In response to the mandates in the MINER Act of 2006, the National Institute for Occupational Safety and Health (NIOSH) conducted refuge alternatives research that included characterizing the utility, practicality and survivability of refuge chambers and outby safe havens. NIOSH also prepared and delivered a report to Congress in late December 2007 that summarized the findings of the re-search, included recommendations concerning the design and performance specifications for refuge alternatives, and focused on specific information that could inform the regulatory process on refuge alternatives. This paper highlights NIOSH?s research and recommendations concerning refuge alternatives, survivability evaluations of refuge chambers and presents a brief review of the current deployment of refuge chambers in underground coal mines in the U.S. The research has lead to the conclusion that refuge alternatives have the potential for saving the lives of mine workers if they are part of a comprehensive escape and rescue plan and if appropriate training is provided.

Blast-induced rock mass displacement can have a significant impact on ore loss and dilution. The mischaracterization of the grade boundaries both prior to, as well as a result of, blasting can lead to significant financial losses due to ore loss and dilution. Ore dilution occurs when waste material is miscategorized as ore and sent for processing or stockpiling, diluting the run-of-mine head grade and recovery. Ore loss takes place when valuable mineral is miscategorized as waste and sent to the waste dumps, thus never being processed and recovered. Extensive research has been conducted at the University of Queensland over the last fifteen years to understand the impact of blast movement on ore loss and dilution. Recent outcomes of this research have led to the development of innovative tools and techniques to measure and model blast movement. These techniques have been implemented at the Ahafo gold mine in Ghana to understand blast movement dynamics and to implement solutions to reduce blast-induced ore loss and dilution. These solutions are validated through controlled trials and have been incorporated into site standard operating procedures.

Tar sand deposits in the. Stare of Utah contain more than 25 billion barrels of in-place bitumen, Although thirty times smaller than the well-known Athabasca tar sands, Utah tar sands do represent a significant domestic energy resource comparable to the national crude oil reserves (31.3 billion barrels), The term "tar sand" refers to a consolidated mixture of bitumen and sand, Based upon a detailed analysis of the physical and chemical properties of both the bitumen and the sand, a hot-water extraction process for Utah tar sands is currently being developed in our laboratories at the University of Utah. This process involves intense agitation of the tar sand in a hot caustic solution and subsequent separation of the bitumen by a modified froth flotation technique., Experimental results indicated that, for a given tar sand sample, percent solids and caustic concentration were the two most important variables controlling the performance of the digestion stage, These variables were identified by means of an experimental, factorial design, in which coefficients of separation greater than 0.90 were realized. Although preliminary in nature, the experimental evidence gathered in this investigation seems to indicate that a hot-water extraction process for Utah tar sands would allow for the efficient utilization of this important energy resource.

The Rochester Mining District is located some twenty-five miles NE of Lovelock, Nevada. The District was discovered and named in the 1860's by prospectors from Rochester, New York. For a number of years, they worked Rochester Canyon, sinking shafts and inclines and making open cuts. After reaching a depth of approximately eighty feet, the miners tapped an underground channel and were driven from these workings by a strong flow of water leaving pumps in the shaft and barely escaping with their lives. With the Rochester workings no longer accessible, the miners went to the southeast and established the Relief Mining District. About 1881, gold was found in the gravel at the mouth of American Canyon, to the east of Rochester. These placers were worked until 1895, first by the Americans who reportedly took out $1 000 000, and later by the Chinese who mined the gravel with great skill. It was estimated that the Chinese took out as much as $10 000 000.

The study of minero-petrographical characteristics and the subsequent textural and structural characterization of an ore mineral represents the first step of each industrial processing whose aim is the beneficiation of each valuable mineral phase present inside the ore. Analytical, mineralogical and petrographical, chemical and multispectral image analyses based investigations, applied to eclogitic ore samples deriving form "in situ" sampling campaigns, were applied. These procedures enabled to point out some textural and structural aspects characteristics of ore litotypes belonging to eclogitic-amphibolitic dikes. Such dikes are characterized by a paragenesis constituted mainly by siliceous gangue and accessory "useful titanium minerals" as rutile [TiO2], "pseudolilmenite", ilmenite [FeTiO3] and sphene [CaTiSiO5]. In the beneficiation process applied to ore s is of preliminary importance to establish a strategy in order to obtain a recovery of high quality "mono-minerals concentrates" as rutile, "pseudolimenite", ilmenite and phene. To reach this goal is thus necessary to develop a preliminary study of the textural, structural, petrochemical and minerochemical characteristics of the samples. Different ore samples (eclogitic rocks), resulting from sampling campaigns carried out in the Piampaludo area (Sassello district: Savona, Northern Italy), are examined.

