This chapter presents an overview of fatal and nonfatal injuries reported to MSHA during 1986-1995. Reportable injuries follow the criteria for reporting specified in 30 CFR 50. These criteria are listed in appendix A. The data used for these analyses came from the MSHA accident, injury, and illness data closeout files for the years 1986-1995. The MSHA data are described in appendix A and methods used in data analysis in appendix B. There may be slight discrepancies between the figures presented here and other presentations of MSHA data for the following reasons: (1) injuries to nonemployees have been excluded from analysis; and (2) injuries identified by MSHA after the files were closed out are not included. Within the MSHA data, there are differences in reporting requirements for operators and contractors. Contractors are required to report only those injuries that occur during certain categories of work (described in appendix A). For this reason, data on injuries among employees of operators and contractors in the MSHA database should not be considered directly comparable. As discussed in the "Introduction," the MSHA data on fatal injuries differ from the CFOI data (chapter 1) because of varying definitions used for the mining industry. As compared to the CFOI data, MSHA data exclude injuries that occurred off of mine property and include injuries occurring at mills and processing plants on mine property. Tables 4-1 through 4-9 and figure 4-1 provide comparisons of injury frequencies and rates among the commodities. These comparisons are followed by sections 4A through 4E, which provide more detailed tables and figures for each commodity separately. The method used to calculate injury rates is described in appendix B. Since denominator data for occupations (lob titles) were available for 1986 only, estimates of rates by occupation for the entire 10 year period are based on extrapolation from 1986 data. For this reason, estimates of injury by occupation should be interpreted with caution. Fatal Injuries A total of 1,078 fatal on-the-job injuries were reported to MSHA during the 10-year period 1986- 1995. The average annual fatal injury rate for all commodities combined during this period was 30.9 per 100,000 full-time equivalent workers. Mine operators accounted for 908 (84%) of the fatalities and had an average annual fatal injury rate of 28.2 per 100,000. While mine contractors comprised only 16% (n = 170) of the total number of fatalities, their average annual fatal injury rate was 2.3 times higher than that of mine operators (65.0 per 100,000). Table 4-1 presents the number and rates of fatal injuries by commodity for operators and contractors. Contractors in metal, nonmetal, stone, and sand and gravel are grouped together, since MSHA data on employment were not available for rate calculations for these commodities separately. The highest average annual fatality rate for the period, 66.5 per 100,000, occurred among metal and nonmetallic mineral mining contractors, followed closely by coal contractors (63.1 per 100,000). Among mine operators, the highest average annual fatal injury rate was in coal (38.9 per 100,000); the lowest was in nonmetal (14.3 per 100,000). Table 4-2 shows the average annual fatal injury rate by type of operation for each commodity for mine operators only. Of commodities with underground mines, all except stone had higher fatality rates at underground than at surface operations of these mines. The highest fatality rate for any operation occurred in surface operations at underground stone mines (124.5 fatal injuries per 100,000 workers). Stone had the second highest fatality rate for underground operations at underground mines (77.9 per 100,000). The highest rate for underground operations at underground mines occurred in metal (87.6 per 100,000). Table 4-3 shows the number and rate of fatal injuries by U.S. Bureau of the Census occupational groups (described in appendix D) for the 10 occupation-commodity groups with the highest fatal injury rates. These data should be interpreted with caution because the denominator used in calculating the rates was based on an extrapolation from data for the year 1986 only, as described in appendix B. Extractive occupations are represented in this group for three commodities: coal, metal, and sand and gravel. Construction trades are included for coal, sand and gravel, and nonmetal. The distribution of fatal injuries by commodity and work activity for mine operators is presented in table 4-4. A description of the work activity categories is in appendix C. Overall, the leading work activity at the time of injury was using or operating tools and machinery (26.4%) followed by constructing, repairing, and cleaning (24.0%), vehicular operations (18.8%), and materials handling (12.6%). The distribution varied by commodity. The largest percentage of fatal injuries occurred while operating tools and machinery in both coal and stone; during vehicular operations in metal; and during constructing, repairing, and cleaning activities in nonmetal and sand and gravel. Table 4-5 shows the distribution of fatal injuries by commodity and MSHA accident classification for mine operators. A description of the MSHA accident classifications appears in appendix E. This report (rind related Appendices) provides a description of the functional design specifications and related rationale for development of a training system for dragline equipment operators. Phase I activities involved a delineation and detailed analysis of specific behavioral requirements for successful dragline operation. Following the field data collection effort, job/activity/task descriptions were formulated into training requirements which can be achieved by a trainee with training hardware having specific functional capabilities. The training system analysis integrated the training requirements and the trainer functional capabilities into recommendations which configure a total training system. The objective of the training system described in this report is to provide new operators of dragline equipment with the requisite skills and knowledge to assure cost-effective development and an adequate supply of our human resources.

