The most frequent causes of accidental injury and fatality are carelessness and ignorance; and the most common accidents are haulage accidents, fall, drowning, and fire. However, in this modern day and age, severe injury has become the dire consequence of an ever-increasing number of industrial and mining accidents as well as sport-, military- , etc. Mine accidents have declined dramatically in number and severity through decades of research, technology, and preventive programs. However, preventing recurrence of disasters like those of the past remains a top priority requiring constant vigilance by management, labor, and government. The goal of the present research is to investigate and identify root causes associated with types of mining accidents (victim being caught by an equipment, bodily motion, over exertion, etc) and nature of injuries such as (sprain and strains, fracture, dislocation, amputation, etc). Further, we hope to discover the impetus of the root causes occurring more and more often in this modern day of technology and quality awareness. With the different dimensions of accidents and injuries brought into the fold, there is the need for researchers and mining operators to comprehend and better still visualize significant patterns of associations among these variables of interest. We assume that, there is a high correlation between many dimensions of nature of injuries and types of accidents. This suggested a principal component analysis to reduce the dimensions for easy and amenable interpretation.

The size of bubbles plays an important role in flotation. Equations used to calculate the size of bubbles all show the effect of liquid/gas surface tension. However, the surface tension does not change much with frother concentration and experiments conducted using single capillaries (or single-hole spargers) do not reveal any clear effect of the frother concentration on bubble size. The size of bubbles does, however, strongly depend on frother concentration when multi-hole spargers are utilized (or when measured in a flotation cell); at low frother concentrations the bubble size is much larger indicating coalescence as a main mechanism determining the size. Coalescence can be prevented at frother concentrations exceeding the critical coalescence concentration (CCC), which in turn was found to be interrelated with the dynamic foamibility index (DFI).

The Climax Molybdenum Mine, located near Leadville, Colorado, is the site of a lengthy mining history spanning more than 80 years. In the 1960’s, extraction of molybdenum from oxide ore located adjacent to the massive molybdenite sulfide deposit resulted in the construction of an earthen core dam to impound fine-grained oxide tailing in the Eagle River Valley. Through recognized value of water storage and reclamation opportunities, a tailing removal project was initiated in 1993 to convert the impoundment facilities to a post-mining beneficial land use of developed water resources. An evaluation of the effect residual materials and lake dynamics would have on in-stream water quality was performed. Eagle Park Reservoir stands as a model for future reclamation efforts that involve water delivery to highly sensitive receiving waters. This paper provides a case study on project development, the evolution of water quality assessment, and the regulatory framework that contributed to this project’s success.

The Mini-DiscTM cutter drill bit is a radical design departure from the traditional tri-cone rockdrill bit. Miniature disc type cutting tools as small as 3.25 inches (82.5 mm) diameter are fitted to a bit body to cut individual concentric kerfs. The action is the same as that commonly used on large tunnel boring machines but miniaturized. The high performance rate and efficiency of a disc cutter are the result of producing a large proportion rock chips rather than sand or powder. The Mini-DiscTM drill bit, referred to as the “Disc Bit” has other unique features as well. It can be re-greased between runs. It can be rebuilt in the field in about 15 minutes, cutters can be replaced and the Disc-Bit appears to require less down hole hydraulic power than conventional bits. Thoroughly tested in the laboratory, the bit has recently been undergoing field trials in well drilling, blast hole drilling and shaft drilling. As any engineer knows, the laboratory can not shake out all the problems but it eliminates the more glaring weaknesses. Performance results have been exhilarating, and a few mechanical problems were discovered and largely overcome. This paper is a progress report.

The National Institute for Occupational Safety and Health (NIOSH) conducted a monitoring field study to optimize the rib control plan in a room-and-pillar coal mine. The study mine is located in the Central Appalachia coal basin. Two monitoring sites were selected at different sections in the study mine. The selected sites have different overburden depths and geologies. The instrumentation plans at study sites include borehole pressure cells installed at various depths in studied pillars, rib and roof extensometers and load cells mounted on number of the rib bolts. The instrumentation results and observed geology for one of the study sites showed great potential for optimizing the rib bolt plan at that section of the mine. Therefore, a rib support optimization study was followed using numerical modeling technique. A calibrated threedimensional model was developed using the instrumentation results of the optimization section. The calibrated model was used to determine the optimum rib support for expected overburden depths and rib heights. Baseline rib bolt patterns were assumed for the optimization study. Additional rib bolts, above and beyond the baseline bolting parameters, were outlined and identified as solutions to contain the rib fracturing identified by the modeling results.

