In the last few years major industrial companies have begun to realize the importance of Reliability as an enterprise strategy that, properly implemented, will improve financial performance. Typical results of an effective Enterprise Reliability Strategy include a 20% -50% reduction in maintenance cost accompanied by a 5% - 10%increase in real production capacity, with no capital investment in production equipment. One factor fueling the race to acquire and implement enterprise reliability is the pressure for companies to be increasingly cost-effective and competitive in global markets. In a recent study, Asset Productivity: The Next Wave, the Boston Consulting Group (BCG)stated that, “In order to compete for funds, companies must offer investor returns that are competitive with other opportunities. Alternative investments such as dot-com and others with non-traditional business models have raised the bar.” (APDF version of the study can be downloaded at www.bcg.com). BCG views asset productivity as the most powerful mechanism for increasing shareholder return. The author believes that the most effective way to increase Fixed Asset Productivity is to maximize asset utilization by increasing the Reliability of the Assets and the associated processes. Developing and implementing an ERS requires significant effort and knowledge. Most companies have difficulty finding a sufficient supply of these valuable commodities to implement an ERS Program internally. Many companies also struggle with figuring out how to get started and what direction to proceed.

Project-finance risk assessment is mature and approached systematically. Mining industry due diligence remains fragmented with specialists addressing different aspects of the project such as reserves/JORC, environmental impact, commodity market, even financial analysis/ modelling. Each professional has a natural tendency to believe that the risk factors they have examined are the key ones necessary to be reviewed, as if their review will stand in isolation from others in the way a project materialises. By first laying out the project risk categories and risk matrices, this paper examines the due diligence process with a keen eye to the risk assessment and overlap/interaction. Some cross fertilisation of due diligence techniques from other industry sectors is placed into the mining context. Six case studies are then presented with regards to due diligence features in each projectÆs feasibility review or financial information/ offering memorandum. A power case first shows the differences between the due diligence expectations of banks versus bond financings. Then some Australian and Indonesian mining development cases are analysed to see the due diligence learning points. Some of these were project/due diligence failures. The conclusion focuses on the value of a systematic risk assessment for project evaluation, all from the project-finance perspective.

A Reconnaissance of the manganese- and chrome-ore deposits of Cuba was made by the writer, as a representative of the U. S. Geological Survey, in company with Mr. Albert Burch of the Bureau of Mines, under instructions of the Secretary of the Interior, in the spring of 1918. The Cuban Government courteously detailed Sr. E. I. Montoulieu, a mining engineer connected with the Treasury Department, to act as escort and associate throughout the work on the island. The object of the study was to obtain authentic information for use of the United States Government as to the location, character, quantity, and availability of the manganese- and chrome-ore deposits in Cuba. Accordingly, the work comprised examination of such deposits as seemed to be of promise, without regard to stage of development, with a view to a determination of the quantity and quality of ore likely to become available for shipment during 1918 and 1919, and of the tonnage of ore in reserve. Wile it was possible to visit most of the deposits that seemed to be of promise, there was not sufficient time to visit all that were brought to our attention, some of which may have merit. Moreover, the necessity for paying particular attention to the ore deposits and the mines precluded the possibility of making more general studies of the geology of the region, and of making satisfactory collections of country rock and of fossils, and so for other details as to the geography and geology of Cuba the reader is referred to the report by Hayes, Vaughan, and Spencer.l Mr. Burch returned to the United States about 10 days in advance of the writer, who continued, in company with Sr. Montoulieu, investigations of reported deposits in Pinar del Rio, Matanzas, and Santa Clara Provinces. On his arrival in Washington, Mr. Burch submitted to the Director of the Bureau of Mines a summary of the results of the work to date. Later the writer published, as Press Bulietin 190. 380 of the U. Gas flow and heat transfer phenomena in two industrial-scale waste-heat boilers of the Outokumpu copper flash smelting process were analyzed with a commercial computational fluid-dynamics (CFD) package Phoenics. Various operating conditions were simulated for the first boiler and particularly for the second boiler. The gas flow pattern was compared qualitatively with the physical laboratory models, and the general flow pattern was consistent with the laboratory visualization. The heat transfer simulation was validated with, temperature measurements for both boilers, and good agreement was reached between the computed temperature profiles and the measured data. The computational results indicate that gas flow in both boilers is of three-dimensional and recirculating nature. The numerical tracer experiments indicate that some dead volume is present in both boilers, and the boiler modification reduced the dead volume from 38% to 15% under basic operating conditions. Radiative heat transfer accounts for 65-90% of the total 'heat transported in the radiation sections of both boilers under different operating conditions. The modified boiler is very efficient for cooling the off-gases at almost doubled volume flow rate. The CFD modeling proved to be a useful tool for numerical experiments and analysis of boiler performance for process: optimization.

