Mining provides a large part of the energy and raw materials that feed this Nation's economy. Copper pipe, concrete, and window glass serve as well known examples of the reliance on minerals in modern society. Less appreciated, but as important, are the hundreds of mined commodities that are an integral and necessary part of everyday products, ranging from medicines and computers to carpet and paper. Coal is a major portion of the country's energy mix today, as it will likely continue to be in a future hydrogen-based economy. The domestic mining industry also provides strategic minerals that are important for the Nation's security. The processes to recover these mineral commodities are among the most demanding and complex in an industrial society, and this inherently dangerous industry has historically had the highest risks of fatality and injury. The mining industry has been undergoing some trying and challenging times because of the recent mine disasters in the coal fields. These events have been alarming in light of the strong and encouraging safety record that has characterized mining in the United States for most of the past decade. While mining communities deal with their grief, mine operators, government regulators, and other safety personnel, set about trying to determine why these fatalities occurred. Subsequently, the mining industry is currently experiencing changes in legislation at an unusually rapid pace. The most recent regulatory changes are the Mine Improvement and New Emergency Response Act of 2006 (MINER Act) and the temporary emergency standard for mine seals. The new mine safety laws call on the nation's mines to adopt certain procedures and safety technologies immediately, or as soon as they become commercially available. Putting all the provisions in place from these new laws is a major undertaking, and will necessitate strong collaborations and partnerships among mine operators, manufacturers, regulatory and research agencies, labor unions, and community groups. It is against this background that Prevention through Design (PtD) components for the Mining Sector are recommended.

The National Institute of Occupational Safety and Health (NIOSH) is dedicated to achieving "safety and health at work for all people -- through research and prevention." An important procedure in pursuing this goal is the NIOSH surveillance studies where the number of injuries are monitored according to demographic, employment, and injury characteristics. These numeric data are then used to identify injury risk factors, develop safety programs, and monitor implemented remediation methods. In essence, ongoing surveillance data form the bases for directing the resources of NIOSH in setting of research and prevention priorities, and in evaluating safety procedures. This surveillance techmque, which is based on the Public Health Service approach to identification and control of disease (1, 2, has been used quite successfully in dealing with relatively high number injuries such as hearing loss, musculoskeletal disorder, respiratory disorder, falls, etc. (2). However a problem arises when extending this paradigm to lower frequency traumatic events; particularly those often referred to as disasters (i.e., high impact events like mine explosions, airplane crashes, etc.). One can ask the question - there hasn't been a coal mine fire fatality in the U.S. for over ten years, to what extent should we be concerned with mine fire safety (resource-wise)? - The National Traumatic Occupational Fatalities (NTOF) system (4J which NIOSH uses to survey occupational fatalities annually, would suggest that underground mine fires would not be a high priority item in occupational safety. Yet, miners' (i.e., the exposed workers) do consider mine fires and explosions to be major safety concerns. I believe that we need to rethink our evaluation of work place disasters, what is worker exposure, and what are the underlying assumptions (both explicit and implicit) as to the impact that economic, societal and degree of hazard factors make on risk evaluation. The answers to all these problems, will require much broader considerations than those in current surveillance safety and health models. This paper discusses some facets of those considerations.

