During the 1980s and early 1990s, the South African coal industry experienced a series of methane explosions in which well over 200 coal miners lost their lives. In addition, a fire and an explosion in the gold mining sector claimed a further 240 lives. These tragic incidents resulted in the establishment of a research facility at Kloppersbos dedicated to explosion and fires research and the funding of an extensive research program, and almost certainly influenced the government of the day in its decision to establish, in 1993, the Leon Commission of Enquiry into Mine Health and Safety. The results of this focused research program, the wide-ranging recommendations of the Leon Commission and the determination of the coal mining industry to improve its safety record all combined to result in the past 20 years without a major explosion.This paper lists some of the research outcomes and, through three case studies, illustrates what can be learnt from investigations following explosions. The paper concludes with an evaluation of some of the findings of the Royal Commission on the Pike River coal tragedy. Many of the Commission’s conclusions are pertinent and relevant to the coal mining industry worldwide and their implementation could significantly improve coalmine safety; however, a careful study of the available facts indicates there could well be an alternative explanation of the source of the methane, the location of the ignition and the size of the explosion at the Pike River coalmine.CITATION:Phillips, H R, 2015. Six recent case studies were selected based on the eight main causes of mine disasters (Table 1; Quinlan, 2014) and where credible information sources, such as Royal Commission Reports, Coronial Investigations and textbooks, were available. Upstream embankment raises often represent the lowest cost option for increasing the capacity of tailings storage facilities (TSF), but also pose the highest stability risk due to constructing on top of previously deposited tailings.This paper presents an overview of key aspects in the geotechnical investigation and design of upstream raises and some recent lessons learnt from experience across a number of TSFs in Australia. A summary of recommended field investigation and laboratory testing methods is provided with a focus on collecting quality information for design purposes. A strong focus is given in developing an accurate assessment of pore water pressures, which, if not correctly understood can have disastrous consequences on tailings strength parameter interpretation and wall stability assessment in static and post-seismic conditions.The assessment of the contractive or dilative shearing behaviour of the tailings material and rigorous interpretation of design characteristic values are presented. Today, I would like to talk about ways to maximize the efficiency of surface coal operations. I doubt I will present any startling new ideas, but my comments may, hopefully, stimulate some conversation today and later back at your own operations. Strip mining is a very old technique used by ancient societies. Only with the coming of the machine age in the early twentieth century did we attempt to increase the magnitude of our operations. The industry, as we know it today, really only got started in the early 1900s with the advent of steam shovels. In the first portion of this presentation, I will attempt to show that we have, in many cases, not changed the basics of strip mining, only the scale. In doing so, quite often we have complicated the operations to the point of being unmanageable and unprofitable.

