On the 5 September 2014 a road train (tractor-trailer) with two trailers carrying ammonium nitrate was involved in a single vehicle accident on the Mitchell Highway, 30 kilometers (18 miles) south of Charleville in Central Queensland, Australia. The ammonium nitrate was being transported to a mine site for use as a blasting explosive. The truck came to rest in the creek bed adjacent to a road bridge and caught fire. The vehicle burnt for over one hour before the load of ammonium nitrate exploded. Miraculously there were no fatalities; however eight people nearby sustained injuries, some seriously. The road and rail bridge over Angellala Creek were destroyed and vehicles in the nearby vicinity were seriously damaged. The truck carrying the ammonium nitrate was completely destroyed with over 2500 pieces of debris scattered up to one thousand yards (one kilometer) from the blast site.

Near misses recorded and reported by workers can provide awarenessto the potential causes of injury and prompt safety management initiatives. Although most companies require near-miss reporting, it is unclear what the value of these reports are, if any, and how they influence subsequent actions or controls to reduce on-the-job risks. Researchers at the National Institute for Occupational Safety and Health (NIOSH) conducted a case study with an aggregates company in which near-miss reports were analyzed at each of their locations over an entire quarter during the summer of 2018. Within that quarter, workers recorded 249 near misses. Of those, 167 were valid near misses that occurred at work. Researchers coded the reports using a qualitative 5 × 5 risk matrix. Of the 167 near misses, 19% were deemed low risk, 25% moderate risk, 30% high risk, and 26% critical risk. Several patterns in the near-miss incidents were documented, including classification of incidents and common corrective actions referenced (i.e., elimination/substitution, engineering control or redesign, work process/procedures, and personal protective equipment). The analysis provides insight into ways that risk communication and management programs can be improved to reengage workers and their situational awareness on the job.

I. INTRODUCTION Lawyers are a cautious breed; and it might be expected that a paper concerning a broad and complex area of legal regulation, prepared by lawyers for presentation to a predominantly non-legal audience, would commence with several caveats. Ours are the following: First, we have written a broad (and, in some instances, simplified) overview of the regulatory system and of some of the more important requirements generally applicable to uranium mine and mill wastes in the United States. Our paper is not a legal opinion setting forth all of the governmental requirements that would have to be met by any particular mine or mill. Similarly, we do not imply that the requirements we mention are necessarily applicable to each and every mine and mill in the country; depending on the facts, some of the requirements may be inapplicable or avoidable through exemption or variance proceedings. Compliance decisions and strategies for particular facilities must be considered on a site-specific basis. Second, we have undertaken merely to describe the regulatory framework and some of the generally "applicable" requirements which have been promulgated and proposed by administrative agencies. We express no opinion on whether, under the U.S. Constitution and statutes, the existing and proposed agency regulations are valid in general or as applied to any particular mining or milling operation. There are, of course, procedures for judicial review of agency action; and newly promulgated environmental regulations have frequently been challenged, with varying degrees of success, before the courts. For us even to begin to consider such issues would expand an already forbiddingly long paper to totally unmanageable proportions. Third, in light of the subject of this Conference we have limited our paper to the legal framework and requirements applicable to uranium mine and mill "wastes" -- primarily mine overburden and waste rock and mill tailings (including waste tailings solution). We treat operating discharges (e.g., emissions from yellowcake drying) only peripherally. February 2, 1999 marks a milestone in earth-moving technology at the San Miguel Lignite Mine. It was on that date that The North American Coal Corporation (NAC) took delivery of three Leica Dozer 2000 GPS grade control systems for use on its bulldozers. The primary reason for the purchase of these grade control systems was to prove that GPS technology could safely and economically be utilized on heavy equipment to assist operators in defining grades. If these properties were successfully demonstrated, the ultimate vision of the project was to expand the system to a point were GPS would become the primary means of providing grade control information to the mine’s heavy equipment operators. Twenty nine months and more than $500,000 later this vision was realized when the final bulldozer operating at the San Miguel Lignite Mine was equipped with Dozer 2000. Presently North American Coal has a total of nine Dozer 2000 GPS grade control systems at San Miguel providing cut and fill information to operators 24 hours a day. These grade control systems have allowed the San Miguel operation to increase safety, reduce costs, improve operator attitude, and better ensure environmental compliance. This article discusses both the nature of these benefits and some of the issues that had to be dealt with along the way.

"Located in the northwestern portion of the Abitibi greenstone belt, Les Mines Selbaie is a copper-zinc (silver and gold) producer. Felsic tuffs host three are zones. Composed of sulphide veins and breccias, they are not exhalative massive sulphide deposits, except, probably, for the massive pyrite (fragments and lenses) localized close to the top of the volcanic sequence.The total of all (mineable and mined) ore in the three zones presently outlined on the property, is of the order of 29.9 x 106 tonnes with average grade of 0.63 g/t Au, 37.02 g/t Ag, 1.21% Cu, 1.91% Zn.The ore reserve calculations and the daily production procedures are computerized to various degrees. The data handling process is highly computerized.Various geophysical methods are, and have in the past, been successfully used on the property. The ""mise-à-la-masse"" surveys were used to perform the geological interpretation of an underground deposit (A-2 zone). Presently the PULSE-EM surveys are performed in all surface and underground exploration drill holes."