The purpose of the studies that form the basis of this report was to learn from microscopic examinations of coal how far and in what ways the grouping of coal by types depends on differences in the kinds of plant material from which it was formed and on the condi- tions of its accumulation, or how far the special characters or qualities of coal of any type were determined by the nature and the state of the plant débris from which the coal was formed. The fact is almost universally accepted that beds of coal represent accumulations of vegetal matter in varying stages of preservation, with, as a rule, very small proportions of the remains of animal life. Mingled with the organic substances are different inorganic mineral sediments, which, together with the mineral matter originally con- tained in the plants themselves, constitute the "ash" of coal. The examination of coal shows that the kinds of ingredient plants range all the way from algee and fungi to large trees of various orders, and that these in turn vary in their own groups according to the depth and the nature of the water in which they grew and according to the other conditions of growth, such as moisture, temperature, soil, light, climate, and the competition of individuals. The species or kinds of plants and the numbers of each kind also differ greatly among themselves, not only during any one geologic period because of the changes of environment, but also from one geologic period to another, and it will be remembered that well-developed coal has been found in the strata of every period since the Silurian." Most geologists now agree that coal is transformed peat. True there is wide difference of opinion as to how the transformation has been accomplished and even as to whether coal started as such peat as is now found. Presented below is a review of how the objectives, as outlined in the statement of the problem, were achieved. 1. Methods of evaluating the performance of grounding systems must be described. These methods are the subject of Chapter III. 2. Field trips to operating mines should be undertaken, and a cri¬tique of their grounding systems made upon the basis of obser¬vation and the application of the above techniques. An analysis of field trip data, along with practical recommendations, is in¬cluded in Chapter IV, while a complete review of all field trips is given in Appendix I. 3. Because of their thoroughness, present coal mine regulations should be evaluated, and their applicability to open-pit environ¬ments appraised. This has been done in consultation with governmental agency, manufacturing, operating mine, and consult¬ing engineers. As stated in Chapter IV, insight gained through field observations has served to amplify and extend the base of knowledge from which the recommendations were made. 4. The results of these investigations should lead to suggested Federal standards. Such recommendations comprise Appendix II. In addition, several important factors, which are discussed in Chapter IV, were brought to light as a direct result of the field research. 5. Development of the Rail-Haulage Sub-assembly for the Production Sub-system (UGMHS)* required the use of ,a newly formulated model in order to best satisfy the requirements. Many of the ideas of the former single-stage rail simulator included in the original version of UGMHS have been used, together with the results of experience gained in applying the earlier model to a variety of operating situations (Sanford, 1969). The sub-assembly is developed as an independent system and, as such, can be used to analyze track systems using production distri¬butions as data input. In addition, since this model is designed to replace the original sub-assembly in UGMHS, the linking procedure is identical with respect to bi-directional information flow. The flow of coal from production sections, for example, fills the mine cars, but a condition of "out of empties" or "full of loaded" at a loading station will shut down inby production sections. A requirements analysis was conducted to determine specifically how the model could best serve the user. Operating management had the largest input in developing the following requirements for the Rail Sub-assembly: a) data input in mixed narrative - numeric form to permit user identification of the input variables; b) a user option to extract several ranges of detail in model output, i.e., an average shift summary of minimum retrieval

 Previous reports of this series/7/8/ presented data on the composi- tion of exhaust gases produced by Diesel engines operating under various conditions possible in underground workings. In all previous experiments, the engines tested were in proper mechanical condition; that is, they were adjusted and maintained in accordance with the manufacturer's instructions. In discussing the results of these tests in relation to the use of Diesel engines underground, the importance of maintaining engines in proper mechanical condition was emphasized, although no data were presented to indicate the effect of engine maladjustments on production of harmful and objectionable constituents of exhaust gas. It is the purpose of this report to present such data and to reemphasize the importance of adequate maintenance for Diesel engines used underground.

The National Institute for Occupational Safety and Health (NIOSH) has been working with diesel emissions control technology and the underground mining industry for a number of years. Underground isolated zone studies provided the emissions reduction performance of all of the alternative technologies currently available. A NIOSH/MSHA guide is available which provides mines with information and a process for selecting diesel particulate filters (DPF). Recent NIOSH experience suggests that more attention may be needed in managing the issues associated with diesel emissions and their control. The author argues that a holistic approach is needed which involves several key mine operations departments or disciplines by positing that workplace air can be considered as the product of a complex manufacturing process and examining how that process could be managed.

To improve electrical safety and lengthen light bulb life on trolley haulageways. Approach A resistor inserted in series with each pair of light bulbs, decreases the voltage applied to the bulbs and eliminates heavy arcing when voltages rise. How It Works Under certain conditions, incandescent light bulbs connected directly to a 300-volt d-c trolley wire can be a hazard, particularly during weekends when electrical equipment is nut in use. When little electrical power is being used, the voltage (on the trolley lines and at the substation) can rise to a value which will destroy the bulb filament, cause arcing, and damage the bulb holder. In one coal mine where light bulbs in certain locations fre¬quently failed, the problem was overcome by inserting a 100-ohm, 50-watt resistor in series with each pair of light bulbs. The light bulbs are rated at 300-volts and 100-watts. Figure 1 shows the light bulbs and resistor circuit. This resistance was added in order to reduce the current through a short circuited bulb holder, from several hundred amperes to a much safer value of 3 amperes. However, the resistor in series with each pair of light bulbs, also reduces the voltage across the lighted bulbs to approximately 240 volts during periods when the trolley line voltage is up to 300 volts. With this arrangement, no light bulb failures occurred over a 6-month period. The resulting light reduction has not been objectionable.

