Poorly judged or under-appreciated change plays a role in many major accidents. In particular, operational changes related to continuous improvement, maintenance and introduction of new technology can lead to unintended safety (and other) consequences. This paper provides an overview of the role of management of change in preventing unwanted impacts on safety, community relationships and the environment. It draws on a range of incidents covering mining, minerals processing, refining and smelting where inadequately assessed or ineffectively implemented change led to significant safety, environmental or community consequences. The paper will also highlight cases of well-managed complex change and the underlying model of decision-making in place to make those decisions. All these examples are drawn from Atturra’s experience in response to accidents or working with sites to improve their management systems. The paper will report on the challenges of applying conventional risk assessment approaches to complex or technically significant change. These typically result in either onerous and lengthy change procedures that are prone to workarounds or poor implementation. Conversely, ‘light touch’ or efficiency driven change processes present the risk of intended consequences by failing to bring appropriate expertise to bear in decision-making. Such balances can be resolved by considering the designation of ‘technical authority’ with appropriate expertise to consider the change and deconflict any production, cost or schedule trade-offs. Finally, the paper will reflect on the key components of management of change systems in the context of major accidents, highlighting good practice in how they are implemented. This will consider the integration of existing corporate processes, risk management and organisational structures into management of change systems, not just the content of change forms and procedures.

Long-term exposure to respirable dust continues to present an occupational health and safety hazard to mine workers in underground mines. Workers who are exposed to mine dust can develop occupational lung diseases including coal workers’ pneumoconiosis (CWP) or ‘black lung’, progressive massive fibrosis (PMF) and chronic obstructive pulmonary disease (COPD). To demonstrate the exposure impact of certain working conditions and improve dust management in longwall development panels, this paper details the development of a computational fluid dynamicsvirtual reality (CFD-VR) based training tool. The tool has been developed as an educational platform to facilitate the effective communication of workplace health and safety (WH&S) dust control measures for continuous miner operators and engineers. The study introduces a promising approach to model ventilation and dust flow patterns based on site-specific data, employing computational fluid dynamics (CFD) for a detailed analysis and visualisation of these mechanisms. Additionally, an algorithm has been developed for processing the large and complex CFD data sets into a form amenable for visualisation within a virtual reality (VR) environment using standalone VR headsets with limited processing power. This training tool has been deployed to mine sites with the specific purpose of educating users on the hazards of dust emissions and the best practices for control.

Whilst employed by Dyno Nobel as the Technical Services Engineer û Brisbane, I was asked, as part of a contractual agreement, to look at optimising PioneerÆs blasting process to provide the most effective rock to the new crushing and screening plant. This paper details the preliminary blast designs and practices that have been in use at the quarry and the initial changes made to these designs, and the effect they have had on the excavation and crusher performance.

The hoisting system at the Newcrest Telfer mine is designed for a final duty of 7.35 million tonnes per annum at a hoisting distance of 1450 m. In terms of hoisting capacity (t/h) times hoisting distance it is one of the largest friction hoist installations. Initially, the hoist will be equipped for hoisting 6.5 Mtpa from 1100 m depth. This new drive technology is now well proven by ABB for mine hoist applications with the first installation in commercial operation since July 2001. To date 17 systems for medium voltage drives and another five for low voltage drives have been purchased from ABB for mine hoists. This paper will also deal with skip design and powerful data loggers with diagnostics for local and remote monitoring that are installed in the Telfer mine hoist.

Monitoring of water infiltration into WhiteÆs waste rock dump at the Rum Jungle mine site in the Northern Territory, Australia, started in 1983, and has since produced a unique and long-running data set. Following closure of the uranium mining operations at the site, a soil cover was constructed on the dump in 1984 primarily to reduce water infiltration into, and the quantity of low quality effluent out of, the waste material. The performance of the cover in reducing water infiltration has been monitored using ten lysimeters installed across the dump prior to construction of the cover. Over the past eight years or so, the average infiltration rate as measured by the ten lysimeters appears to have risen compared to the early years of monitoring. The significance of this apparent increase is discussed given the variability observed in the data from individual lysimeters, and the variability in the rainfall. Estimates of the infiltration rate as measured by each of the lysimeters are examined using statistical tools. It is concluded that the infiltration rate has increased with time and with increasing rainfall. The increase with time is a stronger factor than the increase with rainfall. It is further surmised that the cover system at the locations of the lysimeters is less effective now at reducing infiltration into the dump than it was when newly installed.