(Matt. 23:23) I want to talk about "The Weightier Matters of Law." In a situation where big government is trying to have an effect on even small details of a work activity, otherwise simple situations become complex. Regulations are written to such detail that constant compliance with strict interpretations can be a nightmare-not to mention off the target of the intended purpose for the regulation. One soon recognizes the onerous characteristics of federal statutes. Another admonition the Lord gave those lawyers and pharisees back then was that they tended to strain at a gnat and swallow a camel! He meant that the pharisees were given to devoting themselves to the strict interpretation of law by writing hundreds of subrules or interpretation to law. In doing so they eventually lost sight of the original purpose of the law. I think they were being told, keep the law-but don't neglect to humanize it Keep it simple Recognize the purpose of why the law. "Mix in" a common sense approach. Someone once said, "Common sense is instinct -enough of it is genius." Today, as 2000 years ago, we need more "genius" displayed by legislators in considering the weightier matters of law. First of all true legislature genius calls for a declarative statement about the purpose of the statute and a fair, objective analysis of the ground rules by which it will be implemented. In the Health & Safety Act of 1969, the "findings and purpose" as set forth in Section 2 do define its purpose. I believe most of us affected can be in accord with the reasons advanced for the legislation. However, beyond those well-intentioned few paragraphs, we very quickly run into trouble. Congress complicated the simplicity of the statement of purpose by mandating government intervention into areas that cannot be effectively legislated, nor even should be legislated. The Act provided for broad gaged regulation which is unnecessary, inappropriate, and tremendously costly. Frankly, it detracts from a constructive, cooperative effort by the parties involved. These are strong negatives which characterize inappropriate legislation. Appropriate legislation and subsequent regulation, which is realistically conceived and practically enforced must be born of an honest desire for improvement, based on a working knowledge of common problems which are thoughtfully analyzed. Then the legislation must be written in clear concise language so that there can be little or no doubt as to its literal meaning and intent. To be practically enforced, it must be understood by all who will enforce it and by all who are subject to it. That is to say, an effective law or standard cannot be fixed or inflexible so that it cannot be tempered in human judgment. There must be an elasticity in the statute to provide for consideration of all of the relevant facts of a given situation. Such elasticity makes the compliance effort as meaningful as it can be for the time and dollars invested. Meaningful compliance requires room for common sense. Today, we in business along with our associates in government and labor, urgently need to pause, and assess our current situation. The problems arising from the 1969 Act have been intensified through the Mine Safety & Health Act of 1977. After reading the "findings and purpose" of this legislation, and by process of elimination, we should all clearly see what is and is not the purpose of the Act. 1) It is not to turn the management of private industry over to government. 2) It is not to impair the efficient operation of business. 3) It is not to reduce productivity and profits. 4) It is not to reduce national unemployment. 5) It is not to perpetuate the bureaucracy. 6) It is not to increase the tax burden on an already overburdened people. 7) It is not to increase our balance of payments deficit. 9) It is not to replace management/labor relations in working together to solve problems.

I chose the title "Let's Clear the Air" for this afternoon's presentation because it has two meanings, the literal and the figurative. Most of you know what MSHA is. Most of you know what MSHA does. And, for the most part, most of you know why. What I hope to accomplish today is to "clear the air" around our health standards, to clarify for you how MSHA views its congressional mandate under Title I Section 101(a)(6), (7), and (9) of the Mine Safety and Health Act. Asbestos, silica, radiation, noise, welding fumes, carbon monoxide, lead, hydrogen sulfide, nitrogen dioxide - the list of physical and chemical hazards encountered in the metal and nonmetal mining industry is as diversified as the industry itself, possibly even more so. In addition, miners are often exposed to several of these health hazards simultaneously.

The public relations problems of users of explosives have greatly increased within the past few years. One reason is that explosives and blasting agents are being consumed in increasing quantities. The principal reason, however, is that more people have become physically affected by blasting. Mines, quarries, and construction sites that were formerly in relatively remote locations now find themselves near congested areas. New express traffic ways being constructed to connect the suburbs with downtown areas are being built through the middle of long-established business and residential neighborhoods. The public becomes aware of blasting by two physical sensations-vibration and noise. Vibrations are usually so slight as to be negligible, and in most cases the only sensation noted is noise. The reduction of vibrations to this low level has been primarily achieved by the use of blasting practices and explosive products recommended by vibration engineers based on seismograph recordings. Unfortunately, very little scientific attention was given to the accompanying physical sensation-noise.

Bateman Engineering managed the establishment of a second diamond treatment plant at the Letseng Mine in the Mokhotlong District of northern Lesotho, about 100 km from the town of Buthe Buthe. Letseng Diamonds Proprietary Limited, is owned 70 % by Gem Diamonds and 30 % by the Government of the Kingdom of Lesotho. The second plant, which commenced construction in late 2006, doubled Letseng Mines' hard rock processing capacity from 2.6 million t/yr to 5.2 million t/yr, making it the worlds' seventh largest diamond mine by throughput. Bateman Engineering provided the engineering design, procurement and construction management of the new plant.

