The Work Health and Safety Act 2020 (WA) (WHS Act; DMIRS, 2020) and associated Work Health and Safety (Mines) Regulations 2022 (WA) (WHS Regulations; DMIRS, 2022), supporting guidance materials and other laws, such as the Sex Discrimination Act 1984 (Cth) (SD Act), have imposed new duties on every person or business conducting an undertaking (PCBU) in Western Australia, including those operating in the mining industry. These laws and associated guidance materials combine to require owners, operators and contractors to discharge positive duties and better report and manage psychosocial hazards in the workplace. For mine operators, there is a positive duty to not only identify and eliminate hazards relating to psychological harm, sexual harassment and gendered violence at work, but to also report incidents to the safety regulator that could result in serious harm, even while internal and police investigations are ongoing. Traditional mines safety teams are navigating the evolving frontier of psychological and gendered safety as the duties relating to psychosocial safety and sexual harassment at work continue to expand. This paper will review the legal obligations on PCBUs to conduct internal investigations into critical psychosocial incidents and gendered violence on mines, whilst balancing the demands and sensitivities of investigations by the safety regulator and police. There are emerging approaches to safety investigations that can aid PCBUs in managing their investigations of psychosocial incidents and gendered violence, including trauma-informed investigation techniques, collaboration with psychological experts and the use of limited legal privilege. Well managed and sensitive investigations can foster trust amongst workers, result in improved incident reporting by workers and lead to enhanced safety outcomes for the resources industry whilst preserving the legal positions of involved PCBUs. Specifically traversing the new duty to consult, cooperate and coordinate, this practical session will arm mine managers, safety professionals and human resources professionals, from mine operators and mining contractors alike, with tools and processes to bring to safety investigations, particularly on residential mine sites.

The proposed Rockwater Power Project is a hydropower development on the Waitaki River in the South Island of New Zealand. The proposed development will have an installed capacity of 250 MW with a peak flow capacity of 260 m3/s. The project commences at Lake Waitaki and extends some 35 km downstream on the north bank to a surface power station at Stonewall. Flow is returned to the Waitaki River via an outfall channel.The tunnelling for the project is through Greywacke, which comprises Permian and Triassic age marine sediments that are well indurated and weakly metamorphosed. The rock mass is jointed and sheared and there are frequent faults, which range up to hundreds of metres in width. Three 12 m diameter tunnel boring machines (TBMs) are proposed to excavate the 35 km long tunnel.The paper describes the development of the tunnel alignment, rock mass classification, design of the support, rock and hydraulic loads, support installation options and construction sequencing.The alternatives of rock bolts and shotcrete and segmental lining were both investigated and are discussed. Support using rock bolts and shotcrete offers the potential of rapid construction with minimum support cost when the ground conditions are good, but delays due to support installation increases with deteriorating ground conditions. In contrast, segmentally lined tunnels offer greater certainty in cycle time with reduced program risk. Both options are described and discussed.The horizontal alignment is affected by the presence of major fault zones, which are either avoided, or pretreated by driving conventional tunnels through in advance of the TBM delivery, topographical constraints with the rugged topography and operational hydraulic constraints. Construction requirements, future power station operation and the minimisation of leakage from the tunnel have all impacted on the vertical alignment.The number and sequencing of TBMs to be used is discussed in terms of program time and risk minimisation.

An estimation geologist has many tools in their kit and using the correct one for the job is vital for creating a useful model. Reconciliation is a key step to monitor the performance of a resource model and to ensure the correct tools have been selected and appropriate parameters are set. This paper presents an example of refining the estimation methodology for a Channel Iron Deposit (CID), resulting in an improved model. Rio Tinto Iron Ore (RTIO) CIDs are unique iron ore deposits located in the Pilbara region of Western Australia and contribute a significant volume to the world’s seaborne iron ore market. A problematic feature of these deposits is the irregular dispersion of clay pods that cannot be delineated with confidence within the resolution of the exploration drilling. Resource to grade control reconciliation showed an over-prediction of high-grade material in the resource model. A fresh approach was required to improve grade and tonnage predictions. Historically, clay pods were delineated using traditional wireframing techniques and estimated using ordinary kriging with hard boundaries. The new method utilised indicator kriging to estimate the probability of clay. Probabilities generated from the indicator kriging estimate were used as an additional weighting following the ordinary kriging of grades. Results showed indicator kriging improved global high-grade tonnes reconciliation, however, as expected, it did not improve the spatial prediction of clay. Based on improved reconciliation results indicator kriging has now been adopted as a suitable alternative for analogous CIDs at RTIO. These improvements benefit the mine planning process, but do not replace grade control models.

