Rock masses in deep level mines are subject to high stresses, which can result in unexpected failure of rock into mined-out openings. Historically, various independent methods have been used to discern the causes of rock bursts and to evaluate stress conditions in rock masses susceptible to violent failure. Personnel from two research centers of the former U.S. Bureau of Mines, Spokane and Twin Cities, investigated a rock burst that occurred in May of 1994 in a rock mass between the 4400 and the 4600 levels of the Sunshine Mine, Kellogg, ID. The team evaluated the current state of stress in the pillar and the most likely cause of the rock burst by studying the seismic history of the pillar, examining the geology, analyzing the available in situ stress data, calculating possible fault-plane solutions for the burst, and performing a seismic velocity tomographic survey. The results of the study showed that post-rock-burst seismicity was concentrated about 15 to 30 m west of and about 15 m above the rock-burst location. The pillar is composed of a very hard sericitic-to- vitreous quartzite of a type rarely found in the mine, but that is often found in other district mines having a history of seismic activity. In situ stress analysis of the pillar and the surrounding rock mass showed a probable stress rotation from northwest-southeast to east-west. Of four fault plane solutions, the most probable was either a left-lateral, strike-slip movement with a compressive principal stress oriented nearly north-south, or a right-lateral, strike-slip movement oriented east-west. Finally, tomographic images showed low-velocity areas (interpreted as areas of low stress) associated with the crosscuts and haulageways, and high-velocity areas (interpreted as areas of high stress) associated with the rock-burst location. High velocities were also found in a planar area oriented in an elongate northeast-southwest direction.

General A review of the literature has revealed important areas which should be investigated during the field work. Of the utmost importance is the safety of mine personnel. Should a phase-to-ground fault occur on a shovel or other piece of equipment, current may return to the substation through the machine's contact surface with the mine floor, thereby raising the frame above ground potential and posing a shock hazard to anyone who touches the equipment. If the frame-to-earth resistance is very low, then no dangerous potentials will be developed, and a basis for elimination of the ground conductors from trailing cables could be postulated. However, should the contact resistance be high, then lethal voltages could be present on the machine frame. In this instance, low-resistance ground conductors in the trailing cable would be a necessity. Therefore, field tests should include measurements of the earth resistance of electrically operated portable or mobile mining equipment such as shovels, draglines, and drills. When the electrical distribution system is subject to lightning discharges, the grounding grid must safely carry to earth and dissipate enormous current surges. It is essential that the grounding grid have a low value of earth resistance, because any potential developed across the grounding grid as a result of current flow will appear on the frames of all machinery connected to the grid. Location and Age The main bituminous coal field of Pennsylvania lies in approximately twenty counties west of the mountain area which extends in a northeast southwest direction through the central part of the Commonwealth. The coal fields lie in a dissected peneplain, still in the youthful stage of erosion There are numerous V shaped valleys in the bituminous fields and the tributaries of the larger streams have rather steep gradients The main streams flow in rather narrow valleys with comparatively small flood plains. The coal beds of Pennsylvania are of Carboniferous age, and are confined to two systems, the Permain and the Pennsylvanian The latter system is divided into an upper or Pittsburgh series and a lower of Pottsville series The Peimain system is divided into an upper and lower series The Pottsville series of the Pennsylvanian is represented in western Pennsylvania by the Upper Pottsville only The upper or Pittsburgh series of the Pennsylvanian is represented by the Monongahela, Couemaugh, and Allegheny groups Geologic Structure. This information circular presents data for 1960 on individual coke plants in the United States. All oven-coke plants in existence, whether active or idle at the close of the year, are listed in this report. With two exceptions, only beehive plants on which reports were submitted to the Bureau of Mines are included. Although the Bureau was unable to obtain reports from these two active plants in 1960, the number of ovens and annual coke capacity are estimates, based on previous reports filed with the Bureau of Mines. Accordingly, there were 73 oven-coke plants in existence at the end of 1960 of which 70 were active. Reports were received on 44 beehive plants of which only 13 were in operation at year end. Oven-coke plants are listed alphabetically by States in table 1 which shows the name and address of the company, location of plant, classification or type of ownership, number and type of ovens, annual coke capacity, and coal-chemical materials produced. The terms "furnace" and "merchant" plants used in this and all other reports of the Bureau of Mines designate the type of ownership. Furnace plants are those owned by iron and steel companies whose main business is production of coke for use in their own blast furnaces. Merchant plants are those associated with chemical companies, gas utilities, as well as those that produce for sale on the open market metallurgical, industrial, and residential-heating grades of coke and coal-chemical materials. The annual coke capacity shown for each plant in this table is based on the minimum coking time necessary to produce coke with qualities suitable for its intended use. Therefore, the annual capacity of a plant may change from year to year, depending on age and condition of ovens, the character and quality of coal carbonized, the grade of coke required, and other factors. For example, annual coke capacity of a coke plant primarily making foundry coke with longer coking cycles would be lower than one making blast-furnace coke. Also some ovens must be operated at slightly lower than normal flue temperature because of the condition of the refractories and maximum capacity would be somewhat lower than designed capacity. For this reason, the capacity reported to the Bureau of Mines may differ from the designed or rated capacity estimated by the coke-oven builders at the time of construction. For example, if the generally accepted standard coking rate of one inch per hour were used on all ovens to calculate total annual capacity, it would have been 80.4 million tons or 2 percent higher than the capacity reported to the Bureau. Because of the factors mentioned above, the annual coke capacity shown in this table is believed to be a reliable measure of the practical operating capacity of the industry.

