Incident investigation is of utmost importance in most high-risk industries. This study was undertaken to study the content of past investigation reports to determine the effectiveness of the incident investigations. The study uses a semi-quantitative method to assess the effectiveness of incident investigations in the Ghanaian mining industry by evaluating the quality of past investigation reports. The assessment tool consists of 5 elements with several indicators and rating scales for assessing the quality of an investigation report as a measure of the effectiveness of the investigation. The method was applied to 304 investigation reports of 3 Ghanaian large-scale gold mines, and the results correlated with incidence rates of the mines to determine if any relationship existed. The results showed that the mines differ significantly in the quality of their investigation reports, suggesting differences in the effectiveness of their investigations. In addition, the incidence rates of the mines negatively correlated with some elements of the assessment tool. In general, the method was found useful and revealed areas where improvement is needed. INTRODUCTION The importance of learning from incidents is well documented in the literature. For instance, some scholars regard learning from incidents as important for improving safety [1,2]. Thus, many organisations spend considerable resources in identifying lessons from incidents and subsequently on the process of improvement that follows the identification of lessons. It is therefore important for such organisations to determine the effectiveness of their learning from incidents processes, in order to identify specific areas where improvements are needed, so that interventions can be as specific as possible. The learning from incidents process models the plan-act-check-do approach of management systems, often consisting of four major stages, namely; data collection and incident analysis, planning interventions, implementing interventions and follow-ups and evaluation [3,2]. The investigating and analysis of incidents have been regarded as one of the vital stages of the learning process since it is the stage where lessons are identified [3,2,4]. It has also been noted elsewhere in the literature that results of the previous stage of the learning process are important for the next stage [3]. For example, ineffective incident analyses could result in the planning of ineffective remedial measures. One of the primary aims of incident investigations is to identify lessons from previous incidents which when implemented can prevent future recurrences or minimise the effects of future incidents. There is thus the need for organisations to assess the quality of their investigations periodically since it is one of the means they adopt to improve safety and the quality of remedial measures to some extent depends on the effectiveness of the investigations. Although previous studies [5,2] have assessed the effectiveness of the learning process, and some aspects of the learning process, such as incidents reporting, there is less focus on the effectiveness of incident investigations. The research described in this paper was therefore conducted to contribute to this gap, focusing more closely on incident investigation reports.

Relax: the world today is not running short of many mineral commodities, even though prices are spiraling upward. Einstein?s observation still holds true that everything is still relative, as long as one can pass on the incremental costs of production to the consumer. However, a strong consensus exists among many of the companies comprising the minerals processing industry that it is becoming increasingly difficult to find the easy-to-mine, high-quality resources that make juicy profit margins, which shareholders like. For the most part, the resource cherries have all been picked, and established mineral processing companies that are exhausting their reserves, or in the process of looking to add reserves so as to create longevity for their company, have no recourse but to look long and hard at what were once considered the dregs of the earth. Even end users, such as steel companies, are now looking to become ?miners,? to establish captive iron ore resources. Owning a captive resource facilitates a dependable supply of feed materials, which translates into effective cost control and a guarantee for a thriving sustainable business in the future.

In the opinion of these authors, technology alone can never give an organization an edge over competitors or provide an industry step change. However, history has shown that technology applied with correct logistics and strategy makes a significant difference. Never has the minerals industry faced such daunting challenges and been in such need of a step change. Application of autonomous systems has been steadily increasing in the minerals industry. Automation technology has changed the mining value chain from exploration to mineral processing and refining. The future state will only see additional in-roads of automation into more processes and the whole system design. To better understand the near-future potential of automation, it is useful to explore and review the current state of automation in minerals development. A review of the state of automation research is given. The review will consider three critical areas: automation technology, system’s engineering and management processes around automation, and the role of human factors engineering in automated and semi-automated systems. The review ends with a discussion on the social and political risks of automation in terms of shared value and sustainable development.

