In 2017, Bauer Foundation Corp. (BFC) was awarded the contract to install the deep foundations for the new 17-story Hotel Tower and the Low Rise Casino Expansion at the Seminole Hard Rock Hotel and Casino in Tampa, Florida. The site was underlain by extremely challenging Florida Limestone geology and surrounded by occupied existing buildings and infrastructure. To overcome the challenges of the difficult subsurface conditions, and to successfully complete the foundation installation without impact to the surrounding buildings and casino operation, Langan Engineering (EOR) and BFC were required to modify both the design and installation techniques during construction. The scope of work included 147 temporarily cased drilled shafts to depths up to 230 ft (70 m) below ground surface (BGS), 76 permanently cased micropiles up to 200 ft (61 m) BGS, and more than 300 augered-cast-in-place (ACIP) piles up to 125 ft (38 m) BGS. Voids and zones of high permeability in the limestone created many challenges for execution of the foundation scope including ACIP piles overbreaks of more than 400% above neatline volume, and construction induced ground subsidence/collapses adjacent to structures supported on shallow foundations. This paper will discuss site limitations, design and construction challenges, difficult geology, varied soil conditions, and lessons learned. PROJECT The project is located at Seminole Hard Rock Hotel and Casino Tampa, Florida, as shown in Fig. 1. Seminole Tribe, the owner, started the construction of a new 17-story Hotel Tower and the Low-Rise Casino Expansion at the beginning of 2017.

Although many mining companies apply ergonomic principles, it is usually done either informally and/or reactively. An example of an informal approach is replacing worn equipment (such as seats) with models that have ergonomic features. Another example is modifying a task to eliminate safety hazards. Such approaches reduce risk-factor exposures that can lead to musculoskeletal disorders (MSDs). A reactive approach is followed when modifying a task/ equipment after an MSD occurs. However, neither of these approaches involves a systematic method of proactively applying ergonomic principles to actually prevent injuries or illnesses. This paper discusses how Vulcan Materials Company applied ergonomic principles and implemented interventions to reduce risk-factor exposures.

A study was conducted to investigate the damage potential to underground multiseam openings caused due to vibration generated by open-pit coal mine blasting. The objective was to recommend optimal explosives weight per delay to be detonated in open-pit blasts to ensure the safety and stability of underground openings. Sixteen blasts with varying charge weights and initiation sequences were conducted at Samleshawari open-pit mine and eighty four blast vibration data were recorded in the Hingir Rampur underground colliery in India. Analysis of vibration data showed that roof of the underground openings vibrated with much higher amplitudes compared to those in pillars and floors. The amplifications of vibration in the roof at the junction were upto 4.3 times the pillar vibration. It was observed that coal chips detached from roof when the peak particle velocity reached 131 mm/s or higher. The vibration magnitude recorded was higher in upper-seam openings than in belowseam openings. Rapid attenuation of vibration was recorded in below-seam openings at similar locations. Appropriate particle velocity-charge weight-distance equations were established to calculate the safe explosive weight per delay to be detonated in open-pit mine for the safety of multi-seams workings of underground mine. The support for the galleries and junctions were further modified for the working seams to ensure greater safety.

The Cortez gold mine is located in Lander County, NR, about 130 km (80 miles) southwest of Elko. Cortez Gold Mines (CGM) is a joint venture between Placer Dome US Inc. (60% operator) and Kennecott Corp. (40%). The claims that supply oxide and refractory ore feed to the Cortez milling facility are located in both Lander and Eureka counties. These include the Cortez and Horse Canyon deposits, all operated by the joint venture. Other deposits outside the CGM claim area include Fire Creek, Buckhorn, Gold Bar and Tonkin Springs. These deposits all occur along the Battle Mountain-Eureka Trend. History Mining in the Cortez district began in 1862 with the discovery of high-grade silver ores. Grades in the hundreds of ounces per ton occurred in an area 7 km (4.5 miles) southeast of the current CGM mill site. Underground mining through the Garrison and Arctic tunnels provided the bulk of the ore production between 1864 and 1895. To a lesser extent, the Mill Canyon area also supplied ore until 1930.

