"The current paper presents the results of a study designed to explore the types of practices that are fundamentally important to developing and maintaining comprehensive risk-based health and safety management systems (HSMS) in mining organizations. Eighteen practicing health and safety (H&S) executives, managers, and professionals from a variety of mining commodities were asked to assess the fundamental importance of 20 distinct elements and 133 different practices assembled from a review of several HSMS consensus standards. The research approach assumes that developing and maintaining a robust HSMS requires unwavering attention to activities designed to either: 1) directly contribute to a risk management system’s plan-do-check-act cycle; or 2) build an organization’s internal infrastructure without which the cycle cannot be sustained. The results of this study suggest that the following areas require consistent focus and attention: Leadership Development; Accountability; Knowledge, Skills, and Abilities Development; System Coordination; Culture Enhancement; Behavior Optimization; and Risk Management Studies. Each element and its corresponding top-ranked and top-rated practices are discussed.INTRODUCTION Since the time of the Industrial Revolution, occupational health and safety (H&S) management has been a topic of practitioner thought and academic inquiry (1). In the U.S., decades of regulation and research have resulted in a surplus of prescriptive information (techniques, tools, approaches, standards, programs, practices, policies, etc.). With an abundance of such information, a simple H&S solution to address a problem in the U.S. mining industry may seem straightforward. Counter to this assertion, however, some evidence suggests that the abundance of information may create complexity and cause confusion in the minds of organizational leaders and H&S managers, ultimately discouraging the voluntary pursuit of a sound approach (2). In line with this perspective, in 2006 the U.S. National Mining Association (NMA) formed an independent commission to examine H&S management issues within the mining industry. The findings of the commission called for the mining industry to adopt a risk-based management paradigm. In the presentation of its findings, the commission stated that “…every mine should employ a sound risk-analysis process, should conduct a risk analysis, and should develop a management plan to address significant hazards identified by the analysis” (3). Grounded in the well-known plan-do-check-act cycle, the recommended risk management system allows for mines to ground strategic management choices in their mine-specific hazards and risk profiles"

Mining activities have recently been taken place in Vourinos mountain, northern Greece, exploiting the so-called ?olivinite?. A representative sample was collected from this mining area. Mineralogical analysis was conducted using X-Ray powder Diffraction (XRD). A thin section was prepared and analyzed under optical microscope. X-Ray Fluorescence (XRF) was employed for the chemical analyses of major, mi-nor and trace elements. Environmental Scanning Electron Microscopy coupled with energy dispersive x-ray analysis system (ESEM-EDX) was also applied for the characterization of the sample. Forsterite (Mg-rich olivine) and enstatite (Mg-rich orthopyroxene) were the main mineral phases determined by XRD, with a 56% and 44% content respectively. Trace amounts of chromite spinels were also determined. Based on this characterization, the industrial rock proved to be a hartzburgite (a type of peridotite in petrological terms). Optical microscopy and ESEM-EDX provided similar observations and confirmed the above-mentioned results. XRF revealed a composition of 43.31 MgO, 44.25 SiO2, 9.98 Fe2O3, 1.40 CaO, 0.67 Al2O3, 0.22 MnO, 0.01 TiO2 and negligible amount of LOI (<0.1%). Trace elements with high concentrations were Cr (2678 ppm) and Ni (2044 ppm). The comprehensive characterization of this mining product suggested the suitability for various uses, such as refractories, foundry and slag conditioner. Additional experiments are being carried out for environmental applications, e.g. wastewater treatment.

Large quantities of phosphate rock are mined and processed in Central Florida (Figure 1). The phosphate rock from the mines is further processed in "chemical plants" to produce phosphoric acid (Figure 2). A by-product of the processing&apos; of the phosphate rock to produce fertilizer chemicals is an impure form of gypsum referred to as phosphogypsum. For each ton of phosphate rock processed, approximately 1.5 tons of phosphogypsum is produced (Figure 3). The typical method to dispose of this by-product gypsum is to stack it in large piles, locally referred to as gypsum stacks or gypsum fields (Figure 4). These gypsum stacks have been the focus of many studies in recent years in an attempt to identify the potential for ground water and/or air pollution associated with the stacks. This paper attempts to discuss some of the ground water impacts and current attempts to minimize these impacts.