This study is intended to serve three roles. First, it is an extension of the previous works under the same title that have been the overview and summary chapter of the international volume of Minerals Yearbook since the inception of that volume. Following the traditional format of past chapters, the basic statistical presentations herein deal chiefly with 1989, and in some instances with 1988, the latter only because more recent, comprehensive global statistics for 1989 are simply not yet available owing to delayed data collection and processing in some countries and continued governmental suppression of data in others. The second role of this study is to provide an update to the coverage of events impacting on the world's mineral industry that have occurred between the end of 1989 and the preparation of this study. Inclusion of this update is almost essential because of the potential effect of some of these events on global mineral industry activities.

There are Chinese, Japanese, Russian, Egyptian, and Native American counterparts of the phoenix. All of these birds are identified with the sun and depicted as rising from the ashes ? living again. This is the essence of a community after a natural disaster ? they live again. The Phoenix of Natural Disasters: Community Resilience is edited by Kathryn Gow and Douglas Paton who have done an excellent job of creating a relatively seamless transition between chapters with different authors. The content is organized well with the first chapter providing an excellent overview of what is to come and each chapter including an abstract clarifying the focus of the chapter.

"Haulage accidents usually are responsible for about 17 or 18 percent of all coal-mine fatalities and about 21 percent of all non-fatal coal-mine accidents. When the relatively small number of employees continuously exposed to haulage hazards is considered, it would appear that haulage is more hazardous than any other phase of coal-mine operation.Haulage accidents have increased from year to year, doubtless due to the increase in electrical haulage, and particularly to the use of larger cars, heavier locomotives, longer hauls, and greater speed. Unless very definite steps are taken to eliminate the causes of these accidents, they will continue to increase rather than be reduced, as they can and should be.Underground haulage in many of the large modern mines of today is similar in many respects to railroad practice. In recent years the railroads have made wonderful strides in reducing accidents, and instead. of being one of the most hazardous occupations, as it once was, railroading is now one of the safest. Doubtless, if thought and action, similar to that given to reducing accidents among railroad passengers and employees, were given to safety in mine haulage, under¬ground haulage accidents could be reduced in about the same ratio.Haulage accidents may be classified, according to responsibility, into two groups - those for which management is largely responsible and those for which the employee is largely to blame; there is, however, an overlapping of responsibility from one group to the other. If, therefore, haulage accidents are to be reduced, it is absolutely essential that both management and employees cooperate to this end.In the first group, for which management is largely responsible, may be included accidents due to faulty equipment, moor track, a poorly arranged system of haulage, lack of clearance or of overhead protection, and dirty roadways; even in these features, however, the employee is not without a measure of responsibility, as failure of haulage equipment possibly may be due to lack of care on his part, failure to report when equipment is out of order, or failure to do a good workmanlike job of installing or repairing or operating.To the second group of accidents for which the employee may be largely responsible, belong those caused by such careless practices as jumping on and off fast-moving trips, coupling on the ""fly"", running at excessive speeds, failure to properly block cars, and similar hazardous and unnecessary acts of omission or commission. If, however, management condones such practices, or, as in some instances, insists upon them, it must accept its share of responsibility for the accidents that inevitably result.Some of the most prolific sources of haulage accidents will be discussed briefly, together with the steps that may be taken to minimize their occurrence."

A cooperative research effort was established between the Tilden Mining Company LC, the United Steelworkers of America, and the National Institute for Occupational Safety and Health to lower respirable dust levels in an iron ore grinding and concentrator plant. This cooperative effort involved three field studies to evaluate different techniques to lower respirable dust levels. The greatest impact on lowering respirable dust concentrations throughout this facility was achieved through ventilation changes. The most significant improvement was realized by changing the ventilation profile throughout the entire facility. The ventilation to this grinding and concentrator facility was provided by approximately 40 roof fans operating in either the intake or exhaust mode. The operation of these fans was significantly changed in an attempt to pro-vide more of a directional flow pattern throughout the facility, as well as to more closely balance the intake and exhaust air volume. This change accounted for a 31 pct reduction in respirable dust levels in the primary grinding area of the mill. Another successful modification was to improve the sealing of a reclaim tunnel to the outside and by barricading the access point from the reclaim tunnel to the concentrator plant. This minimized the dust liberated in the conveyor tunnels from traveling through the reclaim tunnel and into the plant. These two modifications illustrate the impact that improvements in ventilation can have on lowering respirable dust concentrations in iron ore processing facilities.