"Selbaie is located northwest of Joutel, in what is known as the fourth east-west volcanic sediment group in northwestern Quebec. The main metals produced are copper and zinc, with important contents of both silver and gold. Production comes from an open pit mine on the A-I zone and from two underground mines on the B zone and the A-2 zone. Each mine gave rise to important capital investment programs for construction and mine development, all of which were managed and staffed with company-hired, non-union personnel. Production from all sources now exceeds 7000 tonnes per day, with a workforce of540 regular employees. Of particular interest technically is the high recovery of B zone reserves underneath 40 m of overburden cover, the low-cost capital development of the A-2 zone, the computerized control of open pit mining, and the successful copper-zinc separation of the A-1 zone ore. Due to the low grades of the mineral deposits, efficiency in all aspects of the production process, is essential to the success of the operation.LocationThe Selbaie Mine is located 82 km northwest of the locality of Joutel in northwestern Quebec (Fig. 1). Access to the mine is by an all-weather road, 32 km of which is paved. A second road access to the site is via La Sarre, 134km to the southwest. Employee access, materials transport and haulage of copper and zinc concentrates take place over both of these roads."

"Les Terrains Auriferes (LIA) Property, formerly the Malartic Goldfield (Goldfield) mine, ceased its remaining milling operations in the fall of 1994. During the early years of mining, the mill processed a non-sulfide ore with substantial carbonate mineralization.In 1977 after an extended shutdown period, ore from a nearby property, Bousquet, was processed at the mill. That ore contained larger quantities of sulfides, and unlike the old tailings, had acid generating potential.Upon closure of the L TA mill, the closure options considered for the L TA site were to cover with:1. a thin cover of 0.5 metres of borrow material (sand and gravel) to achieve basic erosion protection/stabilization; collect acidic seepage and treat with lime.2. a 2 metres of non-acid producing tailings; final surface to be vegetated.3. an engineered oxygen barrier consisting of a multi-layer cover; final surface to be vegetated.The paper presents the technological and economical methodologies that were used to select the closure alternative, describe the associated construction challenges that were faced with the implementation of the multi-layer cover concept and presents preliminary results on the cover's performance.In addition to being the first full scale demonstration of this type of multi-layer engineered oxygen barrier cover technology, this project had the advantage of using non acid producing mine tailings as part of the reclamation solution."

"‘Gas hydrate’ is the common term for a solid material formed from the combination of various gases as ‘guest’ and water as ‘host’. A series of large-scale scientific drilling and coring programmes conducted since 2000 revealed that natural gas hydrate exists in a wide variety of accumulations in the sediments. The scaled-up onshore and offshore field production tests during the last decade show that gas production technology will be realized to make gas production from gas hydrates viable in the future, recognizing its potentials as one of the future sustainable energy resources.The total produced gas was approximately 470 m3 by the hot water circulation method over 6 days of operation in the permafrost-associated Mallik 2002 project in the Mackenzie Delta of Canada. In the Mallik 2007-2008 project, the gas was successfully produced stably by the depressurization method for 6 days, up to 13,000 m3 cumulatively. The depressurization method applied in the Mallik test was shown to be an effective way to produce gas from gas hydrates. The Alaska field trial to inject mixed gas of CO2 and N2 to exchange CH4 in 2012 was successfully done for the first time to produce maximum 1,270 m3 per day. A remarkable achievement was made by Japan when it completed its first offshore production test in the Eastern Nankai Trough, producing approximately 120,000 m3 of methane by the depressurization method over 6 days in March 2013. The technical challenges and scientific uncertainties obtained from Nankai Trough production test provide Korea with more considerations as to the aspects of well completion, reservoir formation and seafloor stability, sand control, flow assurance, environmental monitoring, and etc., due to the different geological setting and geomechanical properties of the Ulleung Basin in Korea."

This paper traces the co-operative initiative undertaken by South Africa?s gold mining companies through the Chamber of Mines Research Organization (COMRO) in development of hydraulic technologies. After a brief description of the structure of these technologies? development under COMRO?s stewardship, the paper turns to an overview of the structure of innovation within which these hydraulic technologies were developed. A central focus of this paper is systematic review on whether similar technologies are likely to emerge in the present system of innovation and the role played by innovation policy.