Your article on page 202 of the August 1993 Journal refers. I was surprised to read that this grant was first introduced in 1984 as it was awarded to me (albeit under the auspices of The Institution of Mining and Metallurgy, London) as long ago as 1964, and I spent two very profitable months in Sweden at that time. Training in job-skills * A South African mine is leading the way in providing alternative job-skills to staff in a bid to upgrade services in local communities and create greater economic empowerment. In recent months, 85 employees of the Associated Manganese Mine (ASSMANG) in the Western Cape have learnt skills as diverse as candlemaking, shoemaking, concrete-product manufacturing, and brick production through a newly established community training centre.

Mr Janisch' s comment' in the August issue on Dick Stacey's remarks about safety factors indeed requires wider debate. The example he quoted to illustrate the successful application of the safety factor concept in coal mining, namely the Salamon formula, is to my knowledge the only recognized application of that concept. Not only that, but that particular formula should be used only for what it was intended, namely to prevent major disasters. It was never intended to indicate long-term pillar stability, and it should not be used for that.

In four years war I have had practically no news. Burch wrote several times but several of his letters were lost and so mine. I believe I even wrote to you. Much has changed since our last days in Telluride; here and there many have disappeared. Of myself I will tell you in a few words: for a year I was in Cadore(?), where I had the good luck to do to the others what they wished to do to me, and had good luck on my side. Then I went to Macedonia for 5 months, did little fighting but lost my faithful fellow officer and companion.

t is both an honor and privilege for me to nominate Dr. Daniel W. H. Su for the inaugural Syd S. Peng Ground Control in Mining Award. I can think of no better candidate than Daniel that is deserving of this award ? an award to recognize technical excellence in advancing ground control technologies [and] approaches. Daniel?s education and career spans over three decades. Having graduated from National Cheng Kung University in Taiwan in 1972 with a B.S. degree in Mining and Petroleum Engineering, he spent the next four years in the Taiwanese military and mining industry, before moving to the United States, where he enrolled in West Virginia University?s Mining Engineering Department. He received his M.S degree in Mining Engineering from WVU in 1978; then in 1982 became the first person to ever receive a Ph.D. in Mining from WVU. His thesis was titled, ?Development of Ultrasonic Techniques for the Measurement of In Situ Stresses?.

Dr. Christopher Mark serves as Team Leader of the Rock Mechanics Section in the Rock Safety Engineering Branch of the NIOSH - Pittsburgh Research Laboratory. In this position he leads a multi-disciplinary team and conducts individual research directed towards fundamental ground control safety issues affecting miners. Dr. Mark is recognized as one of the country?s leading authorities on coal mine ground control. Through his research, he has made numerous important contributions to mine safety that have been widely adopted into mining practice. He is a highly original researcher, and has often developed new and innovative methods to tackle extremely complex problems in his field. In recent years Dr. Mark has also achieved wide international recognition. Five research projects in Canada, Australia, and South Africa have been based largely on his work.

