Within the metal/nonmetal mining sector, fall-related incidents account for a large proportion of fatal and non-fatal injuries. However, the events and contributing factors leading up to these incidents have not been fully investigated. To help provide a clearer picture of these factors, an analysis of imminent danger orders issued by the Mine Safety and Health Administration (MSHA) between 2010 and 2017 at both surface and underground metal/nonmetal mine sites revealed that most orders are associated with fall risks. Of these cases, 84% involved the workers not using fall protection, fall protection not being provided, or the improper use of fall protection. Fall risks for workers most frequently occurred when standing on mobile equipment, performing maintenance and repairs on plant equipment, or working near highwalls. In most cases, a single, basic, corrective action (e.g., using fall protection) would have allowed workers to perform the task safely. Overall, these findings suggest that a systematic approach is needed to identify, eliminate, and prevent imminent danger situations. Furthermore, to protect mineworkers from falls from height, frequently performed tasks requiring fall protection should be redesigned to eliminate the reliance on personal fall protection.

INTRODUCTION During the nineteenth century, the federal mining law was enacted with the objective of promoting the development of the western United States by providing unrestricted access to vast areas of federal land for mineral exploration. Prior to the advent of comprehensive federal, state and local land use plans, there was little, if any, regulation or restriction of mineral exploration activities. Today, the laissez-faire policies of the past that encouraged mineral exploration have gradually given way to the public's legitimate concerns regarding environmental protection, multiple use and planned land development. As a result, the contemporary prospector confronts a maze of statutes, ordinances and regulations with which he must comply to obtain the requisite approvals and permits before commencing mineral exploration on federal, state or private lands. This presentation will provide a broad overview of the exploration permitting process. The first section of this paper recommends certain general management practices to follow when attempting to obtain exploration permits. The second section presents two hypothetical cases, one involving a drilling program on private property in California and the other involving a drilling program and the removal of a bulk sample for metallurgical tests from federal land in Nevada. The cases are used to discuss jurisdictional issues and application of certain federal, state and county laws and regulations that pertain to mineral exploration. The final section contains a brief analysis of three important recent United States Supreme Court decisions. These decisions address the scope of the federal pre-emption doctrine as it applies to activities conducted on unpatented mining claims and the constitutional limits of permissible state and local land use and zoning regulations. PERMITTING PRACTICES The environmental impacts associated with mineral exploration generally result from the construction of drill sites, roads or trenches. Consequently, after the regulatory authority has considered the issues of permissible land use, the primary emphasis of the permitting process is to address problems related to pre-construction site identification and clearance, and post-construction site reclamation. Tailings disposal is a significant consideration for the mining industry, with the majority of the ore processed in most mining operations ending up as tailings. Several tailings dam failure accidents have occurred during the past few years and mine tailings dam failures are occurring at relatively high rates. Tailings dam failure is disastrous with the serious damage and the loss of lives. To increase the tailings dam safety, a tailings dam monitoring and pre-alarm system (TDMPAS) based on the internet of things and cloud computing platform has been accomplished with the abilities of real-time monitoring of the saturated line, impounded water level, rain fall and the dam deformations etc. The TDMPAS has been applied in several mines in China and helped the mine engineers monitor the tailings dam safety and get warning information automatically and remotely in any kind of weather conditions. This work has been demonstrated the tailings dam monitoring and pre-alarm system (TDMPAS) is meaningful to monitor the tailings dam physical safety condition. The TDMPAS could be applied to monitor all the national tailings dams online in the future.

