This Paper describes a sudden spill of rock waste used for backfilling from a waste pass at one of the underground mine in Canada thus causing a major failure/accident. The mine operator immediately launched a detailed investigation to understand the causes of this accident and develop procedures to prevent any recurrence of such events in future. The investigators from McGill University were invited to collaborate with the engineers at the mine to examine this accident. The events leading to the accident were studied and appropriate measurements were taken and samples of the waste material was collected for further test and analysis. The field visit and the interview of the witnesses allowed a detail documentation of the events. The sequences of events related to the incident together with the experimental results obtained in the course of the investigation is detailed. The waste material was characterised in the laboratory with regard to its behaviour in the presence of water. This information is then discussed and the failure mechanism involved in this accident is identified. The full recommendation as well as the immediate operational measures implemented by the mine to prevent future occurrence of such events are discussed and presented.

"Research was performed to assess the feasibility of barrier membrane substances, for use within mining or associated high risk environments, in restricting the diffusion transport of radon gas. Specific tests were conducted, both within laboratory and in situ environments, to determine radon gas concentration ratios across a barrier of interest and emanation rate variation resulting from application of such materials. Several materials have been demonstrated to be effective in restricting the emanation of radon gas into initially radon-free environments. One liquid material shows particularly good promise upon consideration of ideal radon barrier criteria.IntroductionIt has long been known (I.2.3) that inhalation of the short-lived, alpha particle-emitting daughters of radon gas can result in irradiation of lung tissue with the potential for causing lung cancer. Control measures are necessary to maintain concentrations of airborne contamination at a minimum to protect miner health and safety. Typical control practices in mines have been through the application of systematic ventilation and backfilling of excavated opening. As mines become deeper, however, and additional workings become exposed, increased ventilation capacity requirements are needed. As an alternative to other radiation control measures, the use of sealants to prevent radon migration from walls into mine atmospheres may provide a suitable engineering alternative."

This U.S. Bureau of Mines report identifies the types of serious accidents that occur most frequently at small underground coal mines and describes the strategies that could help prevent these accidents. A wide variety of methods for improving safety are suggested, including improvements in the design of equipment, work procedures, work schedules, safety programs, and emergency response plans, as well as techniques for diagnosing the potential hazards associated with new technologies and work procedures. Some of the papers in this volume focus on preventing specific types of mining accidents--ones associated with materials handling (primarily back injuries), equipment maintenance, improper machine guarding, and ground failure during retreat mining. Although the recommendations in this volume are heavily influenced by research performed at underground coal mines in the Appalachian coalfields, most of the papers contain advice that is equally pertinent to almost any type of mine.

Casting, cooling and crushing/sieving of ferroalloys has followed a well-established pattern in the whole ferroalloys industry for many years. Usually liquid metal or alloys are cast into flat open moulds of various shapes and forms, and after solidification the product is removed from such mould to further cool down to a temperature where subsequent handling steps are feasible. Common to all mechanical product fractioning is the generation of dust and fine size material with lower product value. For almost 10 years, Bécancour Silicon has investigated alternative roads to improve technology and/or reduce or solve problems related to those disadvantages. Since 1995 Bécancour Silicon operates a special silicon granulation plant targeted to product for the chemical treatment/conversion of silicon for silicones production. Prior to the development of the granulation process, various systems had been evaluated, among others the vibrating casting bed, now in operation elsewhere but judged inappropriate because of the tapping volumes to be handled at Bécancour Silicon and other disadvantages. After evaluation of all aspects of heat transfer, radiation, and conductivity in relation to transfer media and within the product, a novel system was designed and built.

The 1981 Review: In the late 1970s there was an upsurge of interest in the prospecting and mining of this countryÆs mineral resources. That upsurge of interest was accompanied by a mounting public concern over the damage mining operations might do to the environment. Concern was also expressed about what some people perceived as inadequacies in the legislation which regulates mining and the grant of mining privileges, and inconsistencies in the relationship of that legislation to other legislation regulating land use and the use of resources.

