicated the available solutions are usually quite costly. If information were available during the pile installation so that immediate steps could be taken to correct perceived problems, the concerns of the engineer could be addressed and the system would gain increased reliability. Pile Dynamics, Inc. has developed a quality control system, the Pile Installation Recorder for Augered Cast-in-Place piles (PIR CFA). This system consists of the following devices as shown in Figure 1. 1) The Control Unit powers the sensors, conditions the signals from the sensors, records all measurements, takes input information from the operator, displays the results, and stores the recorded data on a flash card for later permanent storage and processing. During the use of the system all of the results obtained during a test are seen during recording and can be reviewed on the screen again after completion of the installation. The ability to review the measured information makes it unnecessary to print a copy in the field. Since printers often give difficulty in the construction site environment the ability to review the results quickly after completion of the installation is an important capability.

Bag stackers who use semi-automated palletizing machines and air slides may be exposed to high concentrations of dust. To reduce dust exposure, use an exhaust ventilation system in conjunction with an envelope of clean air over the bagstacker provided by an overhead air supply. Description of Hazard Mineral processing plants in the United States and throughout the world process material that is finely ground and placed into bags for shipping to the consumer. These bags normally range from 50 to 100 pounds in weight. Once the material is placed in these bags, a bag stacker loads the bags onto pallets. A bag stacker is exposed to a lot of dust that is mainly released by the force from loading the bag onto the pallet. The two main sources are the dust on the outside of the bag and the dust that escapes from inside the bag as it is loaded. The Mine Safety and Health Administration&apos;s (MSHA) records indicate the bag stacker&apos;s dust exposure is one of the highest of all workers at mineral processing operations. Ways to reduce the dust exposure and strain to the bag stacker are in constant development. Recently, automated systems have been built. These systems range from fully automated, which totally remove the worker from the bag-stacking process, to semiautomated systems, in which the bag stacker still performs some of the functions. This Hazard Control deals with controlling the bag stacker&apos;s dust exposure during use of a semi-automated palletizing machine. The worker slides the bags into position on the palletizing machine. Once an entire layer of bags is positioned, the bag stacker operates the controls on the palletizing machine to lower the layer of bags onto the pallet. An air slide is built into the semi-automated palletizing machine so the bags are easier to move. This slide uses a metal table with air jets that exit through small holes at high velocities, similar to an air hockey game. The dust problem occurs because the air slide causes dust to blow up onto the bag stacker, significantly increasing the dust exposure.