Cobalt, nickel, and manganese can be recovered quickly, cleanly, and selectively from cobalt-rich manganese crust by leaching in dilute acid solutions in the presence of hydrogen peroxide. Common selective precipitation methods can then be used to recover these metals from the leach liquor. The leaching reaction produces only oxygen as a byproduct, and the waste residue from leaching was tested and found to be non-hazardous under existing EPA specifications. The leaching process is effective in either fresh water or seawater. This research is a part of the Bureau of Mines program to identify the mineral resources of the Exclusive Economic Zone and devise methods for recovering their valuable components.

This paper presents an analysis of gold production and prices over an extended period. Production patterns are provided for the world as a whole and for the five major producing countries. Gold prices as a function of interest rates, as measured by municipal bond rates, are given for the period 1947 to the present. World gold production has had a phenomenal increase since 1980, with the exception of South Africa, all major producing nations participated in this increase, even though the price of gold generally has been decreasing since the high level attained in 1980.

In operations that use the sublevel stoping mining method, production scheduling takes into account factors, including ore reserve envelopes, ore grade averages, equipment capacity, feed controls to the mill, and geotechnical restrictions, across the scheduling horizon. Mathematical programming is one of the most effective methods for enhancing the decision-making process in regard to the most viable production schedule in light of a given set of limitations. The majority of research on sublevel stope mining focuses primarily on a single objective of optimisation, which is to maximise the net present value (NPV). Environmental, social, and governance (ESG) considerations are becoming more prevalent in the mining industry. This includes aspects relating to deforestation, land disturbances, gas emissions, community relations, and water usage, among many others. Institutional and other kinds of investors are becoming more interested in the work that resource companies do regarding their ESG obligations. This review article identifies the gap in the existing literature and proposes to simultaneously optimise the production schedule around three objectives. Two of these relate directly to ESG aspects.

This price spread in emission allowances clearly demonstrates the correlation between the quality of fuels versus the price utilities will be willing to pay for them. This paper provides an overview of the effects of the qualities of fuel, coupled with transportation as well as environmental concerns on the price of fuel and the ability to compete in a deregulated electric market.

The Second Narrows Water Supply Tunnel will ensure system reliability in the event of a major earthquake and increase the Greater Vancouver Water District’s capacity to meet a growing population’s longterm needs. It includes a bored tunnel between two deep shafts located in North Vancouver and Burnaby, BC, Canada. The support of excavation for the 16-meter-diameter (52.5 ft) TBM launch shaft consists of 83-meter-deep (272 ft) unreinforced concrete slurry walls. This paper discusses challenges encountered during construction of the launch shaft slurry walls, namely excavation methods and slurry loss, and mitigative solutions implemented to successfully complete the shaft with minimal impact to project schedule.

A hydrogeologic characterization and impact analysis was performed for a proposed quarry at the Hitch Rack Ranch located in the Little Turkey Creek watershed southwest of Colorado Springs, Colorado. Continuous multiple channel tubing, nested piezometers, stream flow measurements, and mapping of known seeps were used to define the three-dimensional potentiometric distribution and areas of groundwater recharge and discharge. Water balance considerations, mass balance calculations, and potentiometric response to earth tides and barometric fluctuations confirmed that the granite fracture system in the ridge area of the proposed quarry has an extremely low bulk hydraulic conductivity resulting in minimal groundwater discharge to sustain the baseflow of Little Turkey Creek. The Little Turkey Creek valley forms the major conduit for groundwater flow exiting from the upper part of watershed underlain by granite. This conclusion has significant implications with respect to mine planning to minimize projected impacts to the hydrologic system and preserve this recharge component to downgradient Denver Basin sedimentary deposits.

The performance of seals in boreholes, shafts, and tunnels is an important concern in mine safety, in the control of effluents from operating and abandoned mines, and in the underground containment of hazardous wastes (Einarson and Abel, 1990; Mining Waste Study, 1988). Despite the clear need for effective seals, few documented test results are available concerning the performance of in-situ seals. Some evidence indicates that horizontal or near-horizontal holes are especially difficult to seal properly (Kelsall and others, 1982). This research involved the installation, extensive hydraulic testing, removal, and post-test examination of a l0-cm-long by 10-cm-diameter cement grout seal in a near-horizontal borehole in limestone. The work was performed in an underground mine on Marble peak in the Santa Catalina Mountains, Arizona. A test hole was diamond cored in the re-crystallized limestone of the Escabrosa formation. The hole is 33.24 m long, 10 cm in diameter, and dips at 10.4 degrees. The hole extends between two drifts and thus is accessible from both ends.