Tailings dam breaches have caused significant life loss, social, economic and environmental damage in recent years. The term "tailings-flow" refers to various forms of tailings outflow movement resulting from the breach of a tailings dam. These flows, comprising a mixture of tailings, water and dam materials, can take different forms, ranging from a debris flood to a flowslide, and can travel long distances and impact large areas. The inundation extents depend on the failure mechanism, tailings dam height, total released volume, sediment concentration (associated with the rheological behaviour of the flow) and downstream conditions (including topography, land cover and presence of a water body). Numerical runout modelling of tailings dam breaches is important for predicting the dynamics and extent of tailings-flows, which are crucial steps in tailings dam consequence analysis and risk management. This paper presents a comparison of numerical simulations of the 1994 Merriespruit tailings dam breach using three numerical models: DAN3D, FLO-2D and MADflow3D. Using compiled site condition information and data from the literature and public domain, model performance was evaluated primarily based on comparisons between the observed and simulated inundation extents. The main purpose of this preliminary model benchmarking exercise was to help us evaluate the capabilities and limitations of each model, and to identify opportunities for future model improvements as part of a broader research initiative known as CanBreach. This exercise also highlights the inherent complexity of tailings-flows and the importance of using judgement to interpret model input and results.

The Telfer Underground sublevel cave (SLC) Operation is located approximately 850 m below surface. The cave was designed to propagate through an old decline which was initially used to monitor the cave. There was a requirement to install air blast plugs at two locations in the old decline in order to isolate any potential air-blasts from the active workings in the event of the rock mass collapsing into the cavity. There was a further requirement for an air-blast plug in another section of the mine where there was a risk of the cave intersecting an old workshop and providing an air-blast pathway through an abandoned incline. The design criteria for the plugs was that they should withstand the design air blast loading, be free draining, cost effective and easily constructed with the mining expertise available on site. A plug consisting of grouted mullock was considered robust and least challenging to construct. For the purpose of estimation of maximum air pressure on plugs, it is supposed as a worst case that the rock above a cave falls as an airtight piston onto the top of a muck pile, forcing air into it and also mine workings intersected by the cave. Airflow through the muck pile is governed by Ergun's equation for turbulent flow. In a time-stepping integration scheme, transient air pressures and velocities are calculated according to equations of thermodynamics, in the cave and at intervals along intersected drives by means of a finite difference spatial discretisation. The ability of plugs to withstand computed pressures is assessed by finite element elastoplastic analysis. This paper outlines the rationale for potential air blast loading, calculation of transient air pressures, plug design and analysis of plug behaviour. The plug construction method is also discussed.

"This paper presents a case study demonstrating active geotechnical pit slope management at the now completed Carina Iron Ore Project operated by Mineral Resources Ltd (MRL). The site is located 105 km north-east of Southern Cross, Western Australia. The paper is primarily focused on the slope stability failure modes and the remedial measures employed to maintain mining.Between a quarter and a third of the excavated slopes were impacted by some form of instability that was beyond expectations of the intended slope design criteria and common practice. Ongoing mining was supported by intensive geotechnical pit slope management including close cooperation between geotechnical, mine planning and operations staff to achieve timely and practical mining solutions.Mining encountered varied slope failure mechanisms which required a combination of remedial solutions comprising: horizontal depressurisation holes, deferral of excavation, unloading, buttressing and catch berms.The root cause of the challenging slope performance was a mismatch between the predicted and encountered geotechnical model, resulting from a failure to recognise the critical mechanisms that control slope stability.Despite the extensive zones of instabilities at completion of mining, full ore recovery was achieved without major delays and in a controlled and safe manner.Citation:Fowler, M, Strang, D and Jaine, R, 2016. Silicomanganese (SiMn), containing approximately 16–30 per cent Si, is produced in submerged arc furnaces using various Mn sources and quartz as the primary raw materials, along with fluxes and reductants. In the high-temperature zone known as the coke bed, a silica-rich slag is formed. SiMn is produced through the reduction of SiO2 from the slag. Dissolution of quartz into slags with different compositions, the primary slag formed from the Mn sources and flux before any MnO reduction occurred, representing conditions at the top of the coke bed, and dissolution of quartz into a partially reduced slag to simulate conditions inside the coke bed has been investigated. Experimental investigations were carried out to study the dissolution of quartz particles of varying sizes, 5 mm and 25 mm in synthetic slags with different compositions at temperatures between 1400°C and 1600°C, with varying holding times. Observed results are compared with liquidus compositions and equilibrium phases for original and formed slags. Particle size has been shown to be of little importance for the dissolution. Quartz dissolved more easily in slag at top of the coke bed than slag in the coke bed. Complete dissolution was observed after 30 mins at 1600°C in all cases, but not after 0 mins.