"Safety performance in many companies and even industries has stalled in the last decade. Accidents rates are at a “plateau” and yet, serious accidents and fatality rates are not. In more dramatic cases, such as in the BP Texas Refinery disaster, organizations that have “exemplary” safety statistics, suddenly have a catastrophic or multi-fatality event occurring. Classic examples are the Piper Alpha disaster and NASA’s Challenger and Columbia disasters, the BP’s Deepwater Horizon Rig disaster in the Gulf of Mexico. Looking into the causes of these accidents provides an insight into the events and deficiencies that led up to the accident, but what are the common features in the organization’s mindset, or its culture? What characterizes these organizations’ decision-making, their approach to safety and to risk and are there features that can be delineated? The research and review presented in this paper covers a period since1995, which started in the Australian resource industry and since then covered international events. The features of these organizations are summarized as the seven deadly delusions of near zero organizations (NZO) that suffered unexpected disasters, based on extensive research by the author. More recently, the massive West Fertilizer plant explosion in Texas, and the Train Disaster in Lac-Mégantic, Quebec, Canada underlined the same trends. Organizations may experience calamities not because they are bad, or unsafe, but because they have become “cult-like”. INTRODUCTION The train disaster of 6 July 2013, in the small town of Lac-Mégantic in Quebec, Canada, in which 47 people were killed, had many strange pre-cursorsso-called “black swan” events that have occurred in many different industries. Black swan events are so rare, that there is almost zero chance that they will ever occur again, but they are so calamitous that the responses to these are emotional and irrational, (but necessary) and in those responses, the seeds for other, different black swan events are put into place. It is not what we don’t do, or do wrong, that causes modern disasters, it is what we do. The same things in the culture that make us successful, may also cause the disasters. We slowly, incrementally and insidiously ‘float’ into disaster. One such Black Swan event: The Piper Alpha Oil Rig disaster, killing 167 men in the North Sea on the 6 September 1988, was one of the most pivotal events in safety around the world. It changed the thinking and focus of governments, whole industries and led to numerous books and papers. It resulted in new legislation, textbooks and a critical self-examination by the oil and gas industry."

Abstract: Action of certain strains of bacteria has been implicated as a significant factor in the oxidation of ore deposits. Other bacteria are thought to be significant in the formation of syngenetic sedimentary ores and in supergene enrichment processes. A normal soil/ground-water regime provides all of the necessary pH and Eh conditions for several groups of bacteria which influence the sulfur cycle in nature. Limiting concentrations of nutrients such as sulfate ions can determine whether a particular microbial species is active at a given place. In the present study, ground water was analyzed from a mineralized area and from a presumed barren area. Trace metal content of the ground water gave no well defined clue as to the presence of a sulfide ore deposit. Sulfide producing bacteria (presumably sulfate users) were found in water from about 50% of wells sampled in the mineralized area and in about 30% of samples from the barren area. These bacteria are readily detected and may provide a useful tool for regional geochemical recon-naissance. Bacterially produced sulfide ions can remove metals such as copper and zinc from solution thus complicating the interpretation of hydrogeochemical prospecting.

This, report presents correlative data on ash and sulfur forms for Pittsburgh seam coal from Greene County, Pa. Prediction equations were developed for sulfur form distributions and washability characteristics. The equations developed were evaluated with an independent set of exploration core samples. It was demonstrated-that both sulfur distributions and washability characteristic's can be predicted from exploration core data ahead of mining. This information can improve the efficiency of selective mining techniques for the production of a uniform metallurgical coal product.

