Incident investigation is of utmost importance in most high-risk industries. This study was undertaken to study the content of past investigation reports to determine the effectiveness of the incident investigations. The study uses a semi-quantitative method to assess the effectiveness of incident investigations in the Ghanaian mining industry by evaluating the quality of past investigation reports. The assessment tool consists of 5 elements with several indicators and rating scales for assessing the quality of an investigation report as a measure of the effectiveness of the investigation. The method was applied to 304 investigation reports of 3 Ghanaian large-scale gold mines, and the results correlated with incidence rates of the mines to determine if any relationship existed. The results showed that the mines differ significantly in the quality of their investigation reports, suggesting differences in the effectiveness of their investigations. In addition, the incidence rates of the mines negatively correlated with some elements of the assessment tool. In general, the method was found useful and revealed areas where improvement is needed. INTRODUCTION The importance of learning from incidents is well documented in the literature. For instance, some scholars regard learning from incidents as important for improving safety [1,2]. Thus, many organisations spend considerable resources in identifying lessons from incidents and subsequently on the process of improvement that follows the identification of lessons. It is therefore important for such organisations to determine the effectiveness of their learning from incidents processes, in order to identify specific areas where improvements are needed, so that interventions can be as specific as possible. The learning from incidents process models the plan-act-check-do approach of management systems, often consisting of four major stages, namely; data collection and incident analysis, planning interventions, implementing interventions and follow-ups and evaluation [3,2]. The investigating and analysis of incidents have been regarded as one of the vital stages of the learning process since it is the stage where lessons are identified [3,2,4]. It has also been noted elsewhere in the literature that results of the previous stage of the learning process are important for the next stage [3]. For example, ineffective incident analyses could result in the planning of ineffective remedial measures. One of the primary aims of incident investigations is to identify lessons from previous incidents which when implemented can prevent future recurrences or minimise the effects of future incidents. There is thus the need for organisations to assess the quality of their investigations periodically since it is one of the means they adopt to improve safety and the quality of remedial measures to some extent depends on the effectiveness of the investigations. Although previous studies [5,2] have assessed the effectiveness of the learning process, and some aspects of the learning process, such as incidents reporting, there is less focus on the effectiveness of incident investigations. The research described in this paper was therefore conducted to contribute to this gap, focusing more closely on incident investigation reports.

During 1998, 62 million tons of iron ore pellets valued at $1.7 billion were shipped from U.S. mines. These pellets are the primary source for domestic iron and steel production. During pelletization, binders are required to improve the green strength of the pellets. Each year, bentonite binder costs a typical plant from $3 million to $5 million. Reducing the bentonite dosage has the advantages of significant cost savings while producing higher quality, lower silica pellets. The authors have recently identified a new and important bentonite binding mechanism, the fiber theory. During this study, procedures were developed that took advantage of the ability of bentonite to form fibers, and resulted in a dramatic decrease in the necessary bentonite dosage. Acceptable quality pellets were produced with the bentonite dosages reduced in half from 14 lb/lt (0.66%) to 7 lb/lt (0.33%).

"The situation of the Ti-V-Magnetite samples reduced with microwave has been investigated in laboratory. Firstly, the paper focuses on the heating behaviors of vanadium-titanium magnetite and characteristics of carbothermic reduction of V-Ti-magnetite in the microwave field. The effect of concentrate's size, quality on heating behaviors and reduction processes were researched. Results show that microwave heating may be a new heating resource replace of carbonic combustion reaction in reducing reaction. When the allocation of carbon is (16.7-30)%, carbothermic reduction of V-Ti-magnetite in the microwave field can proceed; with the increasing of carbon mixed, the metallization rate increases; and Fe, V can be reduced from their oxides in the microwave field; Titanic low oxide can be reduced from TiO2.1. IntroductionThe resource of Ti-V-Magnetite is abundant in the western area of Panzhihua in China, so far a mineral deposit of 4.64 billion tons have been proved up, it contains 14.75 million tons of V2O S which accounted for 87% of the vanadium and 5.93 hundred million tons of TiO2 accounted for 94% of the titanium resource respectively in China. The previous study proved that the major composition of mineral of the Ti-V-magnetite are Ti-magnetite, limonite, iron spinel, magnesium spinel etc, and the main elements of the Ti-V-magnetite in this area are Fe, Ti, V, Cr, Co, Ni, Ga, Sc etc[1] Due to the restrict of the BF-BOF process technical, during the smelting of the iron, the level of extracting the vanadium, titanium as well as other metal elements is limited. This results serious wasting of resources. To develop a new reduction process of Ti-V-magnetite is on the agenda, the microwave assistant reduction could meets this need, based on its characteristic of easy controlling of heating rate. To make use of the microwave energy to substitute the released heat sources of carbon combustion with air, use the solid phase state reduction at a relative lower temperature, make the beneficial element in Ti-V-magnetite transferred into the metal phase, then separated from the impurity. It is well worth to point out that once the microwave based reduction method has its industrial scale, it will take an outstanding position by its less release of carbon dioxide. This new environment-friendly process will play a vital role in the development of social industry and economy."