INTRODUCTION The Washington Metropolitan Area Transit Authority (WMATA) has let a contract for the excavation of three rock stations, spaced about 3550' (1082m) apart, beneath Connecticut Avenue in Washington, D.C. Two previous rock stations have been completed, one of which had a crown pilot tunnel excavated to explore geologic conditions that would be encountered during the station excavation. This pilot tunnel was primarily for inspection by bidders and design was based on core hole information. The other rock station had no pilot tunnel and was also designed on the basis of core hole information. The three rock station areas, which are the subject of this paper, have been explored by crown pilot tunnels and an as-built report, with geologic maps and sections, has been prepared, for use by the design engineer as the basis of design for support of the structures. The geotechnical report describes the geologic structure in detail and relates it to excavation and support of the three crown pilot tunnels and the running tunnels which will be enlarged to create the 58'W x 44'H x 700'(+)L (17.6m x 13.3m x 212m +) stations. The information was complete enough to permit the design engineer to visualize the geologic structure and compute loads for the various types of rock conditions. It was therefore possible to determine the size and spacing of the steel sets for the several different conditions of rock structure encountered in the pilot tunnels.

Each year, hundreds of mine workers are involved in machinery-related accidents. Many of these accidents involve inadequate or improper use of lockout/tagout (LOTO) procedures. To mitigate the occurrence of these accidents, new safety methods are needed to monitor access to hazardous areas around operating machinery, improve documentation/monitoring of maintenance that requires shutdown of the machinery, and prevent unexpected startup or movement during machine maintenance activities. The National Institute for Occupational Safety and Health (NIOSH) is currently researching the application of Internet of Things (IoT) technologies to provide intelligent machine monitoring as part of a comprehensive LOTO program. This paper introduces NIOSH’s two phase implementation of an IoT-based intelligent machine monitoring system. Phase one is the installation of a proof-of-concept system at a concrete batch plant, while phase two involves scaling up the system to include additional sensors, more detailed safety/performance metrics, proximity detection, and predictive failure analysis.

Weak rock mass and poor flow of material can cause excessive ore loss and dilution in sublevel caving. This paper proposes a method to establish design parameters for an inclined deposit within a weak rock mass and evaluates its effectiveness through a case study. In order to solve the problem of low production capacity and poor safety conditions of sublevel caving mining at the Yingfangzi silver mine in China and to ensure a smooth production from the refractory ore body, it is necessary to select appropriate stope structure parameters. Based on random medium theory of gravity flow and the improved equation of ore discharge in random medium, an end wall drawing experiment is carried out to determine the optimum design and flow parameters. It was observed from the experiment that, due to the influence of the randomness of particle movement, there is a mismatch in the flow parameter measurement. The flow characteristics and distribution of the marker particles are different from that of the tested particles resulting in large drawbody shape distortion. The study finds that the flow parameters obtained by the new method are more practical and account for drawpoint width and recommends that the new method for determining the sublevel stope height is used when the drawbody is a non-standard ellipsoid. The outcomes have shown a significant reduction in the dilution and improvement in the recovery rates, and the safety and efficiency of mining have been greatly improved.

Echo Bay’s requirements called for Kvaerner to: •Provide a fixed price for closure with no change orders. •Negotiate, prepare and implement a final closeout plan as outlined by Echo Bay. •Complete a closure program that exceeded current regulatory requirements. •Improve, expand and maintain a proactive public and community relations program. •Hire Echo Bay employees where possible. •Provide adequate performance guarantees and judicial relief of environmental claims.

A variety of modeling techniques can be utilized to forecast metal production at heap leaching operations. These approaches reflect a wide range of complexity, flexibility, time to implement, cost, and accuracy. For many operators, a spreadsheet-based modeling technique is attractive since the calculations are directly accessible, models can often be developed by site staff, and the results are generally easy to extract and interpret. The authors have developed a spreadsheet-based modeling technique that provides a high degree of flexibility, while still considering detailed operating information for ore properties, leach kinetics, scaleup factors, lift height, and in-heap metal inventories. The technique involves establishing kinetic leach curves for each ore type to characterize metal extractions, followed by application of separate metal recovery curves to define the rate at which leached metals exit the heap in pregnant solution, taking into account delays due to the in-heap solution inventory. At any point in time, the difference between total metal extracted and total metal recovered is equal to the in-heap inventory of the leached metal. This modeling technique is particularly useful for larger multi-lift heaps where delays in metal recovery are appreciable due to in-heap solution holdup. The technique is also applicable to sites with multiple heaps, leach cycles, and/or metal recovery plants (e.g., carbon columns for gold; solvent extraction for copper). This paper describes the modeling technique, including input data required and methods for defining kinetic leach curves and metal recovery curves. The technique is compared against an alternative spreadsheet technique using a simplified in-heap solution inventory calculation to forecast metal production, which has proven to be effective at a number of sites.

