This chapter presents an overview of fatal and nonfatal injuries reported to MSHA during 1986-1995. Reportable injuries follow the criteria for reporting specified in 30 CFR 50. These criteria are listed in appendix A. The data used for these analyses came from the MSHA accident, injury, and illness data closeout files for the years 1986-1995. The MSHA data are described in appendix A and methods used in data analysis in appendix B. There may be slight discrepancies between the figures presented here and other presentations of MSHA data for the following reasons: (1) injuries to nonemployees have been excluded from analysis; and (2) injuries identified by MSHA after the files were closed out are not included. Within the MSHA data, there are differences in reporting requirements for operators and contractors. Contractors are required to report only those injuries that occur during certain categories of work (described in appendix A). For this reason, data on injuries among employees of operators and contractors in the MSHA database should not be considered directly comparable. As discussed in the "Introduction," the MSHA data on fatal injuries differ from the CFOI data (chapter 1) because of varying definitions used for the mining industry. As compared to the CFOI data, MSHA data exclude injuries that occurred off of mine property and include injuries occurring at mills and processing plants on mine property. Tables 4-1 through 4-9 and figure 4-1 provide comparisons of injury frequencies and rates among the commodities. These comparisons are followed by sections 4A through 4E, which provide more detailed tables and figures for each commodity separately. The method used to calculate injury rates is described in appendix B. Since denominator data for occupations (lob titles) were available for 1986 only, estimates of rates by occupation for the entire 10 year period are based on extrapolation from 1986 data. For this reason, estimates of injury by occupation should be interpreted with caution. Fatal Injuries A total of 1,078 fatal on-the-job injuries were reported to MSHA during the 10-year period 1986- 1995. The average annual fatal injury rate for all commodities combined during this period was 30.9 per 100,000 full-time equivalent workers. Mine operators accounted for 908 (84%) of the fatalities and had an average annual fatal injury rate of 28.2 per 100,000. While mine contractors comprised only 16% (n = 170) of the total number of fatalities, their average annual fatal injury rate was 2.3 times higher than that of mine operators (65.0 per 100,000). Table 4-1 presents the number and rates of fatal injuries by commodity for operators and contractors. Contractors in metal, nonmetal, stone, and sand and gravel are grouped together, since MSHA data on employment were not available for rate calculations for these commodities separately. The highest average annual fatality rate for the period, 66.5 per 100,000, occurred among metal and nonmetallic mineral mining contractors, followed closely by coal contractors (63.1 per 100,000). Among mine operators, the highest average annual fatal injury rate was in coal (38.9 per 100,000); the lowest was in nonmetal (14.3 per 100,000). Table 4-2 shows the average annual fatal injury rate by type of operation for each commodity for mine operators only. Of commodities with underground mines, all except stone had higher fatality rates at underground than at surface operations of these mines. The highest fatality rate for any operation occurred in surface operations at underground stone mines (124.5 fatal injuries per 100,000 workers). Stone had the second highest fatality rate for underground operations at underground mines (77.9 per 100,000). The highest rate for underground operations at underground mines occurred in metal (87.6 per 100,000). Table 4-3 shows the number and rate of fatal injuries by U.S. Bureau of the Census occupational groups (described in appendix D) for the 10 occupation-commodity groups with the highest fatal injury rates. These data should be interpreted with caution because the denominator used in calculating the rates was based on an extrapolation from data for the year 1986 only, as described in appendix B. Extractive occupations are represented in this group for three commodities: coal, metal, and sand and gravel. Construction trades are included for coal, sand and gravel, and nonmetal. The distribution of fatal injuries by commodity and work activity for mine operators is presented in table 4-4. A description of the work activity categories is in appendix C. Overall, the leading work activity at the time of injury was using or operating tools and machinery (26.4%) followed by constructing, repairing, and cleaning (24.0%), vehicular operations (18.8%), and materials handling (12.6%). The distribution varied by commodity. The largest percentage of fatal injuries occurred while operating tools and machinery in both coal and stone; during vehicular operations in metal; and during constructing, repairing, and cleaning activities in nonmetal and sand and gravel. Table 4-5 shows the distribution of fatal injuries by commodity and MSHA accident classification for mine operators. A description of the MSHA accident classifications appears in appendix E. The Federal Bureau of Mines conducted a study to determine if residues from fractional distillation represent a potential source of mineral values. A literature review and discussions with tar sand specialists failed to disclose the mineralogical nature of residues remaining after bitumen extraction. To develop this information, pilot plant tar sand residues or toluene-cleaned tar sands from 23 deposits were examined. The nature of the residues was determined by standard mineralogical methods and the chemical compositions were determined by X-ray and ICP (inductively-coupled plasma spectroscopy). None of the sands or concentrates examined contained sufficient valuable minerals to be economically important.

