Organisations’ incident classification processes are plagued by the overclassification of events, often leading to excessive workloads and suboptimal outcomes. This presentation addresses the pressing need to rethink how incidents are classified to maximise organisational learning and minimise systemic risk. A major issue in the current incident classification landscape is the tendency to classify events at high levels, overwhelming organisations with investigations they cannot efficiently handle. This results in inadequate risk reduction and learning opportunities. The link between injury classification and investigation methods further compounds these problems, creating unnecessary administrative clutter. Challenging the misconception that every event, no matter how minor, requires a comprehensive investigation, this presentation advocates for a shift in perspective. The key recommendation is to decouple injury classification and risk matrices from the decision-making process regarding what and how to investigate. Instead, the emphasis should be placed on identifying opportunities for organisational learning and risk reduction. Organisations can streamline their incident management processes by adopting a more pragmatic approach that aligns incident classification with the organisation’s investigation capacity. This transformation focuses on achieving meaningful outcomes, enhancing safety and improving performance. This presentation provides an innovative process for organisations to revamp their incident classification practices, enabling them to harness the full potential of incident data for learning and risk mitigation. It highlights the importance of aligning incident classification with the opportunity for organisational learning, ultimately leading to more effective incident management and enhanced safety outcomes.

Segment and gasket lining in tunnel boring machine (TBM) tunnels; and sheet membrane lining systems in driven tunnels are robust waterproofing systems each with a proven history of performance in Australia and further afield. Where an interface between these two systems occurs, the potential for costly failures is vastly increased and additional attention to detail is required in design, planning and construction. TBM cross-passages represent the most common occurrence of this interface, with steel compression flanges and neoprene gaskets being the typical means of connecting the waterproofing systems. There is a large variance in both detailing these flanges and associated outcomes, which makes this an area where there is room for improvement within the industry.The paper will look to tease out the various issues associated with this interface and how they can be dealt with efficiently with appropriate design measures, robust planning and carefully controlled construction methods.The paper will also look to review the new technologies and systems currently being put forward as potential alternatives, including an epoxy tape system, which has recently been successfully trialled on a project in Europe under high-pressure head.CITATION:O’Dwyer, E P, 2014. This paper describes the new legislative regime that will apply to the regulation of the minerals industry in New Zealand.The reform of New Zealand's minerals legislation was initiated in January 1988 when the Minister for the Environment, Rt Hon Geoffrey Palmer, announced the Government's intention to review all the major laws governing New Zealand's resources. The Resource Management Bill is scheduled for introduction to the House in December 1989 and its date of enactment is scheduled for June 1990.

Yampi Sound is a small channel in the Buccaneer Archapelago (latitude 16° 10' S.; longitude 123° 40' E.), on the northern coast of Western Australia (Fig. 1), about 90 miles north of Derby, and 200 miles north-east of Broome.The iron ore deposits of Yampi Sound occur on an approximately S.E.-N.W. trending belt of islands which form the northern boundary of the Sound (Fig. 2). From east to west they are Koolan Island, Tarrant Island, Usborne Island, Cockatoo Island, Irvine Island, Kathleen Island, Flora Island, and Bathurst Island. Bow-tie analysis is a commonly used tool in the Major Hazard Facility sector to identify, display and control risks that have the potential to pose serious harm to personnel. It is a simple but powerful risk management tool that displays traditional risk assessment outputs as easily interpreted ‘bow-tie diagrams’. These are an effective and versatile communication format that can be applied to any type of risk (eg safety, environmental, assets etc) and as such show great benefit in managing operational hazards in mining.

The Mary Kathleen uranium deposit occurs in 1740-1780 M yr-old calc-silicate melasedimentary and mela-igneous rocks of lhe Corella Formalion which have been tightly folded into lhe Mary Kathleen Syncline. The Corella Formation consists of malic and felsic tuffaceous(?) calc-silicates, quartzile, and minor marble, conglomerate and basic volcanics. Garnet-rich rocks form semi-concordant and transgressive masses in the middle and upper portion of the sequence. Granite, gabbro and acid porphyry dykes also occur in the syncline.Uranium-rare earth mineralisation is closely associated with certain malic rocks and occurs wilhin or close to garnet-rich masses. At Mary Kathleen, uraninite grains are enclosed mainly by allanite which is dislribuled through massive garnet as an irregular honeycomb of interconnected shoot and lenses. The mineralisation occurs along strike from dioritic rocks and conglomerate and lies close to the axis of the Mary Kathleen yncline and adjacent to a major faull zone. All other localities allanite-uraninile mineralisation is broadly stratiform.The mineralisation is interpreted to occur at or near centres of basic volcanism. Garnet formation and uranium-rare earth concentrations are believed to be caused by the volcanism, and to be contemporaneous with it. The former is seen as alteration of a hot, saturated pile of volcano-sedimentary material and the latter as intermittent hydrothermal exhalation.Deposition, volcanism, garnet formation, uranium and rare-earth mineralisation, granile and gabbro formation, and tectonism are seen as a continuum of interrelated processes which operated throughout the period of format ion of the rocks in the Mary Kathleen Syncline.

