But what is a “world class” standard and how can it be achieved? Is there a yardstick by which an industrial organization can measure the relative performance of its maintenance activity and compare it with performance levels that universally identify “world class” status? In 1995 I was asked to participate in a litigation matter between a corporation and its insurance carriers concerning insurance coverage for remedial costs at a large, environmentally contaminated closed-down former industrial facility. One of the buildings on this site became known to the lawyers representing the company as “the infamous Building 8”. This building housed millions of documents retrieved from various locations at the facility while it was being shut down. It became apparent that the facility management had no document destruction policy, and thus Building 8 contained internal memoranda, notes, copies of correspondence, permits, operating records, inventory records, and the like from fifty years of operation. Needless to say, some of these documents, all of which were subject to discovery by the insurance companies in the litigation, contained material that was less than helpful to the company’s case. Of course, there were also a few helpful documents, but the overall usefulness of these records was outweighed by the negative impact they had on the conduct of the litigation. A second example involves e-mail. Several years ago, acting on an anonymous complaint that we later discovered was made by a disgruntled former employee who had been fired, a small army of federal environmental investigators descended on a client’s operating facility and, armed with a search warrant, literally commandeered the facility’s EH&S department. The investigators impounded the computers of the EH&S personnel and extracted all of the data from the hard drives of those PCs, as well as all EH&S related files from the hard drives of several servers used by the facility. These computers contained many years’ worth of stored e-mails, draft letters and memoranda, and other documents that provided grist for a four and one-half year criminal grand jury investigation of the facility and its EH&S staff. Both of these companies would have benefited from following a rigorous, enforceable, corporate records policy. Here is a third problem. My client had, during the 1960s, made a brief, and unprofitable foray into the nuclear fuel processing business. It ultimately sold the subsidiary, and was forced to give the purchasing company a broad indemnity against future liability. As might be expected, years later the facility was found to have considerable soil and ground water contamination both by chlorinated solvents and low-level radioactive material. Several years into the remediation program, my client discovered a tractor-trailer load of files, apparently removed from the subject facility, sitting in a parked trailer in a company-owned lot 300 miles away from the facility, where they had been for nearly thirty years. These documents were not helpful to the client, but had to be reviewed, at considerable cost, by paralegals. Moreover, several of the boxes turned out to have been mildly radioactive, setting off a chain of health and safety issues that consumed tens of thousands of dollars of legal expenses before they were resolved. Today, formal, enforceable corporate records management programs have become an extremely important element of overall corporate management. What was for most of the last century little more than a low level warehousing function is now a critical, actively managed, operating function that affects every level of corporate management. The rapid assimilation into corporate culture of electronic document management and the prevalence of e-email as the primary means of internal communication have created records management complexities the likes of which were not even conceived of fifteen years ago. Record preservation and destruction are no longer matters that can be handled on an ad-hoc decision making basis. Because of regulatory requirements affecting various classes of documents and the litigation consequences of ad hoc retention and destruction of corporate business records, and the need to have the ability to quickly retrieve documents in either a litigation or business negotiation context, a set of records management norms have evolved over time that have become the core elements of virtually every corporate records program. To reduce fire hazards from spontaneous combustion of coal in longwall gob areas, a series of computational fluid dynamics (CFD) simulations were conducted by the National Institute for Occupational Safety and Health (NIOSH) to model the spontaneous heating of coal in longwall gob areas. The previous modeling results demonstrate that spontaneous heating of coal usually occurred behind the longwall shields and along the face with a bleeder ventilation system. Assuming a stationary longwall face, the spontaneous heating could turn to a spontaneous fire in several days for the most reactive coal under favorable conditions. When the longwall face advances, the spontaneous heating process will be significantly affected. In this study, the effect of longwall face advance on the spontaneous heating in the gob area is investigated using the CFD model developed in previous studies. One longwall panel with a bleeder ventilation system is simulated. The width of the panel is 300 m (984 ft), while the length of the panel is changed between 1,000 to 2,000 m (3,280 to 6,560 ft). The same permeability and porosity profiles are used for gobs with different lengths. The spontaneous heating first develops in the gob when the longwall face is stationary. Then, the face advances at a certain rate. The face advance is simulated as a series of discrete movements, and the effect of the face advance on the maximum temperature developed during the face stoppage is examined.