The handleability monitor has been specifically designed to provide a simple, rapid and on-site assessment of the whole or part consignment of a coal blend in its ‘as-received’ condition. Evaluation has been carried out at a number of coal-handling plants, on both metallurgical and power station coals from a variety of sources world-wide. The results which are used to predict which coals could routinely pass through a handling plant at line capacity, without causing blockage or delay, have also been correlated with moisture, ash and size analyses of the coal to develop a predictive model of handleability.

Metal mining at great depths presents a number of serious challenges for mine operators. Among them is the high demand for fresh air required to cool down mine openings, evacuate noxious gases and contaminants and provide sufficient amounts of oxygen for miners. At the same time, modern mechanized mines extensively use diesel-powered load-haul-dump (LHD) machines, trucks and other mobile equipment. These vehicles not only emit exhaust gases and diesel particulate matter (DPM), but constitute an additional source of heat. As mine regulations get steadily more stringent with regard to air quality, the use of diesel engines has an additional adverse impact on ventilation costs. In this context, the present paper examines possible alternatives for diesel power trains, both those commercially available and those in development. They are reviewed with regard to their principles, required infrastructure, stage of technology development and progress. The questions of their flexibility, practicality and economic viability are also addressed. Finally, their possible benefits and potential for implementation in the conditions of deep metal mines are discussed. The paper concludes with short- to medium-term predictions concerning promising drive alternatives for vehicles destined to operate in deep metal mines.

Rapidly searching for carbonatite-related rare earth element (REE) mineralized systems is of paramount importance to explorers, because these systems provide the world’s most significant supply of REE. Biotite and barite are suitable indicator minerals that can offer reliable elemental geochemical information used for REE exploration. In this study, we provided an elemental geochemical dataset of biotite and barite from Cenozoic REE deposits in the eastern Tibetan Plateau, and compared the obtained data to those of biotite and barite forming in other geological settings. The F (i.e., fluorine) contents in carbonatite-hosted biotites are highly variable, with biotites from mineralized carbonatite complexes having much higher values (1.05–2.37 apfu, where apfu = atoms per formula unit) than their counterparts from barren carbonatite complexes (≤ 1 apfu). It is therefore believed that the presence of high-F biotites can be regarded as a sign for predicting REE reservoirs. Barites in REE mineralized systems contain much higher (even one or to orders of magnitude) total REE contents (ΣREE >100 μg/g) relative to those species forming in carbonate-hosted fluorite-barite deposits and open marine environments. This means that the high ΣREE concentration of barite can serve as an effective index for REE exploration. The above information is integrated into a REE exploration program in which the areas with strong local tectonism along cratonic edges are priority exploration targets, and high-F biotites and REE-rich barites can serve as direct indices for rapidly identifying carbonatiterelated REE mineralized systems at a strategic distance from other geological settings.

INTRODUCTION In this paper the author will attempt to bridge from the paper work of design and engineering to the reality of implementation. Certain facets of construction are constant to a successful project, no matter the level of technical complexity. Basic to timely, low cost construction are the people assigned to the project. Construction is not accomplished by robots, computers, or speeches; but rather by people. People do however require the avail- ability of equipment and communications to properly complete the work. ORGANIZATION Good organization is the basic requirement of a well implemented construction project. For this paper the author will suggest an organization chart as well as an activity listing that together support the construction activities. There is little difference in the activities or functions needed to construct a small plant or a large one. It is a matter of how much time an individual has to spend on an activity that determines the structure of an organization. The difference between a large and small plant construction job is often described as the multiple hat versus the single hat philosophy. "The 4c segment is a part of the ""Oman National Railway Project” linking the mining area in the surroundings of Thumrait and the port of Salalah. It is 90 km long and the last 40 km stretch crosses a mountainous area near the city of Salalah. The project includes the excavation of a 33 km double and single bore tunnel and 10 km of safety tunnels. This mountainous area is a holiday destination for Omanis, thanks to its beautiful landscapes, especially in summer time due to the monsoon rains. The tunnelling design had to take into account both these environmental constraints and the limestone rock formations crossed, which are affected by significant karst features. Given the context, the conventional tunnelling method has been preferred. INTRODUCTION The Oman Railway Project (Fig. 1) covers the construction of approximately 2,150 km of railway line for freight and passenger transport which will connect together Oman’s main towns and industrial sites and it will form part of the Arabian Peninsula railway system “GCC Project”. The non-electrified double-track railway line must guarantee a speed of 220 km/h for passenger trains and 120 km/h for freight trains and must guarantee the transit of AAR (Association of American Railroad) H and D plates using clearance envelope according to AREMA (American Railway Engineering and Maintenance-of-Way Association) requirements. The design alignment must have maximum radiuses of curvature of 3,250m and a maximum gradient of 1.0%. The project is divided into 9 constructional and functional segments. •,Segment 4c will be constructed in the southern region of Oman, close to the border with Yemen, to connect the town of Thumrait with the Port of Salalah. •,Segment 4c will run through the Dhofar region and will be mainly used by the freight traffic which currently travels by road."