One of the biggest challenges for the mining industry is, quite literally, how to the keep the equipment rolling. There is a limited amount of companies that manufacture the 4.2-m- (14-ft-) tall, 5,670-kg (12,500-lb-) tires needed for the haul trucks that are approaching the 362 t (400 st) pay-load class, such as Terex?s MT6300-AC and Caterpillar?s 797F. And, while these might be the most impressive tires, they are not the only ones that are in short supply. The shortage of tires has affected just about every kind of machine, and the demand for those tires has been extremely high for more than five years with no end to the shortage in site. American Tire Corp. (www.americantire.us) is one of the companies working to supply tires to the mining industry. It announced that the demand for its 1.6-m (63-in.) tires that have a zero failure history globally is great and that the company signed contracts worth more than $500 million at MinExpo. Heap leach technology provides several benefits to combat these issues including low cost, flexibility and diversified process conditions. This paper provides an overview of heap leaching technology with specific emphasis on chemical aspects. Hence, influencing factors, advantages and disadvantages, testing approaches and some fundamentals will be discussed. Finally, heap leach technology for different ore types (copper, nickel, uranium and gold) will be briefly compared.

Ordinary portland cement (OPC) is the key pozzalonic component of the concrete mix widely used as a construction material worldwide. B illions of tons of OPC are used in the construction industry each year. However, calcinations of limestone and silica that produce OPC also produce almost an equal amount of C O2 during the cement manufacturing process. Thus, OPC production accounts for nearly 7% of all anthropogenic C O2 emissions each year. The main objective of this study was to develop a geopolymer-based composite to be used as a substitute for OPC in concrete application to significantly lower the carbon footprint of the construction industry. A waste material produced during the coal combustion process was used as the main raw material for the geopolymer-based composite. It was mostly the nonmetallic content of the F-type fly ash produced from the combustion of bituminous coal. The authors envisioned extracting some of the valuable metal oxides from fly ash prior to using the major nonmetallic portion of the fly ash for geopolymer concrete application. The results obtained from this study showed that the compressive strength of geopolymer concrete compares favorably with that of conventional concrete prepared using OPC.

A crucial step in the acquisition by one company of the assets of another, the acquisition of the stock of another company, or the merger of two com-ponies, is the legal due diligence which needs to be performed in order for the acquirer to ascertain or confirm the obligations, liabilities and exposures to risk that are being assumed along with the assets or corporate stock being acquired. Where the obligations, liabilities, or exposures to risk arise from existing contracts, or from pending lawsuits, administrative proceedings, or threats of these actions, those matters are relatively straightforward to discover by careful review of the files of the party whose assets or stock is being acquired. However, where a company is engaged in ongoing mining or processing operations and has operated or owned mining proper-. ties, there are many sources of liability and exposure to risk which could have major adverse economic consequences to the acquiring party and which are not revealed, or at least are not clearly delineated, by the records of the company whose stock or assets are being acquired.

