This report (Independent Functional Safety Assessment Guidance 7.0) is the last in a nine-part series of recommendations and guidance addressing the functional safety of processor-controlled mining equipment. It is part of a risk-based system safety process encompassing hardware, software, humans, and the operating environment for the equipment’s life cycle. Figure1 shows a safety framework containing these recommendations. The reports in this series address the various life cycle stages of inception, design, approval and certification, commissioning, operation, maintenance, and decommissioning. These recommendations were developed as a joint project between the National Institute for Occupational Safety and Health and the Mine Safety and Health Administration. They are intended for use by mining companies, original equipment manufacturers, and after-market suppliers to these mining companies. Users of these reports are expected to consider the set in total during the design cycle. • 1.0 Safety Introduction (Part 1).—This is an introductory report for the general mining industry. It provides basic system/software safety concepts, discusses the need for mining to address the functional safety of programmable electronics (PE), and includes the benefits of implementing a system/software safety program. • 2.1 System Safety (Part 2) and 2.2 Software Safety (Part 3).—These reports draw heavily from International Electrotechnical Commission (IEC) standard IEC 61508 [IEC 1998a,b,c,d,e,f,g]and other standards. The scope is “surface and underground safety-related mining systems employing embedded, networked, and nonnetworked programmable electronics.” System safety seeks to design safety into all phases of the entire system. Software is a subsystem; thus, software safety is a part of the system’s safety. • 3.0 Safety File (Part 4).—This report contains the documentation that demonstrates the level of safety built into the system and identifies limitations for the system’s use and operation. In essence, it is a “proof of safety” that the system and its operation meet the appropriate level of safety for the intended application. It starts from the beginning of the design, is maintained during the full life cycle of the system, and provides administrative support for the safety program of the full system. • 4.0 Safety Assessment (Part 5).—The independent assessment of the safety file is addressed. It establishes consistent methods to determine the completeness and suitability of safety evidence and justifications. This assessment could be conducted by an independent third party. • Safety Framework Guidance.—It is intended to supplement the safety framework reports with guidance providing users with additional information. The purpose is to assist users in applying the concepts presented. In other words, the safety framework is what needs to be done and the guidance is how it can be done. The guidance information reinforces the concepts, describes various methodologies that can be used, and gives examples and references. It also gives information on the benefits and drawbacks of various methodologies. The guidance reports are not intended to promote a single methodology or to be an exhaustive treatment of the subject material. They provide information and references so that the user can more intelligently choose and implement the appropriate methodologies given the user’s application and capabilities. This surveillance report summarizes data on work-related fatal and nonfatal injuries, illnesses, and hazardous exposures in the mining industry for the 10-year period 1986-1995. The term "surveillance," as used in public health, may be new to many readers of this report. With regard to occupational safety and health, the goal of surveillance is to describe the occurrence of work-related injuries, illnesses, and known hazardous exposures; to identify new hazards that may occur due to the introduction of new technology or other factors; and to assess the effects of preventive measures designed to improve worker safety and health. Surveillance provides an overall picture, which can be used to focus resources on areas most in need of prevention programs or further research. Surveillance generally requires the use of multiple sources of data, all of which have their own strengths and limitations. Surveillance in the U. S. mining industry is complicated by the fact that U.S. national data systems that provide information on mining are based on two different definitions of the mining industry. One is established by the Mine Safety and Health Administration (MSHA), the other by the Standard Industrial Classification (SIC) System [Office of Management and Budget 1987] and the 1980 U. S. Bureau of the Census (BOC) Classification System [U.S. Bureau of the Census 1982]. The mining industry as defined by MSHA is based on the regulatory jurisdiction of that agency. Reporting requirements for injuries, illnesses, and workplace exposures are stipulated under the Federal Coal Mine Health and Safety Act of 1969 and the Federal Mine Safety and Health Amendments Act of 1977. Of note is that the oil and gas extraction industry falls outside of MSHA jurisdiction. The Occupational Safety and Health Administration (OSHA) was charged with enforcement in the oil and gas industry under the Occupational Safety and Health Act of 1970. Under the SIC, the definition of mining covers the extraction of naturally occurring minerals, including liquids (petroleum), gases (natural gas) and solids (coal, metal, and nonmetallic minerals). Exploration and development of mineral properties are also included in the SIC definition of the mining industry. The SIC was developed by the Office of Management and Budget for use in classifying establishments based on the type of economic activity in which they are engaged and serves as the industry classification standard for all establishment based Federal economic statistics. This classification is important for occupational safety and health surveillance because it also serves as the industry classification system for all data on work-related injuries and illnesses collected by the U.S. Bureau of Labor Statistics (BLS). The U.S. Bureau of the Census uses an industry coding scheme similar to that used by the SIC. For the mining industry, the categorization is identical in both systems. Death certificate data made available by the National Center for Health Statistics (NCHS) are categorized under this system. Death certificate data are particularly important for surveillance of mortality from work-related illnesses. Key differences between the SIC/BOC categorization system and the MSHA system can be summarized as follows: • MSHA excludes the oil and gas industry, which is classified as part of the mining industry under SIC/BOC. • MSHA excludes work that is done off of mine property; work off of mine property is included under SIC/BOC if it is performed by an establishment falling into one of the SIC/BOC mining industry codes. • MSHA includes mine-associated mills and processing plants; these are classified under the manufacturing industry, rather than mining, according to the SIC/BOC coding structure. Since the data collected by MSHA and the data collected by BLS and NCHS using the SIC/BOC classification system are both important sources of information on the mining industry and in many cases serve different purposes, data from both systems are presented in this report. In general, data using the SIC/BOC system are comparable across U.S. industries and should be used when comparing mining to other U.S. industries. Much of the data in this report are presented in the form of frequencies or rates. A frequency provides only a count of the number of cases without providing any information about the risk of occurrence. Rates provide an estimate of the risk of injury or illness. The reader is cautioned, however, that rates based on small numbers can be unstable; thus, any inferences should be drawn with care. In this report, no rates are presented for groups in which there were fewer than three cases. The incidence of most injury and illness conditions differs by age, sex, and race. For this reason, data in many surveillance reports are adjusted for these demographic factors. It was not possible to do this for most of the rates in this report because information on demographics was not available. With the exception of the proportionate mortality ratios (PMRs), all data presented are unadjusted. A detailed account of the sources of data is in appendix A. The methods used in data selection and analysis are described in appendix B.