Recent accelerated exploration efforts in southwest China have progressively quantified the vast phosphate resources in the Yunnan Province. To date, according to Chinese geologists, potential reserves in the province may be on the order of 4 Gt, which would equal the entire US reserve base. Development of this vast reserve is underway at two sites located 60-70 km southwest of Kunming, the provincial capital of Yunnan. The first, the Kunyang open-pit phosphate mine, produces an estimated 1.5 Mt/a of 22-30% P20,5. The other project located north of Kunyang is the Haikou mine, which the Chinese consider a possible 2.0-Mt/a operation. Because average ore grades run 3% less than those of Kunyang, beneficiation will be necessary. While the latter project has been initiated on a small scale, it is basically still on the drawing board. Brown and Root and the US Bureau of Mines are assisting on the project. Both operations are under the direction a regional administration which reports to the Ministry of Chemical Industries in Beijing. Combined production from both mines could represent some 70% of China's future productive capacity. Ore reserves in the region are estimated at 400 Mt by local officials. Geology of the District The phosphates are considered Cambrian in age and rest on a Precambrian dolomite. The Indian Ocean, Chinese geologists say, may have been the original source. The line of deposition stretches on a trend from Vietnam in the southwest, through Yunnan, into Sichuan Province to the northeast. Because the area is considered very stable from a geologic viewpoint, large quantities of phosphate have been deposited and large outcrops are known to occur everywhere in the province, according to Chinese sources. The Kunyang and Haikou ore bodies are thought to be two wings of an anticline with the former the southern portion, and the latter the northern portion. Each ore body strikes on an east-west axis. While there are many localized phosphate operations throughout the province, the Kunyang mine is the premium producer. Kunyang's Mining Development The Kunyang open-pit mine was opened in 1966. During the early years, operations were considered somewhat disorganized. Systematic mine planning and normal operations were not initiated until just two years ago, according to mine management. In 1979, Kunyang finally reached its design capacity of 1.5 Mt/a. The ore body consists of two phosphate beds separated by a band of shale. The top layer of phosphate varies from 4 to 8 m, averaging 6 m. The middle shale layer is 2 in. The third layer, which is not as rich as the top, is approximately 4 m. INTRODUCTION Geotechnical investigations for tunneling projects typically use vertical test borings and, to a lesser extent, angled borings. Horizontal drilling applications were often limited to specialty testing programs and to perform probe drilling from a tunnel boring machine (TBM) as part of production tunneling. The former often involved cumbersome setups with skid rigs and cribbing in a pilot tunnel or shaft. The latter usually was limited to generic probing for rock quality and ground water conditions with no sampling. Neither yielded continuous core at production rates suitable for design and construction planning. Conventional vertical investigations still make sense in many instances, particularly for tunnels planned beneath urban settings where truck access is convenient and where the tunnel horizon is not readily accessible via a portal or shaft. However, equipment is presently available to conduct horizontal geotechnicall investigations at reasonable production rates and cost. Advantages include continuously sampling the tunneling horizon and minimizing “access drilling” to get the zone of interest at the underground construction horizon. Horizontal boring lengths of up to approximately 800 m are considered feasible and those in the range of 150 to300 m are now routine. These capabilities make horizontal borings an attractive option to consider when looking at deep tunnels beneath ridges, especially where access is limited. Fewer drilling staging sites can also mean reduced environmental impacts when investigating tunnels proposed beneath sensitive lands such as public wilderness areas. Investment in early construction packages to develop portals for investigation access and, possibly in excavating test shafts for the same purpose should even be considered. Potential benefits in the form of achieving improved overall project costs using horizontal borings exist because of the significant additional data collected in the tunnel horizon. For this reason they should be considered during the routine cost-benefit analyses generally conducted for planning geotechnical investigations for tunneling. The Pipeline ore body was discovered in March 1991. The construction of the new mine and mill did not start until early March of 1996. Upon completion of permitting and corporate project approval, the 289 million USD project was “fast tracked”. The commissioning began 11 months after start of construction and the project ended 40 million USD under the budget figure. The objective of this paper is to review and highlight the Pipeline Project history from discovery, design, lawsuits, permitting, and re-design through to current - 1998operating practices. Particular emphasis will review the process design, design mistakes, and design successes. The paper will review project history, feasibility, design concepts, operator training, construction, financial and the current operation. Additional discussion will review what design features worked and which did not work within the process plant.

Flotation separation and recovery of value minerals from ores containing fibrous silicate minerals are known to be quite challenging. Earlier studies have established that the metallurgical challenges (poor selectivity and recovery, high energy consumption, etc.) arise from the impact of the fibrous minerals (with high aspect ratio) on pulp rheology, which was found be rather complex. The complex rheological behavior of ore suspensions containing particles with high aspect ratio and its influence on flotation outcome are investigated in this study. Flotation tests and rheological measurements were carried out with both ores (containing fibrous minerals) and a model system comprising nylon fibers (chemically inert) added to a copper ore that had no fibrous mineral. These studies allowed the determination of contributions of physical properties of the fiber to pulp rheological behavior. Optical microscopy images of the fiber pulp indicated that the fibers entangled to form two levels of microstructures: two-dimensional (2D) aggregates under semi-dilute (< 20% solids) conditions and three-dimensional (3D) microstructures at concentrations > 30% solids. The yield stress (σ) values determined for the fiber pulp were logarithmically related to the volume fraction of the 2D/3D structures. The relationships developed here for different ARs showed that the yield stress of a flotation pulp changes with respect to shape, size, and the concentrations of the microstructures. Implications to the effects of such entangle networks on selectivity of flotation separation are discussed.