The compliance sampling method for coal mine dust in the United States has not changed appreciably in the last 30 years. A call for more frequent sampling with immediately available results is leading to new instruments that may supplement or replace the existing sampler. This report discusses two such instruments that provide immediate results of dust levels. The first, called the Respirable Dust Dosimeter (RDD), is designed to supplement the existing U. S. cyclone compliance sampling method. It uses the pressure drop across a filter to provide an inexpensive screening type of measurement. The RDD has been tested in the laboratory and a limited number of coal mines. Side by side testing has compared the RDD performance with personal coal mine samplers in triplicate area sample measurements. Results show that the differential pressure of specific filtration media can be an effective surrogate for respirable mass. Data show that there is a dependence on coal type and an effect of relative humidity. There also appear to be two distinct responses related to coal type. For specific coals the coefficients of determination, R2, are better than 0.9. For general use the detector tube, using laboratory generated calibration curves, can be used to estimate respirable dust levels. However, a calibration to a specific coal type may substantially improve the accuracy. Another sampler in development is called the Personal Dust Monitor (PDM). Designed to give compliance quality accuracy, the PDM uses the frequency change of a vibrating element to measure dust and give immediate feedback of results. Results of laboratory comparison of the prototype PDM sampler to personal gravimetric samplers indicate R2 values of better than 0.99 for three different coal types. Work is in progress to package the components into a person-wearable unit combined with a cap lamp system with a total projected weight of less than 1.4 kg (3 lbs).

The U.S. Bureau of Mines (USBM) conducted a series of laboratory tests to investigate the effectiveness of using a flooded-bed scrubber with exhaust ventilation in deep-cut faces of up to 12.2 m in length. An experimental test program to determine the impact on respirable dust levels resulting from changes in face airflow, curtain setback distance, operator positioning, and operating parameters of the external spray system on the miner was completed. Gravimetric sampling was conducted in the immediate return and at three sampling locations on the off-curtain side of the entry. Statistically significant differences in dust levels on the order of 0.5 to 1.2 mg/m3 were observed between specific sampling locations and changes in several test parameters. Dust sampling results indicated that a flooded-bed scrubber and exhaust ventilation can be a viable dust control system for extracting deep cuts up to 12.2 m in length. The impact on methane was not evaluated.

In 1990 the Noranda Technology Centre commenced a project to develop an interactive three-dimensional blast design system using a commerical computer-aided drafting and design package (CADD). The objective was to alleviate the blast engineer from the tedious and time consuming blast layout and design functions normally done by hand. This would allow more time for optimizing blast designs through assessing multiple options. Since several blast design software "tools" already existed at Noranda, the need arose to integrate all the applications into one overall system that shared one common database. The result is BLASTCAD.

"An EXTENDED ABSTRACT is available for download. A full-length paper was not prepared for this presentation. Sea floor hydrothermal activity in the Okinawa Trough is considered a modern analog to ancient Kuroko-type volcanogenic massive sulfide (VMS) deposits. Recent scientific drilling projects at active hydrothermal fields have enabled us to conduct comparative studies on hydrothermal clay minerals between ancient and modern systems. Zonal distributions of clay minerals below the sea floor of an active hydrothermal field have been revealed, which is represented by the distinctive assemblage of clay minerals. Zone 1 consists mainly of montmorillonite and sometimes of kaolinite; Zone 2 consists of chlorite and chlorite-smectite mixed-layer mineral; and Zone 3 is represented by the co-occurrence of illite and chlorite, from shallow to deep. This zonal distribution is likely to be in accordance with that reported previously for the Kuroko-type ore deposit. Similarities are also recognised in chemical compositions of illite and chlorite, which are both abundant clay minerals in the vicinity of the ore deposit. While illite would be formed by interaction with the hydrothermal ore fluid, the formation of Mg-rich chlorite is attributed to Mg metasomatism caused by the involvement of sea water. Further studies, especially focusing on interactions between fluid and solid phases, would significantly promote our understanding of dynamic processes for the formation of VMS depositsCITATION:Ishibashi, J and Miyoshi, Y, 2015. This is the final article in a three-part series on microcomputers and application software in the mineral industry. The first article covered microcomputer terms and concepts. The second discussed exploration and mining software. This article will examine plant, administrative, and geotechnical software. Every effort has been made to be current in the latest software developments. Rapid change, however, is a fact of life in the computer world and any omission is inadvertent. Introduction Geotechnical disciplines have received considerable attention in software programming. Calculations are repetitive and the formulas needed to solve them are not extremely complicated. These programs are ideally suited to the microcomputer and the microcomputer is ideally suited to the consulting geologist or engineer, due to its low cost and reliability. In fact, the advent of the Apple (a registered trademark of Apple Computer Inc.) and, more recently, the IBM personal computer has caused an explosion of inexpensive software programs available to the microcomputing public. Another area that has benefited from computerization, although not in the same way, has been mineral beneficiation. Here, process control for start-up or phase-down operations has been used in newer plants for some time. Programs to perform various calculations are becoming more common as micros increase in number. Tappi Journal has a section with solutions to handheld-calculator and microcomputer problems. Process Modeling Most process modeling is still done only on mainframes. One such program is called ASPEN PLUS (a registered trademark of Aspen Technology), and a one-week course is recommended so the user can best use the program. This program is available through timesharing from University Computing Co. Programs to find an optimal solution using Linear Programming (LP) have been around for some time. Also, plant process simulation has been possible using simulation programs. Both simulation and LP are expensive to program and run. Although there are LP programs available for some micros, the user must be fairly knowledgeable about how to set up the problem. This is because each problem must be set up as a unique program that typically requires custom programming. There are some programs that can handle blending of mine crudes and examine economic parameters over the life of the mine. Plant Applications Areas relating to mine and plant applications with programs available for micros are maintenance, production reporting, inventory, accounting, and personnel scheduling. Maintenance Maintenance typically requires that certain jobs be performed in a particular sequence and parts be ordered with enough lead time for maintenance to take place on time. There is, therefore, a critical sequence of tasks that must be performed or the scheduled maintenance will not take place. Because of complex geological environments, a number of coal reserves occur in inclined or dipping coal seams in certain parts of the world. Longwall top coal caving using split-level a singleentry panel system (SSPS) has been widely used for extracting such coal seams with large thicknesses (greater than 6 m) in China because SSPS can reduce or mitigate problems of coal outbursts, gate road deformations, overturning of shields, downhill sliding of shearer and armored face conveyor (AFC), and so forth, during mining of inclined coal seams. Depth of yield is the range of yield coal from the rib of the pillar to the boundaries where yield ends. It plays a significant role in maintaining the gate pillar stability. This paper presents a study on the stability of an unconventional water-obstruction gate pillar (UWGP) formed by SSPS to prevent water inrush in mining of an inclined thick coal seam in a coal mine in China. As the shape of this pillar is non-cuboidal, nonparallelepiped, a conventional empirical or analytical method is not applicable for studying its stability.