AngloGold Ashanti Australia’s (AGAA) Tropicana gold mine (TGM) geology department seized the rare opportunity of building a new geology department for a new operation: the philosophy, the people and the processes. Not satisfied with the status quo, or simply repeating what was familiar and already known, the vision was for an innovative, agile, multidiscipline, high performance team, capable of delivering on expectations.To achieve this, the geology department has operationalised key components of human resource management (HRM).This paper describes the operational design and implementation of key HRM components: job analysis, organisational design and organisational development. The benefits of this approach are presented in terms of engagement and autonomy in roles, opportunity for role variety and flexibility of the organisational structure. The result is a geology department that provides the functions of mine geology, resource development, regional exploration, survey, data management, geographic information systems (GIS), resource modelling, scheduling, sample preparation, resource coordination and hydrogeology, and even geotechnical engineering for a year!Considerations for application in other geology departments are presented in terms of fundamental requirements and lessons learnt. Fundamental requirements for success have been the sustained support of the general manager, valuing the scrutiny that comes with continuous improvement, high quality HRM systems and tools within the organisation, and a committed leadership group within the department. Alkali emissions are one of the environmental challenges facing alumina refineries. Queensland Alumina Limited (QAL) is undertaking a number of initiatives to continuously improve its environmental performance. One of these is a program aimed at the reduction of alkali emissions. Alkali emissions originate from a number of sources (eg elevated open tanks), and its level depends on a number of process parameters/ mechanisms (ie splash filling, thermal currents in tanks, streams discharged from a pipe, tank temperatures, atomisation) and climatic conditions (eg wind speed and direction, rain, etc). Under normal operating conditions, there is a continuous low level of alkali emissions from the plant. Certain vessels within the plant have been identified as major contributors to this level of emission, while certain activities within the plant contribute to a temporary elevation (æspikeÆ) of alkali emissions. QAL developed a method using phenolphthalein-treated pads to measure emissions within and outside QAL boundaries. These provide no information about mass emission rates but are important for detecting if alkali material is present. Since mass emission rates are important for determining if improvements in emissions are made, QAL identified the need to design an improved sampling method. Following the development and implementation of a sampling and analysis method to monitor alkali emission concentrations, QAL is conducting a number of feasibility studies evaluating a range of alkali emission reduction options. The alkali measurement method will be used to track and demonstrate progress in this environmental initiative. This paper will discuss the measurement method and emission reduction options being evaluated.

Process control applications to the unit operations in both the Lead and Copper Smelters are reviewed. The basic aim of this control is to smooth plant inputs to produce an output of consistent quality in terms of chemical composition, temperature and physical properties. Most effort has been put into the process bottlenecks, namely in the process baghouse of the Lead Smelter and in the Copper Smelter reverberatory furnaces. The method of approach is discussed, firstly extensive plant measurement and data logging plus identification of factors which most affect the unit operation and secondly control of these factors to smooth out variations. This has been possible due to the development in-house of on-line instruments to measure the critical variables and the external use of computers for plant control. Some of these instrument developments are also briefly discussed.

Troy Resources NL acquired a 70 per cent interest in the Mina Sertpo gold project in Goißs State, Brazil, in 2001 following a successful bid for the highly competitive international tender of a tenement package put out by Western Mining Corporation (WMC). The Sertpo deposit is located on cleared grazing land 380 km west of Brasilia and is 30 per cent owned by Amazonia Mineracao Ltda, a private Brazilian company. It is a small high grade open-pittable orebody with a measured and indicated resource of 108 000 tonnes at a grade of 49.7 g/t for 172 500 ounces using a 2.0 g/t cut-off. Troy undertook the project management and development of the project while Amazonia managed the exploration activities and provided other services as required. The project development involved drilling metallurgical test holes, scouring the world to locate an available and suitable second-hand plant, and arranging shipping to andreconstruction in Brazil.

Contemporary mine wastewater flows are subject to stringent water quality standards before discharge to local waterways. Strict criteria are often applied as a result of fears of extensive acid mine drainage; a feature of the many abandoned mine adits around the Coromandel, Central Otago, and the West Coast. In most cases the USEPA water quality criteria have been used to set treated wastewater discharge water quality standards. In future it is possible that the recently published ANZECC water quality criteria guidelines will be used for this purpose. The ANZECC guidelines also provide for the establishment of discharge criteria at specific sites, rather than accept the trigger values. The use of site-specific criteria for treated wastewater mine discharges was pioneered at the Golden Cross gold mine, near Waihi, in the late 1980s. The procedure used to derive water quality criteria was based on USEPA guidelines. In developing the Golden Cross criteria, only species that occur, or are similar to those that occur in the Waitekauri River were used. The mine commenced discharge of treated wastewater in 1991, and macroinvertebrate communities in the vicinity of the discharge point have been regularly monitored twice a year for the past 12 years. Despite fluctuations in community composition and abundance over that period, no permanent significant changes in the macroinvertebrate communities occurred between sites above and below the treated wastewater discharge. The success of the site-specific criteria at Golden Cross has shown how the application of this methodology can result in the protection of aquatic life while allowing for the discharge of treated mine wastewater.