Tailings embankments can develop a high phreatic surface if the original design for the embankment does not include drainage facilities or if existing drainage facilities become ineffective. Subsequent raises can develop a seepage surface due to a high phreatic surface that can lead to stability problems. The U. S. Bureau of Mines amended contract J0100013 to initiate this study of horizontal drains in tailings embankments. This study developed a technique for determining the length of and spacing between parallel drains through the use of the finite element model UNSAT2. The analysis is based on the combined output of a vertical planar mesh and a horizontal planar mesh. The slot opening in the drain pipe and the length of the perforated section are determined from an analogy to water well design. The pipe diameter and gradient can be determined from Manning's equation. To the authors' knowledge, this is the first attempt at horizontal drain design using a finite element model. Additional research is recommended in this area.

The Bureau of Mines, United States Department of the Interior, has established a testing adit on the Government reservation at Mount Weather, Virginia, to facilitate research on the fundamentals of mining under actual metal-mining conditions. A number of sites in several Eastern States were investigated, but Mount Weather was selected because the rock seemed best suited for rock drilling and other mining tests and because the ground was already owned by the Federal Government, the site having been acquired in 1902 for use of the Weather Bureau. The testing adit has an advantage over an active metal mine for certain investigations because variable conditions affecting any particular research problem can be controlled and maintained in a fixed status, whereas in an operating mine this seldom is feasible. Operations were begin in September 1936, with a crew of four laborers under a resident engineer. The laborers were recruited from nearby farms, and although they had no previous mining experience, they have developed into satisfactory miners.

This work shall consist of furnishing and driving foundation piles of the type and dimensions designated on the plans or in the special provisions including cutting off or building up foundation piles when required. This specification also covers providing test piles and performing loading tests. Piling shall conform to and be installed in accordance with these specifications, and at the location, and to the elevation, penetration and bearing capacity shown on the plans or as directed by the Engineer. Except when test piles are required, the Contractor shall furnish the piles in accordance with the dimensions shown on the plans or special provisions. When test piles are required, the pile lengths shown on the plans are for estimating purposes only and the actual lengths to be furnished for production piles will be determined by the Engineer after the test piles have been driven. The lengths given in the Engineer's order list will include only the lengths anticipated for use in the completed structure. The Contractor shall, with-out added compensation, increase the lengths shown or ordered to provide for fresh heading and for such additional length as may be necessary to suit his method of operation.

The separation of certain minerals by electrostatic techniques can be difficult due to their similar electrical conductivity, and any technique to improve this can be useful in certain difficult separation applications. Triboelectric differences between mineral scan result in certain minerals acquiring small positive or negative charges prior to inductive electrostatic separation. By changing the polarity of inductive separation, relative changes in mineral recovery can be observed, which may be beneficial. In this brief study a JKTech mineral liberation analyser (MLA) was used to evaluate the effect of polarity change on selected minerals present in the Richards Bay Minerals deposit, and an approximate triboelectric series based on this behaviour is presented.

Instantaneous outbursts of coal and gas have occurred at various times in several working districts of Metropolitan Colliery, N.S.W.They have occurred in headings only, under both hand and mechanised conditions, and with seam gas ranging from mainly methane to nearly pure carbon dioxide. A modification of inducer shotfiring using a shear cut was introduced in 1955, and in 1961 full face firing both to a machine cut shear and off the solid was introduced. The current series of outbursts -1961 to 1963 - has been induced under these systems with full control of the time of outburst, but without prediction, and with some disappointing production features.Various aspects of current conditions and practices at Metropolitan are considered singular, such as the coarse granulometry of outburst coal, the prevalence of floor heave, the absence of strong structures at outburst sites, the possibility of outburst at any stage in pillar formation up to the last few yards, the effect of rate of advance on outburst occurrence, and the mildness of many outbursts experienced.It is concluded that inducer shotfiring is a reliable control but it suffers by comparison with outburst-free conditions, and that control by preyention would be preferable.INTRODUCTIONThe phenomenon of instantaneous outbursts of coal and gas has been described in detail previously (Hargraves, 1958). Briefly, it is a combined gas and stress hazard of deep coal mining, usually occurring in virgin work and during active face advance, when coal and seam gas burst from the face into the workings.Instantaneous outbursts have occurred sporadically at Metropolitan Colliery, N.S.W., since 1895. The objective of this study was to determine the magnitude and sources of environmental noise from mining operations near populated areas, to de-scribe monitoring techniques, and to identify applicable existing noise abatement techniques. To this end, noise surveys of a representative sample of mines and quarries were conducted. Noise levels at site boundaries and near specific equipment and ensembles of equipment were measured. Noise exposures in terms of annual day-night sound level were calculated for the boundary noise measurement locations. In addition, the relative sound level contributions were estimated for the mining equipment noise sources affecting the boundary locations. Noise abatement methods falling into three categories were analyzed: equipment practices, operational/administrative practices, and equipment quieting. Selected applications of noise abatement methods were evaluated in hypothetical situations with emphasis upon machinery selection and use. Procedures for measuring noise around mining operations were provided.