The aim of this study was to investigate desulfurization and de-ashing of a mixture of subbituminous coal and gangue minerals by the agglomeration method. For this purpose, experimental studies were conducted on a mixture containing subbituminous coal, pyrite, quartz and calcite. The effects of some parameters that markedly influence the effectiveness of selective oil agglomeration, such as solid concentration, pH, bridging liquid type and concentration, and depressant type and amount, were investigated. Agglomeration results showed that the usage of various depressants (Na2SiO3, FeCI3, corn starch, wheat starch) in the agglomeration medium has a positive effect on the reduction of ash and total sulfur content of agglomerates. It was found that an agglomerate product containing 3.03% total sulfur and 25.01% ash with a total sulfur reduction of 56.71% was obtained from a feed that contained 7% total sulfur and 43.58% ash when FeCI3 was used in the agglomeration medium. Wayne Anderson Regarding the technical paper by Thomas E. Finch and Edward L. Fidler, "Recovery of Stray (Thin) Seams in Western Coal Mining." I would like to comment on this subject. In western Canada, most strip mines encounter thin seam coal or rider seams. Although the paper addresses the problems and costs associated with the recovery of this coal, it does not take into account what the savings are. Without looking at the savings, the "Total Objective" is of course not clear. I feel that in a stripping operation the mine can spend per ton (of the thin seam) up to their dragline stripping costs per ton to recover a rider seam or thin seam. The reasoning behind this is simply that this is the price the mine will have to pay to expose an equivalent amount of coal without the recovery of these thin seams. If the mine can recover a rider seam or thin seam for less than the dragline operating costs per ton, a savings will be realized. By recovering a thin seam or rider seam, the life of an ore body is extended by that amount of coal not thrown away. In addition, the cost of land acquisition, associated permits, capital, and reclamation will be reduced as more coal is delivered for a given area. When the above points are considered, the recovery of these thin seams can be very profitable and indeed worth recovering.

Y. Luo and S.S. Peng are to be congratulated for their outstanding paper providing new results from surface subsidence monitoring over longwall panels under different conditions. This writer found the authors&apos; paper very interesting. In reference to this paper, the following contributions are offered: • In the paper, the maximum possible subsidence (?Smax) induced by chain pillar convergence has been related to a parameter (h/I -p) by linear regression. (Notations are expressed in this discussion&apos;s Fig. I (b).) It is a well-known fact that in a goaf the roof lowers onto broken material that gradually compacts. Then the physical behavior of goaf may closely follow the compaction characteristics of broken materials. In light of this assumption, a mathematical means (Fig. 1(a)) of defining the expected compaction (i.e. surface subsidence) represents an exponential expression between ?Smax/m vs. h/1-p. This is based on Luo and Peng&apos;s field observations from the Appalachian, Illinois and Alabama coal fields. (The coefficient of regression "r" under consideration was computed to be 0.883). • According to Wardell&apos;s (1969) findings for board and pillar mining, the maximum possible subsidence can be expressed in terms of maximum pillar loading (depth, ratio of extraction) and the height-to-width ratio of the pillars left. Using Luo and Peng&apos;s 1991 results, this writer discovered a regression equation between the above-mentioned factors. As displayed in Fig. 1(b), the functional expression approximates as closely as possible the observed subsidence data.

Southern Peru Copper Corporation (SPCC) has recently completed the construction of copper leaching, solvent extraction, and electrowinning facilities at their Toquepala and Cuajone plants in Southern Peru. Much of the ore for these new operations is the result of the existing mining operations that have been in existence as early as 1959. At Toquepala, pre-conditioning and bacterial dump leaching of the sulfide ore that was too low in copper content to be processed through the copper concentrator began in 1987. The Cuajone heap leach operation is treating copper oxide ore that has been stockpiled since the Cuajone concentrator began operation in 1976. The pregnant leach solutions from the dump leaching at Toquepala and the permanent pad heap at Cuajone are combined into one plant feed solution. Adjacent to the large dumps at Toquepala, a solvent extraction and electrowinning facility has been constructed. The electrowinning facility has a planned annual production rate of35,000 metric tons per year Grade A cathode copper. Initial production of the cathode copper began in November 1995.