Volume I of this report described the demonstration of cross-ridge mountaintop coal mining. This document, Volume II, is intended for use as a supporting document to Volume I. This report describes the MATHMINE Mine Simulation System, a modularized software system used to support the demonstration of cross-ridge Mountaintop coal mining. The MATHMINE Mine Simulation System was used to generate mine plans for coal extraction at lowest cost. The functional details, data requirements and output of the software system are detailed here. The computer program was written in FORTRAN. One module which generates resource location data required the use of SAS?.

Fires in underground coal mines are persistent problems anywhere in the world that coal is mined. Coal fires pose safety and health hazards in the form of mine subsidence, loss of energy reserves, destruction of property and surface improvements, and air pollution. In 1995, the US Bureau of Mines (US-BM), in cooperation with the Colorado Division of Minerals and Geology (CDMG),entered into a contract with Goodson & Associates Inc. (GAI) to design and field test the effectiveness of a new foamed grout technology called ThermoCell. GAI is a Lakewood, CO geotechnical and mining reclamation engineering firm. The company developed ThermoCell to control and extinguish underground coal fires. The viability of the special grout and the technology of application were established during a full-scale field demonstration at the abandoned IHI Coal Mine fire, near Rifle, CO. Following acquisition of additional funding by CDMG in 1999, Goodson returned to the site in April to continue the grouting to fully extinguish the IHI fire.

SO much has been written recently about the use of silver to replace scarce metals that certain facts about silver and its uses should be of interest. Figures for the production and use of silver, taken from the Handy & Harman annual reviews of the silver market, are given in Table 1. It will be seen that the rate of production has remained fairly constant and has been affected in recent years by the production of base metals, of which silver is a by-product. In each of the years 1937, 1938, and 1939, the U.S. Government acquisitions alone exceeded the entire world production, the difference, of course, being made up by imports of previously accumulated stocks. In 1940 and 1941 the U. S. Government acquisitions plus the United States and Canadian uses in the arts and industries exceeded the Western Hemisphere production. The U.S. Government pays a premium price for silver newly mined in the United States, at present 71.11c. compared with the price of 35'/8c. for foreign silver, with the result that the current U. S. production is not used commercially for the arts and industries. At the time this is written (June 1942) there is much uncertainty as to the future effect of this situation.

Although nickel was in use in alloys long before the Christian era, the metal was not discovered until 1751, when Cronstedt recognized it in niccolite from Sweden. The Chinese apparently used a nickel alloy, known as "pahfong" thousands of years ago and Persian coins dating farther back than 200 B.C. consist of a copper-nickel alloy similar to some of those now in use in coins. The pure metal was first prepared about 1775 and the metal was refined for commercial purposes some years later. The nickel industry did not attain much prominence until the ores in New Caledonia were discovered. Exports of ore from that country began in 1875 and since that date there has been an ever-increasing world output of nickel and a great expansion in the uses of the metal except during short periods of commercial and industrial depression. Previous to 1875, for a number of years, Norway was the chief producer, although the metal was mined also in Sweden, Finland, Italy, Germany, Austria, France, China, the United States and other countries. With the discovery of nickel deposits in the Sudbury district, Ontario, a new factor was introduced into the nickel industry and Sudbury has since become the overwhelmingly dominant feature in the world's nickel markets. Although nickel was first discovered in Canada as early as 1848 near the north shore of Lake Huron and nickel-copper sulfides were found in 1856 by surveyors working close to the present location of the important rnine at Creighton, in the Sudbury field, it was not until 1883 that any importance was attached to this field. In that year copper sulfide attracted attention where the main line of the Canadian Pacific railroad was being projected across Canada to the Pacific coast. Mining began near this discovery in 1889 but in the meantime operations had begun in 1886 in other parts of the field and by 1890 about 100,000 tons of ore had been raised. In the Sudbury field, as in Norway and at the old Lancaster Gap mine in Pennsylvania, the ore was first mined for its copper content and difficully in the refining of the copper led to the discovery of the important proportions of nickel in the ore. ALL the mining machinery of the Penn Iron Mining Co. has been operated by electric power for several years and when another shovel for stockpile loading was required the advantages of an electric shovel were naturally considered. After considerable study, serious objections suggested themselves the use of a shovel operated directly by electrical apparatus by reason of the complicated control, severe service on the motors FIG. 1.-ELECTRO-HYDRAULIC SHOVEL FIG:. 2-THE SHOVEL AS IT WAS OPERATED CINDER CONSTRUCTION. It was finally decided to construct an electro-hydraulic shovel using water under pressure to perform all the necessary operations except that of propelling, for which a separate motor was best suited. A car body with boom, dipper handle and dipper of late design, but without any of the steam machinery, was purchased. A motor-driven centrifugal pump, a pressure tank, an air tank, a small air compressor and water cylinders with plungers, pistons and valves, were installed in place of the; steam equipment. Although known since 1902 , the Capitan iron deposits have not been mined mainly because of the low grade of the ore . A small amount of exploratory work carried on at an early date was reported . C. E. Johnson , district engineer of the Bureau of lines for New, Mexico , visited this propert , and as a result of preliminary sampling and mapping by the Bureau3/ and the Federal Géological Survey , an exploratory project was conducted during March , April , and liay 1944. Further mapping and logging of the vagon -drill samples was done by V. C. Kelly , geologist , of the Survey .