The question "Why can not American kaolins be substituted for English china clay?" has been asked time and time again, and more often since the beginning of the present war in Europe, which has threatened to cut off the supply of European clays imported from abroad. The answer to this question has invariably been, that the domestic kaolins are not as pure and uniform in composition as the imported clays. This demand for white-burning American clays is of such economic importance that it was considered advisable to determine whether some of our vast deposits of impure white clays could not be refined sufficiently to permit their being substituted for the foreign materials. Up to the present time mechanical principles only have been applied in the refining of kaolins, but in order to keep pace with the increasing requirements for better quality and uniformity of product, the application of the principles of colloidal chemistry is necessary. The investigation described in this report was carried on to deter- mine the practicability of applying technical control of clay disperse systems to the refining of kaolins and the utilization of the prepared clay in the manufacture of vitreous china and wall tile. It is hoped that this report will prove a stimulus to the clay- washing industry; will point out a method of refining certain Amer- ican clays, and will lead to the substitution of domestic kaolins for imported china clays by manufacturers of high-grade ceramic wares.

The object of this report is to give briefly information of present value relating to the manufacture and uses of the various commercial alloy steels, with the hope of stimulating the demand for such steels and extending their practical use. The report is issued by the Bureau of Mines as a contribution to the increase of efficiency in the preparation and utilization of the mineral resources of the United States. Alloy steels are included in the so-called special steels, but as the latter term is often used in the mills to designate broadly any steels intended for purposes other than those served by the regular product, it has seemed best to use the more specific term of alloy steels in this report. Alloy steels are bringing about a series of revolutions in various in- dustrial fields in which steel plays an important part. Most elements that could be procured in sufficient quantity have been alloyed with iron in various proportions, either alone or in combination with others, in the search for useful alloy steels. Those steels that have gained and maintained for themselves a place in current use are discussed in this report. Some of them have had an ephemeral life of usefulness which would no doubt have been prolonged had not some other more satisfactory steel been developed. Probably the first useful alloy steel was Mushet's self-hardening tungsten tool steel, patented in 1868. Fifteen years later chromium steel, really containing chromium, was struggling for recognition for some purposes, the chief of which was for the manufacture of solid shot for piercing armor. In both of these steels the effect of the alloying element as used was in a way proportional to the amount contained. In 1882 Hadfield made his epoch-making discovery of manganese steel and demonstrated that in iron metallurgy it is not safe to take for granted anything as to the properties of an alloy of iron with other elements, basing one's opinion on past experience and knowledge, and that the effect of an alloying element may not be proportional to its content. Some of the investigations conducted by the Bureau of Mines have for their object the collecting and disseminating of information regard- ing methods by which the fuels of the country may be most effi- ciently used. As the analysis of flue gases tends to develop better methods of firing, which in turn reduces waste of fuel, the Bureau of Mines in this bulletin presents for the benefit of those in charge of boiler plants and all other persons interested detailed information on methods of sampling and analyzing flue gases, and on the utilization of the analyses in promoting boiler-room economy. This bulletin is intended to be a companion to Technical Paper 80," and is written in plain and nontechnical language, as far as pos- sible, so that it may be readily understood by persons who have not had the advantage of a technical education. Whenever possible, illustrations of apparatus and methods of handling have been used rather than elaborate descriptions. The material presented in this report is arranged in two parts. The first part contains the description of the apparatus and the methods used in sampling and analyzing flue gases. The second part gives experimental results obtained with the different methods of sampling and collecting flue gas that are recommended in the first part of the report. Only simple apparatus and methods are considered, as they are accurate enough to show the large heat losses due to the use of too much air, and are also accurate enough to indicate any incomplete combustion losses of economic importance. Without doubt the loss due to large excess of air is the greatest one in the boiler room, and can usually be greatly reduced by making proper use of gas analysis. Before engineers in the isolated industrial plants can be induced to analyze for small traces of combustible gases, they must be first taught how to analyze for carbon dioxide (CO), and they must learn to appre- ciate the great possibilities of reducing loss from large excess of air. In carrying out a general plan of ascertaining more efficient and more economical methods of utilizing the fuel resources of the United States, in order to prevent unnecessary waste and thus conserve the available supply, the United States Geological Survey made a series of experiments on the combustion of fuel in house- heating boilers. As it was realized that steam boilers of the ordinary types used for heating private houses are often installed and operated under conditions unfavorable to fuel economy and smoke prevention, these experiments were made with briquets, raw coal, and washed coal, so as to determine what is to be expected of these fuels in a representative house-heating plant, properly installed and operated. Briquetted coal had been frequently tested in the hand-fired furnaces under the Heine boilers in the Survey fuel-testing plant at St. Louis. The results were so satisfactory as regards economy and smokelessness that it was decided to conduct a series of tests to determine the value of this fuel for domestic heating purposes. Beginning in October, 1906, a number of evaporation tests were made on the house-heating boiler installed to heat the buildings occupied by the structural-materials laboratory, both briquets and coal being used. After these tests were well under way, it was thought advis- able to conduct some additional tests under conditions more constant than could be maintained at this plant. A carload of briquets was shipped to the University of Illinois engineering experiment station at Urbana, Ill., where two house-heating boilers of a size commonly used in residences were available. This equipment permitted more uniform conditions of pressure and capacity, making the results more valuable for comparison. Tests were made with this fuel at Urbana in June and July, 1907. The present paper is an account of both series of tests, and includes also details of tests made at Urbana on three standard fuels in the spring of 1908. Perhaps the most important result obtained is that showing the relative value of different fuels for domestic purposes. During the mineral survey of the Bob Marshall Wilderness and study areas by the author in 1971, 1973, 1974, and 1976 (Marks, 1978), the area designated Reservoir-North (RARE II No. H1-485) in January, 1979, (U.S. Forest Service, 1979) was examined, along with other lands adjacent to the earlier study areas. However, the 1978 open-file report covered only areas originally designated for study. The present report on Reservoir-North is based on earlier work. No work has been done in the area since 1976. Mineral Appraisal Reservoir-North RARE II area (No. H1-485) is in the Northern Disturbed Belt which has been identified as having high potential for natural gas accumulations and lesser potential for oil accumulations (Mudge, 1978). Oil and gas lease applications in the area, indicate interest by oil companies.