Paul Y. – SELI Joint Venture had been contracted to build the longest section of the Strategic Sewage Disposal Scheme (SSDS) – Stage I. This involved two tunnels under urban and marine areas excavated by high performance hard rock open type TBMs operated from deep shafts. A specific ground treatment program was developed using advance probing and grouting to control water inflow into the excavations. The article describes: the technologies adopted, the experienced problems, the encountered productions and their interrelation with the water inflow amounts, the possible improvement in the technologies for the excavation of the future stages of the same project as well of any other project under similar conditions.

Lime addition to acid mine drainage waters (AMD), possibly accompanied by aeration, has been the basis for their remediation by precipitation of predominantly metal hydroxides. An efficient novel alternative technique causes precipitation of double M(II)-M(III) hydroxy sulfates which readily precipitate and are easily dewatered. When the Fe(II)/Fe(III) + Al(III) ratio of the AMD is appropriate, the Green Precipitate (GP) Process provides a more effective alternative to normal liming/aeration. The process can be adapted to give a continuous in-line treatment by the choice of suitable computer-controlled reactors designed to improve the liming efficiency, reduce scaling and control the rate of treatment. The processes are described in terms of the chemistry involved.

Clean Diesels fuel-borne catalyst, Platinum Plus, helps improve fuel economy for all engines up to 10 percent while reducing particulate emissions by up to 25 percent. When used with a simple diesel oxidation catalyst, particulate emissions can be reduced by 30 to 50 percent. The use of Platinum Plus enhances combustion in the cylinder, burning fuel more completely. This reduces CO2 and other greenhouse gases and lowers total operating costs by improving fuel economy.

Multiple Criteria Decision Making (MCDM) is one of the most considerable branches of Operation Research. MCDM refers to making decisions in the presence of multiple, usually conflicting, criteria. The problems in MCDM are classified into two categories: Multiple Attribute Decision Making (MADM) and Multiple Objective Decision Making (MODM). However, very often the terms MADM and MCDM are used to mean the same class of models and mostly confused in practice. Usually, MADM is used when the model cannot state in mathematical equations and otherwise MODM is used (Hwang and Yoon, 1980). Decision-making (DM) is extremely important stage in Mining Engineering like other engineering branches. Every mining engineer makes several decisions in daily mining applications. Most of the decisions made by mining engineers are completely instinctive according to their past experiences and there is not any DM method applied in DM process. Analytic Hierarchy Process (AHP) is one of the well-known techniques solving the decision problems. The AHP is very simple, and understandable method especially non-numerical data considered in DM process. Equipment selection is one of the prominent problems of mining engineering. Mining engineers have to make difficult decisions when equipment selection stage. As the decisions in the equipment selection have radically influenced the economic life of any mining scenario, these are considered as complex MADM problem. So, the decision maker can evaluate the subjective criteria in the problem of equipment selection. The decision maker wants to maximize more than one objective criterion for equipment selection stage. Among the number of alternatives, the most suitable equipment must be selected according to objectives and alternatives. MADM applications can help the decision maker to reach the optimal solution for the equipment selection. In the mining industry, MADM methods can be applied for the equipment selection because it is a process that includes subjective and objective criteria affecting the selection of equipment among the alternatives.

Ilmenite smelting furnaces must be operated with a freeze lining of frozen slag to protect sidewall refractories from chemical attack by liquid slag. The process has two main control objectives: Firstly the slag TiO2 content must be controlled according to specification, and secondly a safe freeze lining thickness must be maintained. This text only concerns itself with the second objective. Freeze lining thickness cannot be measured directly due to conditions inside the furnace. It is therefore inferred from signals of thermocouples installed into the furnace sidewall. These signals are used as the 'controlled variables' in the freeze lining thickness control loop. The dynamic characteristics of these signals are the focus of this text. During this study a model of the process was constructed to study interactions between the slag bath and the freeze lining. This model was used to conduct simulation-type experiments to study the influence of various parameters (temperature, slag composition, ilmenite feed rate, reductant feed rate, power input, etc.) on the slag bath, freeze lining, metal bath, furnace wall and other parts of the process. In the case of the furnace wall, the modelling results were first plotted in the form of time-dependent isotherms. The dynamic thermal behavior of the furnace wall became evident from these results. To illustrate the dynamics of sidewall thermocouple signals, the results were also plotted as time lags between events occurring in the slag bath, and the time when a thermocouple registered a temperature change. The events studied included sudden changes in slag bath state (composition and temperature), changes in operating setpoints (ilmenite feed rate, reductant feed rate, power input), and operational errors (complete loss of reductant or of all feed). All of the result sets showed significant lags between occurrences in the slag bath and temperature changes in the refractory wall. The delays are such that it appears impractical to rely on sidewall thermocouple signals for warning of impending damage to the refractory wall, or for purposes of actively controlling freeze lining thickness. Some measures are suggested to improve the usefulness of these signals during furnace operation.