The fire started in the development section from methane released into the mine through a roof-bolt hole. The date was 8 May 1998. The flames spread quickly as the coal was ignited. After eight hours the Safety Department decided to seal the vertical ventilation shafts and the slopes. The quality of coal in the Esmeralda Mine is very high quality, and Minerales Monclova (MIMOSA) decided to recover the facilities, however, the Esmeralda Mine coals have a very high gas content of 12 m3/t. Immediately after the mine was sealed, MIMOSA staff implemented a plan to monitor the gas levels underground and contacted specialists for assistance in bringing the mine back on-line. During the next 2.5 months, MIMOSA staff and the specialists observed and analysed the gas behaviour supported by a chromatograph. With the results of the observations and analyses, MIMOSA in consultation with the specialists developed a recovery plan based on flooding the area in which fire might have propagated and in which rekindling was highly probable. Those results were analysed by the command centre (Mexican engineers responsible for mine recovery). At the same time MIMOSA trained rescue teams which included miners who had no previous experience in mine recovery operations. By 20 August 1998, the mine command centre had re-opened the slopes seal. Using a æStep-by-StepÆ system, the rescue team began the recovery process by employing cross-cuts and using an auxiliary fan to establish the ventilation circuit. The MIMOSA team advanced into the mine as far as allowed by the water level and was able to recover the main fan. The official mine recovery date was 30 November 1998.

In 1984, work was started to determine the viability of heap leaching silver-containing waste dumps from Cerro Rico near Potosi - Bolivia, at an elevation of 4300 meters (14 100 ft.). The result today is an unusual heap leach utilizing fine crushing, grinding, and agglomeration to process 1000 tonnes per day of ore containing 200 g/tonne silver. The unique metallurgi- cal, economic, and political factors will be discussed which led to the decision to build a heap leach rather than a conventional agitation leach plant. Capital and operating costs will be given along with some of the uncommon process design and equipment problems encountered while developing the operation.

In his letter of resignation as General Superintendent of the Broken Hill Associated Smelters (BHAS) prior to taking up aconsulting position at imperial Smelting, G.K. Williams wrote that he considered his major contributions at BHAS were:the development of large scale sintering machines;the double tuyere-row blast furnace; continuous lead refining.Despite his well deserved reputation as a scientist, based on his advancement of the metallurgical chemistry underlying lead refining processes, Williams reveals that he considered commercial implementation as the ultimate measure of technological success. Furthermore, he clearly did not differentiate between the scientific and the empirical as a means of achieving that end.

The European Union (EU) have recently issued new Threshold Limit Value-Time Weighted Average (TLV-TWA) for diesel exhaust contaminants, which are CO, NO2 and DPM. The new limits for CO and NO2 were issued in 2017 whilst the one for DPM was issued in 2019. However, in order to allow EU mines to formulate strategies to comply with these new limits, the EU granted a transition period in which mines could use the old limits until the new limits are enforced. The new limit for CO and NO2 will be enforced from 21 August 2023 whilst the one for DPM will be enforced from 21 February 2026. The new limit for NO2 and DPM is very low, 0.5 ppm for NO2 and 0.05 mg/m3 of Elemental Carbon (EC) for DPM. Combination of these two limits would be the lowest among all mining jurisdictions worldwide. It poses a huge compliance challenge to EU mines if they keep using diesel machines. Therefore, a study and a field trial were carried out by the author in 2019–2021 to investigate whether replacing diesel machines with Battery Electric Vehicles (BEVs) would be the best answer to this challenge. Over the last ten years geological and mining software products have evolved greatly. Software companies developing such products have focussed almost exclusively on the development of technical aspects of the applications, addressing geological, ore reserve estimation and mine planning problems. These specialist applications, and the many other new technologies that are being developed by a myriad of niche developers, are an essential part of todayÆs hi-tech mining operations. Technology is also being used in many other areas to improve the mining process and there is no doubt that tremendous advances are being made in all these fields. Yet there is something missing. The manner in which the technologies are used and implemented has led to the creation of many independent and inefficient æislands of technologyÆ where a variety of products from different vendors are used to address different, but related problems. Problems occur with unreliable information, data duplication, data redundancy and transferring data between applications that may be incompatible or even run on different computer platforms. These problems impede the flow and availability of information and hamper day-to-day decision-making by mine management. A solution to this problem is to take a holistic view of all the various technical applications and incorporate them into a single information technology solution. This solution must encompass all of the information requirements of a mining operation and must provide facilities that address much of the modern thinking about mine management, including strategic mine planning, value management, key performance indicators, and maintenance of a mineral balance sheet. Gemcom is developing an Integrated Resource Asset Management System (IRAMS), which is an enterprise-wide integrated computer system that encompasses various applications typically found at a mining operation. The core component of the system is a common, open, industry standard database that can be shared by many users from each of the different departments at a mine. Around that central repository are the primary applications that provide the mine with information it needs to operate. Ensuring that the information is reliable, accurate, shareable, timely and consistent will eliminate many inefficiencies, and provide significant benefits to mining companies. In turn these benefits will lead to significant improvements in decision-making, productivity and profitability for the operation. The initial version of the IRAMS system is currently being implemented at the De Beers Venetia Mine in South Africa. The system is being built around a core repository using the Microsoft æBack OfficeÆ architecture, and incorporates key technical applications from Gemcom, Whittle, De Beers Technical Team, Wenco and JKTech. The benefits of enterprise-wide systems have long been recognised in the commercial arena and enterprise resource planning (ERP) systems such as SAP, JD Edwards and MIMS are becoming commonplace in the mining industry. The more forward looking software developers and mining companies realise the need for efficient management of the information gathered during the mining process and can see that the concepts used in ERP systems apply equally to the technical applications as they did to commercial applications. The use of IT in the mining industry has expanded tremendously over the last five years as enabling technology and management acceptance has become more prevalent, and there is considerable room for dramatic growth over the next five years. The evolution of geological and mining software has now reached the stage where mines will yield the greatest returns by integrating the applications used in each area of their operations.