Its three main objectives are to organize the subject matter according to basic principles and unit processes; to integrate the underlying science of physical chemistry with metallurgical engineering; and to put greater emphasis on analysis and quantitative solution of engineering problems. Vol. I deals with unit processes, stoichiometry, thermodynamics, fuels, fluid flow, heat transfer, pyrometallurgical systems and refractories. Vol. II will take up the individual unit processes. Initiation and Growth of Explosion in Liquids and Solids, by F. P. Bowden and A. D. Yoffe. Cambridge University Press. $4.50, 104 pp., 1952.-The book is a study of decomposition by impact and by friction of explosive substances and deals mainly with the work of the authors and their collaborators. Research is still in progress but results thus far indicate that the mechanism of initiation is relatively simple. Findings are thoroughly discussed, and the latter part of the monograph describes the way in which a small nucleus of decomposition can grow into a large scale explosion.

In a recent investigation of the entire magnesium-yttrium phase diagram, Gibson and Carlsonl report the maximum solid solubility of yttrium in magnesium as 8.0 wt pct Y at 1050°F (565°C). However, independent studies of the precipitation in magnesium-yttrium alloys, 2 conducted prior to the publication of the above phase diagram, indicated a greater solid solubility of yttrium in magnesium. Accordingly, the magnesium-rich region of the diagram was re-investigated to determine more accurately the liqui-dus, solidus, and solvus of the magnesium solid solution in this system. Metallography and thermal analysis were chosen as the best corroboratory methods for accurately determining this region of the diagram. A magnesium-yttrium master alloy was prepared from yttrium of 99.9 pct purity and singly sublimed magnesium of 99.9 pct purity. Subsequently, the twelve magnesium-yttrium alloys shown in Fig. 1 were prepared using standard alloying procedures for magnesium. Metallographic samples of the alloys were heat treated under an SO, atmosphere in an electric furnace capable of control within ±3oF of the desired temperature. Alloy samples for the metallographic determination of the solvus, and for the precipitation studies were solution treated at 1000° F for 16 hr. Subsequent aging times for the solvus determination varied from a minimum of 6 hr for the higher temperatures up to 816 hr for lower temperatures. Periods of 30 min at temperature were used to generate liquation for the metallographic placement of the solidus and eutectic horizontal. The liquidus and the eutectic horizontal were determined by thermal analysis. Simple time-temper- ature heating and cooling curves were obtained from 10-g alloy samples heated and cooled in a well-insulated electric furnace. An Iron-Constantan thermocouple was placed in direct contact with the sample. The thermocouple did not contaminate the alloy sample since metallographic examination showed that the alloy melt did not wet or attack the thermocouple. The heating and cooling rates were approximately 5 to 8OF per min. The standard metallographic procedures for magnesium were used in polishing and etching the magnesium-yttrium alloys studied. TWO hundred enthusiastic engineers and coal, op-erators attended a joint meeting of the A. I. M. E., the National Coal Association, and the American So-ciety of Mechanical Engineers in the auditorium of the Alabama Power Co. building at Birmingham, Ala., on Jan. 21. S. W. Farnham, mining engineer of the Goodman Manufacturing Co., Chicago, in his paper on mechanical loading of coal discussed the feasibility of scraper and machine loading. A. R. Anderson of the Jeffrey Manufacturing Co., in a paper on loading coal with conveyors covered their adaptation for long-wall mining and for loading into cars. Discussion of these papers was hampered because the cheap labor costs prevailing in the district have handicapped gen-eral mechanization of the mines. The Bureau of Mines film, "The Story of a Rock-dusted Mine," showing the New Orient mine, West Frankfort, Ill., was also shown at the meeting. F. H. Crockard, president of the Wood-ward Iron Co., represented the A. I. M. E., S. L. Yerkes, vice-president of the Grider Coal Sales Agency, and J. L. Davidson, secretary of the Alabama Mining Institute, represented the National Coal Association, and Paul Wright represented the American Society of Mechan-ical Engineers.