"Fine-grained hematite ore can be concentrated by the process of selective flocculation and dispersion, which relies on proper reagent selection and control of water chemistry. While many previous studies have been performed analyzing the effects of different reagents on this process in a laboratory environment, this paper focuses on investigating the water chemistry within the process in a pilot-scale continuous deslime thickener.The pH, sodium concentration, calcium concentration and magnesium concentration were varied to determine their effects on the iron concentrate grade and recovery, and the phosphorus concentrate grade and rejection in the pilotscale selective deslime thickener. The ideal pH for the iron grade and recovery of the process using a starch selective flocculant was found to be 10.5. Phosphorus rejection, however, was increased at lower pH values. Minimization of sodium concentration was shown to improve iron grade, iron recovery and phosphorus rejection. Calcium acted as a nonselective flocculant showing higher iron recovery, lower iron grade and lower phosphorus rejection with increasing concentration. Conclusions could not be drawn from the experiments that varied magnesium concentration.The zeta potential of the solid-liquid interface of particles in each sample taken was also analyzed to show relationships between zeta potential and process performance. In all cases, a maximization of the magnitude of zeta potential correlated with increased iron grade and recovery. This supports the hypothesis that a higher level of dispersion enhances the selective flocculation and separation process.IntroductionThe United States produced 53.2 million metric tons out of the 3,000 million metric tons of concentrated iron ore produced worldwide in 2012 (Tuck, 2013). With this in mind, there has been a large increase in iron ore processing research (Bolen, 2014; Carlson and Kawatra, 2008, 2013; Halt and Kawatra, 2014; Halt et al., 2014; Haselhuhn et al., 2012a,b, 2013a,b; Liu et al., 2014; Manouchehri, 2014; Sandvik and Larsen, 2014; Semberg et al., 2014). Of the 53.2 million metric tons produced, roughly 8 million metric tons of hematite (Fe2O3) concentrate were produced using a process known as selective flocculation and dispersion (Cliffs Natural Resources, 2011). This process is the only economically viable method for concentrating low grade (< 40% Fe) fine grain (< 25µm liberation size) hematite ores. It is scarcely used due to difficulty in controlling the surface chemistry of the ore and high reagent costs (Department of Energy, 2001; Haselhuhn et al., 2013a). The process is highly dependent upon both reagent selection and precise control of the water chemistry during separation. This study shows the dependence of the process performance of a pilot-scale deslime thickener on the water chemistry in the feed slurry."

For the past 30 years, process flowsheet design has been accomplished largely through modeling and simulation. This approach provides circuit designers the freedom to test various separation approaches while minimizing physical and human resource requirements. The Limn® software provides a unique approach to process simulation, through a Microsoft Excel interface. While the software comes with standard process models for numerous unit operations, the Excel calculation environment and the accompanying VBA platform provide a convenient interface for custom model integration, even for non-experienced programmers. This feature is especially useful for university students in introductory process modeling and simulation-assisted flowsheet design courses as well as experienced practitioners with proprietary process models. This paper shows how the Limn® software may be used to solve basic process engineering problems as well as more comprehensive circuit design challenges. Industrial examples as well as opportunities for training and education are presented.

Ground calcium carbonate (GCC) is used in the paper, plastics, paint, rubber, caulks, and sealants industries. The favorable properties of GCC include high whiteness and brightness, low hardness, and outstanding rheology in water systems. The quality of calcitic GCC ores, which include chalk, limestone, and marble, is dependent on calcite purity and the nature and quantity of ancillary minerals. Wet and dry grinding is used in the manufacture of GCC. Ore processed by optical sorting, flotation, and/or particle size classification has higher brightness and lower abrasion. GCC markets are a function of the product&apos;s properties, purity, and cost.

Two pilot-scale biological reactor systems were installed to treat metal-contaminated water. The reactors consisted of barrels and tanks filled with spent mushroom compost, within which bacterial sulfate reduction became established. Concentrations of Fe, Zn, Mn, Ni, and Cd were lowered by over 95% as these metals were precipitated as insoluble sulfides by reaction with bacterially generated H2S. The chemistry of the reactor systems is examined, and opportunities for maximizing their metal-retaining and alkalinity-generating potentials are discussed.

An improvement in the present day screen sizing methods calls for knowledge on i) how material in the size range: 0.5 x 1.5 times the through-fall opening of the mesh on a deck, splits between the oversize and undersize streams of the deck, and ii) how near-mesh content of the feed affects the screening performance. In the throughput based sizing method given here, this is accomplished through the introduction of a generalized partition curve and a near-mesh factor. A newly developed efficiency factor is employed in addition to the traditional screen sizing factors.