3D photogrammetry is a powerful tool for data capture and interpretation, utilised at all Australian underground MMG sites. In personnel constrained mines, 3D photogrammetry can free up personnel time, improve data quality for resource modelling, as well as benefit other mining departments.At Rosebery one of the challenges faced by geologists was no longer data capture but, rather, avoiding paralysis by analysis from the sheer abundance of information currently available to the department. Although the data had been interpreted, the structural model was cluttered; the geological block model was not incorporating any geological domains captured by 3D photogrammetry, nor was it being utilised by other departments such as geotechnical engineering.The geology department is now focusing on utilising all the interpreted data gained from each photograph heading into an uncluttered, easily understandable model for geologists and other mining departments.Our solutions involve integrating the Leapfrog modelling suite into the process, streamlining the structural model, tailoring the data interpretations for future updates of the block model and training other departments to improve utilisation of the interpreted data. This process also uncovered other benefits for near-mine exploration, and improving our understanding of the deposit’s origin.CITATION:Robertson, M, 2017. During the last century Allis-Chalmers has supplied many screens of various types to industry, from revolving screens to the large capacity highly efficient screen available today. The features incorporated over these years in horizontal and inclined screens are varied and numerous and include many innovations such as tri-and bi-sloped screen decks. Today the emphasis is on wide vibrating screens with good "G" value, with large "throw", to achieve high capacity. Larger screens lead the way to treating a somewhat neglected area of mining operations - that of concentrator mine flotation tailings to allow effective transportation and stacking by conveyor belt. The benefits from a cost, water saving and environment view point are discussed in this paper.