Paper presented at the conference on Economic geology in Europe and beyond II, models for mineral deposits in sedimentary basins, held in Keyworth, UK, 13-14 April 1994. Paper originally entitled: Processes in a newly discovered sulphide-PGE mineralisation in Norway. The Stormyrplutten mineralisation is hosted by a minor tholeiitic metagabbro of low-Ti composition intruded at a high level in the volcanic lithology. It is thought to be the subvolcanic counterpart of submarine basalts generated during an extensional episode of ensimatic arc evolution. The sulphide-PGE mineralisation occurs as stratabound pyrite, pyrrhotite and chalcopyrite disseminations with average grades of 376 ppb Pd, 313 ppb Pt, 40 ppb Au, 0.5% Cu and 0.08% Ni in a deposit 15-25 m thick. Locally the PGE content peaks at 2 ppm over 1 m. Microprobe analysis isolated 14 different platinum-group minerals, dominated by sperrylite, temagamite and borovskite and occurring as 1-20 micron compounds embedded in the main sulphide minerals or along the sulphide/silicate grain boundaries. Mantle-normalised plots reveal strong depletion in Os, Ir and Ru and enrichment in Pt, Pd, Cu and Au. The general decrease in Pd and Pt and increase in S up through the stratigraphy is similar to PGE distribution in the Darwendale subchamber of the Great Dyke in Zimbabwe, and may reflect strong fractionation of Pd and Pt in favour of the first droplets of sulphide melt; two reversals of the trend at the 4 and 12 metre levels represent replenishment with fresh magma and repetition of the cycle. In contrast to the Darwendale system, Cu and Ni also show peak concentrations in the first-formed liquids and Pt appears to have fractionated more strongly into the sulphide melt than Pd

Adequate load transfer is considered essential for optimum ground support and is increasingly important in weak roof and/or high stress environments. If adequate load transfer is achieved not only can this have a positive impact on the resulting roof behavior, but it may also allow the use of lower and hence more economic densities of roof support. A wide variety of post-groutable cable bolts are currently in use in the Australian coal mining industry, each of varying capacity, design, and grouting methodology. Though all of the cables are supplied with the appropriate steel specification, strength, and modulus data, the Australian coal mining industry does not have access to a comprehensive and comparative database on the corresponding load transfer performance, which is a critical problem. On the basis of the above, this paper describes the methodology and results of a laboratory pull testing program that aims to assess the impact of cable bolt design on load transfer, and specifically, the impact on the capacity and stiffness of the anchorage. As part of the testing program, 19 cables were tested including (1) plain-strand cables installed in holes drilled with 28 to 55 mm diameter drill bits and (2) bulbed and nutcaged cables installed in holes drilled with 42 to 64 mm diameter drill bits. Other parameters assessed included the significance of smooth and profiled wire, wire packing, and expanded geometry bulbs. The main results of the testing indicate that in comparison to plain-strand cables, if the cable is bulbed or nutcaged, the capacity of the anchor can increase by up to 400%, and the peak stiffness by between one and two orders of magnitude. The results also indicate that increasing hole diameter may have a positive impact on load transfer in the case of bulbed or nutcaged cables and a negative impact on plain-strand cables. No conclusive results were obtained with regard to the impact of grout strength on load transfer.