Kaolin of the Georgia-south Carolina "Clay Belt" are used in a wide variety of filler and extender applications. Sane applications specifically require the performance enhancement delivered by incorporating kaolin into the product formulations. Other applications are less specific in requirements for an inert mineral filler. In order to be a feasible candidate for filling and extending, a mineral or mineral product must be generally inert (except where specific interaction with the filled medium is desired), inexpensive, easily handled and transported in a variety of forms, accessible to a broad segment of the desired market, and must exist in sufficiently large ore reserves as to assure production for many years. Of course, more precise requirements are necessary in specific applications of a prospective filler material.

Meramec Mining Company is a joint venture by Bethlehem Steel Corp. and St. Joe Minerals Corp. which mines and pelletizes iron ore at the Pea Ridge Mine. The Pea Ridge property is located near Sullivan, Missouri, about 70 miles southwest of St. Louis. The orebody was delineated in the mid-1950&apos;s by St. Joe during a lead exploration program. The first test holes drilled on the Pea Ridge magnetic anomaly revealed the presence of a large magnetite deposit; further drilling in 1956 and 1957 confirmed that the orebody was mineable, Meramec Mining Co. was incorporated in 1957 and shaft sinking began late in the year. Production commenced in April 1964. The orebody is overlain by about 1300 feet of flat-bedded sediments. It is tabular, about 2600 feet long and up to 600 feet thick. It dips about 80° and is of unknown depth. The ore is mainly high grade magnetite with small zones of specular hematite. The wall rock is a Precambrian rhyolite porphyry. The mine was initially started with five major levels on 150 foot intervals. Crosscuts were driven across the orebody on 190 foot centers. Banks of scopes were mined between the crosscuts by a modified shrinkage stoping method. This was done by undercutting a 40 foot wide by 150 foot long block and blasting horizontal "lifts" drilled on 5 foot intervals. A 65 foot still was left between levels which contained the slushing drift, fingers, crown pillar, and adequate thickness for support. By 1971 a ?lattice? of pillars had been left and mining had progressed to the point that an orderly pillar recovery program was necessary.

A large body of readily available Na-montmorillonite is present about 24km (15 miles) east of Fallon, Nevada. The clay is comparable to Wyoming Na-bentonite, but with somewhat less MgO and somewhat more Fe2O3 in its crystal structure. The physical and industrial properties of the Nevada clay are comparable to those of the Wyoming clay. The Na-montmorillonite is presently mined for use as hazardous waste barrier material and as a sealant for canals. It may be economically feasible to process the clay material for removal of impurities.

This paper presents the results of 18 high-G centrifuge tests performed at the Electric Power Research Institute&apos;s Coal Cleaning Test Facility in January 1985. These tests, using -28 mesh (0.6mm) froth flotation product, show that the high-G, solid-bowl centrifuge can produce clean-coal moistures of less than 12 percent at solids recovery rates exceeding 98 percent. The high-G centrifuge achieved these low moistures by applying centrifugal forces up to 2,000 times gravity. Because these forces are two to three times the forces used in a screen-bowl centrifuge, the high-G centrifuge can use a solid bowl and still produce a low-moisture product. The solid bowl captures ultrafines that would pass through a screen bowl, and thus obtains higher recovery rates than a screen-bowl centrifuge.

Oily and particulate befouled waste water discharges are a common industrial problem. Removing these water bourn emulsions from process discharges become increasingly important for both recycled and discharge water streams. Low cost, efficient and easily maintained systems are important. This paper evaluates design factor of such a water cleaning system -a trough filter. This is a continually operating filter that removes oil droplets by wires that wet the oil droplets.

Rogers Group, Inc., is the nation&apos;s largest privately held aggregates producer (primarily producing crushed limestone and secondarily sand & gravel) and a major regional asphalt paving construction contractor. The company also has interests in building supplies and coal. Facilities are concentrated in five states in the central U.S. (Ohio, Indiana, Kentucky, Tennessee, and Alabama), with an additional location in Southwestern Virginia. At the request of its then CEO and with support from the company&apos;s ownership, Rogers embarked on a strategic planning program in late 1986 by retaining an outside management consulting firm. The ultimate goal of this program was to develop a self-sustaining strategic planning process to inject a strategic dimension into every manager&apos;s business thinking and to improve company performance. Since then, Rogers has internalized the outside consultants&apos; strategic planning framework --initially, using it wholesale, and later, greatly streamlining and tailoring both the planning process and product to meet the specific needs of the company. Implementation of strategies developed from strategic planning has resulted in significant improvements throughout the organization. The latest product of the strategic planning effort is a company-wide (or, "corporate") strategic plan that carries the theme of "Market Leadership through Excellence". This paper provides a case study of successful corporate strategic planning by examining the evolution of strategic planning at Rogers and by summarizing the key elements of a sound planning process and product.