"The industrial Internet of Things (IIoT)—a concept that combines sensor networks and control systems—has been employed in several industries to improve productivity and safety. NIOSH researchers are investigating IIoT applications to identify challenges of and potential solutions for transferability of IIoT from other industries to the mining industry. Specifically, NIOSH has reviewed existing sensors and communications network systems used in U.S. underground coal mines to determine whether they are capable of supporting IIoT systems. The results show that about 40% of the installed post-accident communication systems as of 2014 require minimal or no modification to support IIoT applications. NIOSH researchers also developed an IIoT monitoring and control prototype system based on low-cost microcontroller Wi-Fi boards, to detect a door opening on a refuge alternative, activate fans located inside the Pittsburgh Experimental Mine, and actuate an alarm beacon on the surface. The results of this feasibility study can be used to explore IIoT applications in underground coal mines based on existing communications and tracking infrastructure. INTRODUCTION The nation’s approach to health and safety in mining has historically been reactive in the sense that health and safety regulations and mandates have traditionally been passed as a result of tragic mining accidents. An example of this is the Mine Improvement and New Emergency Response Act (MINER Act) of 2006, which was passed as a result of several mining accidents where tracking of and communications with underground personnel were severely limited. While this reactive approach has led to the development and implementation of highly impactful safety interventions and safety practices, there currently exists a technological revolution that can shift the paradigm toward a proactive approach based on preventative methods: Internet of Things (IoT). IoT is the network of physical objects or ""things"" embedded with electronics, software, sensors, and connectivity to enable that network to achieve greater value and service by exchanging data with the manufacturer, operator, and/or other connected devices [1, 2]. It should be noted that IoT has been defined from various perspectives, and hence numerous definitions exist in the literature. The apparent ambiguity of the IoT definition might be due to the fact that IoT is syntactically composed of two terms: internet and things. The first one pushes towards a network-oriented vision, while the second tends to put the focus on generic objects. The traditional internet can be viewed as an internet of computers —i.e., “things” connected to the internet are computers only. IoT “extends” the internet by allowing other generic things that are commonly found in our daily life, such as refrigerators and cars, to be interconnected via the internet, and thus creates a bridge between the virtual word and the physical world."

Acceptable working environments in production areas and service openings of Mindola Mine on the Zambian Copperbelt are currently maintained with an air volume of 1000 cu m/s and 5500 kW of installed refrigeration plant. It is planned to open up additional deeper areas of the mine for future production. Temperature problems, due to the increased heat generation from various underground sources are recognized as potential hinderances to efficient production. Findings of research work indicate that the present total heat output of 58,000 kW will inevitably increase by the second five-year period of the deepening program to 52,900 kW and by the fifth five-year period to 63,200 k\V. Estimated WB and DB temperatures, representing the total heat generation in stope faces and development headings during these same periods are predicted to increase to 53.8/40.2 °C and 35.5/42.1 °C respectively. It is assessed that the nominal refrigeration capacity needs to be progressively increased to 9,000 k\V This paper deals with the thermal problems in Mindola Mine, current heat output calculations, future prediction of temperature and heat load in various working areas of the mine with the application of computer programs and interpretation of data in order to find the effective solution for long-term planning of ventilation and air-conditioning.