The sorptive and impervious nature of clay materials was recognized long before modem technology allowed us to understand the physiochemistry of clays. In Cyprus, around 5000 BC, sorbent clays were used for fulling woolen cloth. One of the earliest recorded uses of clays as an impervious liner was more than 3200 years ago in the construction of a bitumen-sand-gravel-clay liner built along the embankment of the Tigris River at Assur. Today one of the most common applications for clay minerals in the environmental industry is for use as impermeable barriers to fluid transport. Clay minerals are fine in particle size and thus have large surface areas, which, as a result of intermolecular forces and ionic substitutions, make them excellent materials for both ion exchange and ion sorption. Therefore it was logical to consider certain clay materials for applications in the environmental industry, where they can act as physical and chemical traps for potential contaminants. Certain bentonites have historically been used as sealants for fluids (Grim, 1962), and other clays have been used as sorbents for both water and oil (Haden, Jr., 1972). Prior to the 1980s, disposal of the nation's municipal, commercial, chemical, and nuclear wastes was largely indiscriminate and poorly regulated. This created environmental pollution problems, especially with respect to groundwater contamination. Promulgation of federal regulations like the Resource Conservation and Recovery Act (RCRA) and the Comprehensive Environmental Response, Compensation and Liability Act (CERCLA - commonly known as Superfund) have forced a more careful and costly approach to waste disposal. The US Environmental Protection Agency (EPA) is the government body charged with enforcing and ensuring compliance with federal regulations governing toxic waste production, storage, and disposal. Widespread application of geotechnology to the design and construction of waste containment systems and to pollution remediation has become necessary due to such laws and because of increasing use of the subsurface environment for the disposal of hazardous wastes. In the United States nearly 1.8 kg of trash is disposed of per person per day, which amounts to nearly 181 Mt per year. This amount is more than double the waste produced in 1960, and these numbers are expected to continue to increase. Canada produces approximately 1.68 kg of waste per person per day. The EPA estimates that in the United States 64% of our garbage is landfilled, 18% recycled, and 18% incinerated. Even with continued increases in recycling and incineration efforts, the EPA projects that the United States will still need 82% of today's landfill capacity in the year 2000. Data on the exact number of landfills in the United States and Canada is conflicting in part because of changing definitions of what constitutes a landfill and because of inadequate record- keeping by some states and provinces. An estimate of the number of municipal solid waste (MSW) landfills in the United States in 1990 to 1991 ranged from 4 462 to 10 467 with an average estimate of 6 600. In Canada the number of MSW landfills reported varied from 5 493 in a 1982 survey to 1 766 reported in a 1991 National Solid Wastes Management Association (NSWMA) survey (Repa and Sheets, 1992b). A properly operated MSW landfill on average requires 1 hectare of land per year per 25 000 people (Coates, 198 1). In addition to MSW facilities, there are also waste disposal facilities needed for hazardous waste and low level and high level radioactive waste. In 1983, approximately 266 Mt of hazardous waste was generated in the United States, almost 50% of this by industrial production. About 68% of the hazardous waste generated in the United States and Canada and almost 50% generated in Europe are still landfilled (Egger, 1987). In 1985, approximately 51 000 m3 of commercially produced low level radioactive waste was disposed of in radioactive waste disposal sites across the United States (Jungling and Greeves, 1989). By 1987, the US defense program had produced approximately 10 000 t of high level nuclear waste. In addition, there were approximately 14 000 t of spent fuel assemblies from commercial nuclear power plants in temporary storage; the amount is expected to nearly triple by the year 2000. Presently most commercial spent fuel assemblies are stored in water-filled pools at nuclear power plants. Defense waste is stored on federal reservations in surface and underground steel tanks (Anon., 1987). Under federal law nearly all waste disposal and containment sites are required to have a liner or barrier and a cover system design that meets federal- or state-approved hydraulic conductivity requirements for the type of waste that the site will receive. When approved materials, necessary for constructing the waste containment structure, are not available on site, one of the most economical ways to meet these requirements is through the use of clay, soil/clay, or clay/geotextile materials. In a 1991 survey by NSWMA of its members and other select private sector facilities, 81% of the surveyed landfills had some type of liner system. About 75% of these liners were clay materials (either recompacted or natural). The remaining 30% were using flexible membrane liners as part of a liner system. Clay liner materials are most commonly used in the South, Midwest, West-central, and South-central regions of the United States. About 93% of the landfill facilities had closure plans, and of these 91 % were using clay materials in some way for the final cover material (Repa and Sheets, 1992a). Waste barriers and containment structures take several basic forms, and they are used in a variety of different applications: 1) cutoff walls, 2) soil or soil/clay-admix liners, 3) geosynthetic bentonite-clay/geotextile membranes, 4) radioactive waste containment barriers, and 5) synthetic geomembranes. DURING an investigation into the effect of heat-treatment on the creep properties of the magnesium alloy ZW1, (1 pct Zn, 0.6 pct Zr), the previously published methods of final polishing were found to be unsatisfactory. A new diamond-polishing technique has been developed to facilitate the study of fine precipitates in this and other magnesium alloys, and is described in this note. The sectioning and coarse grinding techniques are those of Fox and Hall,' and Hess and Cieorge.' Pre- liminary mechanical polishing is done with 6-µ, and 1-µ( grades of diamond compound on 'Microcloth' using a lubricant of 'Hyprez' oil. When the conventional technique is used with a 1/4- µ diamond wheel for the final polishing of magnesium alloys, scratching is likely to result from the abrasive action of the 'Microcloth' nap on the soft metal surface. This problem has been overcome by the use of a cream on the wheel in order to separate the specimen surface from the polishing cloth. To prepare this cream, a solution of 6-ml tri-ethanolamine in 75-ml water is stirred into 12.5 g stearic acid at a constant temperature of 80o to 90°C. The cream is allowed to cool, and is then 'whipped' to a smooth consistency with 100-ml 'Hyprez' oil. Before polishing, the diamond loaded 'Microcloth' is moistened with a few drops of 'Hyprez' oil, then a small amount of cream is worked into the nap. For the best results a wheel speed of 500 rpm should be used for polishing, and the specimen should be held by hand with a light pressure while being rotated in the opposite direction to the wheel. After polishing it is necessary to swab the specimen thoroughly in methylated spirits to remove residual cream and oil from the surface in order to avoid uneven etching and drying stains. Fig. 1 shows the surface finish obtained on a specimen by polishing on a 1/4- µ diamond wheel using the cream, and Figs. 2 and 3 the precipitate observed in ZW1 after polishing, and etching for two seconds in nitric acid 15 pct in ethylene glycol. This new diamond-polishing technique has made a valuable contribution to the programme of metal-lographic research on ZW1 and has become a routine laboratory practice for other low alloy content magnesium alloys.