"Evolution Mining is the sole owner of the Mt Rawdon open cut operation, located 75 km south-west of Bundaberg in Queensland, Australia (Figure 1). It is a massive, volcanically hosted, low-grade gold deposit that exhibits excellent characteristics conducive to low cost mining and treatment.In 2014, Mt Rawdon began investigating initiatives to deliver higher mill throughputs without any significant capital expenditure or modifications to the existing processing plant. Various studies have shown throughput increases are possible by applying more blast energy, essentially by reducing feed size through blasting and leaving less work for the comminution circuits (Brent et al, 2013; Ziemski, 2011; Workman and Eloranta, 2003).Given that 53 per cent of energy used on a mine site is consumed by the comminution circuit, with only two per cent being consumed by drill and blast (Smith, 2013), it makes logical sense for a mining operation to align their drill and blast with their processing plant. The term commonly used when exploring this relationship is a ‘Mine to Mill’ program.Over nearly 12 months, various blasting options and Mine to Mill methodologies were tested at Mt Rawdon. Throughout the journey, it was abundantly clear that the Mine to Mill initiative is not as simple as making one technical or physical change and expecting an improvement. Rather, true sustainable improvements demand a paradigm shift across both the mining and processing cultures, where all departments are aligned and working towards the one outcome. This true overarching Mine to Mill mentality, enabled by Vistis™ 250 high energy bulk explosive, ultimately led to a sustained increase in throughput and profitability of the Mt Rawdon operation.Citation:Wallace, G and Morton, D, 2016. Major falls of ground, strainbursts, airblast and mudrush events are the major hazards in block caving environments, resulting in both safety and financial consequences and losses. Rio Tinto mines have developed strategies to reduce the likelihood of these events occurring. However, until recently, there have been no tools available to monitor and manage these hazards in real time.Mira Geoscience and Rio Tinto have collaborated to develop real-time geohazmaps to monitor and manage major cave hazards. These geohazmaps are used by mine planners to integrate multiple hazard parameters that contribute to form a combined hazard rating.The benefits of implementing geohazmaps include safety and cost improvements through the early identification of potential locations for major events. These cost savings are realised in the design phase, where development and critical infrastructure is located in areas of lower hazard rating. Additional improvements can be achieved by taking the hazard level into consideration when planning the production and development schedules and ground support design.CITATION:Talu, S, Savage, E and Howe, S, 2016. Waste blasting in iron ore operations can pose a number of technical challenges, including blasting through variable geology and blasting soft ground in wet conditions. Blast patterns are designed to be loaded with a specific range of explosives with unique characteristics and properties. Each explosive product will produce an optimal blast outcome for a given set of blast parameters and ground conditions. Sites characterised by softer rock typically require low-density, low-energy explosives. Historically, these products have contained a high ratio of porous ammonium nitrate prill and are only suitable for dry blastholes. When blastholes within a pattern are found to be unsuitable for such explosives due to wet, damp or muddy blasthole conditions, a new product is selected. While it is easy to switch products to match blasthole conditions, the different properties of the new non-ideal product can impact blast performance and increase loading times and costs.Flexigel™ is a solids-sensitised emulsion blend suitable for loading at variable densities and energies in dry, damp, muddy or wet blastholes. It is a universal product that can be loaded to match ground and blasthole conditions or complement other explosive products.At Fortescue Metals Group’s Cloudbreak site, Flexigel™ was trialled through a series of matched pair blasts in order to optimise blast outcomes in conditions that were non-ideal for existing explosive products and to reduce the overall cost of drill and blast operations. Flexigel™ yielded improved instantaneous dig rates and improved loading efficiency when compared to a traditional emulsion blend. The results also indicated that variable-density and variable-energy products afford greater flexibility for optimising blast outcomes across all ground and blasthole conditions trialled.Trials of Flexigel™ at Cloudbreak have shown that a variable-density solution is available for conditions where traditional explosive products are non-ideal. As such, Flexigel™ is able to fill a major void at any operation where the desired low-density, low-energy explosive product must be substituted for a more expensive traditional emulsion blend or heavy ammonium nitrate/fuel oil.CITATION:Andrew, D R, Morrison, K T and Tiedgen, J F, 2015. Failures within hard rock underground excavations have historically been defined by a depth of failure, estimated in the field or through individual measurements, with a simplified failure geometry; such as 1/3 span, cubic or paraboloid. This simplified shape, volume and mass of failure is typically used to estimate in situ demand as a simplified discrete point. However, the highly variable failure geometry indicates simplified methods to estimate the mass can be erroneous in comparison to high precision LiDAR scans. Advances in technology have enabled the accurate capture of failure geometry and allow a far more accurate analysis of failures to be completed. This case study investigates a number of real-life failures to accurately define the failure geometry, the distribution of the failure parameters and compares this to traditional methodologies. This paper also outlines a suggested means of standardising failure geometry parameters whilst accounting for their variability.