From October 2000 to April 2001 a local array of fourteen seismograph stations was deployed at the Trail Mountain Mine (TMM), an underground coal mine in Emery County, Utah, to locate mining-induced seismicity (MIS). Original analysis of the data provided epicentral locations that coincided closely with the active longwall panel. The original analysis yielded poor quality hypocenters, however, preventing a useful relation from being made between mining activity and the event depths. For the original analysis, only stations at elevations above mine level were used for locating MIS. As a result, P- and S-arrival times from four of the stations in the local array were not used: an inmine station and three stations at elevations below mine level. This study compares mine workings and active mining to a revised TMM dataset that included P- and S-arrival times from the lower elevation stations that were not used in the previous study. In the revised locations, a modified master event methodology was used to calculate station delays. These station delays correct for errors in the shallow layers of the velocity model. The resulting hypocentral locations have improved depth resolution, determined by a lower root mean square (RMS) for the locations, compared to the original study. Most of the events occur in the roof of the mine and follow the longwall panel. When located using the velocity model determined in the original study, the elevations of the events increase as the longwall panel progresses. One possible explanation for the elevation increase is that it may be related to geology or the caving process. As presented here, a 10% decrease in the shallow velocity structure results in a more plausible location for the hypocenters. Consequently, it is hypothesized that progressive development of the TMM longwall panel(s) may have resulted in a progressive change in the velocity model.

The following minerals found in the residual black sands from gold mining in El Bagre Antioquia (Colombia) were identified as having major economic interest: Ilmenite (FeTiO3), with an average of 49% of TiO2, frequently altered presenting crowns; lamellaes of rutile and alteration textures; titanomagnetite with up to 25% of TiO2; magnetite (Fe3O4) in individual grains or associated with others minerals as hematite and ferromagnesian silicates rutile (TiO2) with considerable quantities of Nb2O5(0.1-3.7%Wt); zircon (ZrSiO4) with 66%Wt of ZrO2 and monazite with an average of 63.43%Wt of REE, this frequently associated to xenotime and thorite forming textures as little drops. Analysis in EPMA and SEM/EDS showed percentages of minor elements and traces, thereby clearly establishing the applications of these minerals in diverse industries. Furthermore, a detailed granumetric study of the different minerals and textural relationships among them also allowed for establishing the basis for possible processing and exploitation.

In mid-1991, the design/build contractor set to work on the Pergau HEP in a remote corner of Malaysia, armed with only a slim volume of conceptual drawings. The next five years saw roads, power lines, buildings, dams, and over 30 kilometers of tunnel constructed, followed by two years of E&M installation and commissioning. Along the way, the project overcame geothermal groundwater, faults, land disputes, monsoon rains, marauding tigers, and those most fearsome of foes, politicians and the press. This paper is a testament to the ingenuity, commitment, and persistence of the construction industry professionals who brought this massive project to fruition.

Halliburton Services and Brown & Root have developed a unique foundation system that blends oilfield and construction technologies to construct deep foundations which offer significant advantages across a variety of soil profiles. This highly efficient system is being developed to meet the construction industry's needs into the 21st Century. Based on a concept initially introduced to Halliburton Services by V.L. Schellhorn in 1987, Halliburton combined their existing cement slurry mixing and pumping technology with oilfield drilling practices and a new tool design to field test the viability of the concept. Results looked encouraging, and Halliburton asked it's sister company, Brown & Root to evaluate and further develop the process. With Brown & Root's knowledge of the construction industry, the original concept has developed into a viable and efficient foundation construction system.

The backfilling of underground stopes is a common practice throughout the mining industry. The stresses imposed by the backfill on the surrounding rock mass need to be assessed because of their influence on ground stability and on the hydro-geochemical response of the fill material. There is, however, a complex interaction between the relatively soft backfill and the much stiffer adjacent rock mass. In recent years, the authors have presented results from investigations conducted on stress distributions (obtained numerically and analytically) in and around backfilled stopes with (sub)vertical walls. In this paper, the influence of stope inclination and of backfill properties is evaluated. Results indicate that, in general, the vertical and horizontal stresses in inclined stopes are close to, but somewhat smaller than the stresses in vertical stopes. Nonetheless, significant differences can be observed in some cases, especially for the vertical stress along the inclined central line and along the hanging wall. It is further shown that neglecting deformability in limit equilibrium analyses is an acceptable assumption. However, Poisson?s ratio and the dilatancy angle, usually neglected in limit equilibrium analyses, may have significant effect on the stress distribution. Results also confirm that the backfill strength parameters (cohesion and friction angle) are critical factors required to properly evaluate the stress distribution, at least within a certain range of parameter values.