"This report is () expansive test program by the Bureau of Mines to determine methods from various and ferrous manganese-grade product () widespread () and manfniferrous iron ores. Among the samples received for testing was one from the pits of the Barytes Mining Co., 1-1/ 2 miles northeast of Cartersville, Ga.. The property is approximately in the center of the Cartersville manganese district and shares a common corner with properties of the New Riverside Ochre Co. and the Etowah Development Co., both producers of manganifereus ores. The deposit is typical of the district and occurs in residual clays resulting from weathering of the Shady limestone and Weisner quartzite. 7/The property is developed by a number of open pits ranging to 60 feet in depth. In 1918 several carloads of manganiferous iron ore was mined from an alit on the property. Since then mining operations have been negligible. The ore is known locally as calico ore.A 4-ton sample representing -re as it occurs in the -pen pits vas furnished for testing. The sample consisted of a mixture of red iron oxides, wad, and clay, with small pebbles of manganese oxides and siliceous fragments scattered throughout."

It was recognized early that a list of petroleum and natural gas publications was needed for the Bureau&apos;s work, and unofficial publication of a ?straight forward? listing was begun. It was at first a chronological list; however, suggestions submitted by its users prompted a format change to show publications under broad, general topics. Author and subject indexes were developed, becoming more detailed with each new edition. This present list supersedes the ones compiled in 1949, 1952, and 1957, and several supplements, which were aside available to the public by the Library Of the Bureau?s Bartlesville Petroleum Research Center. The unofficial field publication came to be widely known nail in considerable demand by those who are primarily interested in the Bureau&apos;s publications on petroleum and natural gas so that it became, desirable to have it published as an Information Circular. It is hoped drat, this official publications will result in a wider and more effective use of the Bureau&apos;s research results which are reported in its publications.

3.1 Criteria for Selection A coal deposit has the following major characteristics: i) wide distribution as a flat-lying, thin, bedded deposit ii) stable nature of its major components and ranking characteristics such as carbon, heat content, volatile components, specific gravity, etc. iii) number of constituents and characteristics which under present regulations limit usage, such as content of ash, sulphur, sodium, etc. iv) effect of characteristics of over- and under- lying host rocks including porosity and permeability, lithology and general geology v) low value, high bulk nature. Because of the relative stability of its major components and its low unit price, coal does not require the same attention to detail as do the metal deposits. However, new regulations combined with social pressures make it imperative that more accurate information be made available concerning factors which affect the quality of coal and which may have an adverse effect on the environment when the coal is mined or burnt. The method selected for the evaluation of a coal deposit must, therefore, be flexible and sensitive enough to be used to interpolate stable parameters whose general distribution character-

This report was limited to a search of available literature on under- ground methods to combat acid water pollution. From this review it is evident that very little work has been done in this specific area. It is also evident that there are specific areas of mining engineering and mining related hydrology and geology that need further research efforts to solve the many water problems. They are: (A) Composition, variation and availability for oxidation of sulfur in strata. (B) Rate of oxidation of sulfuritic materials. (C) In situ limestone and its influence on underground acid drainage. (D) Structural, stratigraphic and hydrologic character of old mining areas as a basis to make decisions about adjacent new mining areas. (E) Operational changes in mining methods to prevent pollution. (F) Sealants for underground coating to exclude water from contact with pyrite to inhibit formation of the contaminants. Such a program has been initiated on refuse piles. The prime purpose of this report is to investigate those practices which have been used to regulate or control water accumulation and prevent harmful mine drainage. A supplemental purpose is to indicate those areas which need further research to prevent underground mine water pollution.