"Cayeli Bakir Isletmeleri is an underground copper-zinc mine in turkey. The ore is highly variable with feed grades ranging from 1–4 per cent copper and 1–10 per cent zinc. Due to this variability, ore characterisation and blending strategies are crucial for performance optimisation.This paper presents a case study for a multidisciplinary approach to process optimisation, integrating Geometallurgical and advanced control methods to achieve optimised plant performance. The results from comprehensive metallurgical tests and surveys were used to develop a mathematical model utilising SGS’s Integrated Geometallurgical Simulator® (version number 2.5.24.0). The model was employed to determine the most appropriate operational settings required to ensure that optimised grades and recoveries are achieved for each ore type.SGS’s Modular Expert Technology (MET) expert system was then implemented to target these optimum operational ranges utilising online grade and froth velocity measurements. In 1983, open cut coal mining operations were suspended at the Tent Mountain coalmine, located in Alberta, Canada in an area called Crowsnest Pass. Montem Resources (‘Montem’), an Australian mining company, purchased the asset in 2016. They conducted several drilling campaigns and undertook environmental monitoring with the intent to restart operations. In 2020, Montem completed a Definitive Feasibility Study, which indicated a mine life of more than 14 years.

Environmental, social and governance (ESG) factors have always been important to the minerals industry and is now considered one of our most important opportunities (and risks). Getting it right from the beginning is critical for the minerals industry, and that starts with mineral exploration. Exploration, or the process of investigation of an economic mineral deposit, is generally low impact and short-term. Exploration operations vary in size and scale and often shift priority, focus and location relatively quickly. Australia is a highly regulated jurisdiction with different regulatory systems and requirements in every state and territory. Most mineral exploration is undertaken by junior exploration companies or ‘explorers’. Exploration companies are lean operations with limited staffing and resources, traditionally with no income apart from funds raised or work through partnerships. Funds are tight and administrative investment is minimised so that investment into activity on the ground is maximised. All these variables mean that understanding and implementation of ESG frameworks for an exploration company can be challenging. ESG is a fast-moving sector with currently multiple frameworks and reporting standards. Interest from community and government stakeholders as well as investors and off-take partners has been growing, with mandatory reporting soon to be implemented. The upcoming changes to the JORC Code will ensure that ESG considerations will be a key component of all public reporting. Resources have been developed to support explorers implement, manage and report on ESG considerations. Due to their scale, agility and direct importance of land access and social licence, many explorers are not the ‘cowboys’ (or ‘cowpersons’) of the industry, but industry leaders in ESG. Strong interest and leadership have led to innovative practices and strongly embedded vertical frameworks that directly contribute to social licence to operate.