I am much pleased by your presentation of my article and flattered to the extent that the cover design refers to it. Incidentally, as between folks who like to work with words, refer to line 6, second full paragraph, page 773 (August Mining Engineering) Would "for" preceding technological have changed the percentages in lines two and three of the bold-face heading on page 770? The intent was coequality among three, (studies of mankind, of science, and of technological empirics). Thirty-three pct for "man" studies is a pretty big bite for engineering schoolmen; 50 pct they won't even examine, let alone nibble at. Arthur F. Taggart For those of you without a copy of the August issue handy, the sentence referred to read: The plea herein is to make the overall time allotment for studies of mankind coequal with those, for science and technological empirics.... Professor Taggart suggests that insertion of the word "for" would clarify the intent of coequality among three divisions of study: The plea herein is ... for studies of mankind coequal with those for science and (for) technological empirics. The engineering personnel shortage, present and impending, so well brought out in the article in Mining Engineering in the August, 1951 number, both in the editorial lead and the article by William B. Plank and Henry J. Petrie, has left untapped and has ignored a potential source of supply that would fill the need ably and with complete satisfaction. This source is comprised of the women engineers- and geologists, both presently, trained and able to handle work at all levels, and the many who would make splendid engineers and geologists if they could hope for even a modest taste of the square deal that should be theirs, but, as we all know, is not. I write from experience. It is not pleasant or sensible to have to urge borderline cases among men students to consider engineering and geology as careers to fill the need, when I have far more able, interested and suited young women students whom I must divert into other channels. It does not make sense to scrape the bottom and sides of the barrel to induce potentially second or third rate male engineers and geologists to enter the profession when we have superior material among the women, eager to do good work in these fields. It does not make sense for these men to become second or third rate in these fields when they might be first rate salesmen, musicians, advertising copywriters or policemen. Nor does it make any more sense to force potentially first rate female geologists and engineers to become second or third rate and very frustrated typists, kindergarten teachers, dress sales-women or clerks. At this point I am willing to admit that where superstitious -miners object to women in underground workings, men are available for the jobs. Moreover, it is not necessary for women to be sent into the most difficult country, if there is reluctance on either side of the contract. There may be fewer of them eager for the roughest assignments, in proportion to the men, but women are so accustomed to discrimination far more complete than that against any racial or religious group, that a reasonably tough assignment would seem like a heaven-descended joy. In my own experience, I have found that my girl students majoring in geology, could keep up with me in the field in the roughest terrain, but. the men, all but the best students and the most physically fit young men, complained that they could not keep up with me and that I always "walked their-legs off. The women, a few men and I always returned with bags heavy with well chosen specimens and proper notes. Most of the men carried no specimens and-but poor scraps of notes on loose bits of paper. Yet the men were urged to prepare for a career in engineering or geology if their examination marks were reasonably good. When we find that we still have a shortage of engineers and geologists, after putting all the qualified women to work, then we may begin to worry. I HIT the ceiling when I opened my November MINING ENGINEERING and after waiting a week to cool off I' still find it necessary to express my resentment of the gross libel embodied in the editorial introduction to Norman Stines' legitimate criticism of my paper on "Russia's Mineral Potential" (p. 949). The underlying thesis of my article, and a point that I tried to make even more clearly in my oral presentation at the St. Louis meeting last February, is that we should not over estimate the present and near-future mineral production of the USSR. The words that I resent in your boxed introduction are: "Opinions range from those presented by Paul Tyler ... to those of Norman C. Stines . . ." This is not merely an understatement that can be defended by admitting that the range goes much farther to the left, beyond me. It represents the appraisal that 90 pct of your members will carry away, namely that I am not only an incompetent observer-which may be true-but always that my bias is to applaud the efficiency of the beasts in the Kremlin-which is not true. Notwithstanding my efforts to clarify the record I now find myself branded as being at the opposite pole from 'Mr. Stines among the red-eyed worshipers of Communism or at least among the timid souls that shake in their boots in the belief that the Soviet system has created an invincible as well as an evil menace to the decent peoples of the world. My ideas are somewhere in the middle ground between Mr. Stines' complaisance or contempt and the exaggerated fears of several other observers whose views have received wide acceptance. PAUL M. In his article he objectively presented an estimate of the mineral position of the Soviet based on the statistics available to him which sources are credited in the first paragraph of his article. We happen to know that, Mr. Tyler was reluctant to prepare the article for the Mineral Economics Division sessions at the 1951 Annual Meeting and did so only because of his loyalty to AIME. It was thought that this article would be a basis for discussion of this important subject at the Annual Meeting. Determination of mineral composition is of significant importance for the mineral sands industry as this information is required for orebody evaluation, the control of mineral processing plants and the determination of product quality. Mineral processing is made more difficult by the presence of pseudorutile, leucoxene, altered-ilmenite and particle inclusions. The paper investigates the properties of such elements from a single product stream using an MLA approach.