Smoky River Coal Ltd. mines low-volatile metallurgical coal by surface and underground methods in the foothills of the Rocky Mountains of Alberta, Canada. Current underground operations are confined to the 5B-4 Mine. Development of 5B-4 began in January 1998; production from depillaring sections commenced in July 1998. This paper describes the history of underground mining on the Smoky River property in terms of extraction methods and pillar design. The development of the present pillar design guidelines is discussed in this context. Recent work to prepare a number of case histories for back-analysis using the Analysis of Retreat Mining Pillar Stability (ARMPS) method is described, along with the modifications developed for calculating the ARMPS stability factor for retreat extraction of thick seams. The design criteria are described, as well as the geotechnical program implemented in order to verify its applicability.

In an underground mine fire, a toxic mixture of combustion product gases and particulate matter is transported by the ventilating system endangering everyone downstream. To determine the magnitude of the problem that these toxic combustion products pose, the U.S. Bureau of Mines is investigating the combustion products of typical materials found in underground mines. The total toxicity of the combustion products depends upon the evolving gas species and particulate matter, the amount of material involved, and the ventilation rate. In a simulated mine environment, the products from smoldering poly- vinyl chloride (PVC) brattice and conveyor belting were analyzed for gas concentrations and smoke characteristics. The primary toxic gases are hydrogen chloride (HC1) and carbon monoxide (CO). Smoldering conveyor belts are more detectable than smoldering PVC brattice cloths. These results, combined with previous analyses, are used to estimate relative toxicities, product levels, and detect-ability of smoldering mine combustibles.

A method to assist in the evaluation and selection of roof bolts using in situ measurements of roof bolt loading has been developed by researchers of the Spokane Research Center, National Institute of Occupational Safety and Health. Both axial and bending forces are measured by strain gauges at many locations along the length of fully grouted bolts during various stages of mining. This information is then used to (1) predict bolt loading for variations in bolt spacing, grade, and diameter, (2) calculate the design changes necessary in zones where bending loads are high, and (3) determine if bolt length is adequate. Results from several case studies of full-column bolt loading in coal mine gate roads are used to illustrate the design method. This knowledge will give design engineers a tool for the selection of roof support systems that will improve underground safety by reducing roof bolt failures.