"This paper is focused on blasting activities undertaken as part of the Pacific Highway Upgrade – Tintenbar to Ewingsdale – project (T2E). This is a ‘priority 1’ Pacific Highway road construction project in New South Wales (NSW), to be completed by mid-2015 (weather permitting). T2E will provide approximately 17 km of dual carriageway road starting at the northern end of the Ballina bypass and extending to the Ewingsdale interchange, and will complete the existing dual carriageway road between Ballina and the NSW/Queensland border.Roads and Maritime Services (RMS) (formerly the Roads and Traffic Authority) is delivering the project on behalf of the NSW state and federal governments. In October 2011, RMS awarded a contract to Baulderstone Pty Ltd (now Lend Lease Engineering Pty Ltd – Lend Lease) to design and construct T2E. One of the key elements in this project is the St Helena Tunnel.In order to avoid the steep grades of St Helena Hill, eliminate a well-known accident black spot and maintain the aesthetics of the Byron hinterland escarpment, RMS and Lend Lease prepared designs to construct a 434 m long dual carriageway tunnel, 46 m below the ridge line of St Helena Hill. Lend Lease elected, where geotechnical conditions permitted, to use drill and blast methods to excavate the full tunnel profile, instead of traditional mechanical excavation techniques.Since all blasting locations (both inside and outside the tunnel) were in close proximity to nearby residents, the NSW Department of Planning and Infrastructure imposed strict air blast overpressure (airblast), ground vibration (vibration) limits and limited working hours. Both RMS and Lend Lease recognised that these restrictions meant that nearby residents may be exposed to blasting activities for longer than was necessary. As a result, after close consultation and agreement with the nearby residents, RMS applied successfully to the NSW Department of Planning to increase the airblast and vibration limits, as well as the working hours.Working under these new conditions, Lend Lease employed a range of best practice drill and blast technologies provided by both Orica Australia Pty Ltd and Atlas Copco Mining and Rock Excavation to improve the safety and efficiency of blasting activities and to minimise the potential negative impacts of the blasting works on affected residents. These technologies included: the use of a continuous environmental monitoring system to record the results of the blast events and provide quick and easy reporting to key stakeholders to address RMS specification requirements electronic detonators to enable accurate hole detonation timing for vibration compliance and to give project engineers the flexibility and precision to alter each design to suit the geological profile of each face the use of two software programs and advanced drill rigs to enable the efficient design of blasts, electronic transfer of designs to the drill rigs, as well as in-hole monitoring and recording of drilling performance the use of a specialised bulk emulsion system to deliver fully coupled bulk explosives into face blastholes to maximise fragmentation, and decoupled bulk explosives into perimeter blastholes, to reduce overbreak and deliver a cleaner profile.By taking a progressive approach towards the environmental restrictions and the work hours imposed, as well as utilising best practice blasting techniques, RMS and Lend Lease were able to blast with confidence and achieve their production deadlines and reduce the total cost of the project.Most importantly, RMS and Lend Lease were able to meet their environmental obligations and work constructively with the nearby residents to ensure that the blasting was undertaken as safely and efficiently as possible without delay or negative impact.CITATION:Dowling, M, Domotor, R and Miller, D, 2014. Otago Schist and Klondike Schist are two Paleozoic-Mesozoic metamorphic belts on the circum-Pacific margin in which there has been a long history of abundant alluvial gold mining, but only minor hard-rock gold mining.   Modern exploration for hard-rock gold is ongoing in both belts, and the active Macraes mine in Otago (>2 Moz production) highlights the hard-rock potential of the geological setting of these belts. Hard-rock gold mineralisation in both belts formed in the latter stages of schist exhumation, after rocks were subjected to major regional ductile folding and then uplifted through the brittle-ductile transition. The majority of known hard-rock gold-bearing systems in both belts consist of well-defined, but generally discontinuous, vein swarms. Most Otago veins are hosted in steeply dipping post-metamorphic normal faults and fractures. However, the Macraes mine is hosted by a late-metamorphic shear zone, and much ore is sulphide-impregnated auriferous schist. Goldbearing veins in the Klondike schist are hosted primarily in extensional sites associated with post-metamorphic compressional structures. Some disseminated sulphide mineralisation is associated with Klondike veins and this is important as it extends the exploration target for these veins. No foliation-parallel late metamorphic mineralisation has been identified in the Klondike but exposure is relatively poor compared with Otago and both schist belts are highly prospective for deposits of this type.