Slope stability accidents are one of the leading causes of fatalities at U.S. surface mining operations. The Spokane Research Laboratory of the National Institute for Occupational Safety and Health (NIOSH) is currently conducting research to reduce the fatalities associated with slope failures. The purpose of this paper to discuss some of this research and to present potential new technologies for slope monitoring and design. The paper also briefly discusses various warning signs of slope instability, introduces the most common slope monitoring methods, describes the limitations of various slope monitoring systems, and presents some field results using some of this new technology.

Well-prepared personnel and an effective emergency response plan are essential components of any emergency command center. Unfortunately, evaluating emergency preparedness is often difficult until a disaster actually occurs. For example, unexpected events may develop during an emergency that were not considered during the planning phase. Also, the command center may need to cooperate with organizations that it has not worked with previously. Preparation can be especially difficult for smaller or remotely located command centers, or those with limited resources to conduct mock drills. This paper explores the potential of using the Internet to teach and evaluate command center skills. The National Institute for Occupational Safety and Health (NIOSH) is developing an Internet-based, computer simulation training package for the mining industry. Trainees will play the role of command center leaders at a mine, while the actions of other individuals and organizations are simulated. Trainees will be able to test their emergency response plans and learn valuable lessons from past mine emergencies. Nationally recognized experts will be able to create new training scenarios and make them available to mines throughout the U.S. This product should better prepare command center personnel for large-scale disasters such as fires and explosions. For all coal mining categories, data on coal mine fires during 1990-1999 have been reported as actual numbers and calculated values. 1. For each mining category, actual numbers include the total number of fires, fire injuries, employees' working hours, lost workdays, and coal production (for underground and surface mines only) for a 1 0-year period (1 990- 1999) and for five successive 2-year periods with the 10-year period. These numbers have also been reported by state ( 10-year period). The actual number of fire fatalities has been reported by time period. Furthermore, actual numbers of fires for the five 2-year periods have been reported by ignition source, method of detection and suppression, equipment involved, location, and burning material. Actual numbers of fire injuries per number of fires causing injuries and total fires have been reported by year, ignition source, equipment involved, and location. 2. For each mining category, the calculated values include the fire and injury risk rates during the 10-year period and the five 2-year periods. The fire risk rate (Frr) values were calculated according to the USBM formula [Pornroy and Carigiet 19951. The injury risk rate (Irr) values were calculated according to the MSHA formula [MSHA 1991a, 1992, 1993, 1994a, 1995a, 1996, 1997, 1998b, 1999c, 20001. Also, risk rate values for individual states (10-year period) were calculated according to the above-mentioned formulas. Of note is that only the risk rate values for the 10-year and five 2-year periods and risk rate values for individual states with the highest number of fires and fue injuries were considered for comparison purposes. The fatality risk rate values were not calculated because of the extremely small number of fire fatalities during the 1 0-year period. 3. Calculations of risk rate values are as follows: a. Fire risk rate (Frr) value: Number of fires per million tons of coal produced [Pornroy and Carigiet 19951. b. Injury risk rate (Irr) value: Number of fire injuries multiplied by 200,000 working hours per total employees' working hours [MSHA 1991a, l992,1993,1994a, 1995a, 1996, 1997, 1998b, 1999c, 20001. The Irr value is the average risk rate value for the number of fire injuries per 200,000 working hours for a given time period.

Improving the air quality in metal/nonmetal mines is important in protecting the health of miners. Air quality can be improved in metal/nonmetal mines by developing proper ventilation techniques. Mine ventilation systems require stoppings to direct the airflow and establish pressure differentials throughout the mine. Due to their large size, stoppings in large-opening mines are cumbersome and costly to construct and maintain. As part of its mining health and safety program, the National Institute for Occupational Safety and Health (NIOSH) designed and tested two novel stoppings (Super Stopping and the EZ-Up Curtain Stopping) at its Lake Lynn Laboratory (LLL). The Super Stopping is designed as a long-term permanent stopping for use in the main entries of the mine, while the EZ-Up Curtain Stopping, although very durable, is designed more for portability and ease of installation. This paper describes the design, materials, and construction methods used for these stoppings as well as their performance and durability when subjected to tests simulating actual production face blast pressures.