Last September, when I was asked to prepare this paper, I was told the subject was "mining," a broad subject indeed! Rather than choose one of the many important technical and economic details which characterize mining today, I turned to a subject which fas- cinates me: mining's image. If we are to survive and prosper into the twenty-first century, and if industry and society are to continue enjoying the material benefits which flow from mining, we must address the melancholy fact that the general public at best does not understand mining, and at worst does not believe it is needed or deserves to continue. While my point of view is largely that of an American mining executive, I would hasten to point out that Homestake has participated in Australian mining for over three decades, and our subsidiary, Homestake Gold of Australia Limited, is now a major producer in this "lucky country." Does society need a viable minerals and metals industry? The answer is obviously "yes," so the challenge is to change some minds: not just those of the amorphous "public," but on a focussed basis targeting journalists, educators, TV commentators, politicians and opinion makers of all stripes (I thought of using "rifle shot" instead of focussed but thought that might be misconstrued). Along the way we miners must also look deeply into ourselves, our attitudes towards the society in which we function, and our willing- ness to change as well as to seek change in others. How did we become such bad guys? More than a century ago mining was not just accepted, it was even romanticised. In America, we had altered the time-honored concept of "Crown" lands into public lands made easily available to settlers pushing back the frontier. Miners even preceded the farmers and stockmen in what was perceived as nation-building, although by the 1890s there was a growing interest in conservation, especially the protection and management of forests and national parks. Even though emphasis was shifting toward government stewardship and away from trans- fer of public lands to private ownership, homesteading and mining were clearly recognised as valid uses requiring not just access, but security of tenure. Indeed, conservation as it was then understood implied management of the public domain as well as its protection. Initially this was no great problem to the U.S. mining industry, but as the nation grew and competing land uses became more important, conflicts began to appear, first over water rights and later over air quality and destructive mining practices. But these were local disputes rather than national issues, and two World Wars, the Great Depression and post-war reconstruction lulled our industry into thinking that nothing had really changed. After the storm began to break in the 1960s, the mining industry found itself confronted by a growing maze of complex and costly laws and regulations. It took us longer than it should have to realise that, in facing this challenge, we seemed to have lost the understanding and support of many of our legislators and a majority of the general public. Sheaves are a critical safety component in a winding system and are expected to survive such extreme loading events as rope break. Sheave designs have often been developed and evolved empirically, consequently many existing sheaves are not necessarily subjected to the detailed analyses that are possible using todayÆs design tools. The design philosophy for sheaves is that the rope should be the weakest link in the system so that associated equipment can function safely for any load up to rope break. A number of failure mechanisms must be considered, but buckling is often prominent. During the course of investigating the feasibility of upgrading the hoisting capacity of a winding system, the capacity of all critical components was reviewed. The sheaves in the system were found to have insufficient spoke buckling capacity for the specified rope. A proposal to increase the spoke buckling capacity by connecting spokes together using a simple tie arrangement was evaluated and found ineffective. An alternative tie arrangement concept was developed and evaluated and shown to be adequate. This paper describes the finite element model, analysis approach and results for the original sheave and the two modifications. The contribution of the sheave rim to the total buckling capacity is evaluated. Some practical issues for spoke tie modifications are discussed.

"It is vitally important in projects where the cut-off grade is low that there is no contamination during the sampling and sample preparation process, both in the ore and the waste zones. Failure to identify contamination could have devastating financial consequences for a project. The use of a variety of blank types is essential for ensuring that contamination is identified and eliminated at every stage of the sampling and sample preparation process. This paper examines an instance where an effective assay quality assurance and quality control (QA/QC) program was able to identify and eliminate a source of significant multi-element contamination in the prefeasibility stages of an open pit project. This examination includes the: evidence for contamination findings of the investigation process of resolving the problem and correcting the data evidence for both addition of metal and dilution through contamination potential impact on the project had this issue not been dealt with. It was through the use of a combination of blank types that the QA/QC process at Mount Isa Mines (MIM) identified a contamination issue at the first crushing phase of sample preparation. Identification of the contamination allowed MIM to stop assaying activities and work with the laboratory to correct the problem. The sampling program could then be continued with no further doubts about the sample preparation process. Contaminated samples were resampled from retained core. With the continuous monitoring provided by the blanks program, it was possible to ensure that only clean results went into the resource modelling data set. CITATION:Webb, J, 2014. As the modification of the 2009 Mill OperatorsÆ Conference theme from æmilling through the boomÆ to æthe boom û experience gained and lessons learnedÆ exemplifies, comminution practitioners are facing a series of challenges in the dramatically changing mining environment. One of the main problems plant managers have been facing during the boom times has been the severe metallurgy skills shortage and high staff turnover, both factors that can cause difficulties in mill liner management. Many of todayÆs mill managers are now also more than ever faced with the need for cost-cutting in order to continue operating viably. Amongst other potential target outcomes such as maximised throughput, MillMapperÖ, a tool for mill condition monitoring and mill performance optimisation, can be utilised to address these issues. This paper demonstrates how MillMapperÆs quantifying decision making tools can serve to be in control of managing mill liner and production issues, and how it can form the basis for mill cost savings without the risks normally associated with conventional methods or the lack of using any condition monitoring methods. MillMapperÖ is currently being referred to at a rate of approximately 150 surveys and associated reports per annum at processing plants in Australasia, South America and Africa. Examples throughout the paper provide insight into how the presented sample set of users have extracted benefits that could have otherwise not been safely or at all achieved.

During the past three years there have been several parties prospecting for oil in the Stat6 of Queensland, and It was: gratifying to read the newspaper headlines last Christmasabout the discovery of oil in the Moohie Nol Well. This discovery was not only a welcome Christmas present for Australia but marked up another success for the seismicmethod in exploration for oil.The purpose of this paper is to discuss the operationaI aspects of seismic work in Queensland with-emphasis on shot-haledrilling. The paper is based upon the experience of one particular seismic survey which began in December of - 1959 and is still inoperation. This survey is being conducted by the Petty Geophysical Engineering - Company of San Antonios, Texas, and the Queensland area involved is in the general vicinity of Quilpie, Dlackall and Charleville.