? A helper stands beside a continuous miner as the operator is at the controls. The operator means to push the stop button but hits the drum rotation start button by mistake. The drum begins to turn and the bits catch the helper's clothing. He is thrown violently to the ground and suffers multiple contusions. ? A farmer's wife helplessly watches her husband drawn into the claws of farm machinery while she frantically and unsucessfully searches for the control to stop the machine. ? A mechanic on the night shift replaces plugs in the lubrication systems of all three engines of an airliner following routine maintenance. In doing so', he fails to notice that "0" ring seals have not been mounted on the plugs. A few hours later, as the fully loaded airliner is cruising high over the ocean, oil pressure on one engine drops precipitously. The pilots quickly shut down the engine and turn for home. Almost immediately, the two remaining engines lose their oil and fail.

"An EXTENDED ABSTRACT is available for download. A full-length paper was not prepared for this presentation. Ore-bearing intrusions in the Central Asian Orogenic Belt (CAOB) were examined from nine mines: Boshekul, Nurkazgan, Kounrad, Borly, Aktogai, Koksai, Baogutu, Tuwu and Erdenet. The intrusions are diorite, granodiorite and tonalite with SiO2 contents ranging from 58 to 75 wt per cent, and show mildly fractionated rare earth elements (REE) ([LREE]N/[HREE]N = 2.7–4.7) – light REE and heavy REE – and varying Eu anomalies ([Eu]/[Eu]* = 0.58–1.4). The ratios of Sr/Y vary from 7.9 to 115 and the contents of Y from 4.6 to 23.9 ppm. The parental magmas of the intrusions are intrinsically oxidised. Zircon grains in the ore-bearing rocks from large- and intermediate-sized porphyry Cu deposits show high Ce4+/Ce3+ ratios characterised by 65–422 at Boshekul (>4.1 Mt Cu), 76–480 at Kounrad (>4.8 Mt Cu), 90–280 at Aktogai (>12 Mt Cu), 80–250 at Erdenet (>11 Mt Cu) 74–370 at Nurkazgan (>3.9 Mt Cu), 100–210 at Koksai (>1.6 Mt Cu), and 71–340 at Tuwu (~2.0 Mt Cu). In two small deposits, the ore-bearing rocks display low values of Ce4+/Ce3+; Borly (0.6 Mt Cu) ranging from 28 to 158, and Baogutu (0.6 Mt Cu) from 29 to 113. The Ce4+/Ce3+ ratio of 120 distinguishes large intermediate-size deposits from small-size deposits. This guide to the understanding of ore reserve estimation is the revised version of a report with a similar title circulated within the CRA Limited (CRA) group of companies in mid-1980. Thesewere not however the only problems to be looked at. Asmentioned more fully later, other ore reserve situationswere known, or were met during the study, in copper,gold and nickel as well, where poor realization: expectationratios had ranged from embarrassing to disastrous.These made it clear that ore reserve estimation had to belooked at afresh.The fresh look has resulted in a broadening of theconcept of ore reserve estimation from its traditionalpurely computational basis to an approach embracingall the many things that can affect expectation andrealization as essential factors. Hence this guiderepresents a gathering together of what has been learnedand what should be known to all those concerned withore reserves in any of its diverse aspects.The study has been marked"by a number of changesof thinking. At first the aspects that appeared to requireattention were two; recognition that an ore reserve statementwas an estimate, not a precise calculation; and asearch for im improved estimation procedure adapted tolow grade ores with narrow economic margins. The US mining industry today is fighting for its existence. We are plagued by the shortage of materials, of equipment, of fuels, of capital, of manpower. If we solve those shortages, we are then frustrated in our attempts to open a new mine or, perish the thought, a new smelter. If we manage to make a profit, demagogues will accuse us of stealing from consumers; and, if we do not make a profit and have to shut down, the same demagogues will accuse us of creating unemployment. Meanwhile, in every aspect of our business - from research, exploration and development, to production, marketing and selling - we are hindered in every way by the bipartisan mismanagement and crippling controls of a government bureaucracy gone amok. We are fighting a war on every front. Some of you realize that ours is a war of ideas. In order to win, our industry must make its ideas heard. But to be heard, we must have something to say - which means: we must know our case logically and consistently, all the way down to essential principles. A principle is a primary truth applicable to a great number of instances. A principle is like a road map: it shows you all the possible alternatives for getting from where you are to where you want to go. Although practically everybody recognizes the usefulness of a map and the need to consult it to find the best way' to get from one city to another, most people are not as careful in charting the course of their life, or the course of their industry, or the course of their nation. And so they act blindly, at random, by the whim of the moment, and get nowhere fast.