Ilmenite-bearing paleo-beach deposits were discovered in Jylland, Denmark using a combination of Ground Penetrating Radar (GPR), portable X-ray fluorescence (XMET), bulk XRF analyses, Computer Controlled Scanning Electron Microscopy (CCSEM), and wire line logging. The Miocene-age paleo-beach deposits are located below a thin Quaternary cover in a region of very moderate relief and essentially no outcrop demanding unique approaches in exploration. Initially, an isopach map of the Quaternary is prepared from existing water well data. A geological model for likely occurrence of Miocene paleo-beach deposits is generated and the coincidence of thin Quaternary cover and potential paleo-beach deposits define targets for detailed exploration. The defined targets are traversed with GPR to identify paleo-beach structures which are then used to define a drilling program. This methodology located a paleo-barrier island at the Vorslunde location with more than 10% ilmenite in the sand. During the drilling process, the TiO2 and heavy mineral contents of the sand were logged every meter with a portable XRF unit (XMET) and subsequently calibrated more accurately by comparison to bulk XRF and CCSEM analyses for selected samples. The bulk XRF data also provided more complete compositional information. Insitu determination of geology, heavy mineral abundance, and corroboration of the XRF logging was done using wire line logs, a combination of spectral gamma and induction conductivity logs. Detailed characteristics (abundance, grain size, shape) and chemical composition of minerals was determined by the GEUS CCSEM methodology (Frei et al, this vol.). The average weight percent TiO2 for all Ti-minerals is 62%, 60% excluding rutile. That elevated TiO2 content (>> ilmenite stoichiometric value, ca. 50 %) indicates that ilmenite experienced an iron-leaching environment. The source of the sand deposit was investigated by dating contained zircon. The results show that the likely sources of zircon are in southern Norway and southwestern Sweden with common zircon ages of ca 1100 Ma. and 1500 Ma.

The U.S. cement industry, including Puerto Rico, again set a record in terms of production and shipments. The increase in shipments came primarily from imports of portland cement as domestic production from produced and imported clinker gained only 1.9 Mt (2.1 million st). Clinker production as reported to the U.S. Geological Survey (USGS) was 88.6 Mt (97.6 million st), an increase of 1 percent. Gray and white portland cement production from domestic and imported clinker, estimated at 93.5 Mt (103 million st), was up 1.9 Mt (2.1 million st). Masonry cement shipments of 5.5 Mt (6 million st), which equates closely to production, was up 6.3 percent. Blended ce-ment shipments were up 35.9 percent from 2004 at 2.35 Mt (2.6 million st).

A unique partnership between the Colorado Mine Safety and Training Program (CMSTP), the Colorado Rock Products Association (CRPA) and the Higher Education and Advanced Technology (HEAT)Center resulted in a video CD-ROM training program for new miners entering the aggregate industry. Cooperation from everyone involved, including the U.S. Mine Safety and Health Administration (MSHA),made this the product against which other training materials in the mining industry will be measured in the 21stcentury. This product’s versatility and wide application make it a tool that is as critical to a mine’s success as any earthmoving machine. The state and federal grant money involved allows this product to be affordable to small operators and independent contractors. The quality of this two CD-ROM set rivals any commercially made CD-ROM training.

The Fairmount Minerals, Inc., Maiden Rock underground sand mine historically delivered air to ventilate diesel-powered equipment at its working faces through high-resistance, 15-inch-diameter PVC distribution lines. In support of plans for expanded production, three new, low-resistance ventilation design alternatives were analyzed. The study determined that adequate ventilation for expanded production was possible with various commercially-available, high-volume/low-pressure fans combined with a systematic layout of control structures. The recommended re-design was implemented in 2001, resulting in a significant increase in flow volumes and a noticeable improvement in the mine environment.

Chemical and physical properties of typical low-rank coal (LRC) ash are significantly different than bituminous coal ash. With the exception of extraneous ash, which is incorporated during mining or is due to ash partings within or between coal seams, LRC ash consists primarily of inherent ash that is bonded into the coal's organic matrix or finely disseminated mineral matter. This paper reviews the nature of LRC ash, both inherit and extraneous, and how this impacts the prospects of washing and utilizing LRCs. A literature review, as well as washability data, proximate and ultimate analyses and ash analysis of 25 Alaskan LRC seams are used as a basis for this discussion. The identification of the major subsidence areas in the Eastern Kentucky Coal Field has been made to distinguish the subsidence phenomena on the basis of geographical location. Further, this identification process is based upon selected critical geological, topographical, and mining features in the coal fields. The proposed identification of the regions is derived from a comparison of the subsidence potential in a particular area. The most likely severe subsidence areas are compared to those with the probable low subsidence severity. The combination of certain geological and mining parameters involved allows for the derivation of a classification of the subsidence potential.