"As the result of the work of an efficient safety organization, the Midwest Refining Co. at its various operations and particularly in the Salt Creek field has been able to show a definite accident reduction. Lost-time accidents during the past year have been reduced to a small number as compared with previous years, and infection cases have been almost eliminated.The accident-prevention work of the Midwest Refining Co. is in charge of the department of industrial relations, with a director as the administrative head; two safety men handle the work directly at Midwest, Wyo. A competent medical staff with a well-equipped hospital at Midwest is also an important factor in the prompt and proper treatment of injured persons when an employee is injured, he is given first-aid and then taken to the hospital with minimum delay.About 1,450 persons are employed in the 4 departments (production, gas, pipe lines, and electrical) and in the 34 districts of the Midwest Refining Co. The work at Midwest spreads over a considerable area, and new drilling operations, pipe work, and similar activities require the services of many small gangs of workers. Complete supervision, of each job, whether shall or large, is possible through the large number of superintendents, foremen, and gang pushers employed; thus, there are 20 superintendents, 78 foremen, and 115 gang pushers. The largest number of men under any one toss is approximately 15, and it is probable that much of the good safety record is due to this intensive supervision. These company officials, including the gang pushers, have been impressed with the idea that it is their duty to see that the men under them do not get hurt; the responsibility of instructing new employees in safety practices is placed directly upon them. Monthly meetings of the field officials, including superintendents, foremen, and gang pushers, and as far as possible the men, of each district are held with the safety engineer. Both officials and men are encouraged to bring up for discussion any practice or proposals looking to the changing of any conditions that will"

Three species of chemosynthetic iron- and sulfur-oxidizing autotrophic bacteria have been isolated from the leaching waters of copper mining areas. These species are: The sulfur-oxidizing Thiobacillus concretivorus, the iron-oxidizing Ferrobacillus ferrooxidans, and a sulfur- and iron-oxidizing bacterium closely resembling Thiobacillus ferrooxidans, INTRODUCTION Among several suggested new processes for treating marginal ores in the United States, none is more intriguing than the possibility of extracting metals from low-grade materials by the action of microorganisms One phase of a Federal Bureau of Mines investigation toward developing such a procedure was to isolate and identify specific chemosynthetic iron and sulfur. oxidizing autotrophic bacteria native to certain domestic copper deposits where leaching is practiced. The bacteria or microorganisms considered important to this investigation are those capable of producing solutions of ferric sulfate and sulfuric acid. Solutions of this type are excellent solvents for some copper minerals By establishing the presence of these bacteria at certain copper-leaching areas, strength would be given to a theory that microbial activity is partly responsible for the oxidation that takes place at these areas. Their presence also advances possibilities for controlled use of microbial action in metal extraction processes. Leathen4/ has demonstrated that one of the chemosynthetic iron-oxidizing bacteria, Ferrobacillus ferrooxidans, can oxidize 200 parts per million of ferrous iron to the ferric state in 3 days,

A relatively simple and inexpensive encapsulated hydraulic device has been developed by the Bureau of Mines to facilitate study of coal bumps , and to measure pressure changes in a coalbed as it is subjected to roof loading during mining . Construction of the device and application techniques are described . Preliminary feasibility tests in the laboratory and field indicate that the device responds satisfactorily to pressure changes in coal created by roof loading .