Australia has, and faces, a growing problem with regard to the ability to meet the demand for liquid fuels from indigenous petroleum resources. The position for natural gas is better but there is no room for complacency. Since the coal resources are relatively extensive, interest in the prospect of producing liquid fuels from coal is in- creasing. The approaches to the production of gaseous and liquid fuels are briefly reviewed and the present status of coal conversion technology is discussed. Many problems remain to be solved and it is estimated (in the USA) that another 8-10 years will be required before any of the processes for liquid fuel production are considered sufficiently efficient and viable to go to a demonstration plant. Coals represent complex feedstocks and the effects of variations in coal properties on oil-from-coal processes are not fully understood. The thermal efficiencies of various coal conversion processes are considered in relation to their impact on coal requirements. The production of Australia's present requirement for liquid fuels by the only commercial process in operation would require a four-fold expansion in the local consumption of coal. Any assessment of the suitability of oil-from-coal processes currently under development overseas for Austra- lian coals and requirements must take heed of the differences in coal properties and motivation. The economic via- bility of processes for producing oil from coal will continue to be uncertain because of the large difference between the World price for crude oil and the actual production cost.

For Australian mining projects, sustainable groundwater use is typically defined as a groundwater extraction regime, measured over a specified planning time frame, which allows acceptable levels of stress and protects dependent economic, social and environmental values (DEH, 2004). This principle is compatible with most stakeholdersÆ understanding of our national environmental objectives, yet the application of this policy on a project-specific scale is not always straightforward. Three examples of sustainability issues in the management of groundwater in mining are presented. These examples highlight each of the three dependent values: environmental, economic and social.

The modern workplace has changed dramatically in recent years with an increased emphasis on the psychological well-being of employees. While adhering to a certain standard of psychosocial safety at work is now not only a moral, but legal obligation in Australia, it begs the question: how can organisations begin to ensure psychosocial safety? A positive psychosocial safety climate is one where employees feel they are psychologically safe, feel supported in their work and are not exposed to work-related stressors that can negatively affect their mental health. Such a climate can help boost productivity and create a more positive working environment. This presentation will provide a twopronged approach to understanding and managing psychosocial safety: pairing a traditional risk assessment with climate measurement. Risk assessments can provide several benefits, including identifying and characterising work-related hazards, identifying the nature and severity of the risks to employees and prioritising necessary interventions for reducing or preventing psychosocial harm in the workplace. However, these assessments can have certain limitations, especially when it comes to capturing some of the less obvious causes of psychosocial harm. Climate measurement can provide valuable insights into employees’ perceptions of the work environment, identify areas of concern that may not have been identified in the risk assessment and help to guide organisational changes that will enhance employees’ psychosocial well-being over time. It will also be argued that climate should be assessed from a broad perspective exploring general, physical, social and psychological aspects of safety climate. This presentation will explore why conducting risk assessments in conjunction with detailed climate measurements is integral to protecting the psychosocial safety of employees in the workplace and demonstrate why employers should view the two approaches as complementary and interconnected. Best practice recommendations will be provided for the risk assessment, climate measurement and psychosocial interventions

Applications of chlorine metallurgy to the extraction of metals often result in the production of volatile metallic chlorides, most of which condense directly to a solid phase at 1 atm pressure. A stirred-bed condenser suitable for the recovery of these materials is described.The unit, developed from the concept of a fluid bed condenser, has the advantages of versatility of operation and ability to control fog and dust formation. Test work using aluminium chloride vapour (as an example of a volatile chloride) gave recoveries approaching 100 per cent. The yield was dependent upon halide content of the vapour, flowrate, and, to a lesser extent, upon coolant temperature. Analysis of the heat transfer dataindicated condensation side coefficients in the range 10 to 30 B.T.U./hr sq. ft. 0p.