Highwall mining is an important surface coal mining method, and it may account for approximately 4% of total U.S. coal production. Highwall stability is the major ground control related safety concern. Ground control plans for highwall mining should specify hole width, web pillar width, barrier pillar width and number of holes between barriers. This paper offers simple design charts for these parameters aimed at providing “ball park” checks. Web pillars containing pre-existing auger holes are analyzed and a design chart for estimating their minimum width is also presented. Finally, close proximity multiple seam highwall mining, which may have caused several serious highwall failures, is analyzed. Web and barrier pillar recommendations for this special situation are presented.

Cost Systems Cost systems are organized methods of collecting, allocating, and accumulating resource utility expressed in terms of money. There are two major spheres of cost accumulation: financial accounting and cost accounting. Financial accounting deals primarily with historical costs and provides a means of measuring and recording cost events which have al¬ready taken place. While there are no legal requirements to control the methods of recording and reporting cost data in the United States, the procedures in use are guided by professional organizations such as the American Institute of Certified Public Accountants. The final pro¬ducts of financial accounting practices are the income statement and balance sheet which describe the financial status of the organization under study. Cost or managerial accounting is an expansion of financial ac¬counting procedures, providing more detailed cost breakdowns on present operations with one objective being the projection of present costs into the future. The projection process tends to limit the accuracy of the information; however, to management and project justification per¬sonnel, data on future operations tends to be of greater value. The basic principle behind cost accounting is the cost cycle. As products are developed from raw materials, the value of the work-in¬process increases due to the cost input of the various fabricating ele¬ments. Thus, from the basic input elements of facilities, labor, and raw materials, costs are added to each unit of production as a function of the work performed on that unit.

It would permit coal mines to operate continuously instead of intermittently . Intermittent operation increases the cost per ton for mining , and makes the yearly return to labor low . These conditions contribute to serious instability and unrest in one of the fundamental industries of the country and this can be changed only by coal buyers doing their part to insure steady work . The railroads are now required to transport the maximum amount of coal during the winter months when traffic is most interfered with by bad weather and when the maximum amount of coal is used by the railroads to handle other coal . The railroads now use nearly 30 per cent of the total output of coal . About one- third of their freight traffic is hauling coal , so that economy of fuel used by the railroads will produce a considerable gross saving .

The success of longwall mining can largely be traced to the development of powered roof support systems. The most significant improvement in powered support design has been the shield support, which improved kinematic stability and promoted the application of longwall mining in difficult-to- control caving conditions where chock and frame supports were inadequate. The most obvious trend in shield design has been an increase in shield size and capacity. This U.S. Bureau of Mines report examines shield design and operation practices and their consequences for the utilization of high-capacity shield support systems. An optimization goal is to minimize support loading by selecting an active shield setting force that is compatible with strata behavior and shield loading characteristics. Shield stiffness is an important design parameter that is often overlooked. A consequence of increasing shield capacity by incorporating larger diameter leg cylinders is a proportional increase in shield stiffness. Setting forces have also increased in direct proportion to the increase in shield capacity. The increased stiffness and higher setting force cause the available capacity to be consumed more quickly, severely limiting the ability of high-capacity supports to last longer and provide reserve capacity for difficult mining conditions.

"The prevalence of accidents in the coal-mining industry has prompted a study of the workmen's compensation laws of the various States and the statistics of accident costs, for the purpose of explaining to operators how the compensation laws are regulated and to show the cost of accidents.The subject of compensation laws and accident costs is by no means new, but the prevalence of coal-mine accidents would indicate that the mine operator in general is not thoroughly conversant with their importance. Unquestionably, compensation laws are a large factor in promoting accident prevention, and it is believed that a greater measure of success would be accomplished in preventing accidents if all mine operators were fully aware of their important bearing on the cost of production.Many operators seem to be indifferent to accident occurrence; possibly they do not real¬ize the seriousness of the accident hazards that exist in their mines, even after the hazards are brought to their attention. There is no question that material financial advantages are to be gained by the prevention of accidents and it would appear that when these advantages are thoroughly understood, operators would be anxious to eliminate, insofar as is feasible, conditions likely to cause accidents.The unsettled state of the mineral industry and the irregular operation of mines during the past several years has probably been an important factor in influencing the apparently indifferent attitude of the operator. The chaotic conditions in the mining industry and especially in coal mining are unquestionably a factor that tends to increase accidents, just as mine idleness increases roof hazards and results in deterioration in mechanical, electrical, and haulage equipment, and promotes disorganization of the personnel. During such times greater efforts should be exerted toward the prevention of accidents because of these increased hazards and of the financial advantages brought about by the various savings to be effected when mine accidents are held to a minimum.This report on accidents in the coal mines of Pennsylvania was made possible largely through the cooperation of William J. Maguire, director, Bureau of Statistics, Pennsylvania Department of Labor and Industry, and Rush N.