The year was 1974. The event - the United States Congress was embroiled over what issues should be included in the proposed national surface mining law. The question - what should be done with underground mines? The leading bills ultimately addressed the environmental impacts associated with underground mines including provisions to protect the surface from subsidence impacts. Amid the fears that this subsidence language might discourage underground mining, sponsors of the bill were asked to explain the meaning of the provision that read "to prevent subsidence to the extent technologically and economically feasible." Was this an attempt to limit mining that caused planned and controlled subsidence? Former Deputy Director of the Office of Surface Mining, Dean Hunt, reviewed some of these historical events in a 1985 presentation before the Eastern Mineral Law Foundation. He reported that in 1974 Congressman Morris Udall took the floor to respond to these concerns expressed by other members of Congress and the industry. Mr. Hunt cited Congressman Udall's July 10, 1974 Congressional Record statement: "The House bill contemplates rules 'to prevent subsidence to the extent technologically and economically feasible.' The word prevent led to fears expressed by the Secretary of the Interior Morton, that the effect would be to outlaw longwall mining, with its obvious subsidence ... in fact the bill's sponsors consider longwall mining ecologically preferable and it and other methods of controlled subsidence are explicitly endorsed." Further, the report that accompanied the front-running bill, HR11500, provided the following clarification on the issue of subsidence prevention: "It is the intent of this section to provide the Secretary with the authority to require the design and conduct of underground mining methods to control subsidence to the extent technologically and economically feasible in order to protect the value and use of surface lands. Some of the measures available for subsidence control include: (1) leaving sufficient original mineral for support; (2) refraining from mining under certain areas---or (3) causing subsidence to occur at a predictable time and in a relatively uniform and predictable manner. This specifically allows for the uses of longwall and other mining techniques which completely remove the coal." It is clear from the preceding statements that Congress considered the removal of all the coal and causing subsidence in a predictable and uniform manner to be equal to or better than leaving support coal and causing no subsidence. While the actual provision in the bills talked of preventing subsidence, a subsequent exception made it clear that the real intent was to control it. This is evidenced in the specific planned subsidence language that appeared in the bills and ultimately in Section 516 (b) (1) of PL95-87, the Surface Mining Control and Reclamation Act. It reads: "...adopt measures...to prevent subsidence ... except in those instances where the mining technology used requires planned subsidence in a predictable and controlled manner..." In spite of this historic record that supports controlled subsidence, something has been lost in the implementation. The primary focus by many regulators over the past few years has been on the "prevent" aspect irrespective of mining method being used. This has appeared to change the original intent of Congress from one of controlling subsidence to one of minimizing damage. The objective of this report is to review some of the current regulatory issues that impede full extraction mining and offer some suggestions on possible ways to deal with these issues.