In an extremely competitive mining industry, onsite experience is a big advantage. Mining education at the universities is mostly focused on theoretical studies without a possibility of practical training in mining sites. Hence, experimental mines suitable for practical education are needed to provide a platform for systematic research and education in industrial scale and for training in real mining conditions. Yet, this kind of mine sites is rare worldwide. Thus, a new educational research project, namely MINETRAIN is introduced in this paper evaluating the transition of the Pyhäsalmi mine in Central Finland from an active base metal mine to a research, educational and training underground facility. The uniqueness of MINETRAIN compared to other test mine programs is that the existing state‐of‐art infrastructure in Pyhäsalmi enables research and training facilities among all disciplines related to the overall mine value chain. Though all the above sound interesting in the context of research and education purposes, in practice Pyhäsalmi will have to become an experimental mine that can be sustainable in the future. Accordingly, a prefeasibility study is being conducted and some preliminary results are presented in this paper. INTRODUCTION Despite being a field of studies that requires practical knowledge and experience, mining engineering education follows the same pattern in most mining universities around the world; consisting of theoretical courses and laboratory-based practical modules (Mischo, 2015). The possibility of having practical training in real mining conditions on actual mine sites or processing plants is limited to none, while the only contact that undergraduate students have with mining operations during their time of studies is usually through short-time internships or visits in the context of educational trips. Yet, real integration between universities and the mining industry may be scarce, which does not help students to observe the actual processes, and thus understand the whole picture of mining operations. Indirect practical training is also possible nowadays that technology is shifting from mechanical to digital, and the use of virtual reality -in the context of Mining 4.0- can provide additional experience to the learning process. However, virtual reality is limited to the visual impression and is also limited in actions (Binder et al, 2018). In any case, when there is not a proper pragmatic view of how the different disciplines link to each other and especially if there is a lack of communication, unfortunately the results could be seen at a later stage in the mines as a poor performance of professionals and workers. Another major challenge for educating specialists in the mining sector is that experimental mines suitable for practical education, where the skills and know-how could be developed and/or enhanced are rare worldwide. Such research and educational (R&E) facilities are practically former metal ore mines that have been transformed into experimental sites. Besides being limited in number, not all facilities of this kind serve both a research and educational purpose.