The Vale Ferro Carajßs S11D project in northern Brazil is based on the movement of unprecedented volumes of waste and ore for mobile in-pit crushing and conveying or ætrucklessÆ systems. In peak years, the system is intended to handle a combined 160 Mt of material.The planning and implementation of the first such system for Vale, in a remote greenfield site, utilising the services of a range of international study, design and supply partners has provided a unique set of technical and project delivery challenges. The lessons from these are of value to the industry as the trend to remote and high-volume operations continues.This paper describes the key features of the mining system and documents the challenges and mitigations for important technical, cultural and project delivery issues that have arisen. The author attempted to concurrently apply the US Uniform Standards of Professional Appraisal Practice and the Australian VALMIN Code to a complex mineral property appraisal assignment. This led the author into many severe difficulties. These difficulties included the high standards needed in the work of others, and information and statements needed from the client. The exercise also highlighted many positive features of both documents. Some conflicts were found between the two documents. Implications and thoughts regarding the development of a set of US minerals appraisal standards are discussed.

Augered-Cast-in-Place piles have been used successfully for many deep foundation applications. The four case studies discussed herein describe ACIP pile projects where problems developed during ACIP pile installation requiring changes to the method or sequence of installation. The case studies illustrate the importance of understanding existing subsurface conditions and project constraints when considering the use of this pile type. Ground loosening and over-excavation of soil during the augering process, a significant concern for fine to medium sands and non-plastic silts below the water table, can significantly impact adjacent structures. This risk needs to be evaluated during the pile selection process and generally can be mitigated, but not eliminated, by requiring the use of piling equipment with high torque and slow auger rotation speed to advance the auger with minimum amount of rotation. In some applications where high lateral stresses in the soil are present such as at the base of retaining structures, the use of ACIP piles may not be appropriate if structure movement is unacceptable. On the other hand, ACIP piles have been successfully used to support deep foundations in preconsolidated varved deposits where the new structure was designed to accommodate recompression settlement.

When originally accepting the assignment, the Client had described the property as vacant land. A prior appraisal was later discovered and revealed that the property included a mineral rights appraisal. Further research disclosed an old dispute with a court ruling based on testimony by AIMA-certified experts. The present appraisal was therefore based on data from findings of the court with a fresh look at discount rate, absorption period, and residual land value. Fortuitously, the property subsequently sold on the market, while meeting all criteria for Fair Market Value. This provided insight into a market-based discount rate. Also, the absorption period could be adjusted in accordance with the current market conditions. The residual land value was minimal (based on comparable sales and projecting trends in market) as the property was affected by a flood zone and the surrounding development was sparse. PROPERTY HISTORY: A GREAT INVESTMENT Briefly, the subject property is located in the floodplain region of the San Jacinto River, a short river in southeastern Texas that flows into Galveston Bay, filled with sand and gravel. This location is on the north side of Houston, north of Beltway 8 on the east side of I-59.

A gold mine in north-eastern Africa has expanded throughput from 5 to 10 Mt/y in recent years. The original carbon regeneration kiln was incapable of achieving required throughput or meeting process conditions to reactivate the carbon due to heavy fouling by high molecular weight frothers and collectors. A new high temperature kiln was required.The design criteria for the carbon reactivation kiln and associated equipment were revised, then implemented in close cooperation with a kiln vender who had previously supplied kilns operating successfully on highly fouled carbon. The result is a whole facility, holistically designed to achieve the technical criteria.The new carbon reactivation facility has been operating since 1 July 2014. What have we learned? What has performed to expectations? What could be done better?This paper presents lessons learnt regarding the design, implementation, operation and maintenance of the Ausenco Carbon Reactivation Kiln. ‘Lessons’ are associated with threats and opportunities (risks) that have that have been realised. The basis of this paper is a compilation of work in the design and construction of copper heap leach facilities, and interviews with various heap leach operations personnel in Chile and Peru. The purpose is not to highlight specific mines and their performance, but rather to discuss geotechnical/civil features of copper heap leach facilities that function well, as well as features that do not. During the 1990s several copper heap leach facilities were brought on-line in South America. Many of these facilities have now been operational for a number of years during which time the operators have been able to monitor the performance of their heaps. This paper serves as a compilation of operators? experiences, and provides a discussion of the lessons learnt over the past decade. Several key geotechnical issues are examined, focusing upon the integrity of the ore, drainage material and pipe network, along with the success of inter-lift liners. This conference provides a timely update of the original paper (Burkhalter et al, 2002).

Mr Janisch' s comment' in the August issue on Dick Stacey's remarks about safety factors indeed requires wider debate. The example he quoted to illustrate the successful application of the safety factor concept in coal mining, namely the Salamon formula, is to my knowledge the only recognized application of that concept. Not only that, but that particular formula should be used only for what it was intended, namely to prevent major disasters. It was never intended to indicate long-term pillar stability, and it should not be used for that.

To benefit two endangered fish species, waterfowl and other wildlife, and water quality in the lake and downstream, the decision was made to remove levees that had been in place since the 1950s. Because the levee material was mainly dredged up lake bottom peat, it was decided to pare down the sites chosen for breaching with dozers and trackhoes. This material was spread out onsite instead of expensive removal by dumping elsewhere. Because of the instability of the final pared down levees and the lake level being at a ten-year low, it was decided to use explosives to breach four of the sites. After over five months of earthmoving and two weeks of loading holes, the levees were detonated the day before Halloween, 2007. Somehow pumpkins showed up to be part of the explosive activities.