"Tailings can be more stable in-site where they are stored and and so the risk of conventional tailings dam accidents can be reduced by alternative methods. In recent years, surface paste disposal (SPD) method is one of the alternative tailings storage methods. According to the results obtained from the previous lab scale experiments, optimum storage design was determined and then, this design was carried out in-site application under atmospheric conditions by a pilot scale tests. In the field study, two separate dams with no environmental hazards were established by the hazardous waste storage directive, whose surface area is measured as 400cm × 200cm. Also physical changes in the paste tailings were measured by the sensors such as volumetric water content, mutric suction and oxygen consumption. In addition, seepage water was collected in the drainage pool and examined for acid mine drainage and metal mobilization risks. As a result of the preliminary data obtained from measurements of pilot scale tests, when the pH value of seepage in the cemented dam (only 1th and 10th layers with cemented) is about 11, the pH value of seepage in the uncemented (reference) dam is measured at 8-9. INTRODUCTION Environmental damages occurred during the deposition of mine process tailings at surface using conventional methods (tailings dam etc.) can be reduced and/or eliminated with the use of new technologies and methods to an important extent. Nowadays, it has been investigated to safely deposit metal mine wastes generally discharged into surface tailings dams, seas, rivers, creeks etc. using Paste Tailing Technology which gains the growing importance in recent years. Meanwhile, Paste Tailings Technology has two applications in industry as an underground paste backfill and surface paste deposition. Particularly scientific studies about deposition of process tailings as paste sample in underground and surface have been increased. These studies showed that surface paste deposition can minimize the free water amount which is the biggest problem in conventional tailings dams. Additionally, it also provides less particle segregation, in other words, the formation of more homogenous material, the development of geotechnical properties which causes to obtain more stable material, increases the strength and stability of the paste with addition of binder where necessary surface paste material, and reduces the risk of Acid Mine Drainage (AMD) (Bascetin et al., 2017a). Theriault et al. (2003), Martin et al. (2006) and Simms et al. (2007) reported that Bulyanhulu mine in Tanzania is the first gold mine to employ surface paste disposal (SPD) worldwide. Here, tailings are deposited alternately via several towers. The tailings flow away from each tower to forming a conical deposit. Each layer had a lower thickness than 30 cm. The fresh tailings were poured on top of drying and hardening layer after a waiting period of 5-30 days. No cement was added into the tailings to prepare paste material. Uncemented paste was deposited by a solid content of 73% which corresponded to a 250 mm slump cone height (Yilmaz et al., 2014). In a study conducted by Yilmaz (2010) illustrates the preliminary results of a field cell testing undertaken to investigate the effect of in situ climate conditions and material properties on the performance and quality of fine-grained, sulphide-rich paste tailings at the LaRonde mine in Quebec, Canada. A total of two field cells are constructed in the surface disposal site of the mine and categorized as cemented cell CC and uncemented cell UC (Bascetin et al., 2017b)."