KEECO has developed the Silica Micro Encapsulation (SME) technology for the treatment of acidic, metal-laden waters. Utilizing a silica-based chemical formulation that entails chemisorption and micro-encapsulation of heavy metals in a permanent silica matrix, encapsulated particles settle quickly and are effectively isolated from the surrounding environment, producing a stable sludge that is characteristically non-hazardous. In a number of independently-verified pilot and full-scale applications (including the Bunker Hill Mine, Leviathan Mine, Wheal Jane Mine, and Meldon Quarry), SME has been proven highly effective. The technology has been featured in a number of mining publications and selected for innovative technology awards.

In concrete lined tunnels, formwork removal timing may be controversial. Contractors seek to strip the form as soon as possible, and designers seek to keep the formwork in place as long as practical to protect the fresh concrete. Formwork removal timing is normally specified as a function to its insitu concrete strength. This approach may work for smaller diameter tunnels, but fallacy occurs when applying this approach to larger diameter tunnels, since a larger diameter would experience more displacement than a smaller diameter tunnel from its own weight. Excessive deformation may fail a tunnel to meet its roundness criterion and overstress the lining causing cracking or falling from the crown. This paper presents a contractual mechanism to mitigate this issue.

On July 24, 2002, a nonfatal entrapment accident occurred at the Quecreek #1 Mine, Black Wolf Coal Company, when an active mining section inadvertently cut into a flooded abandoned mine. This incident, and the dramatic rescue which ensued, brought national attention to a common problem in the coal mining industry. Since 1995, there have been more than 100 inundations at coal mines in the United States. Unavailable, inaccurate, or incomplete mapping of older abandoned mines is typically responsible. Serious problems associated with accidental discharges of fluid mine waste material (slurry) and water from coal refuse disposal facilities have resulted from similar causes. Recent problems related to the inaccurate location of old abandoned mine works have increased the interest in the use of conventional and emerging drilling and geophysical techniques for detection of air-filled and water-filled cavities. The United States Congress has committed significant funding toward investigating the applicability of these technologies. MSHA actively promotes the use of the best available technology to minimize the significant adverse safety and environmental consequences associated with unplanned breakthroughs and inundations. This paper provides background information and a general summary of the MSHA-sponsored field demonstration projects which are currently in progress. Potential benefits and limitations are discussed. The need for field verification/ground-truthing is emphasized. Under appropriate conditions, geophysical methods can be effectively integrated into a comprehensive field exploration program.

The Mine Safety and Health Administration (MSHA) conducted a respirable dust survey to evaluate the effectiveness of a water curtain utilized for dust control on continuous miner sections. The purpose of this survey was to determine if the water curtain was effective in reducing respirable dust concentrations, since several mines currently use water curtains to control miners? respirable dust exposures. The survey was conducted on a supersection of an underground coal mine, which utilized two continuous miners on the same split of air. Each continuous miner alternated taking a cut so that both continuous miners would not operate at the same time. The underground coal mine is located in Knox County, Indiana. The water curtain evaluated in this survey was located in the last open crosscut between the intake side continuous miner and the return side continuous miner. When the intake side continuous miner cuts coal, its exhaust air would be used to ventilate the return side of the section. The intent of the water curtain was to reduce the intake air respirable dust concentration (generated by the intake side continuous miner) before the air stream ventilated the return side of the section. To determine the effectiveness of the water curtain in reducing respirable dust concentrations, respirable dust sampling pumps were placed on both the intake side and the return side of the water curtain. Additional respirable dust sampling pumps were placed farther downstream from the water curtain to monitor dust levels downstream of the water curtain, but in an area where respirable dust would not be created by anything other than the intake side continuous miner or additional section air. The water curtain and the respirable dust sampling pumps were operated only when the intake side continuous miner was cutting coal. The samples were analyzed for respirable dust and quartz content. The water curtain showed minimal effect in controlling respirable dust and respirable quartz dust, but the reduction was so negligible that this survey could not quantify the reduction. The measured reductions are within the accuracy of the sampling equipment.