The National Mining Hall of Fame and Museum (NMHF&M) will induct the 2013 National Mining Hall of Fame class on Sept. 14, 2013. The ceremony will take place at the Brown Palace Hotel in Denver, CO. Representing the domestic and worldwide metal production, education, and health and safety sectors of the industry, this year?s five inductees cover a diverse cross-section of the mining industry. The inductees are: Charles F. Barber, Donald W. Gentry, Patrick H. O?Neill, Milton H. Ward and David A. Zegeer. They will join 218 other mining industry pioneers when they are formally inducted into the National Mining Hall of Fame at the 26th Annual Induction Banquet and Ceremony. They were selected for being visionaries, leaders and ambassadors, both within their own sectors and across the industry at large. Barber was a top executive at leading U.S. metals producer Asarco from 1956 to 1982. During his tenure, Asarco responded successfully to the demanding requirements of new environmental legislation in the United States and to changing concepts of foreign investment in mining, especially in Latin America. Gentry?s career took him from being a working mining engineer, to renowned mining educator, to president and chief executive officer of a mining company. As an educator, he had a positive impact upon hundreds, if not thousands, of new mining engineers and many others who took his short courses in mine economics. During his years in South America, O?Neill did everything from supervising operations of 15-bucket ladder gold dredges to becoming president of International Mining Corp. As an executive, he emphasized provision of education and training to local people working at mine sites for which he had responsibility. Ward?s experience covered the world and spectrum of metals, minerals and ores, ranging from A to Z (antimony to zinc and Australia to Zambia). He was president, chairman and chief executive officer of Cyprus Amax Minerals from 1992 to 1999, and he was president and chief operating officer of Freeport-McMoRan from 1983 to 1992, during which time he was instrumental in developing Freeport?s world-class Indonesian operations. Zegeer dedicated his mining career to protecting the health and safety of miners as a corporate executive, head of the U.S. Mine Safety and Health Administration (MSHA) and community leader. His tenure at MSHA coincided with a period of remarkable progress in U.S. mining safety .NMHF&M Board of Directors Chairman Paul C. Jones said: ?The five outstanding gentlemen being honored this year as inductees into the National Mining Hall of Fame are classic examples of the leadership and work ethic that has made America?s mining industry the world leader in technically sound, environmentally and socially correct, and economical mineral production that sets the standards for mining worldwide. We are very proud to add these five legendary gentlemen, who have demonstrated their sound technical and management skills to advance society on a worldwide basis, to our current list of 218 inductees being honored by the National Mining Hall of Fame.? 2013 Prazen Award winner ? "The mines of the future present a host of challenges, which may appear exciting or daunting, depending on one’s perspective. Whether some sectors of the public recognize it or not, the demand for raw resources is not going away any time soon. In fact, it is rising, as the global population grows, urbanizes and attempts to steadily increase quality of life. Yet, at the same time that much of the highly developed world is calling for greener goods, reduced consumption and cleaner production, in other areas there are cries for clean water and sanitation, consistent energy supply, access to health care and education, personal security and support for basic human rights. The balancing act for sustainable development of our global society is, indeed, precarious and the mining community is a key player. Fortunately, many industry members have accepted this role and the responsibility to provide leadership and a solid foundation for progress. Looking ahead, we need new generations of leaders, including many mine engineers who are ready to tackle all sorts of complex problems.There are tremendous needs just for technological innovations. Increasingly tough deposits will require that the industry continues to expand scale toward “mega pits” and “super caves;” fully develop unconventional recovery techniques like in situ leaching and gasification or deep-sea extraction and seriously consider off-earth endeavors. Such work will take entirely autonomous equipment, novel separations, processing and recycling methods, and systems that can withstand extreme physical conditions, just to name a few. It will also take a level of systems integration never-before seen in the mining industry, complete with big data issues, sophisticated predecitve analytics and modeling efforts and cyber-security challenges.Moreover, increasingly complex environmental problems will require creative solutions. Addressing issues like pollution control across multiple media, emerging contaminants, optimal management of wastes, selective mining and shared use of resources between mine operations and surrounding communities and ecosystems are among the top priorities. Really, a complete paradigm shift must occur, such that the focus is on overall improvement of environmental conditions, rather than minimization of impacts. A similar shift will be required in the social realm. How can miners be safer and healthier on-site and off? How can the industry contribute to communities such that they are better off, now and longterm, for having a mine next door? How can artisanal mining practices be transformed to also contribute to sustainable development? How can the industry’s work be a factor in increasing economic and political stability? How can the public understanding of raw resources and the need for mining enterprises be improved?Given the coming challenges, there is no doubt that the mine engineers of the future will need highly diverse skillsets — meshing traditional and contemporary competencies to a degree not previously considered in mining, or most others fields. To be sure, the scope and scale of mining projects make them wholly different than most other engineering work, aside from perhaps the largest civil and urban development projects. While on-the-job experience will continue to be essential, formal education programs (i.e., at post-secondary institutions) must lay critical groundwork."

Chemical and mineralogical characterization of Malaysian monazite, a phosphate mineral, bearing rare earth elements separated from the tin tailings originated from Ipoh, Perak, Malaysia, was performed in this paper. The study aims to collect detailed information on the chemical composition, crystal phases, and microstructure of the mineral monazite concentrate that would aid to optimize the subsequent hydrometallurgical processes for high-efficient separation of thorium and other associated rare earth elements. A systematic characterization study of the concentrate was conducted using techniques such as optical microscopy (OM), scanning electron microscopy (SEM) equipped with energy-dispersive X-ray spectroscopy (EDS), and Fourier-transform infrared spectroscopy (FTIR). These techniques analyzed the morphological details on the surface, elemental analysis, and mineral association assessment and identified the surface functionalization groups. The bulk composition and the mineral phases in which the elements are present were studied by wavelength-dispersive X-ray fluorescence (WD-XRF) and X-ray diffraction (XRD) studies respectively. The XRF analysis confirmed the presence of Ce, La, Nd, Pr, and Y (rare earth oxides: REO’s ~ 60 wt.%) while thorium dioxide (ThO2) accounted for 7 wt.% of the total composition. Traces of Ca, K, Al, Fe, Ti, and Mn were also confirmed by SEM elemental mapping. The XRD results confirmed that the concentrate was primarily composed of monazite (Ce, La, Nd, Th (PO4)) along with minor impurity phases of quartz. Automated mineralogical analysis was used as a confirmatory tool to corroborate the preliminary evidences. Based on the particle size distribution analysis supported by SEM, the majority of monazite grains were found to be present in the size range of 170–210 μm. Strong bands of PO4 and SiO4 were observed in the IR spectra corresponding to the phospho-silicate matrix of the REE mineral.