The U.S. Bureau of Mines conducted an inventory of the prospects and mineral resources in the Scodies RARE II area (No. 5212) in 1982. No mineral production is recorded from the area, and no mines are being developed. The Scodies RARE II area is comprised primarily of light colored granodiorite with minor inclusions of mafic granitic rocks. Late stage differentiates of magmas resulted in quartz veins and pods. Metasedimentary rocks which have been mineralized in some parts of the Sierra Nevada were not observed within the study area during this investigation but small inclusions have been reported.

This report describes the loading and transportation practices employed at the zinc-lead mines of The Eagle-Picher Co., Jo Daviess County, Ill., and Lafayette County, Wis. The report also gives a brief history of the Illinois-Wisconsin zinc-lead district and the operations of the company in the area. The diesel-powered, rubber-tired loading and haulage equipment is described and illustrated from the operational point of view only, and an analysis is made of the materials-handling system without going into health and safety factors. The costs per ton of the various components of mechanical loading, underground haulage, underground crushing, and hoisting for fiscal year 1960 are tabulated.

The National Institute for Occupational Safety and Health (NIOSH) is conducting a program of research to evaluate and improve remote mine seal construction technology for mine fire control and suppression. The main focus of this work is to develop reliable technology that will remotely seal a mine opening from floor-to-roof and rib-to-rib. NIOSH, in partnership with Strata Mine Services Inc.1, is evaluating the capability of a two-component phenolic foam-based product, called Rocsil®1, that is intended for cavity filling, air and gas sealing, and consolidation of highly fractured strata. Rocsil® can be rapidly applied, has a quick cure time, a high expansion rate, good compressive resistance and excellent anti-static properties. When the resin and catalyst are mixed, an immediate foaming reaction occurs followed by rapid expansion up to 30 times the original volume. By virtue of its fire-resistance properties, Rocsil® foam has application for sealing underground mine areas undergoing heating from spontaneous combustion and it could be useful for remote mine seal construction to aid in mine fire control and suppression. This paper presents an overview of remote mine sealing technology, introduces the use of Rocsil® foam for remote mine seal installations, offers a case study of the use of this rigid foam material to remotely construct a temporary in-mine seal and discusses technology improvements. The low-grade deposits of manganese ores along the Missouri River near Chamberlain, S. Dak., are known to contain one of the largest reserves of manganese in the United States. Manganese occurs as concretionary nodules ranging in size from 13 inches in their longest dimension to particles finer than 65-mesh. The nodules are embedded in soft, wet shale, which contains little manganese. The removal of nodules from the low-grade shale is a simple mechanical -problem; the question is primarily to determine the lowest-cost method of separation. This report purposes to present data acquired by experimenting with samples of the ore, which form a basis for suggesting methods of concentrating manganese nodules.

In 1986 the Bureau of Mines conducted a probability sample survey, Mining Industry Population Survey, to measure such employee characteristics as occupation; principal equipment operated; work location at the mine; present job, present company, and total mining experience; job-related training during the last 2 years; age; sex; race; and education. The population estimates are necessary to properly analyze the Mine Safety and Health Administration (MSWA) injury (includes illness and fatality data) statistics; that is, to compare and contrast injury rates for various subpopulations in order to identify those groups that are exhibiting higher than average injury rates. This report uses the survey's results to characterize the U.S. metallic mining workforce from March through September 1986. Similar reports have been published for the stone, sand and gravel, and nonmetallic mining industries, as well as for the entire metal and nonmetal mining (includes metallic, stone, sand and gravel, and nonmetallic industries) sector and the coal mining sector.

This Bureau of Mines study reviews the geological factors that affect methane in the Beckley coalbed in southern West Virginia, including overburden, coalbed structure, type of and changes in lithology above and below the Beckley coalbed, and presence of fractures in the coal and rock adjacent to the Beckley. Depending on the depth, the Beckley coalbed emits from 216 to 520 ft3 of methane per ton of coal mined, and the Bureau estimates that methane emission in a deep, extensively developed mine in this area may exceed 3,000 ft3 per day per ton of coal mined. Average face cleat and butt cleat trends in the Beckley coalbed measured underground are N 33° W and N 53° E, respectively. Close cleat spacing con¬tributes to the friable nature of the Beckley and provides easy movement for methane and water. Because rider coals, splits, and carbonaceous shales may contribute methane to mine emissions, predictions of methane emission based on gas content of the Beckley coalbed alone may be conservative. Beckley coalbed reserves in the study area are 234 million tons. The gas content of the Beckley coalbed averages 407 ft3/ton, and the coalbed gas reserves are 108 billion ft3.