Analysis of reported injuries for off-highway haul trucks indicated that 42.7% were slip and fall, and of these 34.1% occurred mounting and dismounting the machine. Redesign of the lower steps was concluded to provide the most overall improvement for increased safety and accident reduction. Various existing, innovative and conceptual designs were reviewed and evaluated. The most promising candidate designs were the four-spring supported lower steps and the center tube mounted lower steps. Both configurations proved to be viable designs as they passed successfully human factors and survivability tests conducted at a large copper mine.

The Bureau of Mines' Oil Shale Data Bank (OSDB) was created in 1976 to serve as a centralized source for the storage, retrieval, and dissemination of technical literature, patents, and information on research projects in oil shale. Thousands of bibliographic references, patents, and research projects relating to the mining of oil shale have been identified, abstracted, and indexed according to selected keyword topics. The resulting data bank listings are contained in three sections, each with a separate keyword breakdown for use in rapidly identifying references pertaining to a specific subject. All references cited in the OSDB were published prior to an established cut-off date of May 1, 1981, when a redirection of Bureau of Mines programs resulted in the loss of project funds and the subsequent phasing-out of data bank operations. ACKNOWLEDGMENTS The oil shale data bank listings are the result of a team effort of the project staff. The contributions of Jimaleen Jansma for keypunching, information acquisition, and data entry, and John Brandenburg and Sandra rake for keywording and information acquisition are gratefully acknowledged. This report describes a computer-based model for estimating the tangible costs of injuries and deaths from work-related accidents in underground coal mines. The study undertook to identify and quantify the elements of accident costs, including field visits to coal mines, insurance carriers, and public agencies. The mathematical models developed for each tangible element of accident cost are presented in Volume I -of the report. A listing of the computer programs which comprise the model and instructions for using the programs are presented in Volume II. Statistical distributions of mining family characteristics, data on 95 miners killed in underground accidents, and other basic data used to develop the model are presented in Volume III. Application of the model to a data base of 9,286 accidents which occured in underground bituminous coal mines during 1974 yielded a total of $56,900,000 in tangible costs to industry, mining families, and public agencies. Loss of income to the families of miners injured or killed in these accidents accounted for 47 percent of the total.