Inn mining communities throughout the western United States, cows have always been part of the landscape as they graze on surrounding rangelands. Until recently, mining companies regarded these animals as something to be kept off their properties — especially any freshly reclaimed lands. During the last few years, though, this situation has begun to change. Today, cattle can be seen in a handful of mine settings, moving up and down the slopes of mine tailings and waste rock. These animals are being used by several companies to do some of the reclamation work once performed by large earth-moving equipment, mechanical spreaders, rippers and seed drillers. This is because cows can contour slopes more naturally and produce and spread soil nutrients in ways that tractors and ripping machines cannot.

The surface chemistry of the calcite/oleate flotation system was examined by in-situ FT-NIR/IRS. Spectra of combination and overtone bands of aliphatic stretching vibrations revealed gauche/trans conformational changes that occurred solely as a function of temperature. The conformational changes were attributed to the simple "melting" behavior associated with phase transitions. It was concluded that chemisorbed oleate at calcite surfaces is gel-like at temperatures below 22oC, resembles a coagel between 22oC and 31oC, and becomes micelle-like at temperatures above 31oC. By comparison, chemisorbed oleate at fluorite surfaces also undergoes these phase transitions but is additionally dependent on adsorption density. The conformational changes were observed at high chemisorption densities (> 2.0x10-10 mol/cm2) and attributed to molecular forces, primarily van der Waal&apos;s interactive forces, among neighboring chemisorbed oleate molecules. Similar analysis of in-situ FT-NIR/IRS spectra of surface-precipitated calcium dioleate at calcite surfaces did not exhibit this behavior thereby confirming that surface-precipitated calcium dioleate remains in an all-trans conformation. The results help explain why surface polymerization is not observed at calcite surfaces but is observed at fluorite surfaces.

Modern transportation systems are one of the most important achievements of our civilization and provide us with tools for broadening individual and social freedom of choice. The transportation system initiated today is handed dawn to the next generation, probably with little choice because of the enormous expenditure of monies involved, and influence their way of life as to where they should live and where they should work. The ever increasing antagonism between the citizens and the planners of the transportation systems necessitates a careful review of the entire planning process. The planning process can be broadly classified into: the goal formulation and decision making process; the plan development process; and the plan implementation process. THE GOAL FORMULATION AND DECISION MAKING PROCESS Goal formulation and decision making is one of the vital phases of the planning process and is primarily based upon social values. In measuring social values, the planner must employ an appropriate scale of measurement since in the absence of this yardstick, these values tend to became vague and abstract ideals. The need for major social programs and concerns (poverty, etc.) of the urban areas should be analyzed to determine the type of transportation system which would contribute to the solution.

The proposed 1.83km long twin bore A3 Hindhead road tunnel planned for contruction in 2008 is being delivered under a Highways Agency Early Contractor Involvement (ECI) contract. The proposed construction methodology is progressive mechanical excavation of a horseshoe shaped tunnel with shotcrete installed close behind the face using robotic spraying equipment. The tunneling medium is a ‘soft rock’ sandstone for which there was little previous tunneling experience, requiring extensive geotechnical investigations and an innovative approach to lining design. A 240m long portal section though sandy material utilising a steel pipe umbrella is also required. This paper describes the development of the tunnel planning and design including the most recent developments in UK road tunnels, and the innovation included in the ‘permanent’ primary lining design.

Precipitation of hydrous manganese/iron oxides in streams can be readily observed in much of the northeastern United States, and is apparently a common phenomenon in other glaciated areas (1,2,3,4). Deposition occurs when Mn-enriched ground or surface water enters the stream environment and encounters conditions of higher Eh and pH. The preferred location for deposition is in shallow, fast-flowing segments of stream; the oxides usually form as coatings on detrital material. The experimental studies of Hem(5) and a considerable amount of work on marine and lacustrine manganese nodules(6,7,8) suggest that given an appropriate substrate, deposition of manganese oxides may occur even when the water is undersaturated in Mn with respect to the oxides, due to catalysis by organisms or by silicate, carbonate, or ferric hydroxide surfaces. Once begun, manganese oxide deposition is autocatalytic (9). Studies of the distribution of oxides in various size fractions of stream sediments indicate that the thickness and prevalence of the coatings commonly increases with increasing particle size in the coarse sand and gravel size ranges. Figures 1 and 2 show a fairly typical distribution of leachable metals over nine size fractions from 62 microns up to 8-16 mm. The leaching solution used was neutral ammonium citrate with hydroxylamine hydrochloride, which effectively removes the oxide coatings without significantly attacking most silicates, carbonates or sulfides. Figure 1 shows metal content as ppm of the total sample; figure 2 shows&apos; the same data as micrograms of metal per square millimeter of surface area.