1.0 FRAMEWORK FOR COMPARISON This document describes the decision factors that bear on the selection of the modulation process for the wireless underground medium frequency (MF) communication system. The selected modulation process must satisfy the Bureau of Mines stated objective of improving general communications within an underground mine. The improvement being sought includes the development of a wireless communication technology of measurable benefit to both mine safety and mine productivity. The improvement will allow wireless communications paths to exist between roving miners. The wireless communication paths will also be made compatible with the existing underground telephone system to extend paging and voice communication throughout the underground mine. The new wireless equipment will include man-pack, vehicular, repeater, and base station transceivers. Repeaters and base stations will feature coupling networks for interfacing with the existing telephone circuits. Frequency modulation (FM) and single sideband (SSB) modulation processes are analyzed in view of the special conditions associated with underground mining. Of primary importance is the tremendous electrical noise generated in an operating mine that could adversely affect communications quality. High noise coupled with high attenuation rate of near field signals forces the communication system to be inherently short range. This paper presents an approach for representing and analyzing random motions and hazardous events in a simulated three-dimensional workplace, providing designers and analysts with a new technique for evaluating operator-machine interaction hazards in virtual environments. Technical data in this paper is based upon a project striving to reduce workers&apos; risks from being hit by underground mining machinery in a confined space. The project&apos;s methodology includes human factors design considerations, ergonomic modeling and simulation tools, laboratory validation, and collaboration with a mining equipment manufacturer. Hazardous conditions can be analyzed in virtual environments using collision detection. By simulating an operator&apos;s random behavior and machine&apos;s appendage velocity, researchers can accurately identify hazards, and use that information to form safe design parameters for mining equipment. Analysts must be discerning with the model and not read more from the databases than what the simulation model was designed to deliver. Simulations provided an interesting approach to data gathering in that there was no need for live subjects and logistics - test sites and costs associated with experiments-became insignificant. Collisions versus speed, operators&apos; size, and risk behaviors proved the versatility found in the data obtained from the model. Preliminary results show that response time significantly affects the number of collisions experienced by the virtual subject. Also simulation data suggests that more mishaps occur with hand-on-boom-arm risk behavior.

A nomographical approach is proposed as a fast, easy-to-use, and reason- ably accurate method for estimating expected discounted cash flow (DCF) rate of return and payout time for onshore development drilling and oil production. All costs pertaining to drilling, completing, and equipping development wells, annual expenses of operation, maintenance, and taxes incurred in producing properties, and crude oil prices were considered in developing the nomographs. This report briefly outlines the origin, derivation, and use of the nomographs.

Engineering controls for overburden drilling include dry dust collection systems, wet dust suppression systems, and enclosed cabs [Zimmer and Lueck 1986; Volkwein et al. 1979]. Proper maintenance of the dust suppression system is critically important for drills using dry dust suppression methods. Failure to rigorously maintain these systems will result in inadequate dust control. Acute silicosis has been reported in miners operating equipment that relies on dry dust suppression [NOSH 1992].

This paper discusses a practical method for enhancing the performance of mining work crews. The method, described as the Work Crew Performance Model (WCPM), seeks to define performance variability within similar tasks of an underground work crew. Key components of the WCPM include: (I) job definition through task analyses; (2) the ranking of job elements by perceived cost consequence; (3) observational techniques to establish performance baselines; and (4) cost linkages between adherence to task procedures and measures of consequence for noncompliance. The components of this model emphasize a reliance on learning from a veteran work force and using that information to reinvest in strategies to enhance safety and efficiency. These strategies are not limited to technologic efforts but include consideration of the so-called "soft-skill technologies" aimed at reducing variability in individual performance and at effecting a continuous improvement in overall system effectiveness.

High-temperature thermodynamic data have, in recent years, become increasingly important for the extractive metallurgist. The extraction of metals from their ores previously was based on trial and error methods, involving hours of testing and often costly equipment fabrication. Using thermodynamic data it is frequently possible to predict the feasibility of a proposed metallurgical process as well as the optimum operating conditions for the process with reasonable accuracy. Such predictions can save consider-able tim~ and expense. Widespread use of thermodynamic analyses has been handicapped by lack of dependable experimental data. Investigators, notably K. K. Kelley (4)4. have devoted many years to determining and compiling such data. These investigators, although prolific in their production of thermodynamic values, have been unable to meet all the needs of industry and other branches of science. This becomes obvious when one considers the aeronautical and nuclear fields, in which previously unimportant, therefore uninvestigated, substances have become of prime importance.

A radiotracer technique developed by the Bureau of Mines extends to the parts-per-billion range the sensitivity for determining contributors of deposits formed in stored and thermally stressed jet fuels. The method depends upon measuring radioactivity associated with thermally induced deposits from fuels containing one labeled compound. The technique was applied to 88 test blends, representing 12 fuels and nine carbon-l4 labeled fuel constituents or fuel additives, or both. Data from results of these tests are included in this report. With this radiotracer technique, the contribution of fuel components and additives to thermally induced deposits in a variety of fuel environments was estimated before and after storage.

Many inquiries are received by the United States Bureau of Mines for the names of elementary books on geology, mineralogy, methods of identifying minerals, prospecting, and similar subjects. In response to this demand the following brief bibliography has been prepared. As many of the inquiries are received from those who have limited technical knowledge of the subjects involved, the bibliography includes the simpler texts which present the subjects in nontechnical language. Other texts contain glossaries which define the technical terms used. A short note following the title indicates the character of each book, the number of pages, and the price. Thus, elementary mineralogists or geologists, prospectors, mineral collectors, nature students, or travellers may select the texts that best suit their requirements and their capabilities. To supply the needs of more advanced students, quite a number of the standard texts used in schools and colleges are included in a second group. A short list of books on economic geology and mineralogy has also been added.