Northern Star’s Thunderbox Open Pit C-Zone cutback was designed to maximise extraction in a short life excavation; In fresh rock on the western wall this included 20 m to 30 m bench heights with 6.5 m to 10 m berm widths and a 90° pre-split angle. Rock mass quality in this area is generally fair to good, however, there are sections where the dominant foliation runs parallel with the designed pit wall, and more closely spaced, altered, foliation is present: these areas are difficult to predict and prone to failure. Stringent controls and monitoring were required in order to safely access below these areas, in particular the use of slope stability monitoring radar, Trigger Action Response Plans (TARPs), re-entry procedures, visual inspections, drone inspections and day-to-day personnel awareness of the hazard. Robust and comprehensively planned remediation was also required after two significant slope failure events where long reach equipment and rope access techniques were utilised to remediate the areas in question safely, but also in a timely manner, such that expected production targets were impacted as little as reasonably practicable. This combination of rigorous risk management and well planned remediation resulted in a successful and complete extraction of the C-Zone cutback with no impact to operational safety.

The aim of any rock breaking activity is to achieve success by developing designs that are blasting objective-specific and geology-specific, and then implementing such designs. This paper targets mining geologists with the aim of extending their knowledge of the rock fragmentation processes and providing insights to the significant relationships between geology and blasting. There are two main relationships, and if mining geologists appreciate both these relationships, then they can play a key role in optimising production. The business risk continuum is introduced to support this understanding. The first relationship is between geology and potential blasting hazards, and the operational complexity required to manage those risks. The risks associated with eight different ground types are discussed in terms of ground conditions, hazards and controls. These types are: reactive ground, hot ground, geothermal ground, fibrous ground, cavernous ground, combustible ground, pyritic ground and conductive ground.The second relationship is between the bench geology and blasting results. The blasting results discussed include: muckpile characteristics, remaining rock faces and blasting emissions. In this regard, some detailed insights and graphical examples are given for the influence of geology on blasting emissions, in particular, vibration, airblast and flyrock.A detailed description of the geological materials themselves can convey much useful information for blasting operators provided they are able to utilise this knowledge. Geologists are ideally suited to provide this description more effectively if they are familiar with the blasting process and the rock mass properties that influence blasting hazards and blasting results. It is also important to have clear statements of blasting objectives and associated operational targets, and this will maximise the likelihood of achieving the desired outcomes. The main aim of the paper is to highlight that geology is currently not fully utilised in the blasting process. In this regard, geologists are often not entirely aware of the blasting process and how they can contribute. The two significant relationships discussed above are integral parts of this process, and thus they are discussed as separate issues in the main body of this investigation.

The Ambuklao and Binga Rehabilitation Project is a 175 MW hydro power project currently under construction across dual sites in the north region of Luzon Island, Philippines. When it was built in 1956, the Ambuklao Dam harnessed the second largest hydro-electric reservoir in Asia. However, due to landslide erosion and prolonged silt infiltration caused by an earthquake in 1990, the power plant has been rendered inoperable since 1999.To bring the power generating facility back to life, the project scope entails the rehabilitation and construction of civil structures, shafts and tunnels. A major component of these works is the decommissioning of a section of the seven metre diameter headrace tunnel by installing a concrete plug. The headrace tunnel is part of a closed waterway system that connects the dam via a shaft, exhibiting a hydrostatic pressure of 13 bar (1300 kPa) at the location of the new tunnel plug. In addition, the power station ball valves have seized ajar and the damaged intake riser is unsafe toaccess thereby requiring the plugging operation to be carried out under live flowing conditions.Working under hydrostatic pressure and live conditions, the headrace plug will be an underwater tremie pour operation that includes drilling and concrete injection through blow out preventer caissons installed in the floor of an underground chamber above the tunnel. The purpose of this paper is to describe the key engineering and planning components involved in this operation including pre- and post-grouting, chamber crane, sonar, local water compensation, jet flushing, drilling, caisson installation, and concrete design, production, delivery and placement. The success of this task rests on the strategic preparation and operational management of risk and safety in a challenging rural environment.