Longwall mine instrumentation for measuring abutment stress distribution, mining induced immediate roof stress change and deformation, and primary/secondary support loads and deformations provides critical guidance for the design of gateroad chain pillars and roof support. Field measurement results are also recognized as the best way to calibrate numerical models that have been used to study the failure and support mechanisms. International Conference on Ground Control in Mining (ICGCM) started in 1981 and since then, there were 26 papers published in the proceedings discussing the stress and load measurements as the face advanced using various field instrumentation techniques. The focus of this paper is on summarizing the analyses of the data published in these papers. Stress/load change monitoring included the following categories: gateroad chain pillar, abutment pressure, bolt load, and pre-driven recovery room. The data shows that the magnitude of stress changes as the face advances varies considerably from panel to panel and mine to mine. However, regardless of types of stress changes, pillar stress, abutment pressure, and bolt load, there exists a rough trend of stress increase as the face advances, generally consisting of three stages, i.e., stress/load (1) initially increases steady or slowly, (2) increases rapidly, and (3) increases reaching the peaks and dropping down slowly or sharply. The stress/load increases start earlier during the second panel mining and are larger than the first panel mining. This review makes it easy for the interested persons to find valuable data for other similar projects.

A recent Nevada Gold Mines research program focused on pressure oxidation pre-treatment of a Carlin-type double refractory ore from the Turquoise Ridge Mine. The research team found that both arsenian pyrite and carbonaceous matter were highly oxidized within an hour at 300 °C using a bench-top autoclave reactor and that the gold recovery from the residues after cyanide/carbon-in-leach approached the technical limit set by the amount of gold encapsulated by silicate minerals. Although the 60-min pre-treatment at 300 °C yields robust performance when treating a whole ore, copper additives can catalyze the oxidation rate of carbonaceous matter and reduce the residence time needed at elevated temperature. Certain gangue minerals are susceptible to dissolution in acidic solutions at 300 °C, and this may cause poor gold recovery. This recovery challenge is mitigated by keeping free acid in the discharge solution below a key threshold.

Rock fragmentation size distribution is often used as an important index to account for the blasting effect because it directly affects the subsequent loading, transportation, and secondary crushing. Due to the mismatching of explosive and rock wave impedance, high boulder yield often occurs which affects the blasting effect. In this study, methods of measuring rock acoustic impedance, rock strength point loading, and detonation wave velocity have been used to obtain more accurate input parameters. Then, in the watershed image segmentation technique, the Gates-Gaudin-Schuhmann and Rosin-Rammler distribution functions have been used to analyze and quantitatively describe the rock fragmentation size distribution in the existing muck-pile. Finally, taking the rock properties, explosive performance, blasting parameters, and system characteristic variable into consideration, support vector machine (SVM) regression model has been analyzed on the learning and prediction of samples. The results show that SVM has a good prediction accuracy, high precision, and strong generalization ability. The optimized matching coefficient of rock and explosive wave impedance K ranges from 2.50 to 2.58 times. This study has developed a series of simple, accurate methods for rock properties analysis, detonation wave velocity measurement, and muck-pile model image processing, and a basis for predicting and evaluating rock fragmentation size distribution and optimizing the matching coefficient before carrying out a blasting operation.

The mining industry collects a significant amount of operational data. However, gleaning useful information from the terabytes of data is difficult, and not just because of the sheer volume of the data. Therefore, an automated tool was developed at the University of Alaska Fairbanks to go through data and apply sophisticated statistical and neural network techniques in order to identify the data streams that are important to a process. This paper presents results from the tool as applied to SAG mill data from a gold mine. The results were compared to results achieved earlier with available commercial modeling tools. The comparison indicates that there was little or no loss in performance by automating the very complicated process of neural network modeling. Therefore, the intent of the exercise, to examine if complicated modeling tasks can be automated, was realized. Mining Engineering, 2011, Vol. 63, No. 12, pp. 52-54.