INTRODUCTION. This paper is a discussion of a part of the mineral wealth of the States of Oaxaca and Puebla, Mexico. It does not refer to the precious metals, some miles of which, in these States, are said to have been worked before the advent of the Spanish conquistadores in the sixteenth century, as my purpose is to invite attention to the exploration of deposits of coal and iron-ores in the Mixteca region. The prominence given to coal is warranted by the great effect of the scarcity of fuel upon the industrial development of Mexico. Under the progressive administration of President Porfirio Diaz, the Republic has made wonderful advances in the past three decades; and this progress would have been greater but for the limitations imposed by expensive fuel. In the spring of 1906 I was detailed by the Birkinbine Engineering Offices, of Philadelphia, to accompany A. B. Adams, of New York, in a reconnoissance of reported deposits of iron-ores in the State of Oaxaca. The result of this expedition was the formation, in 1907, of the Oaxaca Iron & Coal Co., of which Mr. Adams became, and has remained, President and General Manager, and I have spent the past three years as Chief Engineer of this company in Mexico, principally in western Oaxaca, where field-headquarters were established. One of the most significant technical and practical challenges facing the tunnel construction team on the Legacy Way road tunnel project in Brisbane, Australia was how to erect the prestressed precast concrete slabs to form the smoke duct in the tunnel boring machine (TBM) tunnels.An essential part of the tunnel ventilation system, the smoke duct extends the full length of the 4.6 km-long twin two-lane tunnels. The vast majority of the tunnels (approximately 4.3 km) were constructed using two 12.4 m diameter double-shield TBMs. The refurbished Herrenknecht machines broke several world records on the way to completing the tunnel excavation works in just over nine months. With the excavation complete the focus turned to solving the complex civil tunnel fit-out challenges. A key component of the civil fit-out of the tunnels was constructing the smoke duct in the TBM tunnels, which was designed as a precast solution.The slabs, which each measure 8.62 m long × 2.0 m wide × 200 mm thick and weigh close to 9?t, need to be erected above cast in situ corbels whilst other tunnel finishing works continue. The space-proofing clearance offered by the design to the support corbels below and the precast segmental lining above was in the order of only 100 mm.This paper tells the story of how the Transcity Joint Venture (JV) tunnel fit-out team comprising some of Europe and Australia’s leading tunnel construction specialists drew upon the global strengths of the existing JV member companies and the global reach of leading TBM manufacturer Herrenknecht to develop an innovative system in solving this challenging problem. A new technique for reaming the holes has been tested. This technique is available for reaming a hole at its deep end creating an enlarged cavity that has helicoidal grooves oriented at less than 40 degrees to the hole axis. This technique increases bolt resistance by a factor 1.3-1.62; its bearing capacity by 3 times and enhances underground roadway stability. This technique is suitable for great depth (1000m and greater) and weak wet rocks.

Paper 1?South African experience with asbestos-related environmental mesothelioma: Is fiber type important? Neil White, Gill Nelson, Jill Murray The purpose of this review was to illustrate the importance of the type of asbestos in environmental mesothelioma. South Africa is a logical place to assess fiber type. The three major types of asbestos (crocidolite, amosite and chrysotile) were mined (all asbestos mining stopped in 2002) in South Africa. The association between asbestos and mesothelioma was discovered in South Africa. The occupational literature indicates high mesotheliomagenic potential for crocidolite, less for amosite and less still for chrysotile. Since South Africa has been a major source of crocidolite, relatively high rates of mesothelioma in the mining population is not surprising. High rates of mesothelioma due to environmental exposure is surprising, except perhaps when the authors? point out potential sources for community exposures (e.g., mine tailings used for road surfacing, golf courses, brick and plaster making; insulation in residential ceilings; children playing on fiber-rich waste rock). The authors? reviewed four studies reporting on mesothelioma cases and their sources of exposure and fiber type. Overall 23% of the known cases were attributed to the environmental (not occupational) exposure and about 90% were due to crocidolite asbestos. The proportion of environmental mesothelioma is considerably higher than in Western Australia where crocidolite is also mined. There the overall rate of environmental mesothelioma is about 6% (and less than 1% from residence in a crocidolite mining area).