OZ Minerals' Prominent Hill copper-gold concentrator is located 650 km north-west of Adelaide in the Gawler Craton of South Australia. The concentrator was built in 2008 and commenced commercial production in early 2009. The Prominent Hill concentrator is comprised of a conventional grinding and flotation processing plant with a design capacity of 8 Mtpa. The flotation circuit includes six rougher cells, an IsaMill for regrinding the rougher concentrate and a Jameson cell preceding the three stage conventional cell cleaner circuit.Since its commissioning, the Prominent Hill flotation circuit has been the focus of a series of projects aimed at continual performance improvement. This has been achieved by a systematic approach of project identification, evaluation and implementation which has led to a progressive improvement in flotation performance. Innovations that have been successfully implemented include aspects of cell design and operation, and reagent suite optimisation.Improvements to the mechanical cells initially focused on the installation of a modified rotor-stator design in the rougher circuit, as recommended by Outotec. Isolated cells were fitted with this technology and proven to provide a benefit for copper recovery before the design was trialled in subsequent cells along the bank. Based on this success, further modifications were made to other aspects of the rougher and cleaner cells.Laboratory trials conducted at Prominent Hill involve a screening process identifying potential reagents from a list of products, which are then rigorously tested in the laboratory before one is selected for trial in the concentrator. Where these reagents have demonstrated a statistical improvement in performance they have typically been incorporated permanently into the flotation circuit. These have resulted in new legislation which will have an impact on mining economic costs as well safety and health of miners. This paper discusses the changes that new legislation and regulations are making to the industry and current research that is producing technologies to make mining safer and will likely impact the future of mining.

There is often a difference in opinion about the allocation of risk in the establishment of mine closure bonds or financial assurances. From the State’s perspective, protection is needed to ensure that the broader community is not left with the liability of properly closing a mine in the event of a default by the mine operator. Such a default might occur where an operator decides to abandon a site, perform incomplete rehabilitation, becomes insolvent or claims limited liability. On the other hand, mine operators who can demonstrate a strong financial position plus a record of social and environmental commitment and performance, argue that there should be some recognition of this in setting a bond that is less than the full cost of early closure. They argue that a mine rehabilitation bond based on the full cost of early closure is overly conservative and ignores the actual risk of the operator being in default. The discovery of economic uranium miner alisation in unique geological conditions at Yeelirrie, 700 km NNE of Perth, presented Western Mining Corporation Limited with a num- ber of sampling and evaluation problems. Carn- otite mineralisation occurred in a porous cal- crete-clay lithology in close proximity to the water table; background information on such a deposit was non-existent and sampling in these conditions could be difficult. The deposit was sampled initially by aug- er drilling with X-ray fluorescence analysis of drill cuttings, followed by reverse circulation and diamond drilling with analyses by X-ray fluorescence and gamma logging of the holes. Two costeans provided lithological and grade distribution data and mining experience in the deposit. Spoil from the costeans was stockpiled according to location in the first costean and according to location and grade in the second costean. Samples from the various stockpiles were analysed by X-ray fluorescence. No. 1 costean was extensively sampled by channel sampling of the walls and by pit sampling of the floor of the benches of the costean by back hoe. No. 2 costean was sampled by Geiger counters mounted in T-probes and by the gamma logging of auger holes to establish the valid- ity of the T-probe results. Samples were taken

The history of the reduction of iron from its ores is lost in antiquity. Archreologists state that man has practised it for over four thousand years. Until comparatively recent times charcoal was the principal reducing agent for making metallic iron from its various oxidised ores. Coke was first used by iron founders in England in 1619, and its use spread gradually throughout the civilised world from then onwards. Indeed, the destruction of forests to obtain the requisite charcoal made the iron founders, both in England and America, extremely unpopular, and this hampered the iron industry to a considerable extent, until coal provided the metallurgist with a new and better fuel--coke which is used by most pyro-metallurgists today. The transition from wood to coal products for iron smelting had a profound effect on the location of iron works. Particularly was this the case in America where, in the infancy of the industry, practically each of the early States had its own iron producers, but as coal and coke replaced charcoal, there came about a concentration of the iron and steel industry around the coking coal deposits, and so today there are huge steel plants clustered around the soft coal basins in Pennsylvania and Illinois.The development of the iron and steel industry in Australia, which is comparatively recent by world standards, has been cradled in the coal basins of New South Wales. The first steel works of any consequence in this country were built near Lithgow, close by the mines which produced the coal for coking.Incidentally, these works were rather unique in that the three essential materials for iron and steel manufacture-ore, coal, and limestone-were found virtually at the works' back door.When, in 1912, the location of the proposed steel works for The Broken Hill Pty. Co. Ltd. for smelting ore from Iron Knob in South Australia was considered, the site ultimately chosen was at Newcastle, almost on top of seams of satisfactory coking coal. One of the major factors which had determined the site of the steel works was the fact that, about 1912, three tons of coal were required to produce one ton of finished steel product, and the relative economics of ore and coal haulage were much in favour of bringing ore to the coal.