On the average, however, the new graduates are, without any doubt, the best trained ever, or at least they have been the most expensive to train in the universities.There appear to be exciting opportunities for competent, well-trained and educated scientists and engineers far beyond the numbers that can be annually produced, and the demands for excellence in depth as well as in breadth of training probably will continue to increase for many years to come. IF you do not make the disc at the same time as the core and as part of it, it is necessary to make it by itself. This is done by spreading on a board or other flat surface a layer of clay as thick and wide as [88] the disc that you made at the feeding head of the first pattern of the gun. B. C. Burgess-—Prior to hearing this paper presented at the San Francisco meeting, I travelled by car from Yuma, Ariz., across south-central California and up through the San Joaquin Valley. After hearing the paper I returned by the coast route to San Diego then along the Mexican border to Yuma. The striking feature of these trips, related to this paper, was the dry river and creek beds at a time of year not the dry season. Contrasted with this was the wasteful use of water in irrigation canals and laterals. Poland says "at least 90 pct of the ground water pumped is used for irrigation." That being the case, it would appear that first consideration should be given to conservation of this use. Probably a small part of this is actually reaching the plants. Some measure of this loss between pump and crops would be revealing. Methods that might be employed to reduce this loss are pipe lines and spraying of some crops such as the banana planters are using in Central America. Another idea which might be investigated is to line canals and ditches with some water-repelling mineral like pyro-phyllite. Perhaps a comparatively thin sprayed-on