"The demand for commercial and residential construction has continued to evolve across the United States over the last decade. Our markets demand larger, deeper and inherently more complex structures, located within increasingly constrained urban locations and with challenging geotechnical conditions. As project demands incrementally escalate, specialty contractors are continuously challenged to extend their state of practice. Development in the size and capabilities of construction equipment, tooling, techniques, monitoring and verification systems over the last decade has consistently allowed contractors to build larger and deeper. For support of excavation (SOE), in addition to scale, selection of the most appropriate system has been a key to success in the most challenging ground conditions. This selection process is based on sound engineering and construction practice, and also prior experience, both positive and negative. Ultimately many Owners have chosen to manage their risk of SOE construction through design-build contracts, which allows specialty constructors to optimize the balance of cost, schedule, scope and associated risk based on their local expertise. Successful implementation of a design-build methodology begins long before a contractor is selected, with identification of key project requirements and risk factors during the feasibility studies and design development phases. In these early stages of the project development, advice and budgets are frequently solicited from Specialty Contractors. The Owner, Construction Manager and Consulting Team will filter this input from multiple sources to provide their specific recommendations, develop construction documents and select the appropriate form of contracting. Only in the final stages, after award of a Design-Build Subcontract, can the successful specialty contractor directly influence risk management approach on the project. A case study is presented from Block 9 in San Francisco’s Transbay District. This project demonstrates the incremental identification, assignment and mitigation of construction risk through the planning and design build process for SOE on the deepest planned excavation in San Francisco.INTRODUCTIONThe mature urban development market in United States results in high demand for complex Support of Excavation (SOE) schemes on constrained sites. The Design-Build approach can be applied to increasingly difficult SOE challenges, combining regional specialty subcontractor knowledge and experience in order to optimize solutions for cost and schedule. The most successful applications will involve early engagement of shoring subcontractors within the project teams, during feasibility and design development stages, in order to identify key scope and risk issues, and their potential solutions. Throughout the project development, it is important to reasonably assign responsibility for construction risks amongst the project stakeholders who are best able to control them.This paper presents a case history for San Francisco’s Block 9, a well-planned and successfully executed design-build SOE project in San Francisco’s Transbay Redevelopment area. Throughout the 4 year period from the Developers initial proposal to the Redevelopment agency, through completion of excavation down to subgrade, the project team developed a series of increasingly refined SOE schemes, with corresponding scope definition and cost and schedule estimates. This program relied on early identification of key challenges and risks for the project, and evolved through multiple phases of proposals as first the General Contractor (GC) and then Specialty Shoring Subcontractor were engaged for the project. The scheme, cost and schedule refinements continued throughout construction in order to accommodate unanticipated conditions and events as they unfolded."

Current methods of determining jet grout column diameters are highly empirical and site specific, and do not take into account the jet hydrodynamics and soil parameters explicitly. A theoretical model which describes the jet excavation mechanism is introduced in this paper, with the aim of providing an improved prediction tool that can be used in jetting design. The model is based on the assumption that the velocity distribution in the cutting jet is equivalent to that of a free jet with boundaries corresponding to the dimensions of the cavity excavated in the ground. The shape of the cavity formed depends on the erosional properties of the soil and jet expansion is limited by the resistance at the jet-soil interface. The model predicts that the limit of jet penetration is reached when the average dynamic pressure at the jet tip becomes equal to the ultimate soil bearing resistance. The reliability of the model, evaluated using results from four case histories of jet grouting field trials, is within ±15%.

OVERPOLED copper, as commonly defined, is copper which has been excessively reduced during the poling period of the refining process. Owing to its porosity, such copper is unfit for commercial purposes. The cause of this phenomenon is undoubtedly the lack of oxides in the copper, yet this does not completely explain it. Refined copper of the same "pitch" and "set" varies considerably in oxygen contents, and analyses of overpoled bars also show similar wide differences; moreover, it is possible to melt copper in crucibles so as to obtain bars that are practically free from oxygen although they show a "tough pitch" and "level set" surface. Frederick Johnson advances the following theory: The regulation of the quantity of oxygen in copper is the keynote of all copper refining. It is the presence of a Well defined proportion of oxygen in the form of cuprous oxide, forming a copper-cuprous oxide eutectic, Which enables copper to be cast free from blowholes. It seems that When oxygen is present, other gases are less soluble in molten copper; When oxygen is absent, or is present in insufficient quantity, the copper dissolves other gases, such as hydrogen and carbon monoxide, Which are insoluble in the copper when solid, and, being rejected during the process of solidification, causes internal porosity and external ridges and excrescences.1 The "level set" of Wire bars, etc., is entirely the result of a compromise between the occluded gases and cuprous oxide. With excess of the former the metal would be "overpitch" and spongy, While an excess of the latter would result in the metal being "underpitch" and brittle.2 The result of the absorption and subsequent expulsion of reducing gases, carbon monoxide and hydrogen, and the effect of this action. on the overpoling of copper are in dispute; the literature on the subject is full of contradictory statements and will continue to be so until better analytical methods have been developed. Sulphur, however, is tangible. The refiner can see it in his button sample and consequently attributes to it any trouble that may happen during the melting of the copper.