Traditionally, mineral processing plants are designed to achieve the highest recovery of valuable material with limited regard for the cost of managing waste products-both solid and water components. As an industry, we are confronting ever more stringent environmental standards and lower grade deposits requiring more intensive processing. The most profitable processing route can, in fact, lead to costly issues downstream. Issues or risks associated with waste management include water treatment and handling and storage of potentially acid generating material. The capital and operating costs of dealing with these issues are significant and should be considered as a consequence of plant design and optimisation. The authors of this paper are involved with a number of projects where process operation changes offer opportunities to mitigate risks associated with water and waste management. Examples of these changes will be discussed along with the estimated benefits to the value of the project and reduction of long-term environmental risks. Process changes include 1) the application of in-plant water treatment as opposed to dealing with the entire tailings management facility and 2) the potential to reduce metal leaching and acid rock drainage through the preferential liberation of carbonate minerals in the grinding process.

An advanced numerical model based upon the Discrete Element Method (DEM) has been developed to provide the engineer with unique detailed information to assist in the design of transfer points in large industrial conveyor systems in mining operations. This modeling technique provides a quantitative description of the bulk solids movement through the transfer point. This information consists of the velocity distribution of the bulk solids and the stresses within the bulk solids. The DEM model also gives detailed information concerning the impact forces acting on the transfer structure and the conveyor belts from the bulk solids flow.

"Despite the pre-eminence of the Jwaneng Diamond Mine as the world’s richest diamond mine, the discovery story has long been clouded in mystery. This is the 45-year old untold story of the Jwaneng discovery and contemporaneous Bechuanaland/Botswana political and socioeconomic history. Diamond exploration by De Beers in Bechuanaland began in 1955, with the first true kimberlite discovery near Letlhakane in 1967, before Clifford’s Rule was applied and before the use of indicator mineral chemistry. Surface deflation soil sampling and bioturbation by termites was the foundation for exploration into the Kalahari. The 1962 reconnaissance programme south of Jwaneng had failed to make any discoveries. Improved sampling methods were applied to the Kalaharicovered areas in 1969 and positive results were received by year-end. The Whateley’s Wish percussion drill-hole in December 1972 was confirmed as Jwaneng 2424D/K2 with drill core in early 1973. Jumper drills were required to penetrate the Kalahari sand cover. Diamond resources were demonstrated despite multiple technical problems. IntroductionThe Jwaneng Diamond Mine is, by value, the world’s richest diamond mine. Harry Oppenheimer is quoted as saying that Jwaneng is ‘the most important primary deposit found anywhere in the world since the discovery at Kimberley more than a century ago.’ (Taylor III, 1982) The mine opened in 1982 after some nine years of evaluation and construction since discovery in February 1973. Debswana (2017) states that 2015 production was 7.87 Mt of ore from which 10.408 million carats were recovered at a grade of 132 carats per 100 t (cpht). Although the value is not reported, it was noted that Jwaneng contributes 60–70% of Debswana’s total revenue from all mines, including Orapa, Damtshaa, and Letlhakane (Debswana, 2017)."

"Safety performance in many companies and even industries has stalled in the few years. Accidents rates are at a "plateau" and yet, serious accidents and fatality rates are not. In more dramatic cases, such as in the BP Texas disaster, organizations that have "exemplary" safety statistics, suddenly have a catastrophic or multi-fatality event occurring. Other classic examples are the Piper Alpha disaster and NASA’s Challenger and Columbia disasters and even the Chernobyl nuclear reactor. Looking into the root causes of these accidents provides an insight into the events and deficiencies that led up to the accident, but what are the common features in the organization’s mindset? What characterizes these organizations’ decision-making, their approach to safety and to risk and are there specific cultural features that can be delineated? The research and review presented in this paper covers a period since 1994, which started in the Australian resource industry and since then covered international events. The features of these organizations are summarized as the seven deadly delusions, based on extensiveresearch since 1997 of organizations that suffered unexpected disasters.The Piper Alpha Oil Rig disaster in the North Sea, on the 6 July 1988, was one of the most pivotal events in safety around the world. It changed the thinking and focus of governments, whole industries and led to numerous books and papers. It resulted in new legislation, textbooks and a critical self-examination by the oil and gas industry.In short, an explosion and fire occurred when a pipe started leaking gas and ignited. A temporary flange, with no safety valve, was used to block off this pipe during a maintenance operation the previous shift. The permit to advise operators not to start the pumps on this line was misplaced and lost. Several deficiencies, problems and system failures coincided. A key factor was that the water deluge system was inoperable at the time and failed to extinguish the large fire that erupted, followed by an even larger gas explosion. The accommodation unit, situated on top of the oil rig, was the main "killing field" - most men gathered here to await instructions (which never came) and where they died from smoke inhalation. Later, a large explosion of the gas pipeline from an adjoined rig exploded and killed more people.The line of events and the associated system failures are all very relevant in the analysis of the accident, and provide the only insight into what caused the accident. But much "further back", inside the organization’s culture, lurks more dark matter. The first inclination is to ask what was "deficient" in the culture. Did they not care enough about safety, or about their people? Did they have a culture where production decisions were more important than safety considerations? Did they ‘coerce’ supervisors and employees to ignore safety precautions or did they just "fly by the seat of their pants"?Not surprisingly, it is very difficult to identify the true nature of the culture in that business because in such an aftermath of death and destruction, no one dare say that the safety culture was a ‘positive one, focused and caring’ or that the company was doing well in safety and that this is only a freak accident. Indeed, there were many deficiencies in the organization’s systems and procedures, such as the permit to work system.But there are some niggling questions: How is then that the Piper Alpha Oil Rig was actually considered the company’s most productive and safest oil rig in the North Sea? Why would this rig win a safety competition, 6 months before the disaster, with the deficient permit as its most outstanding successful system?"