Spontaneous combustion of coal has been studied by laboratory techniques using an apparatus in a small-scale laboratory testing. Coal?s inherent characteristics can be measured using Differential Thermal Analysis (DTA) and Crossing Point Temperature (CPT) methods. The DTA curve obtained after testing is compared with actual inherent moisture and its effect on spontaneous combustion has been investigated. It has been found that the Wits-Ehac Index increases as the inherent moisture of the sample increases. Therefore it can be deduced that liability to spontaneous combustion increases with increased inherent moisture. In addition, volatiles have a catalytic affect on self heating immediately after the inherent moisture is reduced. This is identified as stage two in the thermogram. The inherent moisture has a cooling effect during the initial stages of testing and is visible in the DTA thermogram. In addition, while an increase in carbon content means an increase in Wits-Ehac Index value, an ash content decreases the likelihood of on spontaneous combustion. A three-dimensional finite element analysis was performed of an instrumented section of a double-entry coal mine in order to provide a comparison between field measurements and existing analytic techniques. The BMINES computer code was used to perform the analysis. The sequence of excavation of the entries was considered, as well as the installation of rock bolts and cribbing during the mining cycle. The materials In the region under investigation were idealized into four rock types and coal. The rocks were assumed to be isotropically elastic, perfectly plastic. Coal was represented by an elastically anisotropic (transverse isotropy) and plastically isotropic model. A fracture yield criterion of the Coulomb form was used for all materials. The strength and stiffness of all materials were substantially reduced from available laboratory data to account for in-situ properties. Qualitative trends and stress distributions obtained from the calculation were found to be reasonable. However, computed displacements and closures were greatly exaggerated and an excessive degree of inelastic deformation was indicated in the coal, especially adjacent to the excavated faces. It was established that both these effects were the result of an excessive reduction in the strength and stiffness of the materials. The analysis underscores the need for three-dimensional calculations by identifying a number of important structural response parameters that would be either outside the scope of, or misrepresented by, two-dimensional analyses. The analysis also emphasizes the need for comprehensive and accurate measurement of material properties in order fully to exploit the power of the analytical techniques to generate reliable predictions.

Investigations by the Federal Bureau of Mines of methods of upgrading tar obtained by low-temperature carbonization of lignite showed that ß,ß?-oxydipropionitrile, ß,ß?-iminodipropionitrile, and ß,ß'-thiodipropionitrile were effective solvents for extracting aromatics (aromatic hydrocarbons plus polar compounds) from coal-tar neutral distillate fractions. Batch extraction showed that these solvents extracted approximately the same percentage of aromatics from various neutral oil fractions boiling between approximately 170° and 350° C. In continuous countercurrent extractions using oxydipropionitrile and iminodipropionitrile, approximately 30 to 35 vol.-pct, and 65 to 70 vol.-pct, of the aromatics in 170° to 350° C. neutral distillate oil were extracted at room temperature when solvent-to-oil ratios were 2:1 and 12:1, respectively. As the concentration of aromatics in the neutral distillate oil fraction decreased, the capacities and, to some extent, the selectivities of these solvents for the remaining aromatics decreased. Aromatics were isolated from the extracts and the solvents recovered by washing the extract fractions with a low-boiling paraffinic hydrocarbon such as heptane, using hydrocarbon-to-extract ratios of at least 2:1. The heptane wash removed most of the aromatics but only part of the polar compounds (compounds containing oxygen, sulfur, and nitrogen) from the extracts. The recovered oxydipropionitrile and iminodipropionitrile, containing polar compounds, were effective in extracting aromatics from neutral distillate oils. The contaminated solvents showed essentially the same capacities and selectivities as the pure solvents.

A newly developed chemical conditioning technique, which greatly simplifies the preparation of plating baths for the electrodeposition of titanium diboride (TiB2) coatings, is described in this report. The new plating technique was used to prepare samples of titanium diboride (TiB2) coatings on three types of substrate materials. These samples were then evaluated in both low- and high-speed wear-test programs to determine the wear resistance, microhardness, and adhesion of the coatings. In general, it was found that the wear resistance of the coatings is independent of substrate. Scanning electron micrographs have shown that the coating wear surfaces are very smooth, suggesting slow and continuous removal of the coating during the wear test (that is, adhesive wear). Adhesion between the coating and all substrates was excellent. Microhardness results show that the TiB2 coating is harder than alumina, with a hardness value exceeding 5,000 kg/mm2 measured in one test. The results of the wear tests show that TiB2 coatings have relatively poor wear resistance compared with alumina in low-speed unlubricated sliding environments, but exhibit wear properties comparable with alumina in high-speed unlubricated sliding environments.