In Maryland, coal mining is confined to Garrett County and the western portion of Allegany .County. Terrain in both counties is predominately level with rolling hills; state law prohibits surface mining in steep slope areas. The Conemaugh and Allegany geologic formations contain five major minable fields or basins. These include: Upper Youghiogheny, Lower Youghiogheny, Castleman, Upper Potomac, and Georges Creek. Maryland's coal reserves are estimated to exceed one billion tons. Approximately 490 million tons of bituminous coal are considered recoverable using conventional mining methods. After World War 11, deep mining declined in Maryland, and by 1977 surface mining accounted for 91 percent of the total production. Since then, production at underground mines has recovered, accounting for 69 percent of the coal production in 1991. Maryland's permanent regulatory program was fully approved February 18, 1982. Its Abandoned Mine Land Program was approved June 16,1982.

One of the details which the engineers observe closely in conducting tests on explosion-proof type equipment at the Pittsburgh experiment Station of the Bureau of Mines is the construction of covers and their means of fastening. These covers are of particular concern from a safety standpoint, because they are very liable to be frequently disturbed for the purpose of making inspection or repairs to internal parts of motors, controllers, etc. When depended upon to permit frequent inspections it is desirable from a practical operating standpoint that the cover be easily and quickly removed and replaced. The position that been taken by the Bureau of Mines is that the questio of ease with which covers will permit inspection should be determined by the mamu- facturer and the user. If the scheme used requires considerable effort, as in the removal of several bolts, it has been assumed that the one going to all this trouble to get the covers off could be trusted to put the cover back in place properly, especially as the approval plate generally calls specific attention to this point. Where a cover is simply screwed in place, fastened by a single bolt, or secured by a hing in or other similar arrangement, the bureau has not been willing to assume that the, cover would in all cases be properly fastened and has required further security by means of a lock or seal. In the course of its investigations the bureau has had occasion to inspect and test five types of covers as follows: (1) Covers having two plane surfaces bolted together. (2) Covers having two plane surfaces held together by lugs, clamps, or other means besides bolts. (3) Covers which have screw thread and fit a similar thread in the compartment. (4). Covers having plane surfaces with the addition of a tongue-and-groove joint at about the center of the plane surface and in general secured by hinge pins or similar means of fastening. (5) Covers in the form of an annular ring accurately fitted to a motor shell which can be rotated around the shell, and thus make easy access to inspection openings in the shell and rotated back again to effectively seal these openings. Tests. have also been made of large bolted covers having a smaller and more accessible cover within the large one, which could be opened for inspection purposes.

A preliminary investigation has recently been made by the U. S. Bureau of Mines to determine whether or not the ordinary mine- type telephone is entirely safe in gaseous atmospheres . Two well known makes of mine- type phones were purchased for this investigation . It is not conceivable that the current necessary for talking over telephones could ignite gas . However , such phones are provided with efficient magnetos , and it was believed that danger might result from the ringing of the magneto . Again , while this type of phone has been designed to have a certain measure of dust and moisture tightness , evidently it has not been designed as an explosion- proof device . either with regard to cover joints and fastenings or strength of castings . Therefore , the problem was simplified . First it was assumed that owing to the construction of the telephones the gas would penetrate into their interior , and the question was to ascertain whether or not the gas would be ignited from the operation of the magneto .

The Bureau of Mines, in pursuing investigations looking to greater safety in mining, has analyzed samples of the air in many different coal mines in the United States, and has studied the analyses. This report presents the results of one phase of this study and shows how atmospheric air, after entering a coal mine, loses oxygen and gains carbon dioxide with resulting formation of so-called black damp. Also the report discusses the effects of the constituents of black damp on men, on the burning of oil and acetylene lamps, and on the ex- plosibility of methane. The term "black damp" was and still is widely used to designate accumulations of carbon dioxide, but a more exact definition of black damp, as Haldane, the English physiologist, has pointed out, is an accumulation of carbon dioxide and nitrogen in proportions larger than those found in atmospheric air. The reasons for preferring Hal- dane's definition are given on succeeding pages.