The ability to precondition (weaken) a rock mass to enhance caving (cavability and fragmentation) is becoming increasingly important, as more potential caving deposits are in stronger rock masses, and at greater depth. This paper discusses a full-scale blast preconditioning experiment conducted at the Cadia East mining operation, including quantification of visible and microscale blast damage. From a cave mining perspective, the objectives of the experiment were to quantify the effect of confined blasting on rock mass characteristics relevant to enhanced caving, and to provide calibration data for the purpose of numerically modelling alternative blast preconditioning designs.The experiment was located approximately 360 m below surface, and consisted of two f165 mm blastholes (60 m vertical holes) and eight HQ diamond drill monitoring holes (35 m horizontal holes). The diamond drill holes were used to sample the rock mass before and after the blast at varying distances from the blastholes. Rock mass blast damage was quantified both visually using core logging, and on a microscale using LABYRINTH® statistical analysis of X-ray microtomography data. Results from the experiment indicated four major findings:on the visible scale, the experiment indicated a 147 per cent increase in the number of hairline cracks within 5 m (p value = 0.015), but no change in geotechnical logging parametersmicroscale tomography of intact rock samples indicate significant localised intact rock damage up to 4 m from the blasthole (p = 0.007)preliminary point load test results indicate that blast damage results in a significant reduction in intact rock strength (p = 0.050)the significant increase in porosity and microfracture size associated with damage provides a mechanism for reduced intact rock strength due to confined blasting.CITATION:Stewart, P C, Brunton, I, Francis, D and Chitombo, G, 2016. A unique partnership between the Colorado Mine Safety and Training Program (CMSTP), the Colorado Rock Products Association (CRPA) and the Higher Education and Advanced Technology (HEAT)Center resulted in a video CD-ROM training program for new miners entering the aggregate industry. Cooperation from everyone involved, including the U.S. Mine Safety and Health Administration (MSHA),made this the product against which other training materials in the mining industry will be measured in the 21stcentury. This product’s versatility and wide application make it a tool that is as critical to a mine’s success as any earthmoving machine. The state and federal grant money involved allows this product to be affordable to small operators and independent contractors. The quality of this two CD-ROM set rivals any commercially made CD-ROM training.

In 2004, the engineers at the Deer Creek Mine located near Huntington, Utah initiated the ventilation planning to exploit a remote area of the mine?s reserves. This area was located under a mountain which would not allow for the incorporation of an additional shaft or portal access near the proposed longwall panels. With this constraint along with the desire to maintain a two entry gate road system and the expected condition of frequent coal wash outs a ventilation plan was established. Different options were examined that included; increasing the current exhaust fan power, adding another exhaust fan installation, converting the ventilation fans in the active portion of the mine to a forcing system, and modifying the ventilation system in the active area of the mine to a push-pull system. The push-pull ventilation system was ultimately selected. This paper details the design process of the ventilation system upgrade, the procurement process for the new fans, and the new fan startup testing.

An analytical procedure is presented for evaluating the stresses in linings of bored tunnels caused by kinematic soil-lining interaction. Three cases of interaction as produced by the following conditions are treated: (1) relaxation of in situ soil stresses near a lining following its installation; (2) overburden pressure at the soil surface; and (3) free-field soil response resulting from an earthquake. Non-dimensional formulas and graphs resulting from parametric studies are presented for practical tunnel design applications. The procedure is applied separately to a steel lining and a concrete lining for a site located at the lower end of Market Street in San Francisco through which a BART (Bay Area Rapid Transit)tunnel passes.