Theoreticians in soil mechanics have been pursuing a probabilistic approach to the factor of safety of an embankment or darn for the past 10 years. The motivation for this work is in contrast to the current practice of deterministically computing a factor of safety and treating it as an absolute with no regard to its inherent statistical variability. Basic to the probabilistic approach is the selection of an appropriate statistical model to represent the histogram or probability density function (PDF) of the factor of safety values, Rather than simply assuming which PDF is appropriate, the Bureau of Mines collected data at two waste disposal embankments for consideration, This report addresses three candidate models, using the techniques of nonlinear curve fitting, and identifies the "best" model. A propagation of error formula for estimating the variability of Fellenius? factor of safety is also discussed.

A study to explore the potential of utilizing the transport time required to move coal miners to and from the working sections. Consideration was given to all forms of training and presentation media, taking into account such environmental factors as noise, dynamic interferences, visibility, physical constraints, and contaminants. Indications of managements? and workers' attitudes toward man-trip training were also assessed. The final product was a proposed prototype training system for man-trip training. This report discusses processing and use of the principal forms of manganese. The focus is on trends in the United States in the 20th century and specifics of domestic manganese material flows as of about 1990. To protect proprietary data, quantitative assessments are limited to the iron and steel industry, which accounts for about 90% of consumption. Nonmetallurgical uses are considered qualitatively. Approximate disposition of a U.S. cumulative consumption during 1901-90 of an estimated 69 million st of manganese is estimated to be iron and steel slag, 69%; nonmetallurgical uses of a dissipative na¬ture, 10%; iron and steel scrap, 9%; National Defense Stockpile, 3%; unidentified and/or including items in service, 10%. In slag from iron blast furnaces and steelmaking, manganese is present as a minor, relatively im¬mobile component, which so far has been uneconomical to recover. Construction and road building are main applications for these slags. Steel slag contains the greater amount of manganese. Studies by others of global manganese fluxes and the effect of human activities on them also are discussed. One source concludes that the main effect of human activities, via deforestation and agricultural activities, has been to increase the rate of transport of manganese to the oceans.

This report is one of a series published by the Bureau of Mines describing methods and practices at various mines throughout the United States. A previous program, conducted from 1928 to 1939, was well received by the mining industry and technical institutions and provided much source material for engineering textbooks and technical handbooks. The stringent wartime demands on the mining industry, the rapidly changing postwar economic situation, and greatly accelerated technologic growth have combined to render many of the previous data obsolete. The objective of the present program is to assemble reliable technical information on mining methods and practices throughout the country and present it for the use of the mining industry, technical schools, textbook authors, and other interested persons. This report deals with the design and development of the Berkeley open- pit mine at Butte, Mont., owned and operated by The Anaconda Company. The Berkeley pit is but one facet of a well-organized and integrated copper-mining operation involving the entire Butte district. Vein-type mining, important for 75 years, is still in progress at depths from 600 feet to nearly 1 mile; on intermediate levels, low-grade ore left untouched by earlier vein-type mines is being extracted by block caving entire ore zones,3/ and secondarily enriched but lower grade near-surface portions of the same ore zones are being mined by open-pit operations (fig. 1). The Berkeley pit itself is named for the Berkeley mine, one of the old mines that extracted copper ore from high-grade veins in the area that now will be mined by open-pit methods.

Greensand, which is widely distributed in the Eastern United States, was produced in 1975 only by Inversand Co., a subsidiary of Hungerford & Terry, Inc., near Clayton, N.J. Production and sales information are not available for publication. Raw greensand produced by the company was sold for agricultural use as a soil conditioner. Processed greensand was used by manufacturers as filter media in equipment for the removal of manganese, iron and other elements from water. The University of Delaware received a grant of $17,051 from the Bureau of Mines early in 1976 to continue its studies of the effectiveness of greensand in removing metallic contaminants from waste waters.

Although known since 1902, the Capitan iron deposits have not been tined, mainly because of the low grade of the ore. Only a small amount of exploratory work has been done since the discovery. C. H. Johnson, district engineer of the Bureau of Mines in New Mexico in 1942-4, visited the property; as a result, preliminary sampling of the property was done in 1943.2/ Owing to the results of this preliminary work, an extensive development program by wagon drilling was planned and executed early in 1944. Equipment available at that time was limited in the depths that could be reached; and as geologic studies indicated the possibility that other limestone beds below the surface might have been replaced by iron minerals, a supplementary development program by churn drilling to reach greater depths was prosecuted in late 1947 and early 1948, with the author as project engineer.