The number of extended cut mining systems has increased significantly in coal mines over the last decade, and there is a constant push to extend the face ventilation systems to even greater setback distances. The need to maintain airflow at setback distances of12.2 m (40 ft) and beyond requires that methods for delivering intake air to the face be improved, or alternative ventilation techniques be implemented. The emphasis of this research effort was to examine various techniques to improve the distribution of available air at the mining face in order to reduce face methane levels. A study was conducted comparing the performance of a jet fan, used in conjunction with a machine mounted scrubber, with blowing curtain ventilation in a 27.5 m (90 ft) heading. In addition, a second study was performed on blowing curtain systems examining how scrubber flow, intake air velocity, and curtain setback influence face methane concentrations. Results showed that when the heading advances to a depth of 27.5 m (90 ft) blowing curtain systems were more efficient at diluting face methane levels when compared to a jet fan. Also, increases in scrubber flows and curtain velocity resulted in lower face methane concentrations for blowing curtain systems at 9.2 and 15.2 m (30 and 50 ft) setbacks.

INTRODUCTION The problems involved with moving millions of tons of rock are being addressed by nationally recognized tunnel contractors who have converged in Chicago over the past 5 years. This wealth of expertise has been needed to meet the risk and challenge of constructing the world's largest network of underground tunnels which in turn has required a dramatic advance in the art and science of tunneling. These pioneers, through innovation, adaptation, and hard work, have eliminated the one major concern of The Metropolitan Sanitary District of Greater Chicago - how to move 8 million tons of rock. The Metropolitan Sanitary District of Greater Chicago (MSDGC) serves 122 municipalities covering 866 square miles in Cook County, Illinois. Of these cities, towns, and villages, 53 have combined sewer systems. On an average of once every four days, these systems overflow about 4 million gallons of combined sewage and rainwater into the area's rivers and canals. The overflows occur at approximately 640 outfall locations of which 10% are owned by the MSDGC, with the remainder of the points belonging to various other municipal bodies. All those who own these "point" sources of pollution are in violation of Illinois State and Federal regulations. To enable its member municipalities to come into compliance with State and Federal law, the MSDGC is implementing its Tunnel And Reservoir Plan (TARP). TARP is a regional solution to the problem of combined sewer overflow in the metropolitan Chicago area. Willowstick has successfully employed a new Seismic Acoustic Resonance (SAR) method to improve seepage characterization through, beneath, and around dams. SAR is a passive seismic method that detects zones of higher permeability in the foundation and fill materials of a dam. Just as the ocean has tides, the earth’s crust experiences daily “Earth Tides” and is constantly in motion. Continual motion produces resonance signals that can be measured, interpreted, and modeled to identify zones of higher permeability within a dam and its foundation. The SAR results are analyzed in conjunction with the traditional Willowstick Method to better characterize seepage flow paths and patterns trough, beneath, or around a dam in three dimensions. When both methodologies, which use entirely different scientific principles, identify the same anomalous feature, a higher degree of confidence is gained in the results of the investigation. Willowstick has effectively utilized the SAR method on many successful dam seepage investigations. The two geophysical techniques when used in parallel have yielded complimentary results, with both methods providing beneficial information about seepage locations. This paper will detail seepage flow paths identified using both Willowstick and SAR methods for two case study tailings dams in Canada and the United States.

Introduction The Utah Copper Division of Kennecott Copper Corporation will present its contribution to the A. M. Gaudin Flotation Symposium in four parts. The first two segments will discuss briefly the historical development of the Utah Copper Division and describe the ore body. The third segment will review the facilities and operating procedures for the Utah Copper Division's newest plant, the Bonneville Concentrator. The fourth segment describes flotation process circuitry, including molybdenite recovery procedures, at the Arthur and Magna concentrators. (The Bonneville concentrator is a crushing and grinding unit and does not have its own flotation plant). 1. Historical Development Progress through change has been the key to the successful operation of the Utah Copper Division for more than 70 years. As the original low-grade copper operation, the pressures to remain competitive and profitable have dictated the timing and extent of each change. Much of the change has occurred through technological and economic advancements in the area of ore concentration. The Utah Copper Division of Kennecott Copper Corporation had its origin June 4, 1903, when the Utah Copper Company was organized. The initial mill of 300 tpd (tons per day) capacity was built at Copperton a few miles below the present Bingham Canyon mine site. From 1904 to 1910, the Copperton mill was expanded to a capacity of 1,000 tpd, and all of the known apparatus for gravity concentration were evaluated.