This paper describes the Contractor’s value engineering design and the construction of the Interstate I-90 highway tunnel sections under the South Station railroad yard in Boston, MA for the Central Artery/Tunnel Project, using tunnel jacking and ground freezing techniques. Three tunnel box sections were constructed and installed, with typical cross-sectional dimensions of 11.6 m x 23.8 m, and respective lengths of 50.9 m, 78.7 m and 115.6 m. A 12-compartment concrete shield was built as part of the front of each multi-sectional tunnel box structure. The jacking systems employed consisted of rear and intermediate jacking stations and up to 87 individual jacks, providing a maximum total thrust capacity of 42,300 t. The tunnel alignments pass through a geologic profile consisting of historic fill, which contains remnants of old waterfront structures, building foundations and a forest of 19th century vintage timber piles, underlain by organic sediments and marine clay. Ground cover over the tunnels is in the range of 1.8–7.6 m. The groundwater table is at depths of 1.5–3 m below surface grade. The paper discusses the techniques developed and equipment used to successfully build and install these tunnel box structures with minimal impact on the commuter and long distance railroad operations directly above the tunnel alignments. Figure 1 shows the track area at the I-90 EB and WB tunnels.

In the extractive industry the term ‘continuous improvement’ (CI) usually means procurement of the latest state of the art equipment. This is generally perceived as the only way to add further value to the supply chain, with the driving philosophy of increased tonnage to dilute operational costs. However, not all market places can facilitate this approach. RJB lives in a fiercely competitive market and has targeted the standard efficiency drivers of rationalisation and stock management with a vengeance; and also operate CMMS (Computer Maintenance Management System), RCM (Routine Condition Monitoring)and CBM (Condition Based Maintenance) methodologies. Ultimately though, these systems are only as good as the people who operate and manage them and to add further value without procurement, people must be re-introduced back into the operation. Through TPM (Total Productive Mining), that is what RJB have set out to do.

The Fort Canning Tunnel, Singapore, is an approximately 50 feet (15m) wide vehicular tunnel under shallow ground cover that has been constructed according to the principles of the NATM. Its shallow location in soft ground combined with a high groundwater elevation in close vicinity of historically important features called for special design considerations combined with a rigorous monitoring scheme. Surface, sub-surface and in-tunnel instrumentation was installed to monitor the performance of the ground and tunnel support during construction. Comparison of actual reading data with the results from the computer modeling carried out during the design phase is provided in this paper.

The Detroit salt mine is operated by the Detroit Salt Co. It is one of the oldest underground room-and-pillar mines in the United States still in production. Mining at the Detroit salt mine began in 1906 with the sinking of the No. 1 shaft. Mining continued through the years, becoming progressively more modern in its equipment and techniques, until it was closed in 1983. In 1997, after a period of care and maintenance, the newly formed Detroit Salt Co. decided to reopen the mine. Upon completion of an extensive rehabilitation program, salt production resumed in 1998. Production currently continues at a rate of 910kt/a (1 million stpy). To date, the Detroit salt mine has mined more than 45 Mt (50 million st).This mine is unique because it lies under a heavily urbanized area within metropolitan Detroit. The mine and its history make a good case study for the continuing coexistence between the mining industry and its environs.

A 4200 ton-per- hour marine transfer terminal, where river and oceangoing vessels simultaneously exchange cargos of coal and phosphate rock, is now in operation near the mouth of the Mississippi. Its materials handling machinery includes a bucket ladder barge unloader; a rope operated clamshell bucket unloader; a coal loader; and a rail mounted bucket wheel machine. The design and operation of this equipment is described as are the docks and conveying, breasting and barge-haul systems in use at the facility. Design considerations imposed by soil and river conditions, geography, operating needs and investment requirements are discussed. The economic significance of design and shipping innovations introduced by this facility is noted.

Sequential excavation of the Beacon Hill Station has been accomplished in highly complex and inter-layered sand, silt, and clay soils with multiple perched groundwater tables. Successful excavation has required the installation of a deep dewatering well system used to depressurize sand layers above and within the excavated tunnel faces. In addition, the presence of sand layers in the crown of the Platform Tunnels required jet grouting at depths of up to 49m (160-ft). This paper discusses the design intent, construction, and effectiveness of the dewatering system and the selective jet grouting of sand horizons and shaft break outs for the successful construction of the deepest SEM station excavation ever completed in the United States.