The main objective of the experimental study was to investigate the dissolution kinetics of various forms of carbon under iron bath converting or smelting-reduction conditions, i.e., particulate, free moving carbon in contact with a turbulent iron melt. Iron/carbon melts were contained in cast magnesia-lined silicon carbide crucibles heated by a medium frequency induction furnace. The melts were stirred by injecting nitrogen through 2 mm I.D. alumina lances submerged to a depth of 50 mm in the 100 mm deep baths. Particulate carbon in the form of graphite, brown coal char and petroleum coke, were dropped directly onto the turbulent bath. The dissolution rates of the various carbon types were monitored by taking periodic bath samples for subsequent carbon analyses. The effect of carbon particle size (1-6.25 mm), the degree of bath turbulence (nitrogen stirring rates of 0-20 litres/min), and bath temperature (1300-1500¦C) were assessed. The analysis of the results was complicated by particle buoyancy effects, but it was established that the dissolution rates of the three carbon types investigated were similar, and increased linearly with increases in the gas stirring power inputs. Particle size had relatively little effect on the dissolution rates.

Remotely located in the jungle near mount Puncak Jaya [16,500 ft (5,030 my), Irian Jaya, Indonesia rests Freeport's Indonesian copper mining operation. Shortly after start up of Freeport's copper concentrator in 1972, lower than estimated throughput was experienced within the crushing circuit. Within one year, Freeport had successfully engineered a solution, and was operating a 5,500 dmtpd wet screening circuit. The patterns of occupational health pro- grammes in mining and metallurgy are discussed in relation to the impact of chemical and phy- sical hazards on the health of workers in these fields. Mining and metallurgy are quite unique in that their history and practice embrace virtually the total spectrum of occupational health problems and hazards as understood today. A panoramic concept of the possibilities would comprise the following fields: 1. The Pneumoconioses or Dust Diseases. 2. Physical stress due to exertion, and exposure to heat and humidity in deep mines. 3. The toxic effects of various metals in the mining of ore. The most obvious are lead, mercury, arsenic and manganese. 4. Diseases associated with mining. These include leptospirosis, ankylostomiasis and mycotic infections of the skin. 5. The Beat Diseases of coal miners working in narrow seams. 6. Illumination problems and miners nystagmus. 7. Geographical and climatic problems

One hundred ninety nine beach face sand samples from an equal number of beaches from the Mexican Pacific were studied. Based on petrological and chemical analysis some locations have been studied in more detail. Black sands from the following areas are discussed: (1) west Baja California Peninsula, where black sands rich in magnetite and ilmenite have maximum values of 50% iron and 14% titanium in the very fine sand fraction; (2) east Gulf of California, in its northern portion, the black sands show maximum values of 43% iron and 5% titanium in the fraction 2 to 4 0;-(3) the south Mexican Pacific, characterized mainly by medium sands, moderately well sorted, show that the fine sand fractions reach values of 35% iron and 41% titanium. X-ray fluorescence applied on selected samples constituted by feldsarenites and lithic feldsarenties, showed the presence of rare elements in some of the studied samples.