Agenda 21 calls on the countries on national level as well as governmental and non-governmental organisations on international level to develop a concept of sustainability indicators to measure the progress towards sustainability. This paper deals with the development of an evaluation system for sustainability in the minerals extraction. By their application, aspects of sustainable development in the raw minerals industry are to be systematically conceived and assessed. This project has been carried out on behalf of the German Federal Ministry of Economics and Labour."

The International Tunnelling and Under-ground Space Association (ITA) is a nonprofit organization that represents all aspects of the tunneling and underground space industry worldwide. The United States has had a prominent role in ITA since its 1974 inception. ITA was formed as a result of a worldwide survey conducted in the early 1970s by the Organization for Economic Cooperation and Development (OECD). Bill Lucke was in charge of tunnels for the U.S. Federal Railroad Administration and was a member of the organizing committee that created ITA. He became a vice president of ITA. Jack Lemley served as President of ITA from 1983 to 1986. Dick Robbins was very active in ITA and served for many years both as a member of the Executive Council and as First Vice President. Harvey Parker has served as President of ITA for the last three years and will serve as Past President for the next three years. Moreover, there are many individuals and companies from the United States that have contributed significantly to the activities and the success of ITA. There have been several active Working Group Animateurs from the U.S. and there have been significant contributions from many individuals as well as companies from the U.S. Several engineers from the U.S. are currently serving ITA in a leadership role. The Underground Construction Association of SME (UCA of SME) is the official organization representing the U.S.. SME Executive. [ ] Director Dave Kanagy and UCA President Tom Peyton were instrumental in the U.S.?s role at ITA during the last few years. Peyton has been the official delegate to ITA. Henry Russell and Christian Ingerslev are both Animateurs of ITA working groups. This article will introduce ITA and its activities. A subsequent article will describe the ITA in more detail and will discuss the significant role the U.S. has played in the development and success of ITA.

The Old Hickory heavy mineral ore body, which lies within the Fall Zone of south central Virginia, has been mined since 1997. There are many differences in this deposit when compared to humate-laden mineral sands mines of southern Georgia and Florida. These differences include an abundance of kaolin-rich clayey sands, highly concentrated and iron-colored heavy mineral sand units, and a base of mineralization that is undulating. For these reasons, the selective approach of dry mining with track excavators is ideal. Grade control in this deposit uses traditional panning techniques and requires an intimate working knowledge of near-by drilling data. Mine geologists train equipment operators to recognize the base of mineralization by such characteristics as a stark color change, significant changes in grain size, and/or considerable, recognizable shifts in the concentration of heavy minerals. The regular pattern of mine site drilling does not always prepare the geologist and digging crews for occurrences of near-vertical contacts between the saprolite basement and the ore sands. These deformed areas, combined with undulating and pinching ore zones, require close monitoring and flexibility by the geologist and the mine operators.

Limestone-based material shows great promise for removal of arsenic from water. In laboratory tests, it has removed more than 95% or arsenic and more than 99% of cadmium and lead. In addition, the waste material appears to be suitable for recycling or disposal in an ordinary landfill. However, the efficiency of limestone for arsenic removal could be improved. Recent research has shown that iron treatment of limestone can enhance its arsenic-removal efficiency greatly, increasing its attractiveness as an alternative to other methods.

The U.S. cement industry, including Puerto Rico, again set a record in terms of production and shipments. The increase in shipments came primarily from imports of portland cement as domestic production from produced and imported clinker gained only 1.9 Mt (2.1 million st). Clinker production as reported to the U.S. Geological Survey (USGS) was 88.6 Mt (97.6 million st), an increase of 1 percent. Gray and white portland cement production from domestic and imported clinker, estimated at 93.5 Mt (103 million st), was up 1.9 Mt (2.1 million st). Masonry cement shipments of 5.5 Mt (6 million st), which equates closely to production, was up 6.3 percent. Blended ce-ment shipments were up 35.9 percent from 2004 at 2.35 Mt (2.6 million st).

The importance of high quality underground mine ventilation is readily apparent. The principles of primary and auxiliary mine ventilation, however, are sometimes less readily apparent. The first article in this two-part series discusses some general aspects of primary and auxiliary ventilation. Next month's article will detail considerations involved with auxiliary mine ventilation.