GENERAL DESCRIPTION The mines are located at Broken Hill in the western part of New South Wales, Australia, 930 km west¬northwest of Sydney. Mining Methods Where pillars must be left between stopes, extraction of a block of ore between two or more levels is carried out in stages: 1) Stopes are mined overhand by horizontal cut¬and-fill stoping, or by long-hole stoping. The various methods of horizontal cut-and-fill stoping used are desig¬nated as: -Loader cut-and-fill stoping with horizontal blast¬holes and diesel load-haul-dump (LHD) units. -Open stoping with horizontal blastholes and com¬pressed air or electric scrapers, or with diesel or electric LHD units. Square-set timber stoping, where ground condition precludes open stoping. The backs are sup¬ported with timber square sets. Pattern rockbolting of the backs may be used as an intermediate stage between open stoping and square-set stoping. 2) Vertical pillars are mined by undercut-and-fill stoping or by vertical crater retreat stoping. 3) Where a horizontal pillar has been formed below the level above the primary stope, this level pillar, usually 6.1 to 8.2 m in depth, may be mined by over¬hand square-set stoping or by undercut-and-fill. Long-hole stoping is used at the Zinc Corp. (ZC) and New Broken Hill Consolidated (NBHC) to mine parts of the zinc-rich A and B lodes. Drilling is carried out from drill drives and, in some cases, sills. Hole sizes used are 165 mm (61/2 in.), 110 mm (4&apos;h in.), and 65 mm (21/2 in.) nominal diam with hole lengths being limited to 50, 30, and 20 m respectively. Ammonium nitrate-fuel oil (ANFO) and water gel explosives are used. Broken ore is loaded from drawpoints at the bottom of the stope using rubber-tired diesel-driven LHD units which deliver ore to an orepass. Annual production at Zinc Corp. and NBHC accord¬ing to mining method is given in Table 1. At Zinc Corp. mining is being carried out at depths varying between 200 and 832 m. At NBHC it ranges from 528 to 915 m below the surface. The average travel time to the stope is 30 to 45 min. GENERAL ORE BODY REQUIREMENTS Size, Shape, and Dip There are six separate lodes which carry minable ore in the mines of Zinc Corp. and New Broken Hill Consolidated. The lowest of these and the main ore bodies presently being mined at Zinc Corp. are the lead lodes, consisting of a siliceous fluoritic ore body (No. 3 lens) and overlying it a calcite rhodonitic ore body (No. 2 lens). Each possesses characteristic ratios of lead, silver, and zinc, although the grade within each lens is variable, in places being as high as 30% Pb. There are also low¬grade portions which consist chiefly of poorly mineral¬ized rhodonitic material. No. 3 lens terminates within the Zinc Corp. leases, while No. 2 lens continues into the NBHC leases where it continues to provide a significant proportion of pro¬duction from the mine. Overlying the No. 2 lens are the various zinc-rich ore bodies. In ascending sequence these are lower No. 1 lens, upper No. 1 lens, A lode, and B lode. A typical assay for the small No. 1 lens group ore bodies is 8% lead, 50 g/t silver, and 20% zinc. The `A&apos; lode is a large low-grade ore body in which the higher grade sections assay 4% lead, 30 g/t silver, and 10% zinc; larger quantities of lower grade ma¬terial also occur. B lode, the main zinc lode, typically assays 5% lead, 30 g/t silver, and 17% zinc. B lode increases in size as it extends south, and in NBHC is a major ore body, contributing about 52% of the production. All the lodes pitch south, the average pitch being 0.52 rad (30°) in the Zinc Corp. leases and rather flatter than this in the NBHC leases (see Figs. 1-3). The ground is all competent rock. Production requirements at Zinc Corp. are 900 000 t and 1 200 000 at NBHC.

At Kerr-McGee&apos;s Galatia Mine in southern Illinois, the need for safe and efficient longwall moves prompted a detailed planning effort. Galatia Mine operates two longwalls on separate coal seams with different geologic conditions. An overview of the longwall move planning as related to manpower training, roof control, ventilation, transportation, equipment recovery and setup will be discussed. The impressive safety record achieved during each of the longwall moves at Galatia Mine verifies that health and safety planning does play a major role.