"Conventional base metal milling practice normally consists of crushing, grinding and cyclone classification followed by flotation. It is the authors' belief that the commencement of flotation after cyclone classification leads to inferior metallurgical results and that implementation of flotation of grinding mill discharges can result in increased recovery due to reduced overgrinding, reduced activation of contaminating minerals and improved dewatering circuit results due to the production of coarser concentrates.The concept of recovering minerals from the discharge of grinding mills is not new! Gold concentrators have utilized jigs and flotation cells while iron ore plants have employed magnetic separators to recover gold and magnetite respectively from the discharge of grinding mills. More recently, a Reichert cone has been used to recover sulphide and oxide base metal minerals from screened rod mill discharge. 1To the authors' knowledge, the first application of flotation of base metal minerals from the discharge of a production plant grinding mill was at the Lake Dufault concentrator of Corpor'1tion Falcon bridge Copper. Located 10 miles north of Noranda, Quebec the Lake Dufault concentrator commenced operations in August, 1964.The installation of flot11tion cells in the Lake Dufault grinding circuit proved so successful that an identical system was installed in the Manibridge concentrator of Falconbridge Nickel Mines Limited. Located 90 miles southwest of Thompson, Manitoba the Mamibridge concentrator commenced operations in June, 1971 and due to exhaustion of economic ore reserves discontinued production in May 1977."

At the Orieco mine quartz veins carrying sulphides were localized in parts of an extensive fault zone in the Mathinna Beds during late Middle Palaeozoic mineralization.These deposits were subsequently exposed, and oxidation of the sulphides by percolating groundwaters resulted in the formation of gossans at the surface, and replacement close to the permanent water table of the more soluble sulphides by copper sulphides (chalcocite, coveIlite). A restricted zone containing copper oxides (cuprite, tenorite), copper carbonates (malachite, azurite), iron arsenates (scorodite, pharmacosiderite) and possibly rare copper sulphates occurred above the water table.The renewed oxidation of these deposits following mine development has been facilitated by the ready circulation of air and water combined with the formation of large free surfaces ideal for evaporation of aqueous solutions. Under these new conditions of high initial sulphate concentration, low pH, and relatively high Eh, the previously formed mineral assemblage is unstable and numerous Cu (Fe,Zn)-sulphates and Fe (Cu,Pb)-arsenates are presently forming in fractures in the fault zone and on the adit and stope surfaces for some considerable distance from the Sulphide-rich ore shoots. The observed spatial sequence of the major sulphates with respect to the sulphide-rich zones (Le. chalcanthite-antleritebrochantite and langite) is consistent with decreasing sulphate concentration and increasing pH away from the oxidizing sulphides.

Failure of rock materials is a process of crack propagation. Crack initiation takes place when the crack tip stress intensity K reaches a critical value called fracture toughness, K1C. The rock fracture toughness is known to be affected by the surrounding environment such as temperature, confining pressure and humidity. In order to examine the effect of humidity, a series of semi-circular bend tests were performed under various water vapour pressures in a rock material that is known to be anisotropic. Water vapour promotes stress corrosion of rock and therefore the fracture toughness was found to have a decreasing trend with increasing water vapour pressure. The rate of decreasing the fracture toughness depends on the microcrack density that promotes the migration of water vapour into the rock. Also in an anisotropic rock, the fracture toughness depends on the direction of crack in relation to the anisotropy of the rock material.

Repeated noise exposure and occupational hearing loss are common health problems across industries and especially within the mining industry. Large mechanized processes, blasting, grinding, drilling, and work that is often in close quarters put many miners at an increased risk of noise overexposure. In stone, sand, and gravel mining, noise is generated from a variety of sources, depending on the type of ore being mined as well as the final consumer product provided by that mine. Depending on the source of noise generation, different strategies to reduce and avoid that noise should be implemented. The National Institute for Occupational Safety and Health (NIOSH) has evaluated the noise profile at three operational surface stone, sand, and gravel mines. A-weighted sound level meter data as well as phase array beamforming data were collected throughout the mines in areas with high noise exposure or high personnel foot or vehicle traffic. Sound level meter data collected on a grid pattern was used to develop sound profiles of the working areas. These sound contour maps as well as phase array beamforming plots were provided to the mines as well as guidance to modify work areas or personnel traffic to reduce noise exposure.