Over the years as surface mining operations have become more sophisticated, angle drilling has become a more popular blast design technique, especially in areas where overburden and stripping ratios have increased. Another method of blast design is the staggered pattern, which greatly improves explosives distribution within the blast. While both methods present some engineering challenges, this paper will discuss how the two methods can be combined to enhance fragmentation, improve cast percentage, drill production ratios and highwall control. A practical engineering approach will be presented as well as a discussion on the discipline required to maintain a quality blasting plan.

"AEROPHINE@ 3418A is a unique sulfide mineral promoter produced by Cyanamid, based on phosphine (PH3) chemistry. It has proven effective as a collector for the sulfide minerals of copper and particularly lead (galena), as well as for secondary gold and silver values. AEROPHINE 3418A has widest application as a Cu and/or Pb collector for complex ores of Cu-Zn and Pb-Zn, which frequently have significant quantities of associated precious metal values. Advantages shown in the use of AEROPHINE 3418A are improved selectivities against Zn- and Fe-sulfides, increased precious metals recoveries, reduced total collector consumptions for Cu and/or Pb flotation, and increased flotation kinetics leading to higher recoveries of these values. Operating plant, pilot plant and laboratory test data from treating different ores are presented, demonstrating the advantages in use and preferred application of AEROPHINE 3418A.INTRODUCTIONCyanamid operates the only phosphine and phosphine derivatives plant in North America, located in Niagara Falls, Ontario. Specialty chemicals manufactured at this facility are used in many diverse applications, including solvent extraction reagents and textile flame retardants. A unique phosphine-based sulfide mineral promoter, AEROPHINE 3418A, is also produced. The origins of AEROPHINE 3418A promoter date back to the 1960's, when it was established that sulfide mineral collectors based on phosphine chemistry demonstrated efficacy. This new collector chemistry was found to provide flotation performance differing significantly from those of more traditional thiol collector chemistries such as xanthates, dithiophosphates, mercaptobenzothiazoles, thionocarbamates and those with xanthogen groups. Because of the lack of a universally accepted definition or standard for RCA, we are left to our own conventions in the marketplace as to what is most ?effective? or a ?Best Practice? for our own facilities or organization. This lack of uniformity in the application of RCA produces an equal lack of uniformity in the measureable outcomes of RCA on the bottom-line from site to site and industry to industry. What is the definition of success for an RCA? Is it regulatory compliance? Is it some quantifiable measure of bottom-line performance? Is compliance and bottom-line performance the same thing? Can we measure the ROI we receive by doing compliant RCA? These questions must be adequately addressed if any definition of RCA is to be effective on the bottom-line. Like our various Corporate Vision and Mission Statements, what would be our Vision and Mission for RCA? When pondering the answer to this question, we should consider: ? What Key Performance Indicators will the Success of RCA be Linked to? This provides a direct correlation to the corporate dashboard and therefore garners support for the RCA effort when implemented at the sharp end. Not until we know what KPI?s will effectively measure our maintenance and reliability objectives can we begin to establish which opportunities will afford the greatest returns1. ? What Bottom-Line Metrics will be Tracked to Measure the Effectiveness of Our RCA Effort? Will it be increased production, profitability, regulatory compliance, etc.? Will it be decreased injuries/fatalities, claims, customer complaints, risks, etc.? We should be specific about these metrics and their correlation to the metrics on our corporate dashboard. ? What Will Trigger an RCA Investigation? Normally companies will establish quantifiable thresholds to be surpassed before an RCA will be conducted. Examples may be an OSHA recordable injury, a failure event in which production losses exceed $250,000, an equipment failure in excess of $50,000 or a regulatory violation. This is the norm. However, we should realize this is using our RCA skills totally on a reactive basis. High Reliability Organizations (HRO) strive for proaction and they also use RCA proactively. Why can?t we use RCA to analyze why unacceptable risks from an FMEA are so high? It is perfectly applicable yet rarely implemented in this fashion.
